1.Comparison of the effect of delaying progression of myopia in children with defocus incorporated multiple segments spectacle lenses and orthokeratology

Objective:To compare the effect of delaying progression of myopia in children between defocus incorporated multiple segments (DIMS) spectacle lens and orthokeratology.Methods:A nonrandomized controlled clinical study was conducted.A total of 390 children (390 eyes) with myopia who were treated in the First Affiliated Hospital of Zhengzhou University from January 2018 to December 2020 were included, with the spherical equivalent (SER) of -0.75 to -6.00 D. According to the willingness of patients and patients' guardians, the subjects were divided into DIMS group, orthokeratology group and single-vision spectacle lens group, with 130 cases (130 eyes) in each group, wearing DIMS spectacle lenses, standard or astigmatic design orthokeratology and conventional single-vision full-correction aspheric spectacle lenses, respectively.The SER of the eyes was measured using an automatic computerized refractometer in combination with subjective refraction before and one year after lens wear, and the axial length (AL) of the eyes was measured using IOLMaster.A total of 327 patients in the three groups met the inclusion and exclusion criteria, including 107 in the DIMS group, 112 in the orthokeratology group, and 104 in the single-vision spectacle lens group.All the right eyes were included in this study.The changes in SER and AL before and after wearing lenses for 1 year were compared among the three groups.The relationship between AL and SER changes and baseline data in the DIMS group was evaluated by Pearson linear correlation analysis.The study followed the Declaration of Helsinki, and the study protocol was reviewed and approved by the Ethics Committee of the First Affiliated Hospital of Zhengzhou University (No.2023-KY-0174-002). The subjects and their guardians were fully aware of the purpose and methodology of the study, and voluntarily signed an informed consent form.Results:There were statistically significant overall differences in SER and AL at different time points among the three groups (SER: Fgroup=7.065, P=0.009; Ftime=183.730, P<0.001.AL: Fgroup=6.151, P=0.014; Ftime=175.290, P<0.001). One year later, the differences in SER and AL changes among the three groups were statistically significant ( F=7.065, P=0.009; F=6.151, P=0.014). The SER and AL of each group after 1 year was greater than the baseline, with the SER and AL and their changes significantly smaller in orthokeratology group and DIMS group than in single-vision spectacle lens group and greater in DIMS group than in orthokeratology group, showing statistically significant differences (all at P<0.05). Compared with single-vision spectacle lenses, wearing orthokeratology for 1 year can inhibit SER and AL progression by 58.3% and 59.0%, and wearing DIMS frame glasses for 1 year can inhibit SER and AL progression by 46.9% and 43.6%.The percentage of eyes with no change in SER was 5.77%(6/104), 24.11%(27/112) and 17.76%(19/107) in the single-vision spectacle lens group, orthokeratology group and DIMS group, respectively, and the percentage of AL was 0.00%(0/104), 8.93%(10/112) and 7.48%(8/107), respectively, showing statistically significant differences among the three groups ( χ2=9.316, 8.676; both at P<0.001). The AL change in the DIMS group was weakly negatively correlated with age ( r=-0.252, P=0.006). Conclusions:Wearing DIMS spectacle lenses is not as effective as orthokeratology in delaying myopia in children, but it is significantly better than wearing conventional single-vision spectacle lenses.
2.Research on the risk factors and cumulative risk of myopia in children and adolescents

Objective:To investigate the risk factors and cumulative risk of myopia in children and adolescents, providing a basis for identifying cumulative risk factors in preventing and controlling myopia.Methods:Baseline data from the mental and physical health cohort of children and adolescents established in Inner Mongolia Autonomous Region were used. A stratified random cluster sampling method was adopted to select 138 974 students from fourth to twelfth grade as participants. Distance visual exams, refractive assessments, and questionnaires were conducted on the included students. Logistic regression analysis was used to evaluate each risk factor's impact on myopia's prevalence. The number of risk factors was summed to form a cumulative risk score, and logistic regression analysis was conducted to examine the association between the cumulative risk score and the prevalence of myopia. Additionally, the association between the cumulative risk score of myopic students and their degree of refractivity was analyzed using a generalized estimating equation.Results:The study found a high prevalence of myopia among children and adolescents at baseline (70.2%). Girls exhibited a higher prevalence (74.8%) than boys (65.6%), urban areas (74.3%) surpassed suburban ones (68.6%), and the incidence was greater in high schools (80.3%) compared to middle schools (75.3%), which, in turn, was higher than in elementary schools (57.7%) (all P<0.05). Analysis of risk factors revealed that children and adolescents experiencing improper reading and writing distances ( OR=1.10, 95% CI: 1.07-1.13), excessive homework ( OR=1.09, 95% CI: 1.06-1.12), insufficient sleep ( OR=1.10, 95% CI: 1.07-1.13), having myopic father ( OR=1.98, 95% CI: 1.91-2.05), having myopic mother ( OR=2.04, 95% CI: 1.97-2.10), or using classroom chairs not matched to their height faced ( OR=1.04, 95% CI: 1.01-1.07) increased myopia risks. Additionally, the prevalence and significant odds ratio of myopia increased with the increase in cumulative risk score, with every additional unit of cumulative risk score increasing the right eye's refractive error by -0.10 D. Conclusion:The presence of multiple factors and their comprehensive score increases the prevalence of myopia in children and adolescents.
3.Internet addiction and associated factors of junior high school students in Inner Mongolia

