1.Exploration of CT imaging features of cystic pulmonary nodules and establishment of a prediction model for benign and malignant pulmonary nodules

Objective To explore the CT imaging features and independent risk factors for cystic pulmonary nodules and establish a malignant probability prediction model. Methods The patients with cystic pulmonary nodules admitted to the Department of Thoracic Surgery of the First People's Hospital of Neijiang from January 2017 to February 2022 were retrospectively enrolled. They were divided into a malignant group and a benign group according to the pathological results. The clinical data and preoperative chest CT imaging features of the two groups were collected, and the independent risk factors for malignant cystic pulmonary nodules were screened out by logistic regression analysis, so as to establish a prediction model for benign and malignant cystic pulmonary nodules. Results A total of 107 patients were enrolled. There were 76 patients in the malignant group, including 36 males and 40 females, with an average age of 59.65±11.74 years. There were 31 patients in the benign group, including 16 males and 15 females, with an average age of 58.96±13.91 years. Multivariate logistic analysis showed that the special CT imaging features such as cystic wall nodules [OR=3.538, 95%CI (1.231, 10.164), P=0.019], short burrs [OR=4.106, 95%CI (1.454, 11.598), P=0.008], cystic wall morphology [OR=6.978, 95%CI (2.374, 20.505), P<0.001], and the number of cysts [OR=4.179, 95%CI (1.438, 12.146), P=0.009] were independent risk factors for cystic lung cancer. A prediction model was established: P=ex/(1+ex), X=–2.453+1.264×cystic wall nodules+1.412×short burrs+1.943×cystic wall morphology+1.430×the number of cysts. The area under the receiver operating charateristic curve was 0.830, the sensitivity was 82.9%, and the specificity was 74.2%. Conclusion Cystic wall nodules, short burrs, cystic wall morphology, and the number of cysts are the independent risk factors for cystic lung cancer, and the established prediction model can be used as a screening method for cystic pulmonary nodules.
2.A predictive model of lymph node metastasis after thoracoscopic surgery for lung adenocarcinoma with a diameter≤3 cm

Objective To predict the probability of lymph node metastasis after thoracoscopic surgery in patients with lung adenocarcinoma based on nomogram. Methods We analyzed the clinical data of the patients with lung adenocarcinoma treated in the department of thoracic surgery of our hospital from June 2018 to May 2021. The patients were randomly divided into a training group and a validation group. The variables that may affect the lymph node metastasis of lung adenocarcinoma were screened out by univariate logistic regression, and then the clinical prediction model was constructed by multivariate logistic regression. The nomogram was used to show the model visually, the receiver operating characteristic (ROC) curve, calibration curve and clinical decision curve to evaluate the calibration degree and practicability of the model. Results Finally 249 patients were collected, including 117 males aged 53.15±13.95 years and 132 females aged 47.36±13.10 years. There were 180 patients in the training group, and 69 patients in the validation group. There was a significant correlation between the 6 clinicopathological characteristics and lymph node metastasis of lung adenocarcinoma in the univariate logistic regression. The area under the ROC curve in the training group was 0.863, suggesting the ability to distinguish lymph node metastasis, which was confirmed in the validation group (area under the ROC curve was 0.847). The nomogram and clinical decision curve also performed well in the follow-up analysis, which proved its potential clinical value. Conclusion This study provides a nomogram combined with clinicopathological characteristics, which can be used to predict the risk of lymph node metastasis in patients with lung adenocarcinoma with a diameter≤3 cm.
3.Establishment of a genotyping method for the junior blood group and identification of a rare blood type with partial DVI.3 and Jr(a-)

