1.Prevalence of central obesity among the elderly with different body mass indexes in Xuhui District, Shanghai

		                        			ObjectiveTo investigate the prevalence and influencing factors of overweight, obesity and central obesity among elderly residents in Xuhui District, and to analyze the epidemiological status of central obesity in elderly people with different body mass indexes. MethodsThe third round of health status and health service utilization monitoring data in Xuhui District was used. The information collected from questionnaire survey and physical examination were analyzed. SPSS 20.0 software was used for χ2 test, trend χ2 test and multinominal logistic regression analysis. Results5 096 survey subjects were included. The prevalence of overweight, general obesity, and central obesity in the residents aged 60 and above in Xuhui District were 34.3%, 6.5%, and 29.2%, respectively. There was gender difference in the rates of overweight, obesity, and central obesity among the residents. The overweight and central obesity rates in males were higher than those in females, while the obesity rate was lower than that in females (P<0.05). Multinominal logistic regression analysis showed that in comparison with the normal weight non-central obesity group drinking only at party (OR=1.729, 95%CI: 1.184‒2.525), and hypertension (OR=1.637, 95%CI: 1.305‒2.053), were highly associated with normal weight with central obesity. Aged 60‒ years (OR=1.589, 95%CI: 1.190‒2.120), aged 70‒ years (OR=1.763, 95%CI: 1.327‒2.342), male (OR=1.379, 95%CI: 1.134‒1.676), hypertension (OR=2.231, 95%CI: 1.878‒2.649), former smokers (OR=1.437, 95%CI: 1.027‒2.011), drinking at party only (OR=1.491, 95%CI: 1.107‒2.006), and drinking ≥3 times per week (OR=1.611, 95%CI: 1.116‒2.325), were highly associated with overweight combined with central obesity. Aged 60‒ years (OR=3.817, 95%CI: 2.251‒6.474), aged 70‒ years (OR=3.084, 95%CI: 1.838‒5.175), hypertension (OR=3.683, 95%CI: 2.753‒4.929), diabetes (OR=2.085, 95%CI: 1.511‒2.878), former smokers (OR=1.835, 95%CI: 1.043‒3.226), were highly associated with compound obesity. Central obesity was found in the elderly residents with different BMI categories, and the rate of central obesity increased with the increase of BMI grade. ConclusionThe prevalence of overweight, obesity and central obesity among the elderly aged 60 years and above in Xuhui District is not optimistic. Attention should be paid to the elderly under 80 years old who are with hypertension and/or diabetes, alcohol consumption, low educational level and not doing physical exercise. Especially for the central obesity population with normal BMI, measures should be taken to prevent and intervene the occurrence of obesity and related diseases.

		                        			Microsurgery is not only a technology, but also a science. Microsurgery was enlisted as a tier-three discipline in the Classification of National Clinical Medicine Disciplines (Discipline code 320.2715) in 1992. In the new era, the discipline development of microsurgery should be inherited, innovated, realistic and pragmatic, and it also relies on the other surgical specialties to expand the applications of microsurgery. Microsurgery technology should firmly adhere to its core competitiveness and hold high the banner of microsurgical characteristics by offering the best possible serves to all the clinical surgical disciplines; Pay equal attention to medicine, teaching and scientific researches; Unite, collaborate and strengthen academic exchanges at home and abroad; Focus on talent training (i.e. exports of great country, leading figures, etc.) and support the development of private hospitals specialised in microsurgery. 3.Probability of premature death from 4 major chronic diseases among the registered residents in Xuhui District, Shanghai from 2007 to 2021