Objective:
To explore the current situation of Internet addiction among junior high school students in Inner Mongolia and its associated factors, so as to provide a theoretical basis for the prevention and control of Internet addiction among junior high school students.
Methods:
In September 2023, 53 755 junior high school students were selected by stratified cluster random sampling method from all the flag counties in Inner Mongolia, and questionnaires were administered according to the "Monitoring and Intervention Program of Common Diseases and Health Influencing Factors of Students, by using the Internet Addiction Inventory and Generalized Anxiety Disorder Scale-7 Item. The Chi test and multiple Logistic regression analysis were used to explore related factors and draw nomograms.
Results:
The reported rate of Internet addiction among junior high school students in Inner Mongolia was 2.15%. The results of multivariate Logistic regression analyses showed that students were more likely to be in higher grades (grade eight: OR =1.54; grade nine: OR =1.64), being scolded by their parents ( OR =1.06), living in the school ( OR =2.28), being bullied in the school ( OR =1.91), anxiety symptoms ( OR =3.15), and substance abuse (smoking: OR =2.56; drinking alcohol: OR =2.29; inhaling soluble solvents for pleasure: OR =2.17; using cough suppressant except for coughing: OR =1.50; using sedatives without doctor s permission: OR =1.34) were positively correlated with Internet addiction among junior high school students in Inner Mongolia ( P <0.05). The area under the curve was 0.819, and the calibration curve showed that the predicted values were basically consistent with the measured values.
Conclusions
Internet addiction among junior high school students in Inner Mongolia is related to students living environment and substance abuse behavior. Attention should be paid to the living environment of junior high school students and strengthen the preventive education of substance abuse in order to reduce the occurrence of Internet addiction among junior high school students.
4.Best evidence for perioperative exercise in kidney transplant patients

Objective To summarize the best evidence for perioperative exercise in renal transplant patients and provide evidence-based evidence for clinical staff to perform exercise in renal transplant patients.Methods Evidence-based questions were identified according to the PIPOST model,and computerized searches were conducted on BMJ Best Practice,UpToDate,the World Health Organization,the US National Guidelines,the website of the Chinese Society of Nephrology,the website of the British Renal Association,the website of the American Society of Nephrology,CNKI,the knowledge service platform of Wanfang Data,the Wipo database and other databases for all evidence on perioperative exercise in renal transplant recipients,including clinical guidelines,clinical decision-making,systematic evaluations,evidence summaries,and expert consensus,with a timeframe for searching from the establishment of the database to August 2023.Quality assessment and evidence extraction of the literature were performed independently by 2 nursing graduate students.Results A total of 13 publications were included,including 1 clinical decision,1 guideline,3 expert consensus,and 8 systematic evaluations.A total of 24 pieces of best evidence were summarized in 7 areas:exercise benefits,exercise assessment,preoperative exercise strategies,postoperative exercise strategies,precautions,exercise instruction monitoring,and exercise support.Conclusions This study summarizes the best evidence about perioperative exercise exercise in renal transplantation and provides a reference for standardizing perioperative exercise exercise in renal transplantation.Clinical staff should fully identify the impediments and facilitators of exercise exercise in renal transplant patients,and develop personalized exercise exercise programs based on medical scenarios,patient conditions,and combined with patients'exercise wishes.
5.Integrative nursing experience of a case with chronic refractory wound formation caused by drug extravasation

To sum up integrative nursing experience of a case with chronic refractory wound formation caused by drug extravasation.The essentials of integrative nursing are:structured nursing intervention of"assessment-management-treatment"based on the Triangle of Wound Assessment;determination of the timing for integrative nursing according to the theory of TCM sores and ulcers;implementation of copper board scraping method to promote circulation of qi and blood;use of Huo-long Comprehensive Moxibustion Therapy to promote muscle regeneration.With the help of the cooperation of the multidisciplinary specialist nursing team,the wound was completely healed after 59 days of integrated traditional Chinese and Western medicine nursing interventions.
6.Co-occurrence trend and association study of Internet addiction and depressive symptoms among students in Inner Mongolia Autonomous Region