Objective:To develop a genotyping method for the Junior blood type and report on a rare blood type with Jr(a-).Methods:Healthy O-type RhD+ volunteer donors of the Shenzhen Blood Center from January to May 2021 ( n=1 568) and a pedigree with difficult cross-matching ( n=3) were selected as the study subjects. Serological methods were used for proband′s blood type identification, unexpected antibody identification, and antibody titer determination. Polymerase chain reaction-sequence specific primer (PCR-SSP) method was used for typing the proband′s RHD gene. ABCG2 gene coding region sequencing and a PCR-SSP genotyping method were established for determining the genotypes of the proband and his family members and screening of Jra antigen-negative rare blood type among the 1 568 blood donors. Results:The proband′s ABO and RhD blood types were respectively determined as B and partial D (RHDDVI.3/RHD01N.01), Junior blood type Jra antigen was negative, and plasma had contained anti-D and anti-Jra. Sequencing of the ABCG2 gene revealed that the proband′s genotype was ABGG201N.01/ABGG201N.01 [homozygous c. 376C>T (p.Gln126X) variants], which is the most common Jr(a-) blood type allele in the Asian population. Screening of the voluntary blood donors has detected no Jr(a-) rare blood type. Statistical analysis of the heterozygotes suggested that the allelic frequency for ABCG2*01N.01 (c.376T) was 0.45%, and the frequency of Jr(a-) rare blood type with this molecular background was about 0.2‰. Conclusion:A very rare case of partial DVI.3 type and Jr(a-) rare blood type has been identified. And a method for identifying the Junior blood type through sequencing the coding regions of the ABCG2 gene and PCR-SSP has been established.
4.Application of hierarchical teaching based on team-based learning plus Bandura's role model theory in the training of newly-recruited nurses in department of gastroenterology

Objective:To explore the feasibility and effectiveness of hierarchical teaching based on team-based learning (TBL) combined with Bandura's role model theory in improving the training effectiveness of newly hired gastroenterology nurses.Methods:The 98 nurses recruited in the Department of Gastroenterology from July 2019 to July 2023 were divided into a control group (47) and an observation group (51) according to their order of enrollment. The control group received routine teaching, while the observation group received hierarchical teaching based on TBL plus Bandura's role model theory. The two groups were compared for theoretical and skill assessment scores, clinical communication skills, teamwork ability, training effectiveness, and satisfaction with teaching.Results:After the training, the theoretical and practical assessment scores of the observation group were (81.69±4.26) and (80.29±5.21) points, respectively, which were significantly higher than the (77.57±4.16) and (75.28±5.43) points in the control group ( P<0.05). After the training, both groups showed significant improvements in building harmonious relationships, listening skills, team participation, problem confirmation, information transmission, and validation of feelings, with the observation group scoring higher than the control group. After the training, the trust and support, team orientation, team leadership, and team mental model dimensions scores of both groups were significantly improved, and the observation group showed higher scores than the control group. After the training, the leadership ability, critical patient monitoring, teaching and cooperation, planning evaluation, communication, and professional development scores of both groups were significantly improved, and the observation group demonstrated higher scores than the control group. The observation group scored higher on various dimensions of teaching satisfaction than the control group ( P<0.05). Conclusions:Hierarchical teaching based on TBL plus Bandura's role model theory can improve the quality of training newly hired nurses in the gastroenterology department, enhance their clinical communication skills, teamwork abilities, and clinical nursing quality, and facilitate the cultivation of high-quality gastroenterology clinical nurses.
5.Evaluation system of ideological and political education in clinical medicine courses based on analytic hierarchy process and fuzzy comprehensive evaluation models

Objective:To explore the construction and application of the evaluation system for ideological and political education in clinical medicine courses based on analytic hierarchy process (AHP) and fuzzy comprehensive evaluation (FCE) models.Methods:The evaluation system for ideological and political education in clinical medicine courses was constructed based on AHP and FCE models in the Yaahp software. Ideological and political education checklist teaching that places equal emphasis on medical ethics and skill was implemented in internal medicine course taken by 185 students in The First Affiliated Hospital of Xi'an Jiaotong University. At the end of the course, the evaluation system was used to obtain quantitative comprehensive evaluation results.Results:Based on AHP and FCE models, the evaluation system for ideological and political education in clinical medicine courses was established. The goal was education of students with both medical ethics and skill, the criteria included teachers' ethics, curriculum design, students' experience and development assessment, and thirteen alternatives were used in the system. The implementation effect of ideological and political education in internal medicine course of the first semester in the 2020-2021 academic year was evaluated by the evaluation system. The quantitative comprehensive evaluation score was 4.080 (range 1-5), which showed that the ideological and political education checklist teaching with equal emphasis on medical ethics and skill achieved good educating effect.Conclusion:The evaluation system for the implementation effect of ideological and political education in clinical medicine courses can be constructed based on the AHP and FCE models. The evaluation system is scientific, quantitative, comprehensive, and improvable.
6.Prenatal diagnosis and genetic analysis of three fetuses with duodenal atresia or stenosis.

