1.The role of SENP-1 in chronic intermittent hypoxia induced myocardial injury in rats

Objective :
To investigate the effects and mechanism of small ubiquitin-like modifier ( SUMO) specific proteinase-1 (SENP-1) on chronic intermittent hypoxia ( CIH) induced myocardial injury in rats.
Methods :
32 male SD rats were randomly divided into : control group,CIH group,negative control adeno-associated virus interven- tion group (AAV-shNC) and SENP-1 shRNA adeno-associated virus intervention group (AAV-shSENP-1) ,with 8 rats in each group.After 6 weeks of CIH induction,echocardiography was performed.The levels of cardiac troponin I (cTNI) ,creatine kinase MB isoenzyme ( CKMB) ,myoglobin (Mb) ,lactate dehydrogenase (LDH) in serum and malondialdehyde (MDA) ,uperoxide dismutases ( SOD) ,glutathione ( GSH) ,interleukin( IL) -1 β , IL-6 and tumor necrosis factor-α(TNF-α) in myocardial tissue were detected by ELISA.The pathological changes of myocardial tis- sue was observed by HE staining.The reactive oxygen species ( ROS) level in myocardial tissue was detected by DCFH-DA fluorescence probe labeling.The small ubiquitin-like modifier (SUMO) level of hypoxia inducible factor- 1 α (HIF-1 α) protein in myocardial tissue was detected by kit.The mRNA and protein levels of SENP-1 and HIF- 1 α in myocardial tissue were detected by qRT-PCR and Western blot.
Results :
Compared with the control group, the pathological damage of myocardial tissue in CIH group was serious,the levels of left ventricular end diastolic diameter (LVEDD) ,left ventricular end systolic dimension (LVESD) and serum cTNI,CKMB,Mb and LDH signif- icantly increased (P<0. 05) ,and the levels of ROS,MDA,IL-1 β , IL-6,TNF-α and the mRNA and protein levels of SENP-1 and HIF-1α in myocardial tissue also significantly increased (P <0. 05 ) ,while the levels of LVEF, LVFS,serum GSH and SOD significantly decreased (P <0. 05) ,and the SUMOylates level of HIF-1α protein in myocardial tissue also significantly decreased (P <0. 05 ) .Compared with CIH group,AAV-shSENP-1 group had less myocardial pathological damage,the levels of LVEDD,LVESD and serum cTNI,CKMB,Mb and LDH signifi- cantly decreased (P<0. 05) ,and the levels of ROS,MDA,IL-1 β, IL-6,TNF-α and the mRNA and protein levels of SENP-1 and HIF-1α in myocardial tissue also significantly decreased (P<0. 05) ,the levels of LVEF,LVFS,serum GSH and SOD significantly increased (P<0. 05) ,and the SUMOylates level of HIF-1α protein in myocardial tissue also significantly decreased (P<0. 05) .
Conclusion
Inhibition of SENP-1 expression can alleviate CIH induced myocarditis and oxidative stress in rats,improve myocardial injury and cardiac dysfunction,and its mechanism may be related to the improvement of HIF-1α SUMOylates level,thus inhibiting HIF-1α expression.
2.Effect of continuous nursing intervention on the quality of life of patients with permanent cystostomy

Objective To explore the effect of continuous nursing intervention on the quality of life of patients with permanent cystostomy.Methods A total of 60 permanent cystostomy patients in our hospital were selected as research objects,and were divided into control group and experimental group according to different nursing methods,given routine nursing care,routine nursing and continuous nursing intervention,respectively.Bladder stoma related complications,life quality before and after intervention,nursing satisfaction were evaluated.Results There were significant differences in catheter removal,catheter blockage,QOLQ quality of life score,and nursing satisfaction in two groups (P < 0.05).Conclusion Continuous nursing intervention can effectively reduce incidence rate of complications of permanent cystostomy,improve quality of life and promote harmony of nurses and patients.So it is worthy of clinical application.