Objective:
To investigate the prevalence of Internet addiction and depression of students, and to analyze the co-occurrence and trend, so as to provide a theoretical basis for prevention and controlling measures of Internet addiction and depression.
Methods:
A total of 6 317,7 152,81 808,71 180 and 89 932 students aged 10 to 24 years from 12 leagues (103 banners) in Inner Mongolia Autonomous Region were selected by stratified random cluster sampling in September each year from 2017 to 2021. The Internet Addiction Scale and the Central for Epidemiologic Studies Depression Scale(CES-D) was used to measure Internet addiction and depression. And the annual inspection rate, group difference and annual change trend in students were calculated. Multivariate linear regression and restricted cubic spline analysis were used to estimate the linear and non linear associations between Internet addiction and depression in students.
Results:
The Internet addiction proportion in students gradually decreased from 4.1% in 2017 to 2.1% in 2020, but increased to 3.9% in 2021. And the depressive symptoms proportion increased from 20.9% in 2017 to 28.0% in 2020 and 27.0% in 2021. The detection rate of Internet addiction and depression comorbidities remained at 1.8% to 2.5 %. The Internet addiction proportion in boys was higher than that in girls( χ 2=42.82, P <0.05). The depressive symptoms prevalence in girls was higher than that in boys( χ 2= 553.90, P <0.05). Taking reversal in prevalence of Internet addiction in urban and rural areas was observed in 2019. The detection rates of depressive symptoms and comorbidity were higher in urban areas than these in suburban counties on the whole, and the difference showed a trend of decreasing or even equalizing year by year. Internet addiction was positively correlated with depressive symptoms score ( B=1.67, 95%CI =1.64-1.71), the proportion of depressive symptoms ( OR=1.39, 95%CI =1.38-1.41) and the proportion of major depressive symptoms ( OR=1.35, 95%CI =1.33-1.36) among students in 2021 ( P <0.05). An N-shaped curve was found in the significant nonlinear associations between internet addiction and depression across sex, region and school stage.
Conclusion
Internet addiction and depression in students show significant linear and non-linear associations, which are consistent in different sexes, regions and school stages. Therefore, relevant measures should be made and implemented in each region, especially in suburb areas, so as to prevent the increasingly development of adolescents and children s Internet addiction and depression.
7.Co-occurrence trend of overweight,obesity and elevated blood pressure and its association with lifestyle factors among students in Inner Mongolia Autonomous Region

Objective:
To explore the epidemiological trend of overweight and obesity, elevated blood pressure and their comorbidities in children and adolescents from Inner Mongolia Autonomous Region during 2016-2021, and to analyze its association with lifestyle, so as to provide reference for formulating prevention and control strategies of regional common comorbidities in schools.
Methods:
A total of 8 908, 8 222, 9 448, 127 068, 100 778, and 138 540 students aged 10-18 years in Inner Mongolia were selected by stratified random cluster sampling in September each year from 2016 to 2021. Physical examination and questionnaire survey were conducted on the included students. The prevalence trends of overweight,obesity, elevated blood pressure and their co-occurrence were analyzed. Logistic regression was used to compare the prevalence of elevated blood pressure in different body mass index (BMI) groups. After excluding individuals without lifestyle information in 2021, Logistic regression analysis was used on 136 374 subjects to analyze the association between overweight,obesity, elevated blood pressure and their co-occurrence and lifestyle factors.
Results:
During 2016 to 2021, the prevalence of comorbidity of overweight, obesity with elevated blood pressure among students in Inner Mongolia Autonomous Region were 5.04%,5.14%,4.99%,7.51%,7.60% and 9.45%, respectively . The prevalence of overweight and obesity was 26.94%, 28.07%, 29.62%, 34.19%, 36.71% and 37.53%, respectively. The prevalence of elevated blood pressure were 16.05%, 11.54%, 13.12%, 14.85%, 14.12% and 18.40%, respectively. Except for 2016, the risk of elevated blood pressure in overweight and obese people was higher than that in normal BMI group in other years, and there was a positive correlation between overweight and obesity and elevated blood pressure after gender and urban and rural areas ( P < 0.05 ). In 2021, the detection rate of comorbidity of overweight and obesity with elevated blood pressure among children and adolescents in urban areas was higher than that in suburban counties, and the reporting rate of healthy lifestyle was lower than that in suburban counties ( P <0.05).Skipping breakfast ( OR =1.11,95% CI =1.07-1.16) and non daily moderate and high intensity physical activity( OR =1.27,95% CI =1.20-1.34) were positively correlated with the co-occurrence of overweight,obesity and elevated blood pressure among children and adolescents in Inner Mongolia Autonomous Region. Non daily moderate and high intensity physical activity ≥60 min was positively correlated with elevated blood pressure ( OR =1.11,95% CI =1.07-1.16), and insufficient sleep was positively correlated with overweight,obesity ( OR =1.04, 95% CI =1.01-1.06)( P <0.05).
Conclusion
The prevalence of overweight,obesity, elevated blood pressure and their co-occurrence among children and adolescents in Inner Mongolia Autonomous Region is relatively high. Overweight/obesity is an important risk factor for elevated blood pressure, and unhealthy lifestyles are risk factors for co-occurrence of overweight,obesity and elevated blood pressure. Region specific lifestyle interventions are indispensable for the prevention and control of regional common comorbidities. Urban areas may be a key focus for lifestyle interventions.
8.Research progress on relocation stress of parents of Pediatric Intensive Care Unit children in transition period