OBJECTIVE:
To explore the genetic basis for three fetuses with duodenal atresia or stenosis detected by ultrasonography.
METHODS:
Clinical data of three fetuses identified at the Women's Hospital Affiliated to Zhejiang University School of Medicine between January 2021 and August 2022 were collected. Umbilical cord blood and amniotic fluid samples of the fetuses and peripheral blood samples of their parents were collected and subjected to G-banded chromosomal karyotyping and single nucleotide polymorphism array (SNP array) analysis.
RESULTS:
Prenatal ultrasound of the three fetuses revealed duodenal atresia or stenosis. No karyotypic abnormality was detected, whilst SNP array has identified 1.4 ~ 1.9 Mb duplications at 17q12 in all of them, which were all predicted to be pathogenic copy number variations (CNVs).
CONCLUSION
The 17q12 duplications probably underlay the duodenal atresia and stenosis in these fetuses, and chromosomal CNVs should be considered in duodenal atresia and stenosis.

7.The diagnostic value of different methods for detecting constipation with defecation disorder based on Roman Ⅳ criteria

【Objective】 To study the diagnostic value of balloon expulsion test, anorectal manometry, and colonic transit test for detecting constipation with defecation disorders. 【Methods】 Outpatients with functional constipation were retrospectively analyzed and classified into dyssynergic defecation constipation and non-dyssynergic defecation constipation according to the Rome Ⅳ diagnostic criteria. We studied the differences in symptomatological characteristics and results of the three testing methods between the two groups of patients. 【Results】 Among the 48 patients with functional constipation included, there were 13 males (27.1%) and 35 females (72.9%) with an age of (44.8±12.3) years, including 32 patients (66.7%) with defecation disorder and 16 patients (33.3%) with non-defecation disorder. The percentage of anal distension was significantly higher in patients with dyssynergic defecation constipation than in those with non-dyssynergic defecation constipation (34.4% vs. 0.0%, P=0.021). All the three tests had diagnostic value for dyssynergic defecation constipation, but with low diagnostic agreement between the results of each test. The diagnostic sensitivity of anorectal manometry was 100%, but the specificity (56.3%) was low, and both the positive predictive value (82.1%) and the negative predictive value (100%) were higher; the diagnostic sensitivity (75.0%) and specificity (81.3%) of the balloon expulsion test were both higher. The Youden index of colonic transit test was the smallest. The anal resting pressure and maximum systolic pressure of dyssynergic defecation constipation were lower than those of non-dyssynergic defecation constipation (75.2 mmHg vs. 97.1 mmHg, 168.7 mmHg vs. 251.6 mmHg). The two types of constipation did not significantly differ in anorectal pressure gradients during simulated defecation or rectal sensory function. 【Conclusion】 The balloon expulsion test can be used as a primary screening test for constipation with defecation disorders, and anorectal manometry has the highest comprehensive diagnostic value, but there is a certain false positive rate, and the colonic transit test has the lowest diagnostic efficacy. Thus a comprehensive analysis of the patient’s symptoms and the results of different tests is needed in clinical practice.
8.Investigation and estimation of medical X-ray diagnostic examination frequency in Huai′an city in 2017

Objective:To estimate the frequencies of medical X-ray diagnositic examinations and analyze their distribution in Huai′an City.Methods:Stratified random sampling was used to select 2 tertiary hospitals including 1 general hospital and 1 MCH hospital, 4 secondary hospitals, 20 primary and unclassified hospitals to investigate medical X-ray examination frequencies in the 26 hospitals in 2017.Results:The average number of X-ray diagnostic equipment were 13.5 pieces in tertiary hospitals, 7.3 in second-level hospitals and 1.6 in primary and ungraded hospitals. The fraction of imported equipment accounted for 85.2%, 69.0% and 9.4%, respectively. The higher the hospital level, the greater the proportion of equipment with five years of service history. The number of examinations in 2017 in the 26 hospitals were X-ray photography 350 221, fluoroscopy 71 597, CT 327 404, dental 23 820 and breast 12 849, respectively. The number of examinations per 1 000 population were estimated to be 444.22 by conventional X-ray and 236.40 by CT.Conclusions:The frequency of conventional X-ray examinations in the city is slightly lower than the average level in Jiangsu province in 2016 and the CT examination frequency is roughly same as in Jiangsu province.
9.Application of percutaneous transhepatic gallbladder drainage for acute calculous cholecystitis in special population

