OBJECTIVE To explore the construction of a pediatric pharmaceutical care cloud platform based on Western Pediatric Development Union, to provide a reference for narrowing the difference of pediatric pharmaceutical care in western regions and medical institutions. METHODS Based on the Western Pediatric Development Union, the “1+3+3” pediatric pharmaceutical care cloud platform had been built by relying on the alliance telemedicine service network. That was, with Children’s Hospital of Chongqing Medical University as one center, three core pharmaceutical services, including prescription dispensing, pharmaceutical guidance and health education, were carried out in the union through standardized 3 aspects of management of resource information, service process and component interface. RESULTS & CONCLUSIONS Established “1+3+ 3” pediatric pharmaceutical care cloud platform based on the remote service network of the Western Pediatric Development Union registered 1 208 thousands registrations since its operation, with 112 thousands online prescriptions and 44 thousands circulation prescriptions completed; the total number of medication consultation cases was 10 694, and the number of online training people exceeded 15 thousands. However, during the operation, there are also problems such as limited clinical medication data and insufficient coverage of the region. With the accumulation of clinical medication data on the platform, the artificial intelligence technology will be used to extract the data of prescriptions, medication behavior, and physical indicators after medication, the correlation analysis of data will be conducted under the conditions of different geographical environments, different age groups, different heights and weights in the region to obtain clinical medication characteristics for children in the region, providing decision support for further guiding rational and safe medication in pediatric clinical practice.

BACKGROUND:Current studies have confirmed that Ganoderma lucidum polysaccharides can promote nerve regeneration in neurodegeneration-related diseases. The occurrence of neurodegenerative diseases is closely related to mitochondrial dysfunction, but the role of Ganoderma lucidum polysaccharides on the regulation of apoptosis and mitochondrial function in neurodegenerative diseases is not yet clarified. OBJECTIVE:To explore the regulatory effects and mechanisms of Ganoderma lucidum polysaccharides on apoptosis and mitochondrial dysfunction in H2O2-induced SH-SY5Y cells. METHODS:SH-SY5Y cells were divided into three groups:control group,H2O2 group, and Ganoderma lucidum polysaccharides group.Cells in the control group were normally cultured.Cells in the H2O2 group were treated with 300 μmol/L H2O2 for 24 hours. In the Ganoderma lucidum polysaccharides group, the intervention with 300 μg/L Ganoderma lucidum polysaccharides was conducted first for 1-2 hours, followed by the addition of 300 μmol/L H2O2 for 24 hours. The mitochondrial membrane potential was detected by JC-1 kit.Apoptosis was detected by TUNEL staining kit.The activities of malondialdehyde and superoxide dismutase were detected by malondialdehyde test kit and superoxide dismutase test kit, respectively. The apoptosis and expression of mitochondrial dynamics-related proteins were detected by immunofluorescence staining and western blot assay. RESULTS AND CONCLUSION:(1)Compared with the control group, the mitochondrial membrane potential and superoxide dismutase activity were significantly reduced,as well as apoptotic rate and malondialdehyde levels were significantly increased in the H2O2 group(P<0.05).After treatment with Ganoderma lucidum polysaccharides, the membrane potential and superoxide dismutase activities were significantly increased, and apoptotic rate and malondialdehyde levels were significantly reduced compared with the H2O2 group(P<0.05).(2)The expression levels of pro-apoptotic proteins Bax and Caspase-3 were significantly increased, but the expression of anti-apoptotic protein Bcl-2 was significantly decreased in the H2O2 group compared with the control group(P<0.05).Compared with the H2O2 group, the levels of Bax and Caspase-3 were significantly decreased, but the expression of anti-apoptotic protein Bcl-2 was significantly increased in the Ganoderma lucidum polysaccharides group(P<0.05).(3)Compared with the control group, the expression of mitochondrial splitting proteins Fis1 and p-Drp1 was significantly increased, but the expression of mitochondrial fusion proteins OPA1,Mfn1,and Mfn2 was decreased in the H2O2 group(P<0.05).Compared with the H2O2 group,Fis1 and p-Drp1 expression was significantly reduced, but the expression levels of OPA1,Mfn1,and Mfn2 were significantly increased in the Ganoderma lucidum polysaccharides group(P<0.05).(4)The above results confirm that Ganoderma lucidum polysaccharides can attenuate H2O2-induced oxidative stress damage and apoptosis in SH-SY5Y cells by ameliorating mitochondrial dysfunction.