4.Anti-mullerian hormone in seminal plasma and serum:association with sperm parameters

Objective To explore the association of anti-mullerian hormone(AMH)in seminal plasma and serum with sperm counts and energy for male.Methods For 215 cases of healthy male selected from our reproductive clinic,with women′s reason for infertility,seminal plasma and serum AMH were detected,as semen parameters(sperm density,living rate,vitality and malformation rate),6 items of serum sex hormone.In seminal plasma and serum AMH respectively as the dependent variable,using multiple line-ar regression model to explore its quantitative relation with semen parameters and sex hormone levels.Results 215 cases were en-rolled,aged 34.28±5.70 years,while the median of the seminal plasma AMH was 0.47,quartile 0.05-3.09 pmol/ejaculation.The median of the serum AMH was 53.07,quartile 32.32 -72.20 pmol/L.Through multiple linear regression analysis,after adjusted by age and BMI,the seminal plasma of AMH and total number of sperm,sperm concentration,dynamic motility,total sperm activi-ty,serum inhibin B were positively correlated(P <0.05);The correlation between sperm morphology and other serum sex hormone had no statistical significance(P >0.05);Serum AMH negatively correlated with serum FSH,with serum inhibin B positively(P <0.05);Seminal plasma in various parameters and other related serum sex hormone had no statistical significance(P >0.05).Conclu-sion The seminal plasma of AMH were positively correlated with sperm concentration,sperm counts,sperm vitality,with the asso-ciation for serum AMH not yet found.
5.Evaluation of left ventricular myocardial function in patients with severe aortic stenosis underwent aortic valve replacement with preserved left ventricular ejection fraction by three-dimensional speckle tracking imaging

Objective To evaluate left ventricular myocardial function altemation by three-dimensional speckle tracking imaging (3D-STI) after surgical aortic valve replacement (AVR) in severe aortic stenosis (AS) patients with preserved left ventricular ejection fraction (LVEF),and discuss its clinical value.Methods Forty patients with severe aortic stenosis who were hospitalized or outpatient in Nanjing First Hospital Affiliated to Nanjing Medical University during the period of October 2014 to October 2016 (AS group),and forty healthy volunteers (normal control group) were enrolled in this study.Normal control group underwent conventional echocardiography and 3D-STI measurement,while the AS group underwent conventional echocardiography and 3D-STI measurement at preoperative,1 week postoperative and 3 months postoperative,then we obtained left ventricular end-diastolic diameter (LVEDD),left ventricular end-systolic diameter (LVESD),interventricular septum thickness diameter (IVSd),left ventricular posterior wall thickness diameter (LVPWd),left ventricular ejection fraction (LVEF),global longitudinal strain (GLS),global circumferential strain (GCS),global radial strain (GRS),global area strain (GAS) strain and 3D-strain.The independent sample t test was used to compare the difference of conventional ultrasound parameters and 3D-STI parameters between AS preoperative patients and healthy controls,AS patients at 3 months postoperative and healthy controls.The single factor analysis of variance was used to compare the difference of conventional ultrasound parameters and 3D-STI parameters in severe patients at preoperative,1 week postoperative and 3 months postoperative.LSD-t test was used to compare in different AS groups.Results Compared with normal control group,IVSD,LVPWD and GCS of severe AS patients increased significantly (t=13.824,11.298 and-6.584,all P < 0.001),GLS,GRS and 3D-Strain decreased significantly (t=10.221,-6.237 and-5.674,all P < 0.001),LVEDD,LVESDand GAS had no significant difference.Compared with preoperative AS patients,LVEF,GLS,GAS and GCS decreased significantly (t=-2.205,-2.093,-2.034 and-3.152,all P < 0.05 or 0.01) at 1 week postoperative,LVEDD,LVESD,IVSD,LVPWD,GRS and 3D-strain had no significant difference at 1 week postoperative;GLS,GRS and 3D-strain increased significantly (t=5.446,-4.923 and-4.388,all P < 0.05 or 0.01) at 3 months postoperative,GCS,IVSD and LVPWD decreased significantly (t=-3.988,4.794 and 4.211,all P < 0.05 or 0.01) at 3 months postoperative,LVEDD,LVESD,LVEF and GAS had no significant difference at 3 months postoperative.Compared with AS patients at 1 week postoperative,LVEF,GLS,GRS,GAS and 3D-strain increased significantly (t=-2.631,7.383,-4.719,2.923 and-4.154,all P < 0.05 or 0.01) at 3 months postoperative,GCS,IVSD and LVPWD decreased significantly (t=-2.109,4.747 and 4.323,all P < 0.05 or0.01) at 3 months postoperative.But in AS patients at 3 months postoperative,IVSD,LVPWD,GLS and GCS were still higher than those of normal control group (t=9.809,7.066,4.752 and-2.553,all P < 0.001 or < 0.05),LVEDD,LVESD,LVEF,GRS,GAS and 3D-strain had no significant difference.Conclusion The alternation of left ventricular myocardial function have a certain characteristic before and after aortic valve replacement in severe aortic stenosis patients with preserved LVEF,and 3D-STI can evaluate it more accurately.
6.Analysis of correlation between sperm indicators and early spontaneous abortion after in vitro fertilization

