1.UPLC-Q-TOF-MS Reveals Mechanisms of Modified Qing'e Formula in Delaying Skin Photoaging and Regulating Circadian Rhythm

		                        			ObjectiveTo reveal the active substances and mechanisms of modified Qing'e formula (MQEF) in delaying skin photoaging by ultra-performance liquid chromatography-quadrupole-time of flight mass spectrometry (UPLC-Q-TOF-MS),network pharmacology, and cell experiments. MethodsUPLC-Q-TOF-MS and a literature review were employed to analyze the transdermally absorbed components in mice after the topical application of MQEF. The potential targets of MQEF in treating skin photoaging were retrieved from databases.The compound-potential target network and protein-protein interaction network were constructed to screen the key components and core targets. A photoaging cell model was established by irradiating HaCaT cells with medium-wave ultraviolet B (UVB). The safe doses of bakuchiol (BAK) and salvianolic acid B (SAB) for treating HaCaT cells and the effects of BAK and SAB on the viability of cells exposed to UVB irradiation were determined by the cell counting kit-8 (CCK-8) method.The reactive oxygen species (ROS) fluorescent probe was used to measure the ROS production in the cells treated with BAK and SAB.The expression levels of genes related to oxidative stress,inflammation,collagen metabolism,and circadian rhythm clock were measured by Real-time PCR. ResultsA total of 24 transdermally absorbed components of MQEF were identified,which acted on 367 potential targets,and 417 targets related to skin photoaging were screened out,among which 47 common targets were predicted as the targets of MQEF in treating skin photoaging. MQEF exerted the anti-photoaging effect via key components such as BAK and SAB,which acted on core proteins such as serine/threonine kinase 1 (Akt1) and mitogen-activated protein kinase 3 (MAPK3) and intervened in core pathways such as the tumor necrosis factor (TNF) and phosphatidylinositol-3-kinase (PI3K)/protein kinase B (Akt) signaling pathways.Compared with the model group,the administration of BAK and SAB increased the survival rate of HaCaT cells (P<0.01),down-regulated the mRNA levels of cyclooxygenase-2 (COX-2),interleukin-6 (IL-6),tumor necrosis factor-α (TNF-α),matrix metalloproteinase-1 (MMP-1),and matrix metalloproteinase-9 (MMP-9) (P<0.01),and up-regulated the mRNA levels of heme oxygenase-1 (HO-1) and NAD(P)H quinone dehydrogenase 1 (NQO-1) (P<0.05,P<0.01) in photoaged HaCaT cells.In addition,it eliminated excess ROS production induced by UVB and up-regulated the mRNA levels of brain and muscle ARNT-like 1 (BMAL1) and circadian locomotor output cycles kaput (CLOCK) associated with circadian clock (P<0.05,P<0.01). ConclusionMQEF delays skin photoaging through the coordinated effects of various components,multiple targets,and diverse pathways.The key components BAK and SAB in MQEF exhibit anti-photoaging properties,which involve inhibiting oxidative stress,preventing collagen degradation,mitigating inflammation,and maintaining normal skin circadian rhythms by regulating clock gene expression.

		                        			Exposure to ionizing radiation intervenes in genomic stability and gene expression,resulting in the disruption of normal metabolic processes in cells and organs by causing complex biolog-ical responses.Altered genomic variations,gene expression and metabolite concentrations in blood or tissue samples reflect systemic radiation damage.With the application of new techniques and exten-sive study on the mechanisms for ionizing radiation damage,related indicators such as chromosomal variation,gene expression,lipid and metabolism are being recognized and promise to be the markers for early diagnosis and prognosis of radiation exposure.Therefore,this article reviews recent progress in and potential applications of biomarkers related to ionizing radiation injury.
		                        		
