Objective:To evaluate the efficacy and safety of mitoxantrone hydrochloride liposome injection in the treatment of peripheral T-cell lymphoma (PTCL) in a real-world setting.Methods:This was a real-world ambispective cohort study (MOMENT study) (Chinese clinical trial registry number: ChiCTR2200062067). Clinical data were collected from 198 patients who received mitoxantrone hydrochloride liposome injection as monotherapy or combination therapy at 37 hospitals from January 2022 to January 2023, including 166 patients in the retrospective cohort and 32 patients in the prospective cohort; 10 patients in the treatment-na?ve group and 188 patients in the relapsed/refractory group. Clinical characteristics, efficacy and adverse events were summarized, and the overall survival (OS) and progression-free survival (PFS) were analyzed.Results:All 198 patients were treated with mitoxantrone hydrochloride liposome injection for a median of 3 cycles (range 1-7 cycles); 28 cases were treated with mitoxantrone hydrochloride liposome injection as monotherapy, and 170 cases were treated with the combination regimen. Among 188 relapsed/refractory patients, 45 cases (23.9%) were in complete remission (CR), 82 cases (43.6%) were in partial remission (PR), and 28 cases (14.9%) were in disease stabilization (SD), and 33 cases (17.6%) were in disease progression (PD), with an objective remission rate (ORR) of 67.6% (127/188). Among 10 treatment-na?ve patients, 4 cases (40.0%) were in CR, 5 cases (50.0%) were in PR, and 1 case (10.0%) was in PD, with an ORR of 90.0% (9/10). The median follow-up time was 2.9 months (95% CI 2.4-3.7 months), and the median PFS and OS of patients in relapsed/refractory and treatment-na?ve groups were not reached. In relapsed/refractory patients, the difference in ORR between patients with different number of treatment lines of mitoxantrone hydrochloride liposome injection [ORR of the second-line, the third-line and ≥the forth-line treatment was 74.4% (67/90), 73.9% (34/46) and 50.0% (26/52)] was statistically significant ( P = 0.008). Of the 198 PTCL patients, 182 cases (91.9%) experienced at least 1 time of treatment-related adverse events, and the incidence rate of ≥grade 3 adverse events was 66.7% (132/198), which was mainly characterized by hematologic adverse events. The ≥ grade 3 hematologic adverse events mainly included decreased lymphocyte count, decreased neutrophil count, decreased white blood cell count, and anemia; non-hematologic adverse events were mostly grade 1-2, mainly including pigmentation disorders and upper respiratory tract infection. Conclusions:The use of mitoxantrone hydrochloride liposome injection-containing regimen in the treatment of PTCL has definite efficacy and is well tolerated, and it is a new therapeutic option for PTCL patients.
2.An acute kidney injury risk prediction model for elderly patients with sepsis based on the intensive care medicine information database-Ⅳ

