1.Preparation and Transdermal Absorption Study In Vitro of Zishen Gel Plaster

Objective To prepare Zishen pills as gel plaster according to the prescription,and investigate its transdermal absorption characteristics in vitro.Methods Based on preliminary experiments,the matrix prescription of the gel plaster was optimized by single-factor tests and the Box-Behnken design.Evaluation indicators included initial viscosity,viscosity retention and sensory scores.The modified Franz diffusion cell was used to investigate the effect of penetration enhancers on the transdermal characteristics of gel plaster in vitro,with the permeability of neomangiferin,phellodendrine hydrochloride,mangiferin and berberine hydrochloride as evaluation indicators.Results The prescription dosage of the preferred matrix for the Zishen gel plaster was sodium polyacrylate NP700 2.55 g,glycerin 11.04 g,polyvinylpyrrolidone K90 1.13 g,tartaric acid 0.1 g,glycyrrhizin 0.1 g,kaolin 0.3 g,and distilled water 15 g.Among different types and concentrations of permeation enhancers,5%aminoketone showed the best permeation performance.The permeation rates for neomangiferin,phellodendrine hydrochloride,mangiferin,and berberine hydrochloride were 1.5338,1.7809,2.3247 and 20.0899 μg·(cm2)-1·h-1,and the penetration rates were 2.4319,1.9408,1.9604 and 1.4701,respectively.The percutaneous absorption curve of the drug conformed to the zero-order kinetic equation.Conclusion The preparation process of the obtained gel plaste is stable and feasible,with good adhesive properties,sustained drug release,and favorable in vitro percutaneous permeability,indicating potential clinical application value.
2.Influencing factors for acute necrotizing pancreatitis in Eastern and Western countries: A meta-analysis

Objective To investigate the differences in the influencing factors for acute necrotizing pancreatitis (ANP) and infectious pancreatic necrosis (IPN) between Eastern and Western countries, and to provide a theoretical basis for the prediction and prevention of ANP. Methods Databases including PubMed, Embase, the Cochrane Library, and Web of Science were searched for articles on the influencing factors for ANP and IPN published up to January 21, 2021, and a Meta-analysis was performed. Results A total of 59 studies were included, with 22 studies from Eastern countries and 37 studies from Western countries.The Meta-analysis showed that in Eastern countries, male sex (odds ratio[ OR ]=1.51, 95% confidence interval[ CI ]: 1.18-1.91, P < 0.01), C-reactive protein (CRP)(standardized mean difference[ SMD ]=1.39, 95% CI : 1.06-1.71, P < 0.01), D-dimer ( SMD =0.44, 95% CI : 0.07-0.81, P =0.02), Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE-Ⅱ) score (mean difference[ MD ]=3.51, 95% CI : 1.38-5.64, P < 0.01), alcoholic etiology ( OR =3.57, 95% CI : 2.68-4.75, P < 0.01), and biliary etiology ( OR =0.60, 95% CI : 0.46-0.77, P < 0.01) were the influencing factors for ANP, and in Western countries, male sex ( OR =1.63, 95% CI : 1.30-2.05, P < 0.01), CRP ( SMD =2.09, 95% CI : 1.12-3.05, P < 0.01), APACHE-Ⅱ score ( MD =4.28, 95% CI : 2.73-5.83, P < 0.01), Ranson score ( MD =2.99, 95% CI : 2.50-3.47, P < 0.01), and organ failure ( OR =10.87, 95% CI : 2.62-45.04, P < 0.01) were the influencing factors for ANP.In Eastern countries, age ( MD =2.16, 95% CI : 0.43-3.89, P =0.01), body mass index (BMI)( MD =1.74, 95% CI : 1.23-2.25, P < 0.01), albumin level ( SMD =-0.43, 95% CI : -0.75 to-0.12, P < 0.01), CRP ( SMD =0.58, 95% CI : 0.04-1.11, P =0.03), procalcitonin ( SMD =0.80, 95% CI : 0.56-1.04, P < 0.01), D-dimer ( MD =0.23, 95% CI : 0.15-0.31, P < 0.01), APACHE-Ⅱ score ( MD =2.47, 95% CI : 0.73-4.22, P < 0.01), Ranson score ( MD =1.60, 95% CI : 1.46-1.73, P < 0.01), and extent of necrosis ≥30%( OR =2.52, 95% CI : 1.26-5.06, P < 0.01) were the influencing factors for IPN, while in Western countries, age ( MD =4.07, 95% CI : 1.82-6.31, P < 0.01), APACHE-Ⅱ score ( MD =3.28, 95% CI : 1.39-5.17, P < 0.01), Ranson score ( MD =2.18, 95% CI : 1.75-2.62, P < 0.01), SIRS score ( OR =3.88, 95% CI : 1.58-9.51, P < 0.01), alcoholic etiology ( OR =0.61, 95% CI : 0.42-0.87, P < 0.01), and organ failure ( OR =3.63, 95% CI : 1.11-11.92, P =0.03) were the influencing factors for IPN. Conclusion Current evidence shows that biliary etiology and alcoholic etiology are unique influencing factors for ANP in the Eastern population, while Ranson score is a unique influencing factor in the Western population.BMI and extent of necrosis ≥30% are unique influencing factors for IPN in the Eastern population, while alcoholic etiology is a unique influencing factor in the Western population.
3.Preliminary study of cardiac morphology and function in fetuses with tetralogy of Fallot using fetal heart quantification

