1.Effects of celastrol on drug resistance of liver cancer cells through FAK/MEK/ERK signaling pathway

OBJECTIVE To investigate the effects of celastrol (CSL) on drug resistance of liver cancer cells. METHODS Human liver cancer lenvatinib (Len)-resistant cells Huh7/Len were constructed and divided into control group, CSL low-, medium- and high-concentration groups (1, 2.5, 5 μmol/L), and CSL high-concentration+Zn27 [focal adhesion kinase (FAK) inhibitor] group (5 μmol/L CSL+2 nmol/L Zn27), with 6 holes in each group. The proliferation (by absorbance) and cloning ability, apoptotic rate, the number of invasion cells and migration cells, the level of reactive oxygen species(ROS) as well as the protein expressions of phosphorylated FAK (p-FAK), phosphorylated mitogen-activated protein kinase kinase (p-MEK), phosphorylated extracellular signal-regulated kinase (p-ERK), B-cell lymphoma-2 (Bcl-2), Bcl-2 associated X protein (Bax) and caspase-3 were detected. RESULTS Compared with control group, cell absorbance, clone count, invasion count and migration count , and the protein expressions of p-FAK, p-MEK, p-ERK and Bcl-2 were significantly reduced in the CSL low- , medium- , high- concentration groups; the apoptosis rate, ROS level, and protein expressions of Bax and caspase-3 were significantly increased, in a concentration-dependent manner (P<0.05). Compared with CSL high-concentration group, the changes of above indexes were all reversed significantly in CSL high-concentration+Zn27 group (P<0.05). CONCLUSIONS CSL can enhance oxidative stress, promote cell apoptosis, inhibit malignant progression and chemotherapy resistance of liver cancer cells, and its mechanism may be related to the inhibition of the FAK/MEK/ERK signaling pathway.
2.Application value of the "liftoff" modular method in robot-assisted laparoscopic surgery for complex adrenal tumors

Objective:This study aims to explore the application value of the "liftoff" modular method in robot-assisted laparoscopic surgery for complex adrenal tumors.Methods:We retrospectively analyzed the clinical data of 15 patients with complex adrenal tumors treated at the General Hospital of Southern Theater Command from May 2022 to June 2023. The cohort comprised 5 males and 10 females with an average age of (47.6±7.8) years and a body mass index (BMI) of 26.5 (23.8-27.9) kg/m 2. Among the patients, 3 had a BMI ≥28 kg/m 2, 2 had diabetes, 6 had hypertension, and 1 had coronary heart disease. Preoperative endocrine hormone examination revealed abnormal blood catecholamines in 5 cases and abnormal blood cortisol in 2 cases. Ultrasound and CT scans indicated that 9 tumors were located on the left side and 6 on the right, with 4 cases showing tumor compression on adjacent large blood vessels or organs. The average tumor diameter was (7.61±2.79) cm, with 10 cases having a diameter ≥ 6 cm. All patients underwent laparoscopic adrenalectomy assisted by robots through the transperitoneal approach. The surgeries were performed in a lateral position under general anesthesia. The "liftoff" modular method was utilized to separate the treatment of adrenal tumors into lateral, medial, dorsal, cephalic, and adrenal renal plane sides. Tumors were appropriately manipulated during the operations to achieve a "liftoff" shape. Different modular dissociation steps were adopted based on the size and location of the left and right adrenal tumors. The left adrenal gland was dissected in the order of medial and dorsal, adrenal renal plane side, and lateral and cephalic sides, while the right adrenal gland was dissected in the order of lateral and dorsal, adrenal renal plane side, and medial and cephalic sides. Postoperative related indicators and follow-up status of patients were recorded and analyzed. Results:All 15 surgeries were successfully completed without any conversions to open adrenalectomy, with an average operation time of 118 (102-130) minutes and an average intraoperative blood loss of 102 (69-163) ml. The postoperative drainage time was 4 (3-5) days, and the postoperative hospital stay was 6 (4-7) days. The postoperative pathological diagnoses included 5 cases of pheochromocytoma, 3 cases of macronodular adrenal hyperplasia, 6 cases of adrenocortical adenoma, and 1 case of myelolipoma. Follow-up for 6-12 months after surgery showed good recovery and no recurrence.Conclusions:The application of the "liftoff" modular method in robot-assisted laparoscopic surgery for complex adrenal tumors is safe and feasible. It efficiently aids in tumor removal and holds significant clinical application value.
3.Current status of the radiodiagnostic and radiotheraputic resources and the frequency of medical radiation exposure in Wuxi city in 2020