With the improvement of critical care medical technology, more critically ill children are transferred from Pediatric Intensive Care Unit (PICU) to General Ward, and the transition period of PICU has become a hot spot of clinical research. Paying attention to the relocation stress of parents of children in transition period of PICU is also the focus of clinical nursing practice in critically ill pediatrics. However, the relocation stress of parents of children has adverse effects on their own health and the rehabilitation process of children. This paper reviews the concept, assessment tools, influencing factors and intervention measures of parents of transitional children in the PICU transition period and puts forward the limitations and suggestions of the current research on transitional pressure in parents of children in the PICU transition period, so as to provide references for relocation stress nursing service for parents of children in transition period in PICU.
9.PACK-CXL:corneal collagen cross-linking for infectious keratitis

Infectious keratitis is one of the leading causes of irreversible vision loss.With the emergence of multidrug-resistant pathogen,despite appropriate diagnosis and antimicrobial treatment,infectious keratitis can still result in corneal opacity and even melting and perforation.Therefore,it is imperative to develop novel treatment for infectious keratitis.More recently,several articles have demonstrated the effectiveness of collagen cross-linking (CXL) in treating infectious keratitis via direct antimicrobial activity and inhibition of corneal enzymatic degradation.In order to highlight the use of CXL tor infectious keratitis treatment,a new term was proposed at the 9th CXL congress to rename its use in infections as photoactivated chromophore for infectious keratitis-CXL (PACK-CXL).Based on the current literature,PACK-CXL appears to be effective for bacterial keratitis,especially on inhibiting corneal perforation.The effect was limited for fungal and amoebic keratitis,and maybe aggravative on viral keratitis.In this article,we will review the basic profile,technical operation,prevention of complication of CXL,the principle mechanism of PACK-CXL and its clinical application for bacterial,fungal keratitis,amoebic and viral keratitis.
10.Efficacy of intravitreal lucentis or conbercept injection combined with ahmed glaucoma valve implantation for treatment of neovascular glaucoma

Objective:To evaluate the efficacy of intravitreal lucentis or conbercept injection combined with ahmed glaucoma valve (AGV)implantation on the treatment of neovascular glaucoma (NVG).Methods:A total of 68 NVG patients (68 eyes)were divided into three groups according to the pretreatment (the intravitreal injection of lucentis or conbercept or no intravitreal injection 3 - 7 d before AGV implantation):lucentis group (n = 26, given intravitreal injection of 10 g · L-1 lucentis),conbercept group (n = 21, given intravitreal injection of 10 g·L-1 conbercept)and control group (n=21,given medicine or puncture of anterior chamber to smoothen the IOP and received AGV implantation).The panretina photocoagnlation (PRP)was performed in the patients with clear ocular media before intravitreal injection or AGV implantation.For the patients with cloudy ocular media,the intravitreal injection of anti-VEGF drugs was performed first and the PRP was performed when fundus could be seen.Results:Compared with before operation,the mean IOP and the number of antiglaucoma medications of the patients used in three groups after operation were significantly decreased (P < 0.01).There were no significant differences in the successful rates of operation of the patients in three groups at different time points after operation (P >0.05).1 month and 3 months after operation,the BCVA of the patients in lucentis and conbercept groups was significantly better than that in control group (P <0.05).There were no significant differences in the incidence of complications of the pateints between lucentis and conbercept groups at different time points after operation (P >0.05).Conclusion:Intravitreal injection of anti-VEGF drugs combined with AGV implantation is effective and safe in the treatment of NVG.In short term,it can improve the vision,relieve the pain and reduce the complications.
