1.Monitoring results of drinking water-type endemic fluorosis in Xinjiang Production and Construction Corps in 2022

Objective:To investigate the control situation of drinking water-type endemic fluorosis (hereinafter referred to as drinking water fluorosis) in Xinjiang Production and Construction Corps (hereinafter referred to as the Corps), identify weak links in prevention and control, and provide reference for precise prevention and control of drinking water fluorosis in the Corps.Methods:Monitoring data of drinking water fluorosis areas in the Corps from January to December 2022 were collected from the Corps Center for Disease Control and Prevention, the level of fluoride in drinking water and the prevalence of dental fluorosis in children aged 8 - 12 were analyzed retrospectively.Results:In 2022, the qualified rate of water fluoride in the disease affected TuanChang of the Corps was 100.00% (62/62), and the qualified rate of water fluoride in the disease affected company was 99.44% (705/709). The prevalence of dental fluorosis in children aged 8 - 12 in the disease affected TuanChang was 3.38% (1 412/41 714), and there was no statistically significant difference in the prevalence of dental fluorosis among different age groups (χ 2 = 6.07, P = 0.194). The prevalence of dental fluorosis in children aged 8 - 12 in the TuanChang where the company with water fluoride exceeding standard was located (6.00%, 106/1 768) was higher than that in the TuanChang where the company with qualified water fluoride was located (3.27%, 1 306/39 946, χ 2 = 38.47, P < 0.001). Conclusion:In 2022, the drinking water fluorosis in the Corps has reached the control standard, but attention still should be paid to the water improvement situation and maintenance of water improvement projects in some water fluoride exceeding standard companies, in order to prevent sustained dental fluorosis damage to children in the affected areas, and consolidate the achievements of prevention and control.
2.Efficacy and safety analysis of tislelizumab in the treatment of advanced gastric cancer

Objective:To analyze the efficacy and safety of tislelizumab in the treatment of advanced gastric cancer.Methods:The clinical data of 54 patients with advanced gastric cancer in Baotou Cancer Hospital from July 2022 to December 2023 were retrospectively analyzed. Among them, 27 patients were treated with chemotherapy (control group), and 27 patients were treated with tislelizumab combined with chemotherapy (combination group). The efficacy was evaluated after 2 cycles. The patients were followed up until March 2024, and the survival status was evaluated by progression-free survival (PFS) and duration of remission (DOR). The adverse reactions were recorded. Kaplan-Meier survival curve was drawn, and the compared used log rank test. Binary Logistic regression was used to analyze the independent risk factors of efficacy in patients with advanced gastric cancer.Results:The effective rate and disease control rate in combined group were significantly higher than those in control group: 29.6% (8/27) vs. 7.4% (2/27) and 85.2% (23/27) vs. 59.3% (16/27), and there were statistical differences ( χ2 = 4.42 and 4.52, P<0.05). Kaplan-Meier survival curve analysis result showed that the median PFS and DOR in combination group were significantly longer than those in control group (9.9 months vs. 7.2 months and 8.7 months vs. 6.4 months), and there were statistical differences (log-rank χ2 = 6.58 and 8.47, P<0.05). Among the 54 patients, 10 cases (18.5%) were effective, and 44 cases (81.5%) were ineffective. The efficacy was related to the number of organ metastase, prealbumin and Helicobacter pylori infection rate, and there were statistical differences ( P<0.05). Binary Logistic regression analysis result showed that the number of organ metastase >1, prealbumin <160 mg/L and Helicobacter pylori infection were independent risk factors of efficacy in patients with advanced gastric cancer ( OR = 0.089, 8.418 and 0.153; 95% CI 0.012 to 0.661, 1.255 to 56.449 and 0.025 to 0.944; P<0.05). There was no statistical difference in the incidence of adverse reactions between two groups ( P>0.05). Conclusions:The tislelizumab combined with chemotherapy in patients with advanced gastric cancer can improve the efficacy and prolong the survival without increasing the incidence of adverse reactions.
3.Research on the application of artificial intelligence large models in the field of medical and health care

