1.Application of near-infrared autofluorescence probe in intraoperative parathyroid gland identification

		                        			Objective:To explore the use of near-infrared autofluorescence probe (NIRAF-P) and its application in identifying parathyroid glands during surgery.Methods:A total of 68 patients undergoing thyroid surgery at Peking Union Medical College Hospital and Beijing Longfu Hospital between Dec. 2023 and Jun. 2024 were selected. During the operation, the near-infrared parathyroid gland detector was used to identify the parathyroid gland tissue to be tested, and histopathological examination was performed. The positive predictive value and accuracy of the near-infrared parathyroid gland detector were analyzed.Results:A total of 111 parathyroid glands were identified in 68 patients, and the positive predictive value and accuracy of the NIRAF-P were 95.5% and 94.6%, respectively.Conclusions:The NIRAF-P has high accuracy in identifying parathyroid glands. 2.Screening and content determination of differential quality markers in Zingiber officinale mixed and triturated with Schisandra chinensis before and after processing

		                        			OBJECTIVE To screen and quantitatively analyze differential quality markers (Q-Marker) in Zingiber officinale mixed and triturated with Schisandra chinensis (ZMTS) before and after processing. METHODS HPLC fingerprints of before processing [Z. officinale complicated with S. chinensis (ZWS)] and after processing (ZMTS) (10 batches each) were established. The differences of Q-Markers before and after processing were screened by the chemical pattern recognition method and Q-Marker “five principles”, and the contents were determined. RESULTS A total of 14 common peaks were identified in the fingerprints of ZWS, 22 common peaks were identified in the fingerprints of ZMTS, and 8 components were identified. Differential Q-Marker were screened by chemical pattern recognition and Q-Marker “five principles”, i. e. 6-gingerol, schisandrol A schisandrol B, 8-gingerol, 10-gingerol, schisandrin A, schisandrin B, schizandrin C. The average contents of the 8 differential Q-Markers in ZMTS were 229.46, 244.48, 39.96, 44.12, 61.17, 47.82, 100.11 and 9.70 μg/g, respectively. The average contents of the 4 differential Q-Markers (6-gingerol, schisandrol A, schisandrol B, 8-gingerol) in ZWS were 112.58, 19.01, 26.74 and 5.98 μg/g, respectively. CONCLUSIONS In this study, the differential Q-Markers before and after ZMTS processing are screened. 3.Concept analysis of occupational attractiveness for long-term caregivers

		                        			Objective:To explore the definition and implications of occupational attractiveness among long-term caregivers.Methods:The Walker and Avant concept analysis method was used to define the concept of occupational attractiveness among long-term caregivers.Results:Attributes of occupational attractiveness of long-term caregivers were: (ⅰ) a strength or ability; (ⅱ) mutual attraction between long-term care occupations and incumbents and potential job seekers; (ⅲ) to attract potential job seekers and satisfying their willingness to work; (ⅳ) to retain and motivate in-service long-term care personnel, and enabling them to develop the professional identity and career development.Conclusions:This concept is defined from the perspective of nursing practice in the elderly care service industry in China and combined with the characteristics of long-term caregivers at this stage. 4.Effect of modified Sijunzi Decoction on myelosuppression in moderate- and advanced-stage lung cancer patients with Qi and Yin deficiency and investigation of the underlying mechanism