Objective:To explore the risk factors of acute kidney injury(stage 3)developed within 48 hours in elderly patients with sepsis, and to use them to develop a risk prediction model and then evaluate and externally validate the model.Methods:Clinical data of all elderly patients(age≥ 60 years)with sepsis in the intensive care medicine information database(MIMIC-Ⅳ v1.0)were extracted.Independent risk factors were determined by multivariate logistic regression analysis.A risk prediction model was constructed, a nomogram was drawn, and the receiver operating characteristic curve(ROC)and the Hosmer-Lemeshow(H-L)test were used to evaluate the model's prediction accuracy and R-squared.Clinical data of elderly patients(age≥ 60 years)with sepsis admitted to the Department of Critical Care Medicine of the Second People's Hospital of Hefei from May 2019 to October 2021 were retrospectively collected and fed into the prediction model to conduct external validation.Results:A total of 1 977 elderly patients with sepsis were screened out from the MIMIC-IV database and included in the training set, of whom, 544 developed AKI-stage 3 within 48 hours.Univariate analysis was performed for factors that might be associated with acute kidney injury in elderly patients with sepsis.Compared with the normal group that did not progress to AKI stage 3, there were statistically significant differences in 28 indicators, such as the duration of ICU stay, intravenous fluid intake in 24 hours, and use of vasoactive drugs[5(3, 9)d vs.7(4, 12)d; 2.05(1.17, 3.27)ml·kg -1·h -1vs.2.37(1.47, 4.10)ml·kg -1·h -1; 761(53.11%) vs.375(68.93%), P<0.001]. Based on the results of multivariate logistic regression analysis, a prediction model was finally constructed with 9 variables: albumin( OR=0.983, 95% CI: 0.966-0.999, P=0.040), aspartate transaminase( OR=1.000, 95% CI: 1.000-1.000, P<0.001), APTT( OR=1.005, 95% CI: 1.001-1.009, P=0.028), total bilirubin( OR=1.003, 95% CI: 1.001-1.004, P=0.001), serum creatinine( OR=1.005, 95% CI: 1.004~1.007, P<0.001), Charlson score( OR=1.117, 95% CI: 1.061-1.177, P<0.001), intravenous fluid intake in 24 hours( OR=1.101, 95% CI: 1.034-1.173, P=0.003), weight( OR=1.023, 95% CI: 1.018-1.029, P<0.001), and mechanical ventilation( OR=2.412, 95% CI: 1.843-3.157, P<0.001). Then a nomogram was generated.The area under the ROC curve(AUC)of the prediction model was 0.755(95% CI: 0.731-0.780), and the H-L test was conducted( χ2=10.89, P=0.208>0.05), indicating a good fit.Data from 102 elderly patients were included in the validation set, with 27 cases that had developed AKI-stage3 within 48 hours, and were fed into the prediction model, with an AUC of 0.778(95% CI: 0.676-0.880)and χ2=3.72 and P=0.882>0.05 from the H-L test, consistent with the results of the training set. Conclusions:The model has some predictive value for acute kidney injury in elderly patients with sepsis.
3.Analysis of current situation and influencing factors of self-acceptance in young patients with breast cancer

Objective To investigate the current situation of self-acceptance in young patients with breast cancer and analyse the influencing factors.Methods Convenience sampling method was used to include 250 young breast cancer patients in the study,in a tertiary specialised caner hospital in Guangzhou.General data questionnaire and self-acceptance questionnaire were used in the study.Single factor and multiple linear regressions were employed to analyse the influencing factors.Results All 239 patients had completed with the study.The total score of self-acceptance in young breast cancer patients was(41.10±6.60),the score for self-acceptance factor was(21.31±3.80)and the score for self-evaluation factor was(19.79±3.84).Multiple linear regression analysis showed that educational level,per capita monthly income,main hospitalised caregivers and sexual life status were the main factors affecting the self-acceptance in young breast cancer patients(all P<0.05),which jiontly explained 34.4%of the total variance.Conclusions The self-acceptance in young breast cancer patients is at a middle level.Medical staff should take targeted intervention measures according to the influencing factors,therefore to improve the self-acceptance of young breast cancer patients.
4.Challenges and countermeasures in building national regional medical centers for mental health

The authors summarized the current status of national regional medical centers, and analyzed challenges faced in management and operation mode, government investment and compensation mechanism, expansion of high-quality psychiatric medical resources, as well as research and innovation of mental diseases of these centers. On such basis, the authors put forward corresponding countermeasures and suggestions for the next stage of development.
5.Hepatitis E virus infection and gene polymorphism in murine-shaped animals of plague foci in Yunnan Province