The medical and health field has abundant multimode data and open and diverse medical health scenarios,which is one of the most promising fields for the application of artificial intelligence large models.Based on basic hardware,equipment,and model frameworks,the artificial intelligence model is pre-trained,fine-tuned,evaluated,optimised,and utilized through medical texts,medical images,or medical multimode datasets to complete model construction.The large medical and health model has been successfully applied in the fields of auxiliary diagnosis and treatment,medical imaging research,health management,biomedical research,drug research,medical examination and education,but it also faces challenges such as data security risks,moral and ethical risks,and technical risks.In order to regulate the development of artificial intelligence large model technology,some countries have formulated relevant laws,regulations,and standard guidelines.Opportunities and challenges coexist in the field of medical and health cooperation with artificial intelligence,and artificial intelligence large model technology still needs to be continuously optimised and developed to promote deeper integration with the medical and health field.
4.PTEN-regulated alternative splicing of FoxM1 affects tumor cell migration

Objective·To study the effect of phosphatase and tensin homolog on chromosome 10(PTEN)on alternative splicing of forkhead box Ml(FoxM1),and its impact on tumor cell migration.Methods·PTEN was knocked down by short hairpin RNA(shRNA)in human embryonic kidney 293T cells,human prostate cancer DU145 cells,human colorectal adenocarcinoma RKO cells,and human colon cancer SW480 and SW620 cells.Specific primers were designed for FoxM1 and its subtypes FoxM1B and FoxM1C,and the mRNA expression levels of FoxM1B and FoxM1C were detected by quantitative real-time PCR(qRT-PCR).FoxM1B and FoxM1C were overexpressed in DU 145 cells,and their effects on tumor cell migration were tested by Transwell assay and wound healing assay.Immunofluorescence and dual luciferase reporter gene assay were used to explore the potential mechanism of differential regulation of tumor cell migration by FoxM1B and FoxM1C.Results·① PTEN was knocked down in 293T,DU 145,RKO,SW480,and SW620 cell lines.qRT-PCR results showed that compared with the control cells,the mRNA expression level of FoxM1B significantly increased in PTEN knockdown cells,while the mRNA expression level of FoxM1C decreased or remained unchanged.Knockdown of PTEN did not affect the transcription level of FoxM1,but caused the variable splicing of FoxM1 and promoted the generation of FoxM1B.② Compared with the control cells,the number of DU 145 cells migrating to the below chamber increased in the FoxM1B overexpression group(P=0.024),while the number of migrating DU 145 cells in the FoxM1C overexpression group was lower(P=0.000).The healing ability of DU 145 cells was significantly enhanced in the FoxM1B overexpression group(P=0.001),while the healing ability of DU145 cells was weakened in the FoxM1C overexpression group(P=0.021).Overall,FoxM1B and FoxM1C had opposite effects on tumor cell migration.FoxM1B promoted tumor cell migration,while FoxM1C inhibited tumor cell migration.③ Neither FoxM1B nor FoxM1C overexpression could induce β-catenin to enter the nucleus.Dual luciferase reporter gene assay showed no difference in the transcriptional activity of FoxM1B and FoxM1C.The difference between FoxM1B and FoxM1C in the regulation of tumor metastasis was also not mediated by β-catenin translocation.Conclusion·Knockdown of PTEN regulates the alternative splicing of FoxM1,leading to increasing expression of transcript FoxM 1B,which plays a positive role in tumor cell migration.

H 1-antihistamines are widely used in the treatment of various allergic diseases, but there are still many challenges in the safe and rational use of H 1-antihistamines in pediatrics, and there is a lack of guidance on the prescription review of H 1-antihistamines for children.In this paper, suggestions are put forward from the indications, dosage, route of administration, pathophysiological characteristics of children with individual difference and drug interactions, so as to provide reference for clinicians and pharmacists.
6.Related factors of perinatal Escherichia coli infection

Escherichia coli is an important pathogen causing neonatal infectious diseases, such as neonatal meningitis and sepsis, and poses a great threat to the health of neonates. Currently, no effective screening and prevention measures are available. This article briefly reviews the rates of Escherichia coli colonization in pregnant women and newborns, the risk factors of colonization and transmission from mother to infant and the influences of Escherichia coli on pregnancy outcome, especially the harm to newborns. In clinical practice, more attention should be paid to high-risk pregnant women and the possible adverse pregnancy outcomes. For newborns born to mothers with Escherichia coli colonization, the possibility of Escherichia coli infection should be considered first if early meningitis and septicemia occur, which would be of great significance for the selection of antibiotics.
7.Comprehensive evaluation of the effectiveness of iodine deficiency disorders prevention and control in Xinjiang Production and Construction Corps from 2014 to 2020 using TOPSIS and RSR methods