		                        			ObjectiveTo understand the death status of major chronic diseases among the registered residents in Xuhui District, Shanghai from 2007 to 2021, and to analyze the probability and trend of premature death, so as to provide an evidence for the policy-making of disease control and prevention. MethodsBased on the data from the death registration system of the registered residents in Xuhui District, Shanghai from 2007 to 2021, the mortality rate of major chronic diseases, life expectancy without cause of death, potential years of life lost (PYLL), average years of life lost (AYLL), and premature death rate were calculated. Joinpoint 4.8 software was used to calculate the annual percentage change (APC) of premature death rate and its trend. ResultsIn 2021, the mortality rate and standardized mortality rate of four major chronic diseases, namely cardiovascular and cerebrovascular diseases, malignant tumors, diabetes mellitus and chronic lower respiratory diseases among the registered residents in Xuhui District, Shanghai was 767.75/100 000 and 234.69/100 000, respectively, accounting for 85.63% of the total causes of death among the residents in Xuhui District, Shanghai. The PYLL, AYLL and PYLL rate were 10 710.00 person-years, 1.49 years per person, and 11.43‰, respectively. Life expectancy increased by 14.91 years after removing the four major chronic diseases. From 2007 to 2021, the premature death rate of chronic diseases among the registered residents in Xuhui District decreased from 9.38% to 7.91% (APC=-1.09%,P<0.05). Malignant tumors had the highest rate in premature death, and the premature death rate was higher in males than that in females, with a slower rate of decline than in females. Cardiovascular and cerebrovascular diseases, malignant tumors and chronic lower respiratory diseases showed a significant decreasing trend in premature death rate (APC=-1.30%, -1.20%, -5.42%, P<0.05), while diabetes mellitus showed a significant increasing trend in premature death rate (APC=3.20%, P<0.05). There was a significant increasing trend in the rate of premature death from diabetes mellitus in males (APC=5.15%, P<0.05) and a decreasing trend in the rate of premature death from diabetes mellitus in females (APC=-0.76%, P<0.05). ConclusionCardiovascular and cerebrovascular diseases, malignant tumors, diabetes mellitus and chronic lower respiratory diseases are the main causes of death among the registered residents in Xuhui District. The probability of premature death of the residents in Xuhui District from major chronic diseases is at a low level, indicating that the prevention and control of chronic diseases in Xuhui District has achieved some success, but it is still difficult to realize the goals of the Outline of the "Healthy China 2030" Plan. Therefore, it is necessary to comprehensively carry out the control of risk factors such as tobacco use, harmful alcohol consumption, salt intake, hypertension, hyperglycemia and obesity, especially to strengthen the control of risk factors for male residents, as well as early diagnosis and treatment of malignant tumors, cardiovascular and cerebral vascular diseases and diabetes mellitus, and to implement the disease management mechanism to reduce premature death. 
		                        		
		                        		
		                        		
		                        	
4.Application of machine learning model based on XGBoost algorithm in early prediction of patients with acute severe pancreatitis.

		                        			OBJECTIVE:
		                        			To establish a machine learning model based on extreme gradient boosting (XGBoost) algorithm for early prediction of severe acute pancreatitis (SAP), and explore its predictive efficiency.
		                        		
		                        			METHODS:
		                        			A retrospective cohort study was conducted. The patients with acute pancreatitis (AP) who admitted to the First Affiliated Hospital of Soochow University, the Second Affiliated Hospital of Soochow University and Changshu Hospital Affiliated to Soochow University from January 1, 2020 to December 31, 2021 were enrolled. Demography information, etiology, past history, and clinical indicators and imaging data within 48 hours of admission were collected according to the medical record system and image system, and the modified CT severity index (MCTSI), Ranson score, bedside index for severity in acute pancreatitis (BISAP) and acute pancreatitis risk score (SABP) were calculated. The data sets of the First Affiliated Hospital of Soochow University and Changshu Hospital Affiliated to Soochow University were randomly divided into training set and validation set according to 8 : 2. Based on XGBoost algorithm, the SAP prediction model was constructed on the basis of hyperparameter adjustment by 5-fold cross validation and loss function. The data set of the Second Affiliated Hospital of Soochow University was served as independent test set. The predictive efficacy of the XGBoost model was evaluated by drawing the receiver operator characteristic curve (ROC curve), and compared it with the traditional AP related severity score; variable importance ranking diagram and Shapley additive explanation (SHAP) diagram were drawn to visually explain the model.
		                        		