Objective:To learn about the infection and gene polymorphisms of hepatitis E virus (HEV) in murine-shaped animals of plague foci in Yunnan Province.Methods:From July to August 2019, around 16 natural villages (4 in Mile City, 6 in Mangshi and 6 in Lianghe County), which were the foci of domestic plague in Yunnan Province, the murine-shaped animals were captured by the night-time method. The liver RNA was extracted, and the target gene of rat HEV was detected by one-step real-time fluorescence quantitative PCR, and the positive rate of rat HEV was calculated. The rat HEV positive samples were amplified by PCR for further clone sequencing, and the resulting sequences were compared with the HEV sequences registered in the GenBank and a phylogenetic tree was constructed by using MegAlign and MEGA 7.0.Results:A total of 491 murine-shaped animals were captured from 3 orders, 5 families, 8 genera, 15 species, and the positive rate of rat HEV was 4.89% (24/491). Among them, the positive rate of Rattus tanezumi and Niviventer fulvescens was 9.39% (23/245) and 1/3, respectively; and other species were negative. There was a statistically significant difference in the positive rate of rat HEV between different habitats ( P = 0.014), and the positive rate of rat HEV in the habitats near the dwellings was higher than that in other habitats ( P < 0.05). The sequence comparison analysis showed that the gene sequence of P018 from Lianghe County was 100.0% homologous to the MG813927.1 sequence of the first patient with rat HEV in Hongkong, and it was clustered into the same branch with the sequences of MG813927.1 and LC549185.1 from rat, was the type HEV C. G024 from Mangshi shared a low homology (20.7% - 31.5%) with other virus strains, and it was clustered into the same branch with a HEV sequence from an avian (AY535004.1). Conclusions:Rat HEV is prevalent in murine-shaped animals of plague foci of Yunnan Province, and there may be gene polymorphisms of rat HEV. In addition, the difference infection rate may be related to the habitats.
6. Prognostic significance of early phase donor chimerism after allogeneic peripheral blood stem cell transplantation

Objective:
To evaluate the prognostic significance of early phase full donor chimerism (FDC) after myeloablative allogeneic peripheral blood stem cell transplantation (allo-PBSCT).
Methods:
The clinical data of 72 hematological patients received myeloablative allo-PBSCT from Feb. 2016 to Jul. 2017 were analyzed retrospectively. The median age was 36.5 years (range 4-59), 44 were males and 28 females. Of the donors, there were 35 HLA matched sibling donors, 27 haploidentical donors and 10 unrelated donors. Polymerase chain reaction amplification of short tandem repeat sequence (PCR-STR) was used to detect donor cell chimerism (DC) rate of recipient bone marrow at one, two and three months after transplantation.
Results:
The median follow-up was 462 d (range: 47-805 d), 55 cases were still alive, and 45 cases were disease-free survival (DFS) at the end of follow-up. The 2-year overall survival (OS) and DFS were (68.9±7.7)% and (59.5±6.3)%, respectively. A number of 16 cases underwent relapses, with 2-year cumulative incidence of (24.1±5.3)%. The median time of recurrence was 157(32-374) d. Forty cases (55.6%) developed acute graft-versus-host diseases (aGVHD), with median time of 35.5 (13-90) d. Chronic GVHD (cGVHD) occurred in 23 patients (31.9%), with median time of 169 (94-475) d. Univariate analysis found the following factors were not related to OS, DFS or relapse rate (RR), including age, sex, blood type and sex of donor-recipient, occurrence of aGVHD and cGVHD. The OS and DFS in cases reached FDC and no FDC at two months after transplantation were (85.2±6.9)%
7.Influence of vagus nerve on multiple organ function and immune reaction of T lymphocytes in septic rats

