OBJECTIVE To investigate the clinical characteristics of glucocorticoid allergy, and provide treatment and prevention strategies for patients with concurrent bronchial asthma. METHODS A retrospective analysis was conducted on the clinical data of 250 patients with suspected glucocorticoid allergy admitted to the First Affiliated Hospital of Army Medical University (hereinafter referred to as “our hospital”)from May 1st, 2002, to April 30th, 2022; and a typical case analysis was carried out. RESULTS Among 250 patients with suspected glucocorticoid allergy, 140 were female patients (56.00%) and 110 were male patients (44.00%). The majority of admissions were to the internal medicine department (118 cases, 47.20%). One hundred and seventy patients (68.00%) were allergic to dexamethasone, and 37 patients (14.80%) were allergic to prednisone. Two hundred and thirty-eight patients (95.20%) were allergic to one type of glucocorticoid, and 12 patients (4.80%) were allergic to two types of glucocorticoid. Ten patients had clear records of glucocorticoid administration routes, in which 6 were intravenous and 4 were oral. Among the 250 patients, only 32 cases had clear records of clinical manifestations of glucocorticoid allergy, the most common clinical manifestations were rash (12 cases) and skin itching (8 cases), with 6 patients experiencing both rash and skin itching. The treatment for glucocorticoid allergy mainly involved discontinuing the medication and providing symptomatic treatment. For patients with concurrent bronchial asthma, it was generally necessary to switch to other glucocorticoids in their subsequent treatment to control the condition. The incidence of suspected glucocorticoid allergy among inpatients in our hospital during the same period was 0.019%. The incidence of suspected glucocorticoid allergy with concurrent bronchial asthma was 0.201%. Additionally, a case analysis of bronchial asthma combined with chronic obstructive pulmonary disease in our hospital resulted in glucocorticoid allergy revealed that, based on the patient’s medical history and drug challenge test results, the patient was diagnosed with glucocorticoid (Methylprednisolone tablets and Dexamethasone acetate tablets) allergy, and antihistamine treatment was effective. CONCLUSIONS Glucocorticoid allergy is clinically rare, and its clinical manifestations are predominantly mild reactions such as rashes and skin itching. For patients with concurrent bronchial asthma who experience glucocorticoid allergy, treatment options include switching to other glucocorticoids, altering the route of glucocorticoid administration, symptomatic treatment, and the use of immunosuppressive agents as adjunctive or alternative therapy.

OBJECTIVE To report the diagnosis and treatment process of 1 case of Streptomyces thermoviolaceus pneumonia, and provide reference for the diagnosis and treatment of this type of infection by combining literature on Streptomyces pneumonia. METHODS A case study was conducted on a patient with S. thermoviolaceus pneumonia treated at the First Affiliated Hospital of Army Medical University. Additionally, a systematic literature review of Streptomyces pneumonia cases was performed. RESULTS The patient with S. thermoviolaceus presented with left lung consolidation and mass-like opacity. Initial diagnosis via matrix-assisted laser desorption/ionization time-of-flight mass spectrometry failed, but 16S rRNA gene amplification and sequencing confirmed S. thermoviolaceus as the causative pathogen. Six-month therapy with Amoxicillin capsules (1 g orally, three times daily) resulted in near-complete lesion resolution. The literature analysis of Streptomyces pneumonia revealed that 13 patients with Streptomyces pneumonia were included (including the patient reported in the article), age range of 18-77 years, more males (8 cases), and mostly suffering from underlying diseases. In terms of clinical symptoms, all enrolled cases exhibited cough, and some cases were accompanied by variable dyspnea. Imaging findings included that there was no characteristic predilection site for Streptomyces pneumonia lesions, and CT images commonly showed lung consolidation and bilateral nodules. Definitive diagnosis relied on 16S rRNA sequencing. Treatment regimens included tetracyclines, β -lactam drugs combined with enzyme inhibitors, ceftriaxone, aminoglycosides, macrolides, or carbapenems, administered for prolonged duration (6 months). Follow-up indicated a good prognosis, and only one mortality occurred. CONCLUSIONS 16S rRNA gene sequencing should be prioritized for diagnosing S. pneumonia. Effective antimicrobial options include tetracyclines,β-lactam drugs combined with enzyme inhibitors, ceftriaxone, aminoglycosides, macrolides, and carbapenems. Prolonged therapy correlates with favorable prognosis.