		                        			RESULTS:
		                        			A total of 1 183 AP patients were enrolled finally, of which 129 (10.9%) developed SAP. Among the patients from the First Affiliated Hospital of Soochow University and Changshu Hospital Affiliated to Soochow University, there were 786 patients in the training set and 197 in the validation set; 200 patients from the Second Affiliated Hospital of Soochow University were used as the test set. Analysis of all three datasets showed that patients who advanced to SAP exhibited pathological manifestation such as abnormal respiratory function, coagulation function, liver and kidney function, and lipid metabolism. Based on the XGBoost algorithm, an SAP prediction model was constructed, and ROC curve analysis showed that the accuracy for prediction of SAP reached 0.830, the area under the ROC curve (AUC) was 0.927, which was significantly improved compared with the traditional scoring systems including MCTSI, Ranson, BISAP and SABP, the accuracy was 0.610, 0.690, 0.763, 0.625, and the AUC was 0.689, 0.631, 0.875, and 0.770, respectively. The feature importance analysis based on the XGBoost model showed that the top ten items ranked by the importance of model features were admission pleural effusion (0.119), albumin (Alb, 0.049), triglycerides (TG, 0.036), Ca2+ (0.034), prothrombin time (PT, 0.031), systemic inflammatory response syndrome (SIRS, 0.031), C-reactive protein (CRP, 0.031), platelet count (PLT, 0.030), lactate dehydrogenase (LDH, 0.029), and alkaline phosphatase (ALP, 0.028). The above indicators were of great significance for the XGBoost model to predict SAP. The SHAP contribution analysis based on the XGBoost model showed that the risk of SAP increased significantly when patients had pleural effusion and decreased Alb.
		                        		
		                        			CONCLUSIONS
		                        			A SAP prediction scoring system was established based on the machine automatic learning XGBoost algorithm, which can predict the SAP risk of patients within 48 hours of admission with good accuracy.

5.Training in a wearable exoskeleton improves the lower limb motor recovery of stroke survivors

		                        			Objective:To explore any differential effect of training wearing a unilateral exoskeleton on the lower-limb motor function of stroke survivors.Methods:Forty stroke survivors were randomly divided into an exoskeleton group ( n=20) and a control group ( n=20). The control group performed conventional lower extremity exercise training while the exoskeleton group received exoskeleton-assisted lower-limb physical therapy. Each participant received eighteen 40-minute training sessions over three weeks. Before and after the intervention, the walking ability, lower-limb function, balance and ability in the activities of daily living of both groups were evaluated. Integrated electromyography (iEMG) of the rectus femoris and tibialis anterior of both legs was also recorded during sit-to-stand transitions to assess the activation of the affected muscles and the symmetry of bilateral muscle activation. Results:After the three weeks, significant improvement was observed in all of the measurements in both groups, but with the exoskeleton group scoring significantly better on average in functional ambulation category grading (1.63±0.72). Both groups′ iEMGs had also improved significantly compared with before treatment, but the exoskeleton group′s average result was by that time significantly better than the control group′s average.Conclusions:A wearable exoskeleton can effectively improve the rehabilitation of walking, lower limb movement, balance and skill in the activities of daily living of persons with subacute stroke. 6.Effects of transcranial magnetic stimulation on the swallowing and brain-stem auditory evoked potentials of dysphagic stroke survivors

		                        			Objective:To observe any effects of contralateral repeated transcranial magnetic stimulation (rTMS) of the swallowing motor cortex on the swallowing and brainstem auditory evoked potentials (BAEPs) of stroke survivors with dysphagia.Methods:A total of 83 stroke survivors with dysphagia were randomly divided into an ipsilesional stimulation group ( n=22), a contralesional stimulation group ( n=21), a bilateral stimulation group ( n=20), and a control group ( n=20). In addition to their conventional dysphagia training, those in the three stimulation groups received 3Hz rTMS while the control group was given fake stimulation. The treatment was administered daily for 20 minutes, 6 days a week, for 5 consecutive weeks. Before and after the treatment, swallowing function was assessed videofluoroscopically and using the Dysphagia Outcome and Severity Scale (DOSS). The oral and pharyngeal stages of swallowing were evaluated using the videofluoroscopic dysphagia scale (VDS). Brain stem conduction was assessed using BAEPs. Results:After treatment the average DOSS scores of all 4 groups were significantly better than before the treatment. The average DOSS scores of the contralesional and bilateral sti-mulation groups were then significantly better than those of the other two groups. The sub-item and total VDS scores of all 4 groups had decreased significantly, but the average score of the bilateral stimulation group was significantly lower than the control group′s average. Ipsilesional stimulation significantly improved the VDS sub-item scores for the triggering of pharyngeal swallowing, laryngeal elevation, and pharyngeal transit time compared with the control group. In the contralesional stimulation group the average total score and the VDS sub-item scores for apraxia, premature bolus loss, oral transit times, the triggering of pharyngeal swallowing, vallecular residue, laryngeal elevation, coating on the pharyngeal wall, and pharyngeal transit time were significantly lower than those of the control group, on average. After the treatment the latencies of BAEP waves I, III and V and the I-III, III-V and I-V interpeak intervals had decreased significantly in all four groups, but the average latencies and intervals of the bilateral and contralesional groups were significantly shorter than those of the control group. The latencies and intervals of the bilateral stimulation group were then significantly shorter than those in the ipsilesional stimulation group on average. The average latency of wave V in the bilateral stimulation group (6.53±0.73ms) was significantly shorter than that in the contralesional stimulation group after the treatment.Conclusion:Bilateral rTMS over the swallowing motor cortex combined with conventional dysphagia training can significantly improve the swallowing of dysphagic stroke survivors.