		                        		
		                        		
		                        	
3.Etiological investigation and prognostic analysis of chronic renal failure in middle-aged and elderly people

		                        			Objective  To investigate the etiology of chronic renal failure (CRF) in middle-aged and elderly people, and to analyze related factors influencing the prognosis.  Methods  The clinical data of 456 middle-aged and elderly CRF patients treated in our hospital from January 2018 to January 2021 were collected. The etiology and related factors affecting prognosis were analyzed.  Results  The etiology of 456 middle-aged and elderly patients with CRF was as follows: 220 cases of primary glomerulonephritis (48.24%), 78 cases of diabetic nephropathy (17.14%), 65 cases of hypertensive nephropathy (14.25%), and 37 cases of pyelonephritis (8.11%), 19 cases of drug-induced renal injury (4.16%), 18 cases of obstructive nephropathy (3.95%), 12 cases of lupus nephritis (2.63%), and 7 cases of polycystic kidney disease (1.54%). The age of onset in patients with primary glomerulonephritis and lupus nephritis was lower than that in patients with other causes (P<0.05). The results showed that 94 of the 456 patients died in the hospital, and the mortality rate was 20.6%. Multivariate logistic regression analysis showed that age, severe infection, cerebrovascular disease, arrhythmia, hypertension stage III, left ventricular hypertrophy, and myocardial infarction were high-risk factors leading to poor prognosis (P<0.05).  Conclusion The main causes of CRF in middle-aged and elderly patients are primary glomerulonephritis, diabetic nephropathy, and hypertensive nephropathy. Heart disease is the main factor leading to the poor prognosis of such patients. Efforts shall be put into disease prevention and control work for high-risk groups, so as to reduce the incidence and mortality of CRF.
		                        		
		                        		
		                        		
		                        	
4.Status and influencing factors of knowledge, attitude and practice and influencing factors of catheter-associated urinary tract infections prevention and control among community nurses in Pudong New Area in Shanghai

		                        			Objective:To understand the current situation of knowledge, attitude and practice in the prevention and control of catheter-associated urinary tract infections (CAUTI) among community nurses in Pudong New Area in Shanghai and analyze influencing factors, so as to provide a basis for better community-based CAUTI prevention and control.Methods:Using the stratified random sampling method, a total of 286 community nurses from 4 areas in Pudong New Area in Shanghai were selected as the research objects from April to June 2021. The community nurses were surveyed by general information questionnaire, training situation questionnaire and CAUTI Prevention Knowledge, Attitude and Practice Scale. Univariate analysis and multiple linear regression were used to analyze the current situation and influencing factors of CAUTI prevention and control knowledge, attitude and practice among community nurses. Pearson correlation analysis was used to explore the correlations between knowledge, attitude and practice of CAUTI prevention and control among community nurses.Results:A total of 286 questionnaires were sent out and 283 valid questionnaires were recovered, with the effective recovery rate of 98.95%. Only 52.30% (148/283) community nurses had participated in CAUTI related training. The scores of CAUTI prevention and control knowledge, attitude and practice of community nurses were (85.04±5.31) , (23.28±2.73) and (21.39±2.07) , respectively. Univariate analysis showed that there were statistically significant differences in CAUTI prevention and control knowledge, attitude and practice scores among community nurses with different working years, professional titles, age, whether or not to train, training forms and training times ( P<0.01) . Multiple linear regression analysis showed that professional title and training frequency were the influencing factors of CAUTI prevention and control knowledge and practice of community nurses ( P<0.05) . Pearson correlation analysis showed that there were positive correlations between CAUTI prevention and control knowledge and attitude ( r=0.555, P<0.01) , knowledge and practice ( r=0.501, P<0.01) and attitude and practice ( r=0.417, P<0.01) . Conclusions:The level of CAUTI prevention and control knowledge, attitude and practice of community nurses in Pudong New Area in Shanghai needs to be improved and CAUTI-related training needs to be strengthened urgently. Medical institutions should actively carry out targeted CAUTI prevention and control knowledge training and assessment, improve CAUTI prevention and control knowledge reserves of community nurses, update prevention and control concepts, enhance community nurses' initiative and confidence in prevention and control, correctly implement various prevention and control measures and actively give play to the role of community nurses in the prevention and control of CAUTI.