Objective:To investigate the current status of radiodiagnostic and radiotheraputic resources and the frequency of medical radiation exposure in order to provide basic data for reasonably allocating medical resource and standardizing meical exposure.Methods:According to data available from the Jiangsu Radiation Health Information Management Platform in 2020 and the total number of permanent residents from Wuxi Statistical Yearbook 2019, a statistical analysis was carried out of the staff and equipment and the frequency of medical radiation exposure in Wuxi mecical institutions.Results:There were 923 items of radiodiagnostic and radiotheraputic equipment, 2 480 medical radiation workers in Wuxi city in 2020, which were estimated to be 140.03 items of equipment and 376.24 workers per 10 6 population. The frequency of X-ray diagnosis and treatment were 1 137.53 procedures per 10 3 population, including 646.17 per 10 3 population for conventional X-ray diagnosis, 394.11 per 10 3 population for CT scan, 5.34 per 10 3 population for interventional radiology, 0.79 per 10 3 population for radiotherapy and 7.12 per 10 3 population for nuclear medicine. Interventional radiology, radiotherapy and nuclear medicine was mainly concentrated in tertiary hospitals. Conclusions:The number of radiodiagnostic and radiotheraputic resources in Wuxi is above the national and provincial level, although in a unbalanced state. Conventional X-ray diagnosis frequency in Wuxi is higher than that of provincial level, resultsing in higher collective dose. Radiological diagnosis and treatment activities needs a approach to the process of justification.
4.Preliminary results of low-dose computed tomography screening for lung cancer in asymptomatic participants.

OBJECTIVES:
Low dose computed tomography (LDCT) is the best method for early diagnosis of lung cancer. Even though it has been widely used in clinic, the selection of screening objects and the management scheme of pulmonary nodules are still not unified among research institutions. This study aims to evaluate the effect of LDCT in detection effect and follow-up process for pulmonary nodules in asymptomatic participants.
METHODS:
A total of 1 600 asymptomatic participants (37 to 82 years old), who came from Yantian District People's Hospital, Southern University of Science and Technology, received LDCT. The lung nodules were categorized into positive nodules and semi-positive nodules, and according to the density of positive nodules they were categorized into 4 types: solid nodules (SN), partial solid nodules (pSN), pure ground glass nodules (pGGN), and pleural nodules (PN). The number, detection rate, imaging findings, follow-up change of lung nodules, and the postoperative pathological results of positive nodules were recorded and analyzed.
RESULTS:
Lung nodules were found in 221 cases by LDCT. The total detection rate of lung nodule was 13.8% (221/1 600), and the detection rate in positive nodules was 4.9% (79/1 600). The detected nodules were mainly single (173 cases), solid (133 cases) and semi-positive nodules (142 cases). Most of nodules (177 cases) had no change in the follow-up process. The enlargement and/or increased density of nodules (5 cases) were lung cancer. Pathological results were obtained in 10 cases, 8 cases were malignant (1 small cell lung cancer and 7 adenocarcinomas), 2 cases were benign (cryptococcal infection and alveolar epithelial dysplasia). The detection rate of lung cancer was 0.5% (8/1 600), and the proportion of early lung cancer was 75% (6/8).
CONCLUSIONS
LDCT screening can identify and increase the detection rate in the early lung cancer, which is an effective screening method. It is safe and feasible to take regular follow-up and re-examination for nodules with diameter less than 5 mm. When the size and or density of nodule increases, it indicates the malignant prognosis of the nodule and timely clinical intervention is needed.