Objective:To evaluate the fetal heart shape and function in tetralogy of Fallot (TOF) by fetal heart quantitative analysis (fetal HQ).Methods:A total of 52 fetuses with TOF diagnosed by fetal echocardiography and 200 normal fetuses matched with their gestational weeks from March 2020 to March 2022 at Sir Run Run Shaw Hospital, Zhejiang University were retrospectively evaluated. The basic parameters of fetal cardiac blood vessels in the two groups were measured by fetal HQ technology and conventional M-mode ultrasound technology: aortic valve diameter(AV), pulmonary artery valve diameter(PV), main pulmonary artery diameter (MPA) and Z-score. The overall morphometric measurements including end-diastolic length diameter, transverse diameter, area, and global spherical index (GSI) of the fetal heart in the 4-chamber view(4CV), area and length of the right and left ventricles and their ratios. Measurements of left and right ventricular function included ejection fraction (EF), fraction area change rate (FAC), tricuspid annular plane systolic excursion (TAPSE), left and right ventricular global longitudinal strain (GLS), and left and right ventricular end-diastolic diameter (ED), spherical index (SI), and fractional shortening rate (FS) of 24-segments. The differences of above parameters between TOF group and control group were compared. In addition, the relationships between the absolute value of left and right ventricular GLS of TOF fetus and PV/AV, PV Z-score and MPA Z-score were analyzed. The optimal critical values of GSI, left ventricular EF and left ventricular FAC of TOF fetus were determined by ROC curve, and their corresponding sensitivity and specificity were obtained.Results:Compared with control group, there were significant differences in 4CV end-diastolic length, area, GSI, left ventricular area, left ventricular length, left ventricular EF, left ventricular FAC and left ventricular GLS in TOF group (all P<0.05). There were significant differences in ED between left ventricular 15-24 segments and right ventricular 1-21 segments (all P<0.05). There were significant differences in SI between left ventricular 1-16 segments, 21-24 segments and all segments of right ventricle (all P<0.05). The differences in FS were statistically significant (all P<0.05) when comparing all segments of the left ventricle and 1-2 segments of the right ventricular, and the remaining parameters were not statistically significant (all P>0.05). The left ventricular GLS absolute value of TOF fetuses was positively correlated with PV/AV, PV Z-score and MPA Z-score( rs=0.338, 0.441 and 0.458, all P<0.05), the right ventricular GLS absolute value of TOF fetuses was positively correlated with PV AV, PV Z-score and MPA Z-score( rs=0.418, 0.368 and 0.338, all P<0.05). The optimal critical values of GSI, left ventricular EF, and left ventricular FAC in the diagnosis of fetal TOF were 1.19, 59.05%, and 44.4%, respectively. At this time, the sensitivities of diagnosis of TOF were 0.78, 0.75, and 0.80, respectively. The specificities were 0.88, 0.88 and 0.83, respectively. The areas under ROC curve were 0.89, 0.88 and 0.89, respectively. Conclusions:Fetal HQ technology can provide a simple and reliable quantitative evaluation of fetal heart shape and function, and provide certain theoretical parameters for the study of fetal heart shape and function.
4.Genetic analysis and prenatal diagnosis of a pedigree with methylmalonic acidemia

