1.Correlation of the steady-state minimal concentration with AUC24/MIC of vancomycin and analysis of risk factors for treatment failure in pediatric patients

OBJECTIVE To assess the correlation between the steady-state minimal concentration (cmin) and 24 h area under the drug concentration-time curve (AUC24)/minimal inhibitory concentration (MIC) ratio (AUC24/MIC) of vancomycin in pediatric patients, and analyze independent risk factors for treatment failure. METHODS Data of hospitalized children treated with vancomycin and receiving therapeutic drug monitoring in our hospital from January 2021 to July 2024 were retrospectively collected and divided into success group and failure group according to whether the treatment was successful or not. Spearman correlation analysis was used to analyze the correlation between cmin and AUC24/MIC of vancomycin, and one-way and multifactorial Logistic regression analyses were used to screen the independent risk factors for vancomycin treatment failure. RESULTS A total of 59 children were included, with 41 in the success group and 18 in the failure group. Compared with the failure group, AUC24/MIC of vancomycin was significantly higher in the success group (P=0.038), but there was no statistically significant difference in the cmin of the two groups (P>0.05); cmin of vancomycin was significantly positively correlated with AUC24/MIC (r=0.499, P<0.001), but it has a certain efficacy in predicting the achievement of the AUC24/MIC standard (≥400) (area under the receiver operator characteristic curve=0.696), with an optimal cutoff value of 6.05 mg/L determined by the Youden index. The efficacy of AUC24/ MIC in predicting treatment failure was superior to cmin (areas under the receiver operator characteristic curve were 0.671 vs. 0.523, P were 0.038 vs. 0.684), with higher sensitivity (83.3% vs. 66.7%). Hypoproteinemia and AUC24/MIC≤369.1 were independent risk factors for vancomycin treatment failure (P<0.05). The incidence of nephrotoxicity was 3.4%. CONCLUSIONS There is a significant positive correlation between cmin and AUC24/MIC of vancomycin in pediatric patients; hypoproteinemia and AUC24/MIC≤369.1 are independent risk factors for vancomycin treatment failure in children.

3.Analysis on the status quo of pediatric nurses' cognition of death education and its influencing factors

Objective:To explore the status quo of pediatric nurses' cognition of death education, and analyze its influencing factors, so as to provide experience and reference for carrying out death education in pediatric wards.Methods:In this study, 827 nurses from 15 children's hospitals in China were investigated by using the scale for evaluating the cognition of death education in medical staff prepared by Zhang Yan-gai, and the related factors affecting the cognition of death education were analyzed. SPSS 23.0 was used for independent-samples t-test and ANOVA analysis, and multiple regression equation was used to further analyze the relationship between the cognitive factors of death education in pediatric nurses. Results:The total score of death education cognition of pediatric nurses was (35.61±5.64) points, which was lower than the median score of 36 points. The degree of death education cognition of pediatric nurses was at the medium-low level. The results of correlation analysis showed that the professional title of nurses (regression coefficient: 0.064, P=0.045), the training demand for death education (regression coefficient: 0.300, P<0.001), and the way of understanding (regression coefficient: 0.018, P=0.003) were independent influencing factors of the death education cognition level of pediatric nurses. Conclusions:Pediatric nurse death education related theoretical knowledge should be improved through various professional trainings. The hospitals should include death education in pediatric nurse continuing education training plan to improve pediatric nurses' cognitive level of death education through a variety of ways and means, promote the application of death education activities carried out in pediatric ward in the hospice care, reduce deaths impact to nurse their own emotions, and alleviate parents' anxiety and painful experience, thereby improving medical satisfaction.
4.MDT treatment model for diagnosis and treatment of retroperitoneal soft tissue sarcoma in the context of a regional medical center: report of 1 case

Objective:To explore the clinical value of multidisciplinary team (MDT) treatment model for retroperitoneal soft tissue sarcoma (RPS) in the context of a regional medical center.Methods:The diagnosis and treatment of a patient with RPS in MDT model who were admitted to Xiamen Branch, Zhongshan Hospital, Fudan University in October 2020 were summarized, and the literature was reviewed.Results:The patient was a 59-year-old male, he was diagnosed with retroperitoneal INI-1-deficient poorly differentiated sarcoma with multiple systemic metastases. After several rounds of MDT discussions in Shanghai headquarter and Xiamen Branch of Zhongshan Hospital, Fudan University, the patient received systemic antitumor therapy combined with local therapy. After active treatment, the patient obtained a good curative effect. During the follow-up period, the liver and lung lesions slowly enlarged, and the treatment plan was adjusted. The patient had survived for more than 3 years.Conclusions:The diagnosis and treatment of RPS is complex, and the MDT model in the context of a regional medical center can help to formulate the optimal treatment plan for patients and may maximize the survival benefit of patients.
5.Construction and validation of a predictive model for septic shock based on propensity score matching

