OBJECTIVE To analyze the dose-adjusted concentrations of Posaconazole oral suspension in patients undergoing hematopoietic stem cell transplantation (HSCT) and their influential factors. METHODS Data were collected from hospitalized HSCT patients admitted to the First Affiliated Hospital of Shandong First Medical University (Shandong Provincial Qianfoshan Hospital) from January 2021 to April whtwhm@yeah.net 2023 who took Posaconazole oral suspension for the prevention of invasive fungal disease (IFD) and received blood concentration of posaconazole. The rate of concentration attainment and clinical failure rate of posaconazole for the prevention of IFD were evaluated, and one-way and multiple linear regression analyses were performed for the influential factors of dose-adjusted concentrations (C0/D) of posaconazole. RESULTS A total of 44 patients were enrolled; the mean C0 of posaconazole in patients was (0.99±0.94) µg/mL, and 20 patients had a C0≥0.7 μg/mL, with a concentration attainment rate of 45.45% for the prevention of IFD; 13 cases were clinical failures, with a clinical failure rate of 29.55%. Of 24 patients who did not achieve C0/D of posaconazole for IFD prophylaxis, one patient was a clinical failure despite timely dose adjustment of posaconazole in seven patients; seven of the thirteen patients who did not undergo dose adjustment were clinical failures; and the remaining four patients were switched to other antifungal agents. The results of univariate analysis showed that gender, body mass index (BMI), renal function, combined use of sodium phenytoin, omeprazole and metoclopramide had a significant effect on the C0/D of posaconazole (P<0.05); the results of multivariate linear regression analysis showed that gender, BMI and combined use of sodium phenytoin were the independent factors affecting the C0/D of posaconazole (P<0.05). CONCLUSIONS Significant individual differences are reflected in the blood concentration of Posaconazole oral suspension; gender, BMI and combined use of sodium phenytoin are independent factors affecting the C0/D of posaconazole.

Objective:To study the predictive value of total serum bilirubin (TSB) and the ratio of bilirubin to albumin (B/A) in neonatal acute bilirubin encephalopathy (ABE).Methods:Neonates with extremely severe hyperbilirubinemia (TSB≥425 μmol/L) treated in the Nanjing Maternal and Child Health Hospital, Maternity and Child Health Care of Guangxi Zhuang Autonomous Region, Northwest Women and Children's Hospital, Yinchuan Maternal and Child Health Hospital and Liaocheng People's Hospital from March 2018 to August 2019 were selected as prospective subjects for this study. According to the score of brain injury induced by bilirubin, the subjects were divided into ABE group and non-ABE group, and the predictive value of TSB peak and B/A for neonatal ABE were analyzed. Results:A total of 194 infants with extremely severe hyperbilirubinemia were recruited in this study, including 20 in ABE group and 174 in non-ABE group. The peak value of bilirubin ranged from 427 to 979 μmol/L. The optimal critical values of TSB peak value and B/A for ABE prediction were 530 μmol/L and 9.48, respectively. The sensitivity and specificity of ABE prediction were 85.0% and 92.8% when combined with TSB peak and B/A values. Conclusions:TSB peak combined with B/A value can effectively identify neonatal ABE. When the TSB peak value was greater than 530 μmol/L and the B/A value was greater than 9.48, the neonates had a higher risk of neonatal ABE.

Objective To analyze the influence of a dental caries phototherapy device using neodymium-yttrium-aluminum-garnet (Nd:YAG) laser on the temperature of tooth pulp chamber of different sites and morphologies under different irradiation duration and power. Methods Fifty intact isolated teeth were collected and randomly divided into middle incisor group, upper right first premolar group, upper right second molar group, upper left first premolar group, and upper left second molar group. Each experimental group was irradiated according to the laser power 1.5, 1.6, 1.7, 1.8, 1.9 and 2.0 W, and the irradiation duration 30, 60, 90 s. The temperature rise of the pulp chamber was recorded with a thermocouple thermometer. Results After the laser irradiation, the temperature of the tooth pulp chamber increased, and the temperature rise was less than 5.5℃, i.e. the threshold leading to the dental pulp necrosis. Conclusions The use of laser to prevent dental caries will increase the temperature of the tooth pulp chamber, but the temperature rise in the range of 5.5 ° C is relatively safe and will not cause irreversible damage to the pulp tissue.

OBJECTIVE: To observe the effect of n-hexane subchronic exposure on the serum level of neuron specific enolase(NSE), neurofilament light chain protein(NF-L) and nerve growth factor(NGF) in rat, and to explore the feasibility of using NSE, NF-L and NGF as biomarkers of n-hexane neurotoxicity. METHODS: Specific pathogen free male SD rats were randomly divided into control group and low-, medium-and high-dose exposure groups, with 6 rats in each group. Rats in the low-, medium-and high-dose exposure groups were given n-hexane solution at doses of 168, 675 and 2 700 mg/kg body mass, respectively, while rats in the control group were gavaged with 0.9% sodium chloride solution, once a day, 5 days a week for 6 weeks. At week 0, 2, 4, and 6 of exposure, the body mass of the rats was weighed, the gait scores were performed, and the serum levels of NSE, NF-L, and NGF were detected.RESULTS: Body mass, gait score and serum levels of NSE and NF-L in rats were statistically significant in terms of the n-hexane exposure dose and exposure time(P<0.01). At the 6 th week of n-hexane exposure, the body mass of the three dose exposure groups was lower than that of the control group(P<0.05), and the gait score was higher than that of the control group(P<0.05). Moreover, the abnormal gait of the rats showed a dose-effect relationship with the increasing n-hexane poisoning dose. At week 2, 4 and 6, the serum levels of NSE and NF-L in these three dose exposure groups were higher than that in the control group(P<0.05). In addition, the serum level of NF-L in rats in the medium-and high-dose exposure groups increased with the n-hexane exposure time increasing and showed a time-effect relationship(P<0.05). The level of serum NGF in rats was statistically significant in the main effects of n-hexane dose and duration of exposure(P<0.05). The serum NGF level in the high-dose exposure group was lower than that in the control group, the low-dose and medium-dose exposure groups(P<0.05). NGF level in serum of rats at week 6 was lower than that at week 0, 2 and 4(P<0.05). CONCLUSION: Both NSE and NF-L in serum can be used as biomarkers for the early effect of n-hexane on peripheral nerve injury. The feasibility of using serum NGF as a biomarker for the early effect of n-hexane on peripheral nerve injury warrants further investigation.