5.Diagnostic value of 18F-fluorodeoxyglucose PET/MRI multimodal imaging in grading of adult diffuse gliomas

Objective:To investigate the diagnostic value of integrated 18F-fluorodeoxyglucose (FDG) PET/MRI multimodal imaging in the grading of adult diffuse gliomas. Methods:A retrospective analysis was performed; 29 patients accepted cranial 18F-FDG PET/MRI multimodal imaging and diagnosed as having adult diffuse gliomas in our center from January 2020 to December 2021 were chosen; their clinical and image data were collected. These patients were divided into low-grade group (WHO grading II, n=13) and high-grade group (WHO grading III and IV, n=16) according to the pathological results confirmed by surgery or biopsy. T1WI, T2WI, DWI, arterial spin labeling (ASL), magnetic resonance spectroscopy (MRS) and PET were performed in these patients. Differences in T1WI and T2WI signs between the two groups were compared. Differences in maximum standard uptake value (SUV max), apparent diffusion coefficient (ADC), choline/ N-acetylaspartate (Cho/NAA), NAA/creatine (Cr), Cho/Cr, relative cerebral blood flow (rCBF) between the two groups were compared. The diagnostic efficacies of above parameters in differentiating high-grade from low-grade adult diffuse gliomas were determined by receiver-operating characteristic (ROC) curves. The predictive probabilities for differentiating high-grade from low-grade adult diffuse gliomas using combined parameters were obtained by binary Logistic regression analysis. Results:There were no statistical differences in T1WI and T2WI signs, proportion of adjacent parenchyma edema, proportion of crossing the midline into the contralateral side, proportion of mass effect, proportion of cystic degeneration/necrosis, or site of growth between low-grade and high-grade groups ( P>0.05). The values of SUV max, ADC, Cho/NAA, Cho/Cr and rCBF were statistically different between the two groups, and the order of diagnostic efficiency was SUV max>ADC>Cho/NAA>rCBF>Cho/Cr>NAA/Cr. SUV max had the highest diagnostic efficacy in differentiating high-grade from low-grade adult diffuse gliomas with an area under the curve (AUC) of 0.894, a sensitivity of 75% and a specificity of 100%; the diagnostic efficacy of combined parameters (mean AUC>0.9) was higher than that of single parameter, and the combined parameters of SUV max and Cho/NAA showed the highest diagnostic efficacy with an AUC of 0.933, a sensitivity of 81.3% and a specificity of 92.3%. Conclusion:The 18F-FDG PET/MRI multimodal imaging is helpful in the grading diagnosis of adult diffuse gliomas, and combination of PET and MRS may play a greater role.
6.The 497th case: fever, anterior hypopituitarism, lymphadenopathy

A 57-year-old woman was admitted to hospital with fever. She still had fever treated with multiple antibiotics, and no definite evidence for infection was found. Hypothermia and hypotension developed, and magnetic resonance imaging (MRI) examination showed enlarged anterior pituitary and multiple small nodular lesions with mild enhancement on the left side. Hormone replacement and anti-infection treatment were administrated, but fever did not improve. Remarkable lymphadenopathy was found in left supraclavicular area. The pathology of lymph node biopsy indicated peripheral T-cell lymphoma (not otherwise specified, NOS). Positron emission tomography-computed tomography (PET-CT) revealed hypermetabolism in multiple lymph nodes, infiltration of the liver and spleen. The final diagnosis were peripheral T-cell lymphoma with involvement of liver and spleen (stage Ⅳ) and anterior hypopituitarism. After chemotherapy, fever alleviated and the function of anterior pituitary recovered gradually.