Objective To explore influence of vagus nerve on multiple organ function and immune reaction of T lymphocytes in septic rats.Methods Forty Sprague-Dawley rats were divided into sham injury group,sepsis group,vagus nerve stimulation (VNS) group,and vagotomy (VGX) group,according to the random number table,with 10 rats in each group.Rats in sepsis group,VNS group,and VGX group were inflicted with sepsis by cecal ligation and puncture (CLP).Rats in VNS group were given electrical stimulation on left cervical vagus nerve for 15 min right after CLP.Rats in VGX group underwent vagotomy of left cervical vagus nerve at 30 min before CLP.At 24 h after CLP,serum of rats was collected to detect levels of alanine transaminase (ALT),aspartate aminotransferase (AST),glycocholic acid (CG),creatine kinase (CK),myocardial creatine kinase (CK-MB),blood urea nitrogen (BUN),and serum creatinine by fully automatic biochemistry analyzer.The left lung of rats was collected to determine wet or dry mass,and wet to dry (W/D) ratio was calculated.The right lung of rats was collected to measure the activity of pulmonary myeloperoxidase (MPO) by enzyme linked immunosorbent assay (ELISA).Spleen of rats was collected to determine the proliferative activity of CD4 + T lymphocytes by cell counting kit 8,and real-time fluorescence quantitative polymerase chain reaction and ELISA were used to quantify mRNA expressions and levels of interleukin 2 (IL-2),interferon-γ,and IL-4,respectively.Data were processed with one-way analysis of variance and Tukey's honest significant difference test.Results (1) The levels of serum ALT,AST,CG,CK,CK-MB,BUN,and creatinine,pulmonary W/D ratio,as well as MPO activity of rats in sepsis group were significantly higher than those in sham injury group and VNS group (P <0.01) and were significantly lower than those in VGX group (P <0.01).(2) The proliferative activity of CD4 + T lymphocytes of rats in sepsis group was 0.93 ± 0.03,which was significantly lower than 1.54 ± O.07 of rats in sham injury group (P <0.01).The proliferative activity of CD4+ T lymphocytes of rats in VNS group was 1.15 ±:0.15,which was significantly higher than that of rats in sepsis group (P < 0.01).The proliferative activity of CD4 + T lymphocytes of rats in VGX group was 0.75 ± 0.06,which was obviously lower than that of rats in sepsis group (P < 0.01).(3) In comparison with those of rats in sham injury group,the levels of IL-2 and interferon-γ in CD4 + T lymphocytes of rats in sepsis group were markedly decreased (P < 0.01),while the level of IL-4 was significantly increased (P < 0.01).In comparison with those of rats in sepsis group,the levels of IL-2 and interferon-γ in CD4 + T lymphocytes of rats in VNS group were obviously increased (P < 0.01),while the level of IL-4 was markedly decreased (P < 0.01).As compared with those of rats in sepsis group,the levels of IL-2 and interferon-γ in CD4 + T lymphocytes of rats in VGX group were markedly decreased (P < 0.01),while the level of IL-4 was significantly increased (P < 0.01).(4) As compared with those of rats in sham injury group,expressions of IL-2 and interferon-γ mRNA in CD4+ T lymphocytes of rats in sepsis group were markedly decreased (P <0.01),while expression of IL-4 mRNA was significantly increased (P < 0.01).Expressions of IL-2 and interferon-γ mRNA in CD4 + T lymphocytes of rats in VNS group were obviously increased when compared with those of rats in sepsis group (P <0.01),while expression of IL-4 mRNA was markedly decreased (P <0.01).In comparison with those of rats in sepsis group,expressions of IL-2 and interferon-γ mRNA in CD4 + T lymphocytes of rats in VGX group were markedly decreased (P < 0.01),while expression of IL-4 mRNA was significantly increased (P <0.01).Conclusions Electrical stimulation of vagus nerve can significantly improve multiple organ dysfunction and reverse immunosuppression of T lymphocytes in septic rats,while vagotomy of vagus nerve may enhance the susceptibility of rats to sepsis.
8.The Immunomodulatory Effects of All-trans Retinoic Acid on AChR-specific Lymphocytes