Each of the fasting group and the postprandial group was enrolled in 52 healthy subjects. 6.A study on the correlation between semantic distance and cognitive function in schizophrenia

		                        			Objective:To investigate the relationship between lexical semantic processing abnormalities and cognitive function and clinical symptoms in schizophrenia based on the activation diffusion model and semantic distance.Methods:All subjects matched with demographic data were randomly divided into experiment group( n=27) and control group( n=30). The semantic distance of words of different semantic categories and lexical categories were assigned at a level of 1-7.The semantic distance between the two groups was analyzed by repeated measures of variance, and the correlation analysis was conducted on the scores of the MATRICS Consensus Cognitive Battery (MCCB) and positive and negative syndrome scale (PANSS) in the experimental group. Results:The main effects of the groups in semantic distance were statistically significant( F(1, 55)=7.460, P=0.008, ηp2=0.981). The semantic distance in the experimental group (3.672±0.105)was lower than that in the control group (4.068±0.100). The main effect of the semantic category was statistically significant( F(1, 55)= 833.37, P<0.01, ηp2=0.938), and the semantic distance in the relative condition (1.965±0.074) was lower than that in the unrelative condition(5.775±0.117). Under the relative condition, concrete semantic deviation was negatively correlated with the total score of MCCB( r=-0.405, P=0.036)and speech learning ( r=-0.569, P=0.002). Abstract semantic deviation was negatively correlated with speech learning( r=-0.429, P=0.026). Under the unrelative condition, concrete semantic deviation was negatively correlated with MCCB total score ( r=-0.597, P=0.001), speech learning( r=-0.399, P=0.039)and other dimensions.Abstract semantic deviation was negatively correlated with MCCB total score( r=-0.593, P=0.001) and speech learning( r=-0.285, P=0.050). Under the relative condition, the abstract semantic deviation was positively correlated with the general symptom scale score( r=0.448, P=0.019). Under the unrelative condition, abstract vocabulary semantic deviation was positively correlated with PANSS total score( r=0.120, P=0.010), negative scale score( r=0.047, P=0.030) and general symptom scale score ( r=0.306, P=0.010). Conclusion:The semantic distance of lexical processing is abnormal in schizophrenia. 7.Research progress of interbody fusion cage and its materials

		                        			BACKGROUND:With the rapid development of technology, most interbody fusion cages have been applied in clinic, but some are stil in experimental stage. Al fusion cages hold their own distinct characteristics as wel as advantages and shortcomings. OBJECTIVE:To clarify the development course of interbody fusion cages and analyze their types in detail, thereby paving ways for its improvement. METHODS:A computer-based search for literatures in CNKI and PubMed databases published from January 1989 to March 2016 was performed using the keywords of“interbody fusion cage, PEEK, TMC”in English and Chinese, respectively. A total of 106 articles were retrieved and 38 articles were in accordance with the inclusion criteria. RESULTS AND CONCLUSION:Intervertebral fusion is a major method to treat spinal degenerative diseases. There are a variety of fusion cages in clinic, such as pyramesh, polyetheretherketone and absobable fusion cages. Implants used for fusion cages include autogenous, al ogenic and artificial bones as wel as bone morphogenetic protein. We introduce the development process of fusion cages in detail, emphasize the commonly used fusion cages and analyze its development in future. 8.Clinical study of Dahuang-Mudan decoction by retention enema asan adjuvant treatment for the patients with acute pelvic inflammation disease and toxic heat flourishing type

