1.Comparing the effects of clear aligners and fixed braces on the quality of life of adolescent patients

Objective To investigate the differences in treatment experience and quality of life between adolescent orthodontic pa-tients using clear aligners and fixed braces.Methods A total of 104 adolescent patients were selected who underwent orthodontic treat-ment with either clear aligners or fixed braces at Nanjing Medical University Affiliated Stomatological Hospital from January 2022 to June 2022.The patients were divided into two treatment groups based on the type of orthodontic appliance,with 52 patients in each group.Within a 6-month period of using clear aligners or fixed braces,adolescent orthodontic patients were surveyed using the child oral health impact profile-short form 19(COHIP-SF19)and other individual items.The differences in mean satisfaction,quality of life,and statistical scores were compared using independent samples t-tests,while the differences in subjective responses were compared using the Wilcoxon rank-sum test.Results There were no significant differences in mean quality of life and statistical scores between the two groups,but patients in the clear aligner group reported higher satisfaction.The clear aligner group reported greater difficulty with eating,while patients with fixed braces were more likely to experience negative emotions.Additionally,there were no significant differences between the two groups in terms of adaptation time to the appliance,maintaining dental hygiene,and feeling attractive.Conclusion Patients in both groups were generally satisfied with their treatment.Adolescent orthodontic patients treated with clear a-ligners or fixed braces for at least 6 months exhibit similar overall quality of life.
2.Study on the Design and Construction Method of Syndrome Differentiation Knowledge Graph Integrating TCM Facial Color Diagnosis

Objective To design and construct a syndrome differentiation knowledge graph that integrates TCM facial color diagnosis information;To explore the hidden relationships between the knowledge.Methods The literature data came from ancient classics,textbooks,as well as standard literature related to inspection included in the CNKI,VIP and Wanfang Data from the establishment of the databases to December 31,2022.The clinical data was sourced from 30 elderly individuals who underwent TCM health examinations at the Community Health Service Center in Jiading Industrial Zone,Shanghai in September 2022.Facial image acquisition was performed using TFDA-1 digital tongue and facial diagnostic instrument.By following the steps of knowledge extraction,knowledge fusion and quality assessment to construct a graph,and with the assistance of TCM experts for interpretation,using Access 2019 to integrate qualitative textual data and quantitative objective image digital information,a syndrome differentiation knowledge graph integrating TCM facial diagnosis was designed and completed in the Neo4j graph database.In addition,a method was designed to shift facial diagnosis knowledge from qualitative to quantitative.Results There were a total of 194 nodes under 8 entity term types and 12 entity term labels,as well as 361 relationships under 13 semantic relationships in knowledge graph.The Neo4j graph database provided a visualized TCM facial color diagnosis and differentiation,which could be queried and fed back using Cypher language.Conclusion The knowledge graph constructed based on the theory of TCM facial color diagnosis visually shows the complex correlation between facial color diagnosis and syndrome differentiation diagnosis,with a knowledge representation model that forms qualitative data of image features → semantic relationships → syndrome differentiation diagnosis forms.
3.Mechanism of curcumin in the treatment of diabetic retinopathy based on network pharmacology and molecular docking

Objective:To investigate the mechanism of curcumin in the treatment of diabetic retinopathy (DR) by network pharmacology and molecular docking.Methods:The compounds targets of curcumin were predicted by SEA and SwissTargetPrediction databases, and the DR target genes were obtained by CTD database.The different genes were mapped and matched by Venny database to screen their intersections.The intersecting genes were submitted to GeneMANIA database to construct a protein-protein interaction network.WebGestalt database was used to conduct enrichment analysis and AutoDock Vina was used to perform molecular docking of the core targets.Results:A total of 52 targets of curcumin, 1 599 targets of DR and 48 intersecting targets were detected.The core targets were serine/threonine-protein kinase 1 (AKT1), tumor necrosis factor-α (TNF-α), epidermal growth factor receptor (EGFR), signal transduction and activator of transcription 3 (STAT3) and heat shock protein 90 alpha family class A member 1 (HSP90AA1). Enrichment analysis suggested that these targets were mainly associated with signaling pathways, including the EGFR tyrosine kinase inhibitor resistance signaling pathway, hypoxia-inducible factor-1 (HIF-1) signaling pathway, interleukin (IL)-17 signaling pathway and advanced glycosylation end product-the receptor of advanced glycosylation end product (AGE-RAGE) signaling pathway.Conclusions:Curcumin may play an important role in the treatment of DR by regulating multiple signaling pathways to inhibit the inflammatory response and combat oxidative stress.
4.Chronic disseminated paracoccidioidomycosis: the first case reported in China

