1.Investigation and Ethical Consideration of Perioperative Complications in Patients with Liver Transplantation

To explore the prevention and treatment of perioperative complications of adult liver transplantation patients from the perspective of ethics, and carry out ethical thinking in order to provide theoretical support. Through a cross-sectional study, 189 patients selected by strict admission criteria who received liver transplantation in the department of hepatobiliary surgery of the First Affiliated Hospital of Xi’an Jiaotong University from January 2018 to May 2019, to explore the incidence and ethical problems of perioperative complications in adult liver transplantation. The results showed that 87 patients had complications among 189 patients, the incidence was 46.03%. Among them, 28 patients with pleural effusion, the incidence was 14.81%; 15 patients with biliary complications, the incidence was 7.94%; 14 patients with diabetes mellitus, the incidence was 7.41%. The incidence of complications after liver transplantation is high, mainly including pleural effusion, biliary complications and diabetes mellitus. Thus, the prevention and intervention from the perspective of nursing ethics is worth exploring.
2.Preventive and therapeutic effects and mechanisms of Caryopteris incana in a rat model of primary dysmenorrhea with cold coagulation and blood stasis syndrome

Objective To explore the preventive and therapeutic effects of Caryopteris incana decoction in a rat model of primary dysmenorrhea with cold coagulation and blood stasis syndrome.Methods Sixty healthy specific pathogen-free SD female rats were randomly divided into six groups of ten rats each:normal group,model group,ibuprofen group,C.incana high-dose group,C.incana medium-dose group,and C.incana low-dose group.All groups except the normal group were treated with cold stimulation combined with estradiol benzoate and oxytocin to establish a rat model of primary dysmenorrhea with cold coagulation and blood stasis syndrome.On the fifth day of modeling,the rats were intragastrically administered the study drugs for 10 days.Their symptoms were observed and recorded.The writhing response and hemorheological indices were measured.The serum levels of TXB2,6-keto-prostaglandin F1α(6-Keto-PGF1α),estradiol(E2),and progesterone(PROG)were measured.Additionally,the levels of prostaglandin F2α(PGF2α),prostaglandin E2(PGE2),nitric oxide(NO),and calcium(Ca2+)in the uterine tissues were measured.The organ indices of the uterus and ovary were calculated,and histopathological changes were observed.Results Compared with the normal group,the rats in the model group showed obvious symptoms of cold coagulation and blood stasis syndrome and writhing reaction.The morphology of uterus and ovary showed obvious hyperplasia,inflammation,edema and other lesions.The plasma viscosity,packed cell volume and whole blood viscosity were significantly increased(P<0.01).The serum levels of thromboxane B2 and E2 and the E2/PROG ratio were significantly increased(P<0.01),the levels of 6-Keto-PGF1α and PROG were significantly decreased(P<0.01).The uterine index and ovarian index were significantly increased(P<0.01).The levels of PGF2α and Ca2+and the PGF2α/PGE2 ratio in uterine tissue were significantly increased(P<0.01),while the levels of PGE2 and NO were significantly decreased(P<0.01).Compared with the model group,Caryopteris incana significantly improved the symptoms of model rats,improved the morphological lesions of the uterus and ovary,prolonged the latency time of the writhing reaction,and reduced the number of writhing episodes(P<0.01);significantly reduced the plasma viscosity,packed cell volume,and whole blood viscosity(P<0.01);significantly reduced the serum levels of TXB2 and E2 and the E2/PROG ratio,increased the serum levels of 6-Keto-PGF1αand PROG,and reduced the uterine and ovarian indices(P<0.01,P<0.05);significantly reduced the levels of PGF2αand Ca2+and the PGF2α/PGE2 ratio in uterine tissue(P<0.01,P<0.05);and significantly increased the levels of PGE2 and NO in the uterine tissue(P<0.01).Conclusions Caryopteris incana decoction can effectively improve the clinical symptoms of primary dysmenorrhea in rats with cold coagulation and blood stasis syndrome,and it has a good control effect.Its mechanism may be correlated with the levels of TXB2,6-Keto-PGF1α,E2,and PROG in serum and PGF2α,PGE2,NO,and Ca2+in uterine tissue.

Diabetic osteoporosis(DOP)is a serious complication of diabetes mellitus(DM),with bone loss and microstructure destruction.The onset of DOP is insidious,and early symptoms are not obvious.As the condition progresses,the disability and mortality rates increase in DM patients.The advanced glycation end products receptor(AGEs-RAGE)axis can mediate different signaling pathways involved in regulating osteoblasts.This article reviews the mechanism of AGEs-RAGE axis mediated p38 mitogen activated protein kinase signaling pathway in regulation of osteoblasts in DOP.

Gastroesophageal reflux disease (GERD) is a frequently and commonly occurring disease in clinic. In recent decades, with the development in pathophysiology and drug researches, modern medicine has achieved remarkable progress and results in diagnosis and treatment. However, the treatments for non-erosive reflux disease, refractory gastroesophageal reflux disease, proton pump inhibitor resistance, overlap of disease symptoms, and extraesophageal symptoms are limited and ineffective. Traditional Chinese medicine (TCM) was widely used in clinical practice, which has been proved effective in relieving symptoms and improving the quality of life. Sponsored by China Association of Chinese Medicine (CACM) and undertaken by the Spleen and Stomach Disease Branch of CACM, "the 12th Youth Salon of Clinical Predominance Disease Series (GERD)" invited 18 authoritative digestive experts of TCM and western medicine to discuss "the difficulties of clinical diagnosis and treatment of GERD and TCM advantages". The focus issues such as modern medical diagnosis and treatment achievements and contributions, improvement and maintenance of symptoms, response to overlapping disease symptoms, reduction and withdrawal of acid suppressors, and treatment of extra-esophageal symptoms were discussed in depth. TCM and western medicine exchanged and complemented each other's strengths, combing the difficulties of modern medical diagnosis and treatment, which clarified the positioning and advantages of TCM and provided guidance for clinical and scientific research.