Objective To explore the correlation between the sperm indicators and early spontaneous abortion after in vitro ferti ‐lization(IVF) .Methods 251 couples receiving IVF ,were retrospectively analyzed and divided into the research group (98 couples) and control group (153 couples) according to whether spontaneous abortion occurring .The male′s indicators of sperm DNA frag‐mentation index (DFI) ,sperm nucleoprotein immaturity rate ,percentage of normally morphologic sperms ,etc .in the two groups were analyzed .Results DFI ,percentage of normally morphologic sperms ,sperm concentration ,motility and forward motile sperm had no statistical differences between the research group and the control group (P> 0 .05) ;while the sperm nucleoprotein immaturi‐ty rate of the research group was significantly higher than that of control group ,the difference was statistically significant (P <0 .05) ;the sperm nucleoprotein immaturity rate had weak correlation with spontaneous abortion in early pregnancy (P < 0 .05) . Conclusion The sperm nucleoprotein dysmaturity is correlated with the spontaneous abortion in early pregnancy .The sperm nucle‐oprotein immaturity rate could be used to evaluate the risk of spontaneous abortion in early pregnancy after IVF .
7.Application of digital medical technologies in percutaneous transhepatic cholangioscopy lithotripsy for the treatment of hepatolithiasis

Objective To investigate the clinical value of medical image-three dimensional visible system (MI-3DVS) in percutaneous transhepatic cholangioscopy lithotripsy (PTCSL) for the treatment of hepatolithiasis.Methods The clinical data of 66 patients with hepatolithiasis (55 were from the First People's Hospital of Shunde City and 11 were from the Zhujiang Hospital) were retrospectively analyzed.The images of computed tomography were three-dimensionally reconstructed with MI-3DVS.The location,number,size,shape of the stones were figured out,and the operation strategies of PTCSL were designed according to the types of the disease.The clinical value of MI-3DVS was evaluated according to the operation results and postoperative recovery of patients.All the patients were followed up via telephone or out-patient re-examination.Results Liver,biliary system,stones and blood vessels were three-dimensionally reconstructed with the MI-3 DVS,and the size,number,shape,location of the stones and location,degree,length of the biliary stricture and its anatomical relationship with adjacent blood vessels were clearly displayed.The coincidence rate of planned and actual operations was 95.5% (63/66).The mean operation time,intraoperative blood loss,rate of stone clearance,complication rate and duration of hospital stay of 63 patients were (117 ± 9) minutes,(18 ± 1) ml,92.4% (61/66),6.1% (4/66) and (15 ± 4) days,respectively.All the patients were followed up till September 2012,the median survival time was 16 months (range,1-69 months),and the recurrence rate of hepatolithiasis was 9.1% (6/66).One patient died of tumor metastasis and multi-organ dysfunction syndrome postoperatively.Conclusion MI-3DVS could effectively improve the safety and efficiency of PTCSL for patients with hepatolithiasis.
8.A prospective phase Ⅰ / Ⅱ study of recombinant endostatin (Endostar) combined with concurrent radio-chemotherapy in patients with unresectable stage Ⅲ non-small cell lung cancer

Objective To evaluatc the efficacy and safcty of recombinant endostatin (Endostar)combined with concurrent radio-chemotherapy (CRCT) in patients with unresectable stage Ⅲ non-small cell lung cancer (NSCLC).Methods From March 2009 to November 2011,47 patients received threedimensional conformal radiotherapy of 60-66 Gy in 30-33 fractions over 6-7 weeks And concurrent chemotherapy of docetaxel 65 mg/m2 and cisplatin 65 mg/m2.Endostar was administered once a week before and on week 2,4,6 during CRCT at a dose level of 7.5 mg/m2/d.Tumor response was evaluated with thoracic CT scans performed 4 weeks after completion of treatment in accordance with RECIST 1.1 criteria.Acute toxicities were evaluated in accordance with CTCAE 3.0.Results Forty-four patients completed treatment and toxicity evaluation,42 patients completed evaluation of efficacy.Five patients achieved complete response,29 partial response,3 stable disease,and 5 progressive disease,2 were net assessed.Overall response rate was 77%.One-year overall survival rate was 81%,and one-year progression-free survival rate was 51%.Twelve patients died,2 died of treatment related toxicities,8 of cancer,and 2 of unknown causes.Nineteen patients developed grade 3/4 neutrocytopenia,grade 3 acute esophagitis and pneumonitis were observed in 4 and 4 patients,respectively,and 1 patient died of pneumonitis.No patient developed cardiovascular toxicities and hemorrhage.Conclusions Endostar combined with CRCT for unresectable stage Ⅲ NSCLC was safe and the short term outcomes were promising.Further investigations are warranted.
9.Involved-field radiotherapy versus elective nodal irradiation in combination with concurrent chemotherapy for locally advanced non-small cell lung cancer: A prospective randomized clinical study