Objective To construct a predictive model for septic shock based on the propensity score matching (PSM) method and validate its effectiveness. Methods A total of 114 patients with sepsis were enrolled as study objects, and were divided into septic shock group (40 patients) and sepsis group (74 patients) according to whether they developed septic shock. PSM was performed with a ratio of septic shock to sepsis of 1∶2, resulting in the inclusion of 30 patients in the septic shock group and 60 patients in the sepsis group after matching. The levels of C-reactive protein (CRP), procalcitonin (PCT), interleukin-6 (IL-6), serum amyloid A (SAA), soluble endothelial protein C receptor (sEPCR), endothelial cell-specific molecule 1 (ESM-1), clusterin (CLU), and the Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ) score and Sequential Organ Failure Assessment (SOFA) score at admission were compared between the two groups. Cox proportional hazards regression analysis was used to identify the factors influencing septic shock, and a predictive model for septic shock was constructed and internally validated using the receiver operating characteristic (ROC) curve. Kaplan-Meier survival curves were plotted to analyze the differences in survival prognosis among patients with different expression levels of the indicators. Objective:To evaluate clinical efficacy and safety of topical compound oleum lithospermi in the treatment of mild to moderate diaper dermatitis.Methods:A multicenter, randomized, positive-drug parallel-controlled clinical trial was conducted in 19 hospitals from July 2019 to August 2020. Children aged 0 - 12 months with mild to moderate diaper dermatitis were enrolled and randomly divided into 2 groups using a random number table: test group topically treated with compound oleum lithospermi, and control group topically treated with zinc oxide cream. The treatment was carried out 6 - 8 times a day for 7 days. Visits were scheduled on days 0 and 7, and total response rate and clinical healing time were evaluated. Changes in the dermatitis family impact (DFI) score were compared between the test group and control group, and adverse events were recorded. Statistical analysis was carried out by using independent-sample t test for normally distributed continuous data, Wilcoxon rank sum test for non-normally distributed continuous data, and chi-square test or Fisher′s exact test for unordered categorical data; survival curves were drawn, and log-rank test was used for comparisons between two groups. Results:A total of 343 children with diaper dermatitis were enrolled in this study. Among them, 31 children violated the protocol, so 312 were included in the per protocol set, including 157 in the test group and 155 in the control group, and all completed the visits on days 0 and 7. The total response rate was significantly higher in the test group (87.26%, 137/157) than in the control group (78.71%, 122/155; χ2 = 4.04, P = 0.044) . The clinical healing time was significantly shorter in the test group (5.33 days) than in the control group (6.13 days; χ2 = 4.67, P = 0.025) . After 7-day treatment, the DFI score significantly decreased in both the 2 groups compared with that before the treatment, but there was no significant difference in the DFI score between the 2 groups (test group: 4.02 ± 6.96, control group: 3.58 ± 5.90, Z = -0.39, P = 0.686) . The incidence of adverse events was 2.92% (5/171) and 5.45% (9/165) in the test group and control group respectively, and there was no significant difference between the 2 groups ( χ2 = 0.03, P = 0.865) . Conclusion:Compound oleum lithospermi can markedly reduce the clinical severity of diaper dermatitis, improve the total response rate, shorten the clinical treatment period, and improve the quality of life of children′s families with a favorable safety profile.
7.Application of micro-video combined with bilingual standardized patients on dermatovenerology clinical practice in international medical students

Objective:To investigate the efficacy of micro-video based on WeChat platform combined with bilingual standardized patients on clinical practice of dermatovenerology in international medical students.Methods:A total of 56 international clinical medical students from China Medical University who had their clinical practice class from May 2017 to June 2017 in Shengjing Hospital of China Medical University were selected as research objects, and were randomly divided into the experimental group and the control group. New teaching method of micro-video based on WeChat platform combined with bilingual standardized patients was applied in the experimental group and the traditional teaching method was used in the control group. After courses, the theoretical examination, clinical case assessment and satisfaction degree of the two groups were compared to assess the learning effect. All statistical analyses were performed with the independent sample t-test and Chi-square test with SPSS 21.0 software.Results:There was no significant difference in the theoretical test scores between two groups ( t=1.48, P=0.144), but the scores of the clinical case test in the experimental group were significantly better than those in the control group ( t=2.22, P=0.031). Results of satisfaction survey showed that more foreign students preferred new teaching methods, with a statistically significant difference ( χ2=7.24, P=0.007); in terms of consolidating theoretical knowledge ( χ2=4.766, P=0.029), motivating learning interest ( χ2=4.073, P=0.044), enhancing clinical practice skills ( χ2=4.667, P=0.031), promoting communication ability with patients ( χ2=6.411, P=0.011) and improving learning effect ( χ2=4.667, P=0.031), foreign students were more satisfied with new teaching methods, with a statistically significant difference. Conclusion:In the dermatovenerology clinical practice among international medical students, application of micro-video based on WeChat platform combined with bilingual standardized patients can improve the teaching effect, which can be further popularized and applied as a new teaching method.
8.Rapid Screening of Illegally Added Chemical Fungicide in Pesticide Formulations by Ultra Performance Liquid Chromatography- Quadrupole-Time of Flight Mass Spectrometry