6.The effect of bedside chest radiograph in the diagnosis and follow-up of severe and critical COVID-19

Objective:To explore the value of bedside chest radiograph in the diagnosis and follow-up of severe and critical COVID-19.Methods:Twenty-nine patients with severe or critical COVID-19 were collected from January 23 to February 23, 2020,from four COVID-19 designated hospitals in Guangdong Province. Bedside radiography was taken in all the 29 patients, ranged from 1 to 16 times for each patient. Twenty-seven patients underwent follow-up, and the number of re-examination ranged 1 to 15 times, and the interval of review is 1 to 8 days.The imaging findings of bedside chest radiography and the imaging changes on follow-up chest radiography were analyzed retrospectively.Results:Twenty-nine patients were collected. The radiography showed the lesions involved all more than 3 lung fields. The films showed consolidation shadow in 19 cases, multiple patches of shadow in 23 cases, reticular pattern in 12 cases, strips shadow in 14 cases, interlobar fissure thickening in 18 cases, and "white lung" in 4 cases.The complications included pleural effusion in 4 cases, pneumothorax in 2 cases, mediastinal and subcutaneous emphysema in 1 case. The radiography showed the lesions progressed in 15 cases, with expanded involvement of the lung.The increase of lesion density was found in 6 cases, new lesions were noted in 5 cases, while both of them were found in 4 cases. Nine cases showed improvement, with reduced range and decreased density. Patchy or consolidation shadow turned to strips shadow or articular pattern shadow in 8 cases.There was no significant change in 3 cases with large consolidation shadow.Conclusions:Bedside chest radiography has a good value in the follow-up of severely and critically ill patients with COVID-19, and can provide great help for clinicians to evaluate their condition.
7.Thyroid hormones affect alveolar bone density

BACKGROUND:Overproduction of thyroid hormones is shown to increase the activities of osteoblasts and osteoclasts, stimulating bone resorption and transformation. Inadequate compensation of increased bone resorption by bone transformation results in an increased loss of bone mass.
OBJECTIVE:To investigate the effects of hyperthyroidism on the density of the alveolar bone.
METHODS:Twenty-four New Zealand rabbits were equal y randomized into hyperthyroidism group and control group. Rabbits in the hyperthyroidism group or control group were daily injected intraperitoneal y with 50μg/kg levothyrocine diluted in physiological saline solution or equal volume of physiological saline. At 8 weeks after treatment, serum levels of thyroid hormones (FT3 and FT4), alkaline phosphatase, magnesium and calcium, phosphorus were determined;meanwhile, the bone densities of the lumbar vertebra, mandible, bilateral distal femur were determined by dual energy X-ray absorptiometry and the correlation analysis was performed.
RESULTS AND CONCLUSION:At 8 weeks after treatment, serum levels of FT3, FT4, alkaline phosphatase calcium, phosphorus, and magnesium in the hyperthyroidism group were significantly increased (P<0.01), while the bone densities of the lumbar vertebra, mandible, bilateral distal femur were significantly decreased (P<0.05), compared with the control group. Bone density of the mandible was positively correlated with the bone density the lumbar vertebra and bilateral distal femur. These findings suggest that the changes in FT3 and FT4 are sufficient for the diagnosis of hyperthyroidism. Hyperthyroidism results in the decreased density of the alveolar bone, indicating the occurrence of osteoporosis.
8.Effect of quality control circle on reducing incidence of fecal incontinence dermatitis of intensive care unit patients

Objective To investigate the effect of quality control circle on reducing incidence of fecal incontinence dermatitis of intensive care unit (ICU) patients. Methods Quality control circle team was set up and theme was determined then retrospective analysis was carried out for the occurrence of defect in patients with fecal incontinence dermatitis from January to May 2014 before quality control circle activities. Occurrence reasons were explored and corrective measures were formulated and implemented.After the implementation of quality control circle,the occurrence of defect from June to October 2014 was observed. Results After the implementation of quality control circle,the occurrence of fecal incontinence dermatitis reduced from 24.5%(24/98) to 10.4% (10/96) and the difference was statistically significant,χ2=6.65, P<0.05. Conclusion Quality control circle is effective for reducing the occurrence of ICU fecal incontinence dermatitis and continual improvement of nursing quality.
9.Effect of health belief model on compliance behavior of PICC patients after discharge

Objective To explore the effect of health belief model on compliance behavior of patients with PICC(peripherally inserted central catheter).Methods According to admission order,75 cases of PICC were divided into control group(n=36)and observation group(n=39).Patients in the control group received conventional health education and regular visits after discharge and besides the regular methods,patients in the observation group were intervened by health belief model for the compliance behavior.All patients of two groups were investigated with self-designed scale for compliance behavior.Result The compliance behavior of the observation group was better than that of control group with significant difference(P<0.05).Conclusion The health belief model of education can improve the compliance behavior of PICC patients after discharge from hospital.
10.Clinical observation on fatty liver treated by wild apricot in Xinjiang Uighur nationality

Objective:To observe the clinic effect of wild apricot in Xinjiang Uighur nationality on fatty liver.Methods:With a western medicine group as the control group,the treatment courses of both two groups were 12 weeks.The trial group was given wild apricot decoction orally,the control group was given vitamin B,C and Glucurolactone tables.Results:Wild apricot exerted a significant effect in lowering the fat and treating the fatty liver without any toxic and side-effects.Conclusion:Wild apricot in Xinjiang Uighur nationality served as a vital treatment to fatty livers,which was worth being further promoted in the clinical therapy.