This article reports a pedigree with two previously deceased neonates. Both neonates did not experience asphyxia but passed away on their 5th and 13th day of life. The chromosomal analysis of the parents' karyotype revealed no abnormalities. Clinical manifestations of the two deceased cases and relevant medical records were recollected. Whole exome sequencing was conducted on the stem blood sample of Neonate 2, revealing a c.729_730insTT homozygous mutation (p.D244Lfs*39) in the methylmalonyl-CoA mutase gene (NM_000255). It was confirmed that Neonate 2 was affected with methylmalonic acidemia. Amniocentesis was performed at 20 +3 weeks in the current pregnancy. Sanger sequencing of amniotic fluid indicated that the fetus carried the same gene mutation as Neonate 2. Consequently, the fetus was expected to be a patient with methylmalonic acidemia and to exhibit the same phenotype as Neonate 2. Termination of pregnancy, therefore, was selected at 24 weeks of gestation.
5.Ethical regulation and supervision of clinical stem cell research and application

In the paper, national and international ethical requirements and supervision on stem cell research and clinical application were analyzed after literature review. It is suggested that the development of stem cell technology should be timely tracked, prospective ethical research related to stem cells should be enhanced, and that stem cell research and clinical translation should comply with relevant international ethical guidelines and national regulations in China. In addition, it is indicated that the oversight of stem cell products and their clinical use will be continuously improved in our country.
6.Effectiveness and safety of clinical use of stem cells and its implications for regulations

ObjectiveTo analyze the effectiveness and safety of clinical use of different types of stem cells to provide evidence for governmental supervision of key issues in clinical utilization of stem cells. MethodsSix literature databases in China and abroad were searched for relevant literature published from January 2010 to July 2020, and a systematic review was conducted. ResultsThe study showed 72 studies concerning the effectiveness and safety of different types of stem cells in clinical utilization. Although clinical utilization of stem cells had some therapeutic effects for certain diseases, their long-term effect and safety need to be further evaluated, especially their potential risk of tumorigenicity. ConclusionTo protect the patients’ interest, physicians should fully weigh the benefits against the risks of clinical utilization of stem cells, and relevant governmental departments should strengthen supervision of ethics of clinical research and therapies involving stem cells as well as supervision of stem cell products.
7.Comparison of the Effects of Remimazolam and Propofol on Perioperative Cellular Immune Function in Patients underwent Radical Mastectomy