		                        			Objective:To explore the therapeutic characteristics of population with gout achieving treat-to-target (T2T) indicators through real-world research and evaluate their safety.Methods:A total of 3 287 patients diagnosed with gout by rheumatologists in 21 first-class tertiary hospitals in 10 provinces, municipalities, and autonomous regions in China from January 2015 to December 2021 were included in this polycentric cross-sectional study. The database included patients′ general information, disease characteristics, and clinical application of traditional Chinese and Western medicine treatment measures. SPSS and Excel software were used for data analysis. Frequency analysis, cluster analysis, and factor analysis were used to summarize the characteristics and rules of treatment measures for patients with gout who achieved the target after treatment. The occurrence of adverse events (AE) was recorded during treatment.Results:After treatment, 691 visits (7%) achieved the serum urate (SUA) target, and the most frequent use of urate-lowering therapy (ULT) was febuxostat, followed by benzbromarone. The most common treatment options were following: GroupⅠ: traditional Chinese medicine (TCM) decoction-TCM external treatment-physical exercise-proprietary Chinese medicine; GroupⅡ: ferulic acid-nonsteroidal anti-inflammatory drugs (NSAIDs); Group Ⅲ: allopurinol-sodium bicarbonate-benzbromarone; Group Ⅳ: glucocorticoid-colchicine; Group Ⅴ: febuxostat. A total of 5 898 visits (60%) chieved manifestations of joint pain VAS scores target, and the most frequently used drug to control joint symptoms was NSAIDs. The frequency of use of drugs to control joint symptoms were 2 118 times (usage rate reached 35.9%), while the frequency of ULT were 2 504 times (usage rate reached 42.5%), which was higher than the joint symptom control drug. The most common treatment options were following: Group Ⅰ: proprietary Chinese medicine-TCM decoction-TCM external treatment-physical exercise; Group Ⅱ: NSAIDs-colchicine hormones; Group Ⅲ: allopurinol, Group Ⅳ: benzbromarone; Group Ⅴ: febuxostat. A total of 59 adverse events occurred during treatment.Conclusion:The proportions of gout patients who reach target serum urate level & good control of joint symptoms are both very low, and ULT and anti-inflammatory prescription patterns are very different from international guidelines, so it is necessary to strengthen the standardized management of gout patients. 10.A healthy neonate born at the third pregnancy of a woman with ornithine carbamoyltransferase deficiency under multidisciplinary management

		                        			We reported a women with omithine carbamoyltransferase deficiency who delivered a healthy boy after two pregnancies with adverse outcome with the help of a multidiscipline team.The woman was admitted to Xinhua Hospital Affiliated to Shanghai Jiaotong University School of Medicine with an acute prenatal hyperammonemic episode at 28 gestational weeks of her first pregnancy in 2013 and was diagnosed with ornithine transcarbamylase deficiency.Her hyperammonemic complications were controlled under a well-planned multidisciplinary management including a low-protein diet and appropriate medications assisting nitrogen removal.A boy was delivered by cesarean section at 32 weeks of gestation but died three days later.Mutation analysis revealed a hemizygous c.583G>A (G195R) mutation in the neonatal omithine carbamyltransferase gene and his mother was a heterozygous carrier with the same mutation.Two years later in 2015,the patient was pregnant spontaneously.However,she received an induced abortion at 21 weeks of gestation because amniocentesis and DNA analysis showed that the male fetus had the same omithine transcarbamylase gene mutation.The index pregnancy was assisted by in vitro fertilization-embryo transfer and preimplantation genetic diagnosis in 2017 and the woman delivered a healthy boy with the management ofa multidisciplinary team.