		                        			Objective:To investigate the effects of modified Sijunzi Decoction on myelosuppression in moderate- and advanced-stage lung cancer patients with Qi and Yin deficiency and analyze the underlying mechanism. Methods:A total of 100 moderate- and advanced-stage lung cancer patients with Qi and Yin deficiency who received treatment in Lishui Hospital of Traditional Chinese Medicine, China were included in this study. They were randomly assigned to receive chemotherapy with paclitaxel combined with cisplatin (control group, n = 50) or treatment with modified Sijunzi Decoction based on chemotherapy with paclitaxel combined with cisplatin (observation group, n = 50). Myelosuppression, traditional Chinese medicine symptom score, cellular immune function, serum levels of granulocyte-macrophage colony-stimulating factor and granulocyte colony-stimulating factor, and the dosage of recombinant human granulocyte-colony stimulating factor. Results:After treatment, the incidence of leucopenia, thrombocytopenia, hemoglobinopenia and neutropenia in the observation group were 60% (30/50), 18% (9/50), 18% (9/50) and 62% (31/50), respectively, which were significantly lower than those in the control group [90% (45/50), 30% (15/50), 32% (16/50) and 92% (46/50), χ2 = 6.979, 7.025, 6.534, 6.134, all P < 0.001]. The complete remission rate in the observation group was significantly higher than that in the control group [30% (15/50) vs. 8% (4/50), χ2 = 9.018, P < 0.001]. The traditional Chinese medicine symptom score in the observation group was significantly lower than that in the control group ( t = 6.982, P < 0.05). CD 8+, CD 4+ and CD 3+ levels in the observation group were (25.16 ± 2.87)%, (38.76 ± 4.16)%, (48.83 ± 5.61)%, respectively, and they were (28. 89 ± 4.02)%, (34.10 ± 4.59)%, (41.12 ± 77)%, respectively in the control group. There were significant differences in CD 8+, CD 4+ and CD 3+ levels between the observation and control groups ( t = 6.392, 6.235, 5.983, all P < 0.05). The dosage of recombinant human granulocyte-colony stimulating factor in the observation group was significantly lower than that in the control group [(2 567.34 ± 308.25) μg vs. (3 917.82 ± 411.67) μg, t = 11.258, P < 0.05]. Serum levels of granulocyte-macrophage colony-stimulating factor and granulocyte colony-stimulating factor were (25.53 ± 7.86) ng/L and (278.34 ± 28.74) ng/L, which were significantly higher than those in the control group [(21.30 ± 3.12) ng/L, (204.17 ± 11.98) ng/L, t = 9.136, 8.856, both P < 0.05]. Conclusion:Modified Sijunzi Decoction for the treatment of moderate- and advanced-stage lung cancer patients with Qi and Yin deficiency can decrease the incidence of myelosuppression possibly through increasing serum levels of granulocyte-macrophage colony-stimulating factor and granulocyte colony-stimulating factor and improving the immune function.

		                        			Netrin-1, an axon guidance factor, and its receptor UNC5B play important roles in axonal development and angiogenesis. This study examined netrin-1 and UNC5B expression in kidneys with diabetic kidney disease (DKD) and investigated their roles in angiogenesis. Netrin-1 and UNC5B were upregulated in streptozotocininduced DKD Wistar rats, and their expression was compared with that in healthy controls. However, exogenous netrin-1 in UNC5B-depleted human renal glomerular endothelial cells (HRGECs) inhibited cell migration and tubulogenesis. This effect was likely associated with SRC pathway deactivation. Netrin-1 treatment also eliminated the pro-angiogenic effects of exogenous VEGF-165 on UNC5B-silenced HRGECs. These results indicate that UNC5B antagonizes netrin-1 and that UNC5B upregulation contributes partly to enhancing angiogenesis in DKD. Therefore, introducing exogenous netrin-1 and depleting endogenous UNC5B are potential strategies for reducing the incidence of early angiogenesis and mitigating kidney injury in DKD.
		                        		
		                        		
		                        		
		                        	
7.The potential role of calnexin in the activation of cardiac fibroblasts.