ObjectiveTo evaluate the local failure and the impact on survival by prospectively comparing involved field radiotherapy (IFRT) and elective nodal irradiation (ENI) in combination with concurrent chemotherapy for locally advanced non-small cell lung cancer ( LA-NSCLC ).Methods LANSCLC patients were treated with 2 cycles of carboplatin ( AUC =5 - 6,d1 ) combined with paclitaxel ( 175mg/m2 ),followed assessment without distant metastasis,then randomized into IFRT (45 patients) or ENI (54 patients) arm.IFRT included primary tumor,ipsilateral hilar and positive mediastinal lymph nodes;ENI included the primary lesion,ipsilateral hilar,hilateral mediastinal lymph node drainage and bilateral supraclavicular area.The prescription dose was given as high as possible with V20 ≤35% and spinal cord dose ≤50 Gy,combined weekly paclitaxel 40 mg/m2 concurrent chemotherapy.The Kaplan-Meier method was used to estimate survival data and the log-rank method was used to test distribution of survival time between arms.ResultsThe follow-up rate was 99%.49,29 and 17 patients were followed-up for 1-,2-and 3-year,respectively.More patients from group IFRT received >60 Gy than ENI (49% vs.26%,x2 =5.59,P =0.018 ).The local failure rates were 29% and 36%,respectively ( x2 =0.46,P =0.497 ).The 1-,2-and 3-year local tumor progression-free survival rates were 76%,69%,65% and 80%,53%,49% ( x2 =0.74,P =0.389),respectively; the 1-,3-and 5-year overall survival rates were 80%,41%,33% and 69%,32%,13% (x2 =3.97,P =0.046),respectively.There were no significant differences in acute and late toxicities between the arms ( x2 =3.910 - 0.155,P =0.142 - 0.925 ).ConclusionsIFRT improved radiation dose and survival rate and did not increase the failure of elective lymph node region compared with ENI.The toxicities were no differences between IFRT and ENL Further investigation with big size sample is warranted.
10.Severe acute radiation pneumonitis after concurrent chemoradiotherapy in non-small cell lung cancer

ObjectiveThe study is to investigate the predictive values of dosimetric parameters and patient related factors in severe acute radiation pneumonitis (SARP) after concurrent chemoradiotherapy in non-small cell lung cancer (NSCLC).Methods In all,147 NSCLC patients treated with concurrent chemotherapy and 3DCRT between 2006 and 2010 was collected.Independent sample t test was used to compare parameter values between patients with SARP and those without SARP.Logistic regression was used to identify significant determined factor.Predictive value of each parameter was tested by ROC analysis.Pearson correlation was used to analyze correlations between parameters.Represent factors were identified by factor analysis.ResultsThe incidence of SARP was 9.5% ( 14/147 ).The means lung dose (MLD),V20,V30,V40,and V50 ( x2 =4.87 -6.84,P =0.009 -0.025,respectively ) were determining factors for SARP.Our datasets shows that for SARP <5%,MLD,V20,V30,V40 and V50 should be ≤16.77 Gy,V20≤34.15%,.V30 ≤23.62%,.V40 ≤ 18.57%,V50 ≤ 13.02%.ROC analysis show that areas under MLD,V20,V30,V40 and V50 curves was corresponding to 0.678,0.661,0.667,0.677,and 0.651,respectively.In addition,the sensitivity and specificity of each parameter at cutoff values are:78.0% and 48.1% for MLD;42.9% and 82.0% for V2o ;78.6% and 52.9% for V30 ;71.4% and 61.7% for V40,and 57.1% and 67.7% for V50.Factor analysis suggest that we can choose 1 or 2 parameters from MLD,V20,or V30,and another from V40 or V50 for predicting.The incidence of SARP was greater in patients with tumorsin right lower lung than other locations ( 22.2% vs 6.7%,x2 =6.19,P =0.0 2 3 ).Conclusions The MLD,V20,V30,V40 and V50 are determining factors for SARP.As predictive value of each parameter alone is relatively week,using two or more parameters to predict SARP is recommended.