A method of ultra performance liquid chromatography-quadrupole-time of flight-mass spectrometry ( UPLC-Q-TOF MS) was developed to determine 35 kinds of illegally added chemical fungicides in pesticide formulations. The samples were pretreated based on the ultrasonic extraction by the solvent of methanol, and then separated on a Zorbax C18(100 mm×1. 8 mm, 2. 1 μm) column by a gradient elution with 0. 1% formic acid aqueous solution and acetonitrile as the mobile phase. The mass spectrometer was operated under positive mode. Under the optimal conditions, the recoveries at three spiked levels (0. 2, 0. 4, and 2. 0 mg/kg) were in the range of 81. 0%-101. 3% and the RSDs were 1. 0%-4. 4%. Based on the developed method, 100 samples were analyzed, and among which 6 samples were screened out chemical fungicides. The proposed method was high-efficient, accurate and reliable for the qualitatively screening of illegaly added chemical fungicides.
9.Association between glycosylated hemoglobin and non-alcoholic fatty liver disease in health examination population

Objective:To investigate associations between glycosylated hemoglobin (HbA1c) levels and nonalcoholic fatty liver disease (NAFLD) in population who underwent periodic heath examination.Methods:A total of 912 subjects were selected from the health examination adults in University of Hong Kong Shenzhen Hospital from June to November 2013,and they were assigned into a NAFLD group,an else hepatic disease group,and a control group.The physical examination results of these subjects were analyzed retrospectively.Results:The prevalence of NAFLD was 29.7%.The related factors such as age,body mass index,and the levels of systolic blood pressure,diastolic blood pressure,total cholesterol,triglyceride,low density lipoprotein cholesterol,fasting glucose,HbA1c and alanine aminotransferase in the NAFLD group were higher than those in the else hepatic disease group and the control group (P<0.05).Aspartate aminotransferase levels in the NAFLD group were higher than those in the control group (P<0.05).High density lipoprotein cholesterol levels in the NAFLD group were lower than those in the else hepatic disease group and the control group (P<0.05).According to the quartile of HbAlc level,these subjects were divided into 4 groups,Q1,Q2,Q3,and Q4,and the prevalence of NAFLD in the Q1,Q2,Q3,and Q4 were 17.5%,21.5%,31%,and 59.4%,respectively.The prevalence of NAFLD increased along with the increase in the level of HbAlc (x2=100.092,P<0.05).Logistic regression analysis showed that the risk factor for NAFLD was increased in the subjects of Q4 as compared with the subjects of Q1 after adjusting for the related factors including age,gender,body mass index,etc.Conclusion:There is a positive correlation between the prevalence of NAFLD and HbAlc level,and the risk of NAFLD increases with the elevating level of HbA1c.High HbA1c level is an independent risk factor for NAFLD.Thus it should be listed as a routine medical test in the health examination.
10.Practice of modular teaching in the microbiological test experiment of medical laboratory science

In order to adapt to the requirements of the modern clinical laboratory to medical laboratory technology personnel operation ability,our hospital has modified professional microbiology experiment course to modular teaching from the traditional teaching mode.In the process of teaching experiment,the experiment course and clinical practice class are arranged,and the experiment course of microbiology is divided into five modules:basic skills,application,comprehensive training,extension,and clinical practice module.Basic skills module focuses on the foundation that students learn to observe the microbial morphology and master the basic operation technology,at the same time,cultivates the students' sterile ideas and biological safety;Application module pays attention to the detection of various types of bacteria,lets the student have as many times of trying as possible,battle-hardened;Comprehensive training module emphasizes students' ability of analyzing and resolving problems;Extension module guides students actively to make diffusing thinking and comprehensive analysis of problems;The final clinical practice module that combines theory and practice,further consolidates the basic operation skills,cultivates students' comprehensive ability,improves students' the independent working ability and professional thinking and habits.Five modules link up with each other closely and have progressive layers of the process.In sum,modular teaching motivates the students' interest in learning,solves the problem of students' insufficient operating ability,improves the teaching effect and provides a reform method for improving the quality of microbiological test experiment.