OBJECTIVE:To c ompare the effects o f intravenous anesthesia with remimazolam and propofol on perioperative cellular immune function in patients underwent radical mastectomy. METHODS :Eighty patients underwent selective radical mastectomy were collected ,and then randomly divided into remimazolam group (group R )and propofol group (group P ). During anesthesia induction ,group R was intravenously injected with remimazolam 0.2 mg/kg+sufentanil 0.3 μg/kg+cisatracurium 0.2 mg/kg;group R was intravenously injected with propofol 2 mg/kg+sufentanil 0.3 μg/kg+cisatracurium 0.2 mg/kg. During anesthesia maintenance,group R was intravenously pumped with remimazolam 0.4-1.2 mg/(kg·h)+remifentanil 0.1-0.2 μg(/ kg·min);group P was intravenously pumped with propofol 4-10 mg/(kg·h)+remifentanil 0.1-0.2 μg(/ kg·min). Both groups were given intravenous injection of cisatracurium intermittently. The anesthesia depth was monitored during the operation and the pumping speed of remimazolam,propofol and remifentanil was adjusted accordingly. The intraoperative infusion volume ,blood loss ,operation time , opioid dosage ,and visual analogue scale (VAS)scores at 24 and 72 hours after operation were recorded in 2 groups;at the same time,the levels of T lymphocyte CD 3+,CD4+,CD8+ and NK cells were measured 30 min before anesthesia induction ,24 h and 72 h after operation ;CD4+/CD8+ was also calculated. The incidence of ADR was recorded in 2 groups. RESULTS :There was no statistical significance in intraoperative infusion volume ,blood loss ,operation time ,opioid dosage ,VAS score at 24,72 hours after operation and the incidence of ADR between 2 groups(P>0.05). Compared with 30 min before anesthesia induction ,the levels of CD 3+,CD4+,NK cells and CD 4+/CD8+ ratio in 2 groups at 24 hours after operation were significantly decreased (P< 0.05);compared with group P ,the levels of CD 3+,CD4+ and NK cells as well as CD 4+/CD8+ ratio in group R increased significantly in group R (P<0.05). CONCLUSIONS :For anesthesia maintenance ,the inhibitory effects of remimazolam on perioperative cellular immunity in patients underwent radical mastectomy are poorer than propofol.
8.Effects of Dexmedetomidine on Postoperative Delirium in Liver Tumor Resection Elderly Patients with Sleep Disorders

OBJECTIVE:To investigate the effects of d exmedetomidine on postoperative delirium (POD) in liver tumor resection elderly patients with sleep disorder (SD). METHODS :Totally 80 patients undergoing liver tumor resection with preoperative Pittsburgh sleep quality index (PSQI)score ≥7 were selected from the Affiliated Cancer Hospital of Zhengzhou University from Jan. 1st,2020 to Oct. 31st,2020. They were randomly divided into group SD and group Dex according random number table ,with 40 cases in each group. At the same time ,40 patients with preoperative PSQI score <7 were selected as group C. Thirty min before anesthesia induction ,Dexmedetomidine hydrochloride injection 0.4 μg/kg was injected intravenously in group Dex. Etomidate emulsion injection ,Sufentanil citrate injection and Rocuronium bromide injection were used for anesthesia induction in 3 groups,and Ropofol medium/long chain fat emulsion injection + Remifentanil hydrochloride for injection was used to maintain anesthesia. The drug use ,operation time ,PACU stay time and postoperative hospital stay were recorded in 3 groups. The cognitive function was evaluated 2 h before operation and 1,3,5,7 days after operation. The occurrence of POD was observed. The plasma levels of IL- 6 and S 100β were measured 2 h before operation ,2 h after operation ,1,3,5 days after operation. The occurrence of ADR was recorded. RESULTS :There was no statisti cal significance in intraoperativ e drug use and operation time among 3 groups (P>0.05). The PACU stay time , the incidence of POD and the duration of POD in group SD an d lixxi18@126.com group Dex were significantly higher or longer than group C , while the Dex group was significantly lower or shorter thangroup SD (P<0.05). The postoperative hospitalization stay ofgroup SD was significantly longer than group C and group Dex (P<0.05),and there was no statistical significance between group Dex and group C (P>0.05). Before operation ,there was no statistical significance in MMSE scores or plasma levels of IL- 6 and S100β among 3 groups(P>0.05). MMSE scores of group C 1,3 days after operation ,those of group SD and group Dex 1,3,5 and 7 days after operation were significantly lower than those before operation. MMSE scores of group SD and group Dex 1,3,5 and 7 days after operation were significantly lower than group C at corresponding period ;the group Dex was significantly higher than the group SD at corresponding period (P<0.05). The plasma levels of IL- 6 and S 100 β at different time points were significantly higher than before operation ,and the group SD and group Dex were significantly higher than the group C ,and the group Dex was significantly lower than group SD at corresponding period (P<0.05). There was no statistical significance in the total incidence of ADR among 3 groups(P>0.05). CONCLUSIONS :SD can promote the occurrence of POD in liver tumor resection elderly patients. Dexmetomidine can reduce the incidence of POD in elderly patients with preoperative SD ,the mechanism of which may be associated with the inhibition of IL- 6 and S 100β expression and the alleviation of brain injury with good safety.
9.Study on Quality Standard of Yao Medicine Pileostegia tomentella