		                        			Objective To study the effect of Dahuang-Mudan decoctionby retention enemafor the thepatients with acute pelvic inflammation disease (APID) and toxic heat flourishing type. Methods Ninety patients with APID (toxic heat flourishing type) were randomly divided into treatment group and control group (45 case per group). The patients in both groups were given cefamandolenafate injection and tinidazole injection;and the Dahuang-Mudan decoction by retention enema was added to the treatment group.The C-reactive protein (CRP), tumor necrosis factor-α(TNF-α), interleukin-6 (IL-6) and clinical symptoms scores in both groups before treatment and after 2 weeks of treatment were observed. Results After treatment, CRP (3.16 ± 1.25 mg/L vs. 8.68 ± 1.45 mg/L, t=19.126), TNF-α(4.02 ± 1.06 ng/L vs. 9.64 ± 2.08 ng/L, t=15.966), IL-6 (60.34 ± 14.35 ng/L vs. 88.24 ± 12.16 ng/L, t=9.839), temperature recovery time (3.22 ± 1.13 d vs. 4.73 ± 1.36 d, t=5.638), abdominal pain disappeared time (5.26 ± 1.43 d vs. 7.45 ± 1.32 d, t=7.418), leukocyte recovery time (7.34 ± 2.06 d vs. 9.58 ± 2.26 d, t=4.834), hospital stay time (10.24 ± 3.42 d vs. 13.43 ± 4.38 d, t=3.791) in the treatment group were significantly lower than those in the control group (P<0.01). The clinical effect rate (77.27% vs. 48.84%, χ2=7.562) in the treatment group were significantly higher than that in the control group (P<0.01). Conclusions The treatment of APID (toxic heat flourishing type) with Dahuang-Mudan decoction by retention enema can reduce the inflammatory response of pelvic, promote the absorption of exudate and adhesion in pelvic.
		                        		
		                        		
		                        		
		                        	
9.Clinical Effect of EaLeSu on CIN Ⅱ~ⅢPatients with HPV Infection Treated with LEEP

		                        			ObjectiveTo investigate the clinical effect of EaLeSu on cervical intraepithelial neoplasia(CIN Ⅱ~Ⅲ)patients with HPV infection treated with loop electrosurgical excision procedure(LEEP). Methods 165 cases of CIN Ⅱ~Ⅲpatients with HPV infection who underwent LEEP operation were randomly divided into observation group(n=83)and control group(n=82). In the observation group,patients were given a piece of EaLeSu in cervical wound at once in postoperative and two vaginal EaLeSu per day from the first postoperative day for eight consecutive days. The patients in the control group were treated with routine debridement and wound dressing with iodine. We observed the vaginal drainage times within 4 weeks after operative,bleeding rate,cervical wound repair,and cervical HPV DNA infection after 6 months. Results There were significant differences in the bleeding rate,vaginal drainage time,cervical wound repair and HPV DNA infection between the two groups(P < 0.05). 10.Application of “patient-family-community”health promotion mode in patients with impaired glucose regulation

		                        			Objective To investigate the effects of “patient-family-community”health promotion mode on patients with impaired glucose regulation(IGR).Methods Using the function of random case samples in SPSS,we randomly chose an experimental village and a control village respectively from all villages in Zhuqiao of Pudong New Area.Altogether 75 IGR patients who met the requirement were filtered in the two villages. According to the villages they belonging to,39 of them were divided into experimental group and 36 to control group.From March 201 4 to August 201 4, subjects in the experimental group were intervened by“patient-family-community”health promotion mode,while participants in the control group used the regular health guidance.We compared the test score of knowledge-attitude-practice (KAP) questionnaire,family APGAR questionnaire and blood glucose between two groups.Results Relevant indexes of the two groups before intervention showed no difference (P >0.05).After intervention,the knowledge(K),attitude(A)and practice (P)test scores of the experimental group were (8.85 ±1 .31 ),(8.87 ±1 .77)and (6.92 ±2.67),higher than (5.86 ±2.37),(6.78 ±2.92)and (3.89 ±3.02)of the control group(Z =-5.1 89,-3.299,-4.042;P <0.05).The APGAR test scores of the experimental group(7.56 ±1 .90)was higher than(6.47 ±1 .77)of the control group(t =2.571 ,P <0.05).The FPG,2hPG and HbA1 c of the experimental group were (5.86 ± 0.50)mmol/L,(8.02 ±0.72)mmol/L and(5.73 ±0.42)%,which were lower than (6.1 1 ±0.45)mmol/L, (8.60 ±0.97)mmol/L and(5.97 ±0.53)% of the control group (t =-2.293,-2.383,-2.1 70;P <0.05).Conclusions Applying “patient-family-community”health promotion mode can improve the KAP, family function of IGR patients,control their blood glucose better and prevent diabetes.