		                        			Calnexin is a lectin-like molecular chaperone protein on the endoplasmic reticulum, mediating unfolded protein responses, the endoplasmic reticulum Ca homeostasis, and Ca signals conduction. In recent years, studies have found that calnexin plays a key role in the heart diseases. This study aims to explore the role of calnexin in the activation of cardiac fibroblasts. A transverse aortic constriction (TAC) mouse model was established to observe the activation of cardiac fibroblasts , and the cardiac fibroblasts activation model was established by transforming growth factor β1 (TGFβ1) stimulation. The adenovirus was respectively used to gene overexpression and silencing calnexin in cardiac fibroblasts to elucidate the relationship between calnexin and cardiac fibroblasts activation, as well as the possible underlying mechanism. We confirmed the establishment of TAC model by echocardiography, hematoxylin-eosin, Masson, and Sirius red staining, and detecting the expression of cardiac fibrosis markers in cardiac tissues. After TGFβ1 stimulation, markers of the activation of cardiac fibroblast, and proliferation and migration of cardiac fibroblast were detected by quantitative PCR, Western blot, EdU assay, and wound healing assay respectively. The results showed that the calnexin expression was reduced in both the TAC mice model and the activated cardiac fibroblasts. The overexpression of calnexin relieved cardiac fibroblasts activation, in contrast, the silencing of calnexin promoted cardiac fibroblasts activation. Furthermore, we found that the endoplasmic reticulum stress was activated during cardiac fibroblasts activation, and endoplasmic reticulum stress was relieved after overexpression of calnexin. Conversely, after the silencing of calnexin, endoplasmic reticulum stress was further aggravated, accompanying with the activation of cardiac fibroblasts. Our data suggest that the overexpression of calnexin may prevent cardiac fibroblasts against activation by alleviating endoplasmic reticulum stress.
		                        		
		                        		
		                        		
		                        	
8.Clinical features of esophageal foreign bodies difficult to remove by gastroscopy

		                        			Objective To analyze the clinical features of esophageal foreign body(EFB) difficult to remove by gastroscopy.Methods The clinical data of 159 patients with EFBs treated in Renmin Hospital of Wuhan University from January 2013 to April 2017 were analyzed retrospectively.Among 159 cases,the EFBs were removed by gastroscopy in 101 cases(gastroscope group)and removed by non-gastroscope method in 58 cases (non-gastroscope group),including 42 cases removed by esophagoscopy and 16 cases by surgery.The general information of patients,the types of EFB,the condition of impaction,treatments and outcomes of the two groups were analyzed.Results There was no significant difference in age and sex between gastroscope group and non-gastroscope group (P > 0.05).The food typeEFBs included fish bone (n =77),chicken bone (n =45),jujube seeds (n =15),food bolus (n =4);and the non-food type FEBs included spectecleslens (n =2),bottle cap (n =11),denture (n =5).More than one half of FBs were located in the upper esophagus:63.4% (64/101) in gastroscope group and 66.7% (28/42) in esophagoscope subgroup;however,EFBs were mostly located in the middle esophagus in surgery subgroup (62.5%,10/16).In gastroscope group and esophagoscope subgroup,the sharp EFBs of length-diameter > 2 cm were rare(n =4,3.9% and n =3,7.1%),but were common in the surgery group (n =9,56.3%).The EFBs were successfully removed in all patients without massive hemorrhage or death.In gastroscope group (n =101),93 cases had mucosal abrasion and bleeding,5 cases had mild active bleeding,and 1 case had esophageal fistula.In surgical removal (n =16) there were 4 cases of esophageal perforation and postoperative inflammation,3 cases of mediastinal inflammation and 2 cases of mediastinal abscess.Inesophagoscopic removal group (n =42),there were 2 cases of esophageal muscle layer abscess,1 case of esophageal fistula and 1 case of pulmonary infection.Conclusions The most common type of EFBs is of food source.Foreign bodies in the upper and middle segments of the esophagusare difficult to remove with gastroscopy.In case of a sharp foreign body embedded in the middle esophagus,surgical removal is preferred to avoid severe sequences.
		                        		