OBJECTIVE:To establi sh the quality standard of Yao medicine Pileostegia tomentella,and to provide reference for its quality control. METHODS :Totally 10 batches of P. tomentella were collected from Guangxi area. The properties of P. tomentella were observed and microscopic identification was conducted for the transverse section of its stems ,roots and powder. TLC method was established by using 7-hydroxycoumarin as reference. The contents of water ,total ash ,acid-insoluble ash and alcohol-soluble extract in P. tomentella were determined. The contents of skimmin ,7-hydroxycoumarin and 7-hydroxy-8- methoxycoumarin in P. tomentella were determined by HPLC. RESULTS :The root and stem surface of P. tomentella possessed obvious wrinkles ,hard texture ,slight smell and bitter taste ,and there were secretory cells and needle crystal cells in the cross section. The medicinal powder contained calcium oxalate needle crystal ,calcium oxalate square crystal ,sclerotic cell and starch granules. The results of TLC showed that the spots of the same color were found in the corresponding positions of chromatograms of test samples and 7-hydroxycoumarin control. In the 10 batch of P. tomentella ,the contents of water were 9.1%-11.8%;those of total ash were 3.6%-6.7%;those of acid-insoluble ash were 0.10%-0.44%;those of alcohol-soluble extract were 8.9%-14.0%.The linear range of skimmin ,7-hydroxycoumarin and 7-hydroxy-8-methoxycoumarin were 6.088-182.640,1.568-47.040 and 1.096- 32.880 μg/mL(all r not less than 0.999 7). The average recoveries were 99.47%,101.07% and 99.89%(RSD were all less than 2%). RSDs of precision ,stability(24 h),reproducibility and durability tests were all lower than 2% . The contents of 3 components such as skimmin were 1.337-9.534,0.348-2.236, 0.083-1.088 mg/g,respectively. CONCLUSIONS :It is preli- 201705) minarily proposed that the content of water in P. tomentella is not more than 12.0% ;that of total ash is not more than 7.0%;that of alcohol-soluble extract is not less than 8.0%; E-mail:luguoshou@foxmail.com that of ski mmin is not less than 1.30 mg/g;that of 7-hydroxy- coumarin is not less than 0.30 mg/g;that of 7-hydroxy-8-methoxycoumarin is not less than 0.06 mg/g.
10.Optimization of Ultrasonic-assisted Ethanol- (NH4)2SO4 Aqueous Two-phase Extraction of Citrusinol from Desmo- dium caudatum by Box-Behnken Design-response Surface Methodology

OBJECTIVE:To optimize ultrasonic-assisted ethanol-(NH4)2SO4 aqueous two-phase extraction technology of citru- sinol from Desmodium caudatum . METHODS :Using the content of citrusinol as indexes ,with ethanol volume fraction , solid-liquid ratio ,(NH4)2SO4 addition amount ,ultrasonic time and ultrasonic temperature as factors ,based on the single factor tests,Box-Behnken design-response surface methodology was used to optimize the extraction technology of citrusinol. RESULTS : The optimized extraction technology of citrusinol included that ethanol volume fraction was 95.35%,the solid-liquid ratio was 1∶50.35 (g/mL),(NH4)2SO4 addition amount was 4.49 g,ultrasonic time was 48.7 min,ultrasonic temperature was 57.6 ℃. In 3 times of validation tests ,the extraction rates of citrusinol were 0.637 8,0.638 4,0.625 4 mg/g,respectively,which was close to predicted value(0.630 5 mg/g). CONCLUSIONS :The optimized ultrasonic-assisted ethanol- (NH4)2SO4 aqueous two-phase extraction technology is stable and feasible ,and can be used for the extraction of citrusinol from D. caudatum .
