1.Research progress in role of composition and structure of basement membrane in epithelial cell polarization and its mechanism

The basement membrane is a specialized extracellular matrix between the epithelium and the mesenchyme.In stratified epithelium,only the basal cells in contact with the basement membrane exhibit the apical-basal polarity,whereas the epithelial cells do being not in contact with the basement membrane do not exhibit the apical-basal polarity.The basement membrane plays an important role in epithelial cell polarization.It is an important extracellular matrix(ECM)structure in the multicellular organisms,is situated between the epithelium and the mesenchyme,and is produced jointly by the epithelial cells and mesenchymal cells.Its components mainly include Laminin,type Ⅳ collagen(Col-Ⅳ),nidogen(NDG),and heparan sulfate proteoglycans(HSPG),and each component plays the different role in influencing the epithelial cell polarity.The network scaffold formed by Col-Ⅳ and Laminin is the main structure of the basement membrane,and the integrity of the structure affects the epithelial cell polarization.This review summarizes the composition and structure of the basement membrane,focuses on its role in epithelial cell polarization and its mechanism,and compiles the current status of biomimetic basement membrane materials that promotes the epithelial cell polarization,and provides the theoretical foundation for further exploration of the establishment and maintenance of epithelial cell polarity.
2.Research progress on fall risk and prevention in elderly population

Fall is an important cause of serious injury and even death in the elderly. With the increasing proportion of the elderly in China, fall in the elderly has become a major public problem in society. There are many causes of fall in the elderly, and age, gender, chronic diseases, physical function and living environment are closely related to the fall risk in the elderly. Therefore, it is of great significance to understand and analyze the risk factors of fall in the elderly in detail, formulate targeted and rational preventive interventions and reduce the incidence rate of fall in the elderly, so as to relieve the personal and family burdens, save the social resources and enhance the quality of life of the elderly.
3.The Association between GLP-1 Receptor-Based Agonists and the Incidence of Asthma in Patients with Type 2 Diabetes and/or Obesity:A Meta-Analysis

Objective Recent studies have indicated potential anti-inflammatory effects of glucagon-like peptide-1 receptor agonists(GLP-1RAs)on asthma,which is often comorbid with type 2 diabetes mellitus(T2DM)and obesity.Therefore,we conducted a meta-analysis to assess the association between the administration of glucagon-like peptide-1(GLP-1)receptor-based agonists and the incidence of asthma in patients with T2DM and/or obesity. Methods PubMed,Web of Science,Embase,the Cochrane Central Register of Controlled Trials,and Clinicaltrial.gov were systematically searched from inception to July 2023.Randomized controlled trials(RCTs)of GLP-1 receptor-based agonists(GLP-1RA,GLP-1 based dual and triple receptor agonist)with reports of asthma events were included.Outcomes were computed as risk ratios(RR)using a fixed-effects model. Results Overall,39 RCTs with a total of 85,755 participants were included.Compared to non-GLP-1 receptor-based agonist users,a trend of reduced risk of asthma was observed in patients with T2DM or obesity using GLP-1 receptor-based agonist treatments,although the difference was not statistically significant[RR=0.91,95%confidence interval(CI):0.68 to 1.24].Further Subgroup analyses indicated that the use of light-molecular-weight GLP-1RAs might be associated with a reduced the risk of asthma when compared with non-users(RR=0.65,95%CI:0.43 to 0.99,P=0.043).We also performed sensitivity analyses for participant characteristics,study design,drug structure,duration of action,and drug subtypes.However,no significant associations were observed. Conclusion Compared with non-users,a modest reduction in the incidence of asthma was observed in patients with T2DM or obesity using GLP-1 receptor-based agonist treatments.Further investigations are warranted to assess the association between GLP-1 receptor-based agonists and the risk of asthma.
4.Design and application of a multifunctional flushing toothbrush for critically ill patients

Due to diminished self-immunity and oral self-cleaning ability,critically ill patients experience a significant increase in the risk of oral infection.These infections can lead to complications that not only extend hospital stays but also raise mortality.Effective oral care is very important in reducing the occurrence of such infection.Traditional method of using cotton balls in oral care are less effective and can damage the fragile oral mucosa with tweezers.Currently,clinical use of suction toothbrush needs the help of syringe or infusion device.To address these issues,we have designed a multi-functional flushing toothbrush and have obtained the National Utility Model Patent of China(ZL 2021 2 1584512.9).The toothbrush is composed of a brush head,a brush rod,a brush handle,and a water storage mechanism.The side of the toothbrush is provided with a suction tube,a water storage bag and a water guide pipe,and a one-way valve with an open direction to the right is arranged in the guide pipe and the suction pipe,medical staff can control the suction and discharge of liquid by squeezing the water storage bag,thus reducing the use of consumables such as syringes or infusion devices.The device is designed for one-handed operation,making it convenient for medical staff.The brush handle is made of transparent material,allowing medical staff to observe the color and properties of mouthwash and deal with abnormal situations in time,thereby improving safety.The material is mainly made of silicone,natural rubber and plastic,which is green and can be used repeatedly.This device has a certain practical value and is worth popularizing in clinical practice.
5.Predictive value of heart rate variability for long-term prognosis of patients with stable coronary heart disease