		                        		
		                        		
		                        	
9.Effect of continuous positive airway pressure on glycosylated hemoglobin levels in patients with obstructive sleep apnea syndrome

		                        			Objective To observe the effect of obstructive sleep apnea syndrome (OSAHS) patients with glycosylated hemoglobin (HbA1c) levels, assessment of continuous positive airway pressure (CPAP) effect on the level of HbA1c in patients with OSAHS. Methods From January 2013 to December 2016, eighty patients with OSAHS were divided into mild group, eighteen cases, moderate group twenty cases, severe group forty-two cases according to the apnea hypopnea index (AHI). Thirty healthy subjects were chosen as the control group. All the treatment groups were treated with CPAP for three months. Overnight polysomnography and HbA1c test were performed before and after treatment, and the control group was examined by HbA1c before treatment. Results The level of glycosylated hemoglobin in the middle and severe group was higher than that of the control group(P<0.05). After treatment, the glycated hemoglobin levels in the moderate and severe groups were significantly lower(P<0.05), and the sleep apnea hypopnea index, the lowest SaO2 and the average SaO2 level were significantly improved(P<0.01). Conclusion Glycated hemoglobin levels increased significantly in patients with moderate and severe OSAHS, and decreased HbA1c levels in patients with moderate and severe OSAHS after CPAP treatment.
		                        		
		                        		
		                        		
		                        	
10.Correlation between sentinel polyps and proximal colon carcinoma and analysis of its clinical features

		                        			Objective To explore the clinical features of sentinel polyps (rectal polyps with proximal colon carcinoma) and its correlation with proximal colon carcinoma.Methods From January 2003 to December 2013,the clinical features of 963 hospitalized patients with rectal polyps were retrospectively analyzed.According to whether the patient with colon carcinoma,the clinical data of rectal polyps patients were divided into pure polyps group (n=855) and sentinel polyps group (n 108).The characteristics under endoscopy,clinical pathological features,treatment and prognosis of the two groups were observed.Chi square test was performed for differences comparison between groups.Results The length of stay in hospital of 963 patients was from four to 33 days,the average age was (49.7 ± 9.4) years,and the majority of the patients were male (n=610,63.3%).A total of 785 patients (81.5%) had non-specific abdominal symptoms,such as frequency or habit change of defecation,hematochezia,abdominal pain,abdominal distension.The relatives within three generation of 78 patients (8.1 %) were diagnosed with tumor and some relatives of the patients diagnosed with familial adenomatous polyposis (2.2%,21/963).The positive rate of tumor marker of sentinel polyps group was higher (69.4%,75/ 108) than that of pure polyps group (6.8%,58/855; x2 =316.285,P<0.01).The proximal colon carcinoma of patients with sentinel polyps appeared as neoplasma with circumferential growth,there was no specific appearance between distal rectal polyps and proximal colon carcinoma.When compared with pure polyps group,most polyps of sentinel polyps group were in maximum diameter over 1 cm (61.1% (66/108) vs 46.9% (401/855)),multi polyps (n>5,38.9% (42/108) vs 11.8% (101/855)) and adenomatous polyp (83.3% (90/108) vs 35.6%(304/855),x2=7.752,55.595 and 90.544,all P< 0.01).Majority of the proximal colon carcinoma with rectal polyps was papillary adenocarcinoma and tubular adenocarcinoma which was account for 75.9% (82/108).Some were mucinous carcinoma and signet ring cell carcinoma.Most of the proximal colon carcinoma with rectal polys did not penetrate the serosa layer (Duk A-+Duk B,59.3%,64/108) and with little distant metastasis (Duke D,17.6%,19/ 108).Ninty-five point six percent (817/855) of patients with pure polyps underwent endoscopic treatment and all of them were cured and discharged.Forty-one point seven percent (45/108) of patients of sentinel polyps group accepted the radical operation and 19.4% (21/108) received endoscopic submucosal dissection.Conclusions If multiple,maximum diameter over 1 cm and adenomatous rectal polyps were detected under colon endoscope,the possibility of carcinogenesis of the polyps or the proximal colon should be awared.If the endoscope is difficult to go further or the patient can not tolerate the whole colon examination,the patient should be followed up in short-term and complete the whole colon examination.