Objective:To observe the effects of All-trans retinoic acid (ATRA) on the immune functions of AChR-specific lymphcytes via in vitro assays,and investigate the possibility of ATRA in the clinical treatment of myasthenia gravis (MG).Methods:CFA control group and EAMG experimental rats were established to obtain single lymphocytes suspension and cells were followed by AChR97-116 peptide with or without ATRA stimulation for 72 h,and then viable cell population,cell apoptosis,cell cycle and the distribution of Th cells were determined by flow cytometry.CCK-8 assay was selected to evaluate the effects of ATRA on proliferatory ability of lymphocytes.ELISA was used to detect the antibody secretion of B cells affected by ATRA.Results:Compared with CFA group,lymphocytes obtained from EAMG rats had higher ratios of living cells,and this ratio was obviously decreased after ATRA treatment,P<0.001.Different concentrations of ATRA promoted the apoptosis of AChR-specific cells (P<0.001),and the promoted effects were ATRA dose-dependent,however,cell cycles were not changed.ATRA markedly inhibited the proliferation of cells from both CFA and EAMG groups,moreover,AChR-specific cells were more sensitive to ATRA treatment (P<0.01) than that of cells from CFA rats (P<0.05).The ratio of AChR-specific CD4+T cells was reduced by ATRA (P<0.01),and ATRA incubation significantly promoted the percentages of Th2,(PCD4+-4IL-4+<0.001),Treg (PCD4+-Foxp3+<0.001) cell types,but markedly inhibited the percentages ofThl7 (PCD4+-IL-17+<0.05),Thl (PCD4+-IFN-γ+<0.001) cells.ELISA data showed us that ATRA obviously down regulated the antibody secretion of AChR-specific B cells,P<0.01.Conclusions:ATRA not only inhibited the functions of AChR-specific T cells,but also suppressed the roles of AChR-specific B cells,predicating a therapeutic effect of ATRA on myasthenia gravis therapy.
9.Study on the relationship of combined detection of HE4 and CA125 in differential diagnosis and pathological classification of ovarian tumors

Objective To investigate the clinical value of combined detection of glycoprotein 125 (CA125) and human epididymis protein 4 (HE4).Methods 46 patients with ovarian malignant tumor (malignant tumor group) and 48 patients with benign ovarian tumors (benign tumor group) treated in our hospital from June 2013 to August 2015 were selected.The serum levels of CA125 and HE4 were detected in all the patients and its diagnostic value was evaluated by ROC curve.The levels of CA125 and HE4 in patients with different pathological types were compared.Results The best diagnostic value of CA125 was 47.9 U/L,The serum level of CA125 ≥47.9 U/L predicted the specificity of ovarian malignant tumor was 87.34% and that the sensitivity was 76.69%.The best diagnostic value of HE4 was 55.68 pmol/L.The serum level of HE4 ≥ 55.68 pmol/L predicted the specificity of ovarian malignant tumor was 90.34% and that the sensitivity was 83.01%.There was significant difference in CA125 and HE4 between the patients with benign and malignant ovarian tumors (P<0.05).No significant difference in the diagnosis of malignant ovarian tumor and the specificity by using combined detection of HE4 and CA125 (P>0.05)However,the sensitivity was significantly higher than that of single detection,the difference was statistically significant (P<0.05).The levels of CA125 and HE4 in patients with epithelial ovarian tumors were higher than those with non epithelial ovarian tumors,and the difference was statistically significant (P<0.05).The levels of CA125 and HE4 in patients with mucinous ovarian cancer were significantly lower than those in patients with serous ovarian cancer (P<0.05).Conclusion The combined detection of serum CA125 and HE4 can significantly improve the value of differential diagnosis of ovarian tumors,and CA125 and HE4 may play an important role in the pathological classification of malignant ovarian tumors.
10. A multicenter, retrospective study of pathogenic bacteria distribution and drug resistance in febrile neutropenic patients with hematological diseases in Shanghai

Objective:
To investigate the pathogen spectrum distribution and drug resistance of febrile neutropenic patients with hematological diseases in Shanghai.
Methods:
A retrospective study was conducted on the clinical isolates from the febrile neutropenic patients hospitalized in the departments of hematology in 12 general hospitals in Shanghai from January 2012 to December 2014. The drug susceptibility test was carried out by Kirby-Bauer method. WHONET 5.6 software was used to analyze pathogenic bacteria and drug susceptibility data.
Results:
A total of 1 260 clinical isolates were collected from the febrile neutropenic patients. Gram-positive bacteria accounted for 33.3% and Gram-negative bacteria accounted for 66.7%.