Objective To explore the predictive value of heart rate variability(HRV)for long-term prognosis in elderly patients with stable coronary heart disease(CHD)based on dynamic electro-cardiogram(ECG).Methods A retrospective trial was conducted on 402 elderly patients with sta-ble CHD admitted to our hospital from January 2015 to December 2018,and all of them were fol-lowed up for 5 years.According to occurrence of major adverse cardiovascular events(MACE)or not,they were divided into a MACE group(n=102)and a control group(n=300).The main clin-ical characteristics and differences in HRV were compared between the two groups.Multivariate logistic regression analysis and ROC curve analysis were used.Results Advanced age,larger ratio of diabetes,more severe coronary artery stenosis,and higher low-frequency power and ratio of low-frequency power/high-frequency power,but lower left ventricular ejection fraction(LVEF)and high-frequency power were observed in the MACE group than the control group(P<0.01).There was also statistical difference in use of hypoglycemic drugs between the two groups during the follow-up period(P<0.01).Multivariate logistic regression analysis showed that diabetes,age ≥80 years,LVEF<50%,coronary artery stenosis ≥70%and ratio of low-frequency power/high-frequency power>1.16 were independent influencing factors for MACE in elderly patients with stable CHD within 5 years(P<0.05,P<0.01).ROC curve analysis indicated that the AUC value for low-frequency power and ratio of low-frequency power/high-frequency power in predic-ting MACE occurrence within 5 years was 0.801(95%CI:0.749-0.854,P=0.000)and 0.798(95%CI:0.752-0.844,P=0.000),respectively,and the value of high-frequency power in predic-ting the absence of MACE was 0.629(95%CI:0.566-0.692,P=0.000).Conclusion HRV is an independent influencing factor for MACE occurrence within 5 years in elderly patients with stable CHD,and it shows certain predictive value for MACE occurrence within 5 years in these patients.
6.Longitudinal study on quality of life during postoperative 6 months in patients with implanted cardiac pacemaker

Objective To investigate the change trend of quality of life(QOL)during postoperative 6 months in the patients with implanted cardiac pacemaker(PM)in order to provide a basis for the medical staff to implement the targeted nursing measures.Methods A prospective study design was used.The stratified sampling was adopted to select 252 inpatients with PM implantation in the cardiology department of a class 3A hospital in Guizhou Province as the respondents.The pacemaker patient quality of life measuring scale(QLIP-Pv1.0)was used to conduct the survey on the patients at discharged,in 1,3,6 months after PM implantation.Results There were statistically significant differences in the physical function,general cognitive function,psychological function,social function and total score of QLIPPv1.0 among different measuring times(P<0.001).By using LSD afterwards pairwise comparison,the scores of physical function,general cognitive func-tion and total score were:6 months after implantation>3 months after implantation>1 month after implantation>discharge day(P<0.05),the scores of psychological function and social function were:6 months after implantation,3 months after implantation,1 month after implantation>discharge day(P<0.05),6 months after implantation>3 months after implantation(P<0.05).The QLIPPv1.0 scores at post-operative different times in the patients with implanted PM were different and showed the constantly ascend-ing trend with time passing.For each additional measurement,the QLIPPv1.0 score was increased by 6.527 points(β=6.527,P<0.001).QOL of the patients with PM implantation was affected by many factors,inclu-ding marital status,educational level,employment status,family monthly income and pacemaker type.Conclu-sion QOL of the patients with PM implantation shows the dynamic change over time.The psychological function and social support in 1-3 months after implantation could not be ignored.Medical staff could take the targeted intervention measures according to the changing characteristics and influencing factors during dif-ferent periods.