Objective To evaluate the application of percutaneous transhepatic gallbladder drainage (PTGD) in the treatment of acute calculous cholecystitis in 3 kinds of special population including the elderly, cirrhosis or mid and late-stage pregnant women. Methods Clinical data of 292 patients with acute calculous cholecystitis among 3 kinds of special population who underwent PTGD in Baoji Central Hospital of Shaanxi between January 2009 and December 2015 were analyzed retrospectively. The informed consents of all patients were obtained and the local ethical committee approval was received. There were 105 males and 187 females, aged from 22-91 and with a median age of 47 years old. 176 cases were elderly patients, 77 were with cirrhosis and 39 were mid and late-stage pregnant women. Ultrasound-guided PTGD was performed in the patients to relieve gallbladder tension rapidly, and laparoscopic cholecystectomy (LC) was performed according to the patient's condition. Efficacy of cholecystitis control in elderly patients before and after PTGD as well as the efficacy of surgical treatment in patients with liver cirrhosis or mid and late-stage pregnant women were observed. Cholecystitis indexes before and after PTGD were compared using t test. Results Symptoms of elderly patients significantly improved 3 d after PTGD. The average WBC, C-reactive protein and procalcitonin were (9.8±0.5)×109/L, (22.0±1.3) μg/L and (0.15±0.02) μg/L, which were significantly lower than preoperative (12.5±0.4)×109/L, (35.0±2.8) μg/L and (0.25±0.03) μg/L, respectively (t=-18.725,-29.062, -21.287; P<0.05). Cholecystitis in 77 patients with liver cirrhosis were effectively controlled within 1 week after PTGD, including 66 received sequential LC and 1 converted to open cholecystectomy, with a length of operation (31±9) min, intraoperative blood loss (21±5) ml and postoperative length of hospital stay (4.3±0.6) d. Cholecystitis in 39 mid and late-stage pregnant women were effectively controlled 1 week after PTGD. These patients received elective LC during the period after PTGD to 1 month after delivery, including 1 converted to open cholecystectomy. Conclusions For patients with acute calculous cholecystitis in 3 kinds of special population including the elderly, those with cirrhosis or mid and late-stage pregnant women, PTGD can effectively control the cholecystitis with the advantages of simple operation, minimally invasive, safety and effectiveness, and sequential elective LC can reduce the risk of emergent surgery.
10.The Changes of Neurocognitive Function in Early Stage in Patients with First-Episode Schizophrenia

Objective:To evaluate the changes of neurocognitive function in early stage in patients with first-episode schizophrenia.Methods:In this study,73 cases of patients with first-episode schizophrenia (research group) and 75 cases of health person (control group) were selected from January 2015 to January 2016 in our hospital.The neurocognitive function was evaluated by neuro-psychological testing tool and the data between two group were compared.Results:Scores of delayed recall,total recall,3 trial,2 trial and 1 trial of research group were lower than those of control group in BVMT-R test,and the difference was statistically significant (P<0.05).In HVLT-R test,the scores of total delay,3 trial and 2 trial of research group were significantly lower than those of control group (P<0.05).The consuming time of dominant hands and subdominant hands in pegboard tasks were significantly higher in research group than in control group (P<0.05).Conpletion time of connection test A,color connection 1 and 2 in connection test of research group were significantly higher than those of control group (P<0.05).Attempt number and correct number in research group in PASAT test were significantly lower than in control group (P<0.05).Number of search errors in research group was higher than in control group,while number of search correct,search total score and digital sign score were significantly lower than in control group (P<0.05).Total number of words,color and color / word count in research group were lower than in control group in Stroop color word test,and the difference was statistically significant (P<0.05).WMS-Ⅲ test results between two group had no significant difference (P>0.05).Conclusion:The neurocognitive function in early stage in patients with first-episode schizophrenia has been extensively damaged.