Objective:To discuss the comprehensive evaluation of the effectiveness of iodine deficiency disorders prevention and control in Xinjiang Production and Construction Corps (Corps for short) using technique for order preference by similarity to an ideal solution (TOPSIS) and rank-sum ratio (RSR) methods, and identify weaknesses in the prevention and control of iodine deficiency disorders in the Corps at present.Methods:The monitoring data on iodine deficiency disorders in the Corps from 2014 to 2020 were collected from the Disease Control and Prevention Center of the Corps. Six indexes were selected, including the consumption rate of qualified iodized salt, the coverage rate of iodized salt, the qualified rate of iodized salt, the appropriate percentage of urinary iodine in children, the goiter rate of children and the appropriate percentage of urinary iodine in pregnant women. The TOPSIS method and RSR method were combined to comprehensively evaluate the prevention and control effect of iodine deficiency disorders in the Corps.Results:The results of TOPSIS method showed that the control effect was from superior to inferior in the order of 2020, 2018, 2016, 2019, 2015, 2017 and 2014. Based on the RSR method, the control effect was divided into three levels: 2018 and 2020 (with an estimated RSR value of ≥0.525) in the first level (good), 2014 (estimated RSR value < 0.007) in the third level (poor), and other years (0.007≤estimated RSR value < 0.525) in the second level (medium). Referring to the monitoring data, the appropriate percentage of urinary iodine in children and pregnant women in the middle years was the weak point. Conclusions:TOPSIS method combined with RSR method can accurately and reasonably evaluate the effectiveness of prevention and control of iodine deficiency disorders in the Corps, the evaluation results are consistent with the actual situation. It is found that the appropriate iodine nutrition levels of children and pregnant women are the weak points in the prevention and control of iodine deficiency disorders in the Corps.
8.Iodine nutrition level of children aged 8 - 10 years and pregnant women in Southern and Northern Xinjiang of Xinjiang Production and Construction Corps in 2020

Objective:To fully learn about the iodine nutrition level and the status quo of prevention and control of iodine deficiency disorders (IDD) among children aged 8 - 10 years and pregnant women in Southern and Northern Xinjiang of Xinjiang Production and Construction Corps (hereinafter referred to as the Corps), and to provide a basis for consolidating the prevention and control effect of IDD and proposing sustainable prevention and control strategies.Methods:From May to November 2020, IDD monitoring was carried out within the Corps. ⑴Salt iodine monitoring: in 14 divisions of the Corps, 1 regiment was selected in each division from five directions of East, West, South, North and Middle, and 4 companies were selected from each regiment and edible salt samples were collected from the homes of 15 Corps employees to test for salt iodine. ⑵Monitoring of urinary iodine and goiter: in 13 divisions of the Corps, 1 regiment was selected from each division according to five directions of East, West, South, North and Middle, and 40 children aged 8 - 10 years (age balanced, half male and female) from one primary school and 20 pregnant women were selected as the survey subjects. Once random urine sample was collected from children and pregnant women to detect urinary iodine; the thyroid volume of children was measured and the goiter rate was calculated.Results:⑴A total of 4 149 salt samples were tested, the median salt iodine was 27.3 mg/kg, the coverage rate of iodized salt was 99.9% (4 146/4 149), the qualified rate of iodized salt was 98.1% (4 069/4 146), and the consumption rate of qualified iodized salt was 98.1% (4 069/4 149). Among them, 1 197 and 2 952 salt samples were tested in Southern and Northern Xinjiang of the Corps, the median salt iodine was 26.7 and 27.5 mg/kg, respectively; the coverage rate of iodized salt was 99.7% (1 194/1 197) and 100.0% (2 952/2 952), respectively; the qualified rate of iodized salt was 95.0% (1 134/1 194) and 99.4% (2 935/2 952), respectively; the consumption rate of qualified iodized salt was 94.7% (1 134/1 197) and 99.4% (2 935/2 952), respectively; and the differences between regions were statistically significant ( P < 0.05). ⑵A total of 2 602 urine samples of children aged 8 - 10 years were detected, and the median urinary iodine was 186.5 μg/L; the median urinary iodine of children (800 and 1 802) in Southern and Northern Xinjiang of the Corps was 181.9 and 188.5 μg/L, respectively. A total of 1 247 urine samples of pregnant women were detected, and the median urinary iodine was 192.6 μg/L; the median urinary iodine of pregnant women (397 and 850) in Southern and Northern Xinjiang of the Corps was 182.2 and 200.3 μg/L, respectively. ⑶A total of 2 602 children aged 8 - 10 years were examined for thyroid volume, and the goiter rate was 1.2% (31/2 602); the goiter rate of children in Southern and Northern Xinjiang of the Corps was 1.4% (11/800) and 1.1% (20/1 802), respectively. Conclusion:In 2020, the Corps has reached the elimination standard of IDD by divisions, and children aged 8 - 10 years and pregnant women are generally at an appropriate iodine level.
9.Clinical efficacy of the combined diagnosis and management for children with airway allergic diseases