Objective:To explore the clinical efficacy of adult donor dual kidney transplantation. Methods:Retrospective analysis of case data of 13 adult donor kidney dual kidney transplantation (DKT) performed in the The Second Xiangya Hospital of Central South University from September 2016 to December 2020. For 13 donors, the average age and BMI were (53.5±12.4)years and (24.3±2.8) kg/m 2, respectively. Their mean Serum creatinine (SCr) at admission and before procurement was (132.9±54.1)and (228.7±112.4)μmol/L, respectively. 3 of them had diabetes mellitus history, and 8 had hypertension history. 11 met the United Network for Organ Sharing (UNOS) DKT criteria and 6 met Remuzzi score DKT criteria. For 13 recipients, the average age and BMI were (39.3±8.9)years and (20.2±2.4)kg/m 2, respectively. All of them received ABO blood type-matched kidney transplants. 2 of them had their grafts transplanted in the bilateral iliac. In 12 cases, the grafts filled rapidly and urinated immediately when opening blood flow. In 1 case, the grafts were dark in color and vascular showed weak pulsation after opening blood flow. The time to recovery of perioperative graft function (from the day of surgery to the natural reduction of SCr to the normal range 44-133μmol/L), the occurrence of delayed graft function (DGF), acute rejection (AR), ureteral and surgical incision complications, as well as the recipients’ final follow-up SCr, eGFR, urinary protein, and grafts outcome were observed. Risk factors affecting outcomes were assessed by univariate logistic regression analysis. Results:The SCr dropped to the normal range at discharge in 10 recipients, and the average recovery time was (13.8±13.0) days. In other 3 cases SCr at discharge were 300.0, 149.0, 152.5μmol/L. 4 cases had DGF, 4 had AR, 1 experienced urinary fistula, and 1 experienced incisional dehiscence, which were treated with anti-rejection, J-tube implantation, continuous catheterization to maintain bladder void, secondary suturing, respectively. The follow-up time ranged from 4 to 54 months, with a median of 28(15.5, 31.0) months. At the final follow-up time, 10 cases had good graft function, 2 suffered impaired kidney function, and 1 experienced graft failure. The average SCr and eGFR except for graft failure patient were (144.2±101.3)μmol/L and (52.9±21.2)ml/min, respectively. 4 had positive urine protein. Univariate logistic regression analysis showed that donor age, BMI, history of diabetes mellitus and hypertension, and SCr were not significantly correlated with recipients’ DGF and graft impairment ( P>0.05), and due to the small sample size, multifactorial logistic regression analysis was not performed. Conclusion:The short to medium-term effects of adult donor DKT coule be safe and feasible.

Objective:To investigate the current situation of developmental coordination disorder in preschool children in Taiyuan, providing evidence for developing health strategies for preschool children in this city. Methods:We selected 11 126 preschool children aged 3-5 years from 24 kindergartens in Taiyuan using the cluster random sampling method during November-December 2018. Each preschool child was tested with the developmental coordination disorder questionnaire for young children. Results:The incidence of developmental coordination disorder was 3.4%, and the incidence of suspected developmental coordination disorder was 5.4%, and the incidences of which were higher in boys than in girls ( χ2 = 9.46, P < 0.01). The incidence of developmental coordination disorder decreased with aging ( r = -0.06, P < 0.01). The incidence of developmental coordination disorder was negatively correlated with parents' education level and maternal passive smoking during pregnancy ( r = -0.11, P < 0.01; r = -0.12, P < 0.01; r = -0.03, P < 0.01). Lower parents' education level and maternal passive smoking during pregnancy led to a higher incidence of developmental coordination disorder in children. Conclusion:Preschool children from Taiyuan had a lower incidence of developmental coordination disorder than those from economically developed south areas. The higher the gross domestic product in the areas where the kindergartens locate, the lower the incidence of developmental coordination disorder ( r = 0.03, P < 0.01). Environmental factors have a certain impact on developmental coordination disorders in preschool children. Parents, kindergarten teachers, and child health workers should take active intervention measures against the adverse factors in the early development environment of children.

Irreversible destruction of bronchi and alveoli can lead to multiple incurable lung diseases. Identifying lung stem/progenitor cells with regenerative capacity and utilizing them to reconstruct functional tissue is one of the biggest hopes to reverse the damage and cure such diseases. Here we showed that a rare population of SOX9 basal cells (BCs) located at airway epithelium rugae can regenerate adult human lung. Human SOX9 BCs can be readily isolated by bronchoscopic brushing and indefinitely expanded in feeder-free condition. Expanded human SOX9 BCs can give rise to alveolar and bronchiolar epithelium after being transplanted into injured mouse lung, with air-blood exchange system reconstructed and recipient's lung function improved. Manipulation of lung microenvironment with Pirfenidone to suppress TGF-β signaling could further boost the transplantation efficiency. Moreover, we conducted the first autologous SOX9 BCs transplantation clinical trial in two bronchiectasis patients. Lung tissue repair and pulmonary function enhancement was observed in patients 3-12 months after cell transplantation. Altogether our current work indicated that functional adult human lung structure can be reconstituted by orthotopic transplantation of tissue-specific stem/progenitor cells, which could be translated into a mature regenerative therapeutic strategy in near future.