To report the first case of chronic disseminated paracoccidioidomycosis in China. A 49-year-old male patient presented with papules and nodules of the skin for 1 year, and papules and ulcers on the oral mucosa for 2 months. Skin examination showed the edema of the left foot, multiple crusting ulcers on the sole of the left foot, ulcers with a granular base in the interdigital regions between the third and fourth toes as well as fourth and fifth toes of the left foot, accompanied by punctate hemorrhage and exudation; there were multiple papules, nodules, and plaques on the dorsum and medial side of the left foot and the left knee, with ulcers and crusts in the center; 2 papules were observed on the left wrist, and 1 papule on the left upper lip with a crusted surface; red plaques with ulcers and punctate hemorrhage were observed on the gingival mucosa, buccal mucosa, labial mucosa, and palate, and the lesions mainly occurred on the left side. Ultrasonography of superficial lymph nodes showed bilateral cervical and supraclavicular lymph node enlargement, which was more obvious on the left side. Computed tomography of the chest and abdomen showed diffuse miliary nodular shadows, and cordlike, cloudy flocculent and nodular high-density shadows in both lungs, as well as obvious thickening of the left adrenal gland in the abdomen. Yeast cells were observed by immunofluorescent staining of biopsy tissues from the oral mucosa and left lower limb. Histopathological examination of biopsy tissues from the oral mucosa and left lower limb showed granulomatous inflammation, and refractive double-membrane yeast cells could be observed inside or outside the multinucleated giant cells, without or with a single bud or multiple buds; periodic acid-Schiff staining and hexamine silver staining of the above biopsy tissues were positive. Fungal culture of the left lower limb lesion in Sabouraud dextrose agar medium at 25℃ and 37℃ both yielded fungal hyphae. Metagenomics sequencing of the oral mucosal tissue and alveolar lavage fluid indicated the infection with Paracoccidioides brasiliensis. The diagnosis of chronic disseminated paracoccidioidomycosis was confirmed. After 1-month oral treatment with itraconazole capsules at a dose of 400 mg/d, the lesions on the skin and oral mucosa markedly improved, and computed tomography imaging of the lung and left adrenal gland also showed obvious improvement. The dose of itraconazole was reduced to 200 mg/d after 3 months. The patient′s condition further improved during a 10-month follow-up.

Schizophrenia is a serious mental disease. The diagnosis of schizophrenia so far relies heavily on subjective evidence, including self-reported experiences by patients, manifestations described by relatives, and abnormal behaviors assessed by psychiatrists. The diagnosis, monitoring of the disease progression and therapy efficacy assessment are challenging due to the lack of established laboratory biomarkers. Based on the current literature, clinical consensus, guidelines, and expert recommendations, this review highlighted evidence-based potential laboratory biomarkers for the diagnosis of schizophrenia, including genetic biomarkers, neurotransmitters, neurodevelopmental-related proteins, and intestinal flora, and discussed the potential future directions for the application of these biomarkers in this field, aiming to provide an objective basis for the use of these biomarkers in the early and accurate diagnosis, treatment, and prognosis and rehabilitation assessment of schizophrenia.
6.Diagnostic value of probe-based confocal laser endomicroscopy for indeterminate biliary stricture