Objective:To explore the effect of group education intervention strategy based on harmonious nursing theory on understanding and application of pelvic floor knowledge and exercise skills in first gestation women, so as to promote the early recovery of pelvic floor function in postpartum women. Methods:This study was a quasi-experimental study. A total of 80 first gestation women in the pregnant women′s school of the First Hospital of Shanxi Medical University were selected as the research objects, 40 women from April to May 2023 were involved in the control group and 40 women from June to July 2023 were involved in the intervention group. The control group was given routine health education, while the intervention group was given group educational nursing intervention based on harmonious nursing theory. The Primary Prevention of Female Pelvic Floor Dysfunction-self-care Ability Scale was used to evaluate and compare the primary prevention self-care ability of pelvic floor dysfunction among pregnancy 1-4 weeks between the two groups before and after pregnancy intervention. Pelvic floor muscle strength was measured and pelvic floor function exercise compliance was evaluated during outpatient follow-up at 42-49 days postpartum (hereinafter referred to as postpartum 42 days) to evaluate and compare the rehabilitation effect of pelvic floor skills exercise between the two groups at 42 days postpartum. Results:Finally, the control group was (29.97 ± 3.68) years old involved 38 cases, and the intervention group was (29.79 ± 3.66) years old involved 38 cases. There was no significant difference in self-care ability score of primary prevention of pelvic floor dysfunction between the two groups before intervention ( P>0.05). After pregnancy intervention, the scores of self-care knowledge, self-care responsibility, self-care skills, self-regulation and total score of the intervention group were (58.13 ± 6.46), (11.73 ± 1.18), (10.16 ± 0.86), (29.21 ± 2.74) and (109.21 ± 8.92) points, respectively, which were higher than the control group (47.76 ± 4.25), (11.05 ± 1.09), (8.82 ± 1.78), (27.34 ± 2.18), (94.97 ± 6.46) points, and the differences between the two groups were statistically significant ( t values were - 8.26 - - 2.63, all P<0.05). In the intervention group, the maximum value of the fast muscle stage and the mean value of the slow muscle stage of the comparison of pelvic floor muscle strength between the two groups 42 days after delivery were 48.25(44.75, 53.45)μV and 31.00 (27.33, 34.23) μV, respectively, which were higher than the control group 36.80 (32.25, 44.28) μV and 22.40 (19.28, 25.88) μV, the differences between the two groups were statistically significant ( Z = - 4.98, - 5.37, both P<0.01). There was no significant difference in the average value of the pre-resting stage and the average value of the post-resting stage between the two groups ( P>0.05). Comparison of pelvic floor functional exercise compliance between the two groups showed that in the intervention group, the proportion of good compliance was 73.7% (28/38), and the proportion of general compliance was 26.3% (10/38), while in the control group, the proportion of general compliance was 86.8% (33/38) and the proportion of good compliance was 7.9% (3/38). The difference between the two groups was statistically significant ( χ2 = 34.46, P<0.01). Conclusions:Application of the theory of harmonious nursing group educational nursing intervention was carried out on the first gestation women, can effectively improve the first gestation women during pregnancy pelvic floor functional exercise primary prevention self-care ability level, strengthen puerperal women pelvic floor muscle, improve the puerperal pelvic floor functional exercise adherence, coordination and optimization of the first gestation women early for pelvic floor functional rehabilitation nursing process.