COVID-19 is caused by a novel coronavirus-severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2), which has being spreading around the world, posing a serious threat to human health and lives. Neutralizing antibodies and small molecule inhibitors for virus replication cycle are the main antiviral treatment for novel coronavirus recommended in China. To further promote the rational use of antiviral therapy in clinical practice, the National Center for Infectious Diseases (Beijing Ditan Hospital Capital Medical University and the First Affiliated Hospital, Zhejiang University School of Medicine) invited experts in fields of infectious diseases, respiratory and intensive care to develop an Expert Consensus on Antiviral Therapy of COVID-19 based on the Diagnosis and Treatment Guideline for COVID-19 ( trial version 10) and experiences in the diagnosis and treatment of COVID-19 in China. The consensus is concise, practical and highly operable, hopefully it would improve the understanding of antiviral therapy for clinicians and provide suggestions for standardized medication in treatment of COVID-19.
8.Scoping review of prone position ventilation in pediatric patients with hypoxemia following extracorporeal cardiac surgery

Objective:To systematically review the application of prone position ventilation in pediatric patients with hypoxemia following extracorporeal cardiac surgery.Methods:A scoping review methodology was adopted. Databases including PubMed, Embase, Web of Science, Cochrane Library, CINHAL, CBM, CNKI, Wanfang, and VIP were searched for relevant studies on the use of prone position ventilation in pediatric patients post-extracorporeal cardiac surgery. The search was conducted from the establishment of the databases until August 31, 2022. After literature screening and data extraction, research results were summarized.Results:Nine studies were ultimately included. Intervening factors in the application process of prone position ventilation included intervention timing, head-of-bed elevation angle, frequency and duration, and pediatric patient positioning. All nine studies reported positive outcomes; three articles reported associated complications.Conclusions:The timing of intervention, the angle of head-of-bed elevation, frequency and duration, patient positioning of prone position ventilation in pediatric patients with hypoxemia after extracorporeal cardiac surgery have not yet been standardized. Future high-quality research is required to further explore these intervention factors, offering references for the standardized and effective clinical application of prone position ventilation.
9.A qualitative research on the work experiences of nurses participating in COVID-19 vaccination in China′s western communities

Objective:To understand the work experiences of vaccination nurses in western China under the background of free vaccination with COVID-19 vaccine of the entire population, and provide references for these nurses to better their performance in large-scale COVID-19 vaccination.Methods:Semi-structured interviews were conducted on working experience of COVID-19 vaccination with 11 community outpatient nurses who participated in the " Second Western Immunization Planning and Health Summit Forum" held in Chongqing on April 16 to 18, 2021. The sample size was set as information saturation, and the 7-step Colaizzi analytical methods was used in data clarification and analysis.Results:Such interviews extracted three themes: difficulties and challenges, psychological experience, organization and coordination. Difficulties and challenges included shortage of nursing human resources, shortage of resources for routine vaccination, imperfect online appointments, and epidemic prevention and control risks brought by mass vaccination; psychological experience included strong sense of responsibility, accumulation of anxiety, and lack of social identity; organizational coordination included organizational and the overall integration of vaccination resources.Conclusions:Community vaccination nurses have demonstrated a high sense of responsibility and mission in the work of the COVID-19 vaccination. Software and hardware facilities should be enhanced for higher efficiency; vaccination nurses deserve more psychological assistance to help ease their tension; the localities are recommended to organize their vaccination work to fit local conditions.
10.Qualitative study of self-care experience in patients with chronic heart failure: a Meta-synthesis

Objective:To systematically review and integrate the self-care experience of patients with chronic heart failure, so as to provide a reference for the construction of self-care plan for patients with chronic heart failure.Methods:Qualitative studies on the self-care experience of patients with chronic heart failure were retrieved by computer in Chinese and English databases such as PubMed, PsycINFO, Web of Science, Embase, CINAHL, Cochrane Library, China National Knowledge Infrastructure (CNKI) , China Biomedical Literature Database, WanFang Med Database and VIP, and the retrieval time limit was from the establishment of the database to January 31, 2021. The quality of the studies were assessed using the Critical Appraisal Tool for qualitative studies of Joanna Briggs Institute Evidence-Based Health Care Centre in Australian. A pooled method was used to integrate the included articles.Results:A total of 13 articles were included. A total of 50 research results were extracted, and 7 new categories were obtained by integrating similar results, which were combined into 2 integrated results. Integrated result 1, that was, multiple factors affected the implementation of self-care in patients with chronic heart failure. Integrated result 2, that was, over time, the patient began to coexist with the disease, and realized self-worth by exploring positive psychological feelings and choosing positive responses using personal beliefs.Conclusions:Chronic heart failure patients are constrained by a variety of factors in the process of disease self-care. Medical and nursing staff should pay attention to the needs of patients in the process of self-care management, provide targeted support and help, and arouse their enthusiasm for self-management.