Objective:To evaluate the diagnostic performance of probe-based confocal laser endomicroscopy (pCLE) for indeterminate biliary strictures.Methods:Twelve patients with indeterminate biliary strictures who underwent pCLE and brush cytology from April 1, 2013 to December 30, 2016 were enrolled. Clinical data, the results of endoscopic retrograde cholangiopancreatography, pCLE examination and brush cytology were collected. Compared with post-operative pathology and follow-up over 12 months, sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV), and accuracy of pCLE and brush cytology of the diagnosis of malignant biliary strictures were analyzed.Results:The final diagnosis were 9 malignant and 3 benign. The sensitivity, specificity, PPV, NPV and accuracy of brush cytology were 3/9, 3/3, 3/3, 3/9 and 50.0%(6/12), respectively. The corresponding indicators of pCLE were 9/9, 2/3, 9/10, 2/2, and 91.7%(11/12), respectively.Conclusion:pCLE can be used for differential diagnosis of indeterminate biliary stricture.
7.Effect of comprehensive psychological therapy on illness perception, negative emotion, quality of life and medication compliance in patients with chronic hepatitis B

Ninety eight patients with chronic hepatitis B (CHB) admitted to Department of Infectious Diseases, Yongkang First People′s Hospital from January 2017 to June 2018 were randomly divided into two groups. On the basis of entecavir+dicyclool treatment, patients in control group (48 cases) received health education and weekly telephone follow-up from the nurses, while patients in intervention group (50 cases) received health guidance from doctors and psychological counseling and communication from the nurses. All patients were followed up for 8 weeks. The illness perception, negative emotions, quality of life, and medication compliance were evaluated with the Revised Illness Perception Questionnaire (IPQ-R), Self-rating Anxiety Scale (SAS), Self-rating Depression Scale (SDS), SF-36 and Morisky Medication Adherence Scale (MMAS-8) in two groups, respectively. The results showed that, after 8 weeks, the disease identity, serious consequences, treatment control and disease correlation; SAS and SDS scores; physiological function, emotional function, energy and social function were significantly improved in control group( t=2.30, 3.45, 2.35, 2.23, 5.04, 6.03, 2.03, 2.79, 2.02, 2.34, all P<0.05). All dimension of illness perception; SAS and SDS scores; physiological function, physiological function, emotional function, energy, mental health, social function and general health were also improved significantly in intervention group ( t=4.06, 5.44, 6.91, 8.53, 5.65, 8.77, 7.25, 6.06; 12.71, 11.91, 2.27, 2.60, 4.48, 2.82, 3.65, 3.461, 2.82,all P<0.05). And the personal control, disease correlation and emotional expression; SAS and SDS scores; emotional function and mental health in intervention group were better than those in control group ( t=1.49, 2.74, 2.60, 5.02, 5.18, 5.56, 13.72, all P<0.05). In addition, the medication compliance of patients in both groups was significant improved ( Z=5.56, 13.72, all P<0.01), and the improvement was more marked in the intervention group ( Z=3.22, P<0.01). The study indicates that the comprehensive psychotherapy can more significantly improve the degree of illness perception, negative emotions, quality of life and medication compliance in patients with chronic hepatitis B.
8.Effect of leukocyte common antigen-related phosphatase receptors on the regulation of visual plasticity in rats