Objective:To investigate the safety and feasibility of the CHESS endoscpic ruler (CHESS ruler), and the consistency between the measured values and the interpretation values by endoscopic physician experience. Methods:From January 2021 to January 2022, a total of 105 liver cirrhosis patients with portal hypertension were prospectively enrolled from General Hospital, Xixia Branch Hospital, Ningnan Hospital of People′s Hospital of Ningxia Hui Autonomous Region (29 cases), and the First People′s Hospital of Yinchuan (25 cases), General Hospital of Ningxia Medical University (18 cases), Wuzhong People′s Hospital (10 cases), the Fifth People′s Hospital of Ningxia Hui Autonomous Region (10 cases), Shizuishan Second People′s Hospital (6 cases), Yinchuan Second People′s Hospital (5 cases), and Zhongwei People′s Hospital (2 cases) 8 hospitals. The clinical characteristics of all the patients, including gender, age, nationality, etiolog of liver cirrhosis, and Child-Pugh classification of liver function were recorded. A big gastroesophageal varices was defined as diameter of varices ≥5 mm. Endoscopist (associated chief physician) performed gastroscopy according to the routine gastroscopy procedures, and the diameter of the biggest esophageal varices was measured by experience and images were collected, and then objective measurement was with the CHESS ruler and images were collected. The diameter of esophageal varices of 10 randomly selected patients (random number table method) was determined by 6 endoscopists (attending physician or associated chief physician) with experience or measured by CHESS ruler. Kappa test was used to test the consistency in the diameter of esophageal varices between measured values by CHESS ruler and the interpretation values by endoscopic physician experience. Results:Among 105 liver cirrhosis patients with portal hypertension, male 65 cases and female 40 cases, aged (54.8±12.2) years old, Han nationality 82 cases, Hui nationality 21 cases and Mongolian nationality 2 cases. The etiology of liver cirrhosis included chronic hepatitis B (79 cases), alcoholic liver disease (7 cases), autoimmune hepatitis (7 cases), chronic hepatitis C (2 cases), and other etiology (10 cases). Liver function of 32 cases was Child-Pugh A, Child-Pugh B 57 cases, and Child-Pugh C 16 cases. All 105 liver cirrhosis patients with cirrhotic portal hypertension were successfully measured the diameter of gastroesophageal varices by CHESS ruler, and the success rate of application of CHESS ruler was 100.0% (105/105). The procedure time from the CHESS ruler into the body to the exit of the body after measurement was (3.50±2.55) min. No complications happened in all the patients during measurement. Among 105 liver cirrhosis patients with cirrhotic portal hypertension, 96 cases (91.4%) were recognized as big gastroesophageal varices by the endoscopists. Totally 93 cases (88.6%) were considered as big gastroesophageal varices by CHESS ruler. Eight cases were recognized as big gastroesophageal varices by the endoscopist, however not by the CHESS ruler; 5 cases were recognized as big gastroesophageal varices by the CHESS ruler, but not by the endoscopists; 4 cases were not recognized as big gastroesophageal varices both by the endoscopists and CHESS ruler; 88 cases were recognized as big gastroesophageal varices both by the endoscopists and CHESS ruler. The missed diagnostic rate of big gastroesophageal varices by the endoscopists experience was 5.4% (5/93), and the Kappa value of consistency coefficient between the measurement by the CHESS ruler and the interpretation by endoscopists experience was 0.31 (95% confidence interval 0.03 to 0.60). The overall Kappa value of consistency coefficient by 6 endoscopists measured by CHESS ruler in big gastroesophageal varices diagnosis was 0.77 (95% confidence interval 0.61 to 0.93).Conclusion:As an objective measurement tool, CHESS ruler can make up for the deficiency of subjective judgment by endoscopists, accurately measure the diameter of gastroesophageal varices, and is highly feasible and safe.

Objective:To explore the effect of aromatherapy in the management of symptom clusters of oral cancer patients at different time points during perioperative period. Methods:In the first phase of the study, 132 patients with oral cancer who were admitted to the West China Hospital of Stomatology of Sichuan University from September to December 2021 were selected as the subjects by the convenient sampling, and the symptom cluster was extracted by exploratory factor analysis. In the second phase of the study, 45 patients with oral cancer admitted to the West China Hospital of Stomatology of Sichuan University from January to March 2022 were selected as the observation group by convenient sampling. According to the matching principle of nested case-control study, 132 patients with oral cancer in the first phase of the study were matched as the control group with age, sex and other information as the matching conditions. The control group was treated with routine nursing, while the observation group was treated with aromatherapy on the basis of the control group. The severity of symptom cluster at admission, one day before operation, one day after operation, three days after operation, 7 days after operation and 30 days after operation were compared between the two groups. Results:There were 7 symptom clusters in patients with oral cancer during the perioperative period, which were rest activity, gastrointestinal activity, mental psychology, oral motor function, oral secretory function, respiratory function and neurosensory function. The mixed effect model analysis showed that there were inter-group, time and interaction effects between the two groups of patients with oral cancer in terms of the severity of gastrointestinal activity, mental psychology, and oral motor function, and the differences were statistical differences ( P<0.05). There were inter-group effects and time effects in the severity of rest activity and respiratory function, and the differences were statistically significant ( P<0.05). There were time effects in the severity of oral secretory function and neurosensory function with statistically significant differences ( P<0.05), but there were no statistically significant differences in the inter-group effect ( P>0.05) . Conclusions:Aromatherapy can improve the rest activity, mental psychology and oral motor function of patients with oral cancer. Nurses can use aromatherapy to actively intervene the symptom clusters of oral cancer patients during the perioperative period.