Diabetes have been shown to cause progressive neuronal injury with pain and numbness via advanced glycation end-products (AGEs)-induced neuronal cell apoptosis; however, the valuable drug targets for diabetic neuropathy have been poorly reported so far. In this study, we discovered a natural small-molecule schisandrol A (SolA) with significant protective effect against AGEs-induced neuronal cell apoptosis. ATP6V0D1, a major subunit of vacuolar-type ATPase (V-ATPase) in lysosome was identified as a crucial cellular target of SolA. Moreover, SolA allosterically mediated ATP6V0D1 conformation via targeting a unique cysteine 335 residue to activate V-ATPase-dependent lysosomal acidification. Interestingly, SolA-induced lysosome pH downregulation resulted in a mitochondrial-lysosomal crosstalk by selectively promoting mitochondrial BH3-only protein BIM degradation, thereby preserving mitochondrial homeostasis and neuronal cells survival. Collectively, our findings reveal ATP6V0D1 is a valuable pharmacological target for diabetes-associated neuronal injury via controlling lysosomal acidification, and also provide the first small-molecule template allosterically activating V-ATPase for preventing diabetic neuropathy.
8.Gross α and gross β radioactivity of environmental samples in Wuxi, China, 2014—2019

Objective To study gross α and gross β radioactivity in environmental samples in Wuxi, China, and perfect the radioactivity data, and to discover possible radioactive contamination in time. Methods The BH1227 low-background α and β measuring instrument was used to determine gross α and gross β radioactivity in sedimentary dust, aerosol, drinking water, and food samples in Wuxi. Results Among environmental samples collected in Wuxi, 2014—2019, the mean gross α and gross β radioactivity levels were 0.01~5.98 Bq/kg and 9.47~547.86 Bq/kg in food samples, respectively, 1.2~22 mBq/L and 0.1~141 mBq/L in water samples, respectively, 39~44790 mBq/m3 and 743.1~101500 mBq/m3 in sedimentary dust samples, respectively, and 50~1125 μBq/m3 and 38~9510 μBq/m3 in aerosol samples, respectively. Conclusion We should strengthen radioactivity monitoring of environmental samples in Wuxi through increasing the scope and frequency of monitoring, to perfect baseline radioactivity data of Wuxi.

The expert consensus reviews current literatures and provides clinical practices for thermal ablation of GGN. The main contents include: (1) clinical evaluation of GGN, (2) procedures, indications, contraindications, outcomes evaluation and related complications of thermal ablation for GGN and (3) future development directions.

10.Effects of parathyroidectomy on heart rate circadian rhythm in patients with stage 5 chronic kidney disease combined with severe secondary hyperparathyroidism

Objective:To observe heart rate circadian rhythm in patients with chronic kidney disease (CKD) stage 5 and to analyze the effects of parathyroidectomy (PTX) on heart rate circadian rhythm in severe secondary hyperparathyroidism (SHPT) patients.Methods:A cross-sectional observation was performed in 213 patients with CKD stage 5 and 96 controls, and the patients were divided into those with severe SHPT (PTX group, n=70) and without severe SHPT (non-PTX group, n=143). Forty-six PTX patients were followed up prospectively. The baseline data were compared among these groups. Holter electrocardiogram was performed for each participant. Non-dipping heart rate was defined as night/day heart rate ratio greater than 0.9. Multiple linear regression analysis was used to analyze the related factors of heart rate circadian rhythm in patients with CKD stage 5. Results:The 24-hour, daytime and nighttime mean heart rate in patients with CKD stage 5 were all higher than those in controls, especially in PTX group (all P<0.05). The night/day heart rate ratios of controls and CKD stage 5 patients were (0.81±0.08) and (0.91±0.08) respectively ( P<0.01). Correlation analysis showed 24-hour and daytime or nighttime mean heart rate in patients with CKD stage 5 were positively correlated with serum levels of phosphorus and ln(alkaline phosphatase), while nighttime mean heart rate and night/day heart rate ratio were positively related with serum intact parathyroid hormone level. After adjusting with postoperative follow-up period (median time: 10.9 months), 24-hour and nighttime mean heart rate, and night/day heart rate ratio in PTX patients all decreased significantly (all P<0.01). Conclusions:Heart rate is increased and circadian rhythm is abnormal in patients with CKD stage 5, which are related with mineral and bone disorder. PTX significantly decreases 24-hour and nighttime mean heart rate in severe SHPT patients, and improves the heart rate circadian rhythm.