Objective:To investigate the effects of leukocyte common antigen-related phosphatase receptor (LAR) on the regulation of visual plasticity in rats.Methods:Fourty newborn Wistar rats were randomly divided into 5 groups, with 8 rats in each group, and were executed at 1 week, 3, 5, 7 and 9 weeks postnatal, respectively.Thirty-two healthy and 10-week-old Wistar rats were randomly divided into a normal control group, a fluoxetine group, a binocular form deprivation (BFD) group and a BFD+ fluoxetine group, with 8 rats in each group.Rats in the fluoxetine group drinked water with the dosage of 0.2 mg/ml fluoxetine for 4 weeks.The rats in the BFD group received eyelids suture for 2 weeks to form BFD model.And the combination of fluoxetine administration and BFD was performed in the BFD+ Fluoxetine group.No intervention was conducted in the normal control group.The expressions of LAR and chondroitin sulfate proteoglycans (CSPGs) were detected by immunofluorescence.The expression of LAR in the visual cortex of rats was detected by Western blot.Results:LAR was distributed in the cell membrane, plasma and axon.CSPGs were distributed in the intercellular substance in visual cortex of rats.The fluorescence intensities of LAR and CSPGs increased with the postnatal development.The relative expression of LAR was (100.00±3.20)%, (108.37±2.26)%, (113.69±2.33)%, (131.83±3.78)% and (140.11 ±4.02)% at 1 week, 3, 5, 7 and 9 weeks postnatal, respectively, the difference was statistically significant ( F=31.70, P=0.001). The relative expression of LAR was increased according to the growth of age ( β=0.961, P=0.007). The relative expression of LAR was(100.00±2.96)%, (81.02±2.77)%, (71.99±3.09)% and (52.90±2.01)% in the normal control group, fluoxetine group, BFD group and fluoxetine+ BFD group, respectively, with a significantly difference among the groups ( F=18.16, P=0.015), the relative expressions of LAR protein in the fluoxetine group, BFD group and BFD+ fluoxetine group were significantly lower than that in the normal control group ( t=31.30, 36.10, 41.72; all at P<0.01). Conclusions:LAR may be involved in the regulation of visual plasticity as a specific receptor of CSPGs.
9.Lifestyle and obesity of migrant children in Shanghai

Objective:
To investigate the correlation of lifestyle with obesity among migrant children in Shanghai and to provide reference for preventing and controlling obesity of the migrant children.
Methods:
Stratified cluster sampling was adopted to conduct a routine physical examination and investigate lifestyle among 3 894 students (7-13 years old) in June 2018. Association of lifestyle with obesity was analyzed. Chi-square test was used to compare differences in obesity prevalence among migrant children with different lifestyles. Logistic regression was used to explore association with lifestyle and obesity.
Results:
Chi-square test showed that obesity rate in males (18.6%) was significantly higher than that of females(10.7%)(χ2=24.79, P<0.01). Sleep duration, screen time, exercise frequency correlated with obesity(χ2=35.13, 9.29, 7.98, P<0.05). Regression results illustrated that insufficient sleep duration associated higher risk of obesity, the odds ratio for obesity was (OR=2.61, 95%CI=1.64-4.11) and (OR=1.54, 95%CI=1.27-1.86) among children with sleep duration less than 6 h/d and 6-8 h/d, compared with children with more than 8 h/d sleep time, after multiple covariates controlled.
Conclusion
Sleep deprivation positively associated with obesity, ensuring adequate sleep time might help prevent obesity among migrant children.
10.Profession fit and its influencing factors among clinical nurses with master's degree in Class Ⅲ Grade A hospitals

Objective? To explore the profession fit among clinical nurses with master's degree in ClassⅢ Grade A hospitals and analyze its influencing factors. Methods? From January to July 2018, we selected nurses with master's degree from 6 Class Ⅲ Grade A hospitals in Zhejiang Province as subjects by convenience sampling. The investigation was carried out with the Fitting Scale, Perceived Organizational Support Scale in Nurses and Nursing Postgraduates' Scientific Research Capacity Questionnaire. A total of 164 questionnaires were sent out and 152 valid of them were collected with 92.7% for the valid recovery rate. Results? Among 152 nurses with master's degree, the scores of person-organization fit, person-jot fit, perceived organizational support, need for organizational support and scientific research capacity were (36.92±5.42), (58.43±8.81), (3.68±0.86), (4.23±0.79) and (88.31±18.49) respectively. Multiple linear regression showed that the influencing factors of person-jot fit included work lives, ways of taking degree; the influencing factors of person-organization fit were authorized strength, perceived organizational support and scientific research capacity. Conclusions? Clinical nurses with master's degree have the high level of organization fit. Nursing managers should formulate an individualized training plan according to characteristics and capacity by reasonably assessing the research capacity of master's degree nurse, and perfect core value system of nursing human resource management, increase organizational support for staff and improve nurses' identification with organizational core value as well as fit.