Objective:To evaluate the clinical efficacy of the combined diagnosis and management in children with airway allergic diseases(bronchial asthma, allergic rhinitis).Methods:This observational study belongs to cluster sampling cases, which included the clinical data from children with airway allergic diseases in Allergy Department and Otorhinolaryngology Department of Beijing Children′s Hospital from April to December in 2015. They were followed up every three months during 12 months. All the subjects were required to continuously record daily symptom by diary card. ACT/c-ACT, VAS, treatment steps to control asthma, respiratory infections, wheeze, pulmonary function(FEV1%pred,FEV1/FVC,PEF%pred,FEF25%pred,FEF50%pred,FEF75%pred,MMEF%pred), FeNO were assessed in every visiting. The mean±standard deviation was used for the measurement data in accordance with normal distribution. Comparing the pulmonary function indexes at every point, the measurement data with normal distribution and uniform variance were analyzed by single factor analysis of variance, and the measurement data with uneven variance were tested by non-parametric rank sum test.Results:Among 147 recruited participants, 106 completed the combined diagnosis and management. The airway allergic diseases control rate was 87.7% at 12 months after the combined diagnosis and management. At every point, the average daily symptom score and VAS score which were significantly lower than at the baseline( H=35.854, P=0.000)[ 1.2(0.7,2.2),0.6(0.2,1.5),0.4(0.1,1.0),0.5(0.1,1.1) vs 2.0(1.0,3.5)],( H=39.559, P=0.000)[2.5(0.5,4.7),2.2(0.3,4.4),1.8(0.2,4.6),1.6(0.3,3.8) vs 6.9(4.1,9.8)]. ACT/c-ACT score at 3, 6, 9, 12 months were significantly higher than at the baseline ( H=79.695, P=0.000) [25.0(22.5,27.0),26.0(24.0,27.0),25.0(23.0,27.0),25.0(24.0,27.0) vs 20.0(17.0,22.0)]. FEV1%pred and FEF25%pred at 3, 6 months were significantly higher than at the baseline ( F=3.563, P=0.007)(104.7±12.6 vs 96.8±14.5,103.0±10.3 vs 96.8±14.5),( F=2.456, P=0.046)(96.6±22.0 vs 85.0±21.9,93.3±18.0 vs 85.0±21.9). PEF%pred at 3, 6, 9, 12 months after the combined diagnosis and management were significantly higher than at the baseline( F=5.497, P=0.000)(105.1±18.1,101.2±15.3,99.7±17.1,99.8±17.5 vs 90.3±17.8). FeNO at 3, 6, 9, 12 months respectively were no significantly differences at the baseline( F=0.751, P=0.558)(25.7±23.6 vs 30.7±25.6,25.9±16.5 vs 30.7±25.6,27.5±20.2 vs 30.7±25.6,30.6±19.6 vs 30.7±25.6).The respiratory infections rate were 69.8%(74/106),67.0%(71/106),60.4%(64/106),51.9%(55/106) at 3, 6, 9, 12 months respectively. The wheezing rate was 24.5%(26/106),14.2%(15/106),11.3%(12/106),7.5%(8/106) at 3, 6, 9, 12 months respectively. Conclusions:The combined diagnosis and management can significantly improve the control level of children′s airway allergic diseases, which should be implemented in the management of children′s airway allergic diseases.
