1.Effects of mask-wearing exercise on heart rate,blood oxygen saturation and end-expiratory carbon dioxide:a Meta-analysis

OBJECTIVE:Masks are one of the most important defenses against the virus.However,the impact of wearing masks during daily activities or sports on respiratory and circulatory function remains controversial.A comprehensive quantitative evaluation of the effects of mask-wearing on human heart rate,oxygen saturation and end-expiratory carbon dioxide by Meta-analysis was conducted.The effects of wearing different types of masks at different exercise intensities and time of exercise on the human respiratory and circulatory system were explored. METHODS:By February 2023,with"mask,face mask,N95,training,sports,running,walking,cycling"as the Chinese search terms and"masks,respiratory protective devices,N95 respirators,surgical face masks,exercise,resistance training,explosive training,muscle exercises"as English search terms,the experimental studies addressing the influence of exercise with a mask on hemodynamic indexes were retrieved from CNKI,Web of Science,PubMed,Cochrane Library and WanFang databases.The outcome indicators included three continuous variables-exercise center rate,blood oxygen saturation and end-expiratory carbon dioxide.Stata16.0 software was used to analyze the outcome indicators of the included literature.The PEDro scale was used as a quality assessment tool,and the funnel plot was used to analyze the impact of publication bias. RESULTS:Totally 25 articles involving 857 healthy children and adults were included in this Meta-analysis.The overall methodological quality was high,with 22 studies scoring 6 points on the PEDro scale,2 studies scoring 7 points and 1 study scoring 8 points.The meta-analysis results showed that compared with the control group,exercise with masks had no significant effect on heart rate(SMD=0.02,95%CI:-0.11 to 0.15,P=0.81),but increased end-expiratory carbon dioxide(SMD=0.60,95%CI:0.37 to 0.83,P=0.00),decreased oxygen saturation(SMD=-0.28,95%CI:-0.47 to-0.09,P=0.03).Intensity and duration were the factors that affected the heterogeneity between studies.Wearing a mask during high-intensity exercise significantly increased heart rate(SMD=-0.20,95%CI:-0.36 to-0.04,P=0.02).The effect of high-intensity and short-time exercise on blood oxygen saturation was significantly higher than that of other exercises(SMD=-0.40,95%CI:-0.70 to-0.10;SMD=-0.25,95%CI:-0.45 to-0.04).For end-expiratory carbon dioxide,maintaining a certain intensity and increasing the exercise time or increasing the intensity further increased the index significantly,reaching a moderate effect size(SMD=0.61,95%CI:0.06 to 1.15;SMD=0.58,95%CI:0.04 to 1.13). CONCLUSION:Existing evidence suggests that exercise with masks may have the adverse effect of increasing end-expiratory carbon dioxide and decreasing blood oxygen saturation.The influence of different exercise test time and intensities on the three outcome indexes was different.Wearing a mask during high-intensity exercise can significantly increase heart rate and decrease blood oxygen saturation.Maintaining moderate intensity for a long period or further increasing the intensity of exercise will lead to increased end-expiratory carbon dioxide levels.
2.Colorectal cancer with β-catenin protein expression deficiency: a clinicopathological analysis

Objective:To investigate the clinicopathological features and molecular characteristics of β-catenin-deficient colorectal cancer.Methods:The clinical, pathological and molecular features of 11 colorectal cancers with β-catenin protein loss diagnosed at the 960th Hospital of People′s Liberation Army of China, from January 2012 to November 2022 were analyzed.Results:Among the 11 patients, 3 were males and 8 were females. Their age ranged from 43 to 74 years, with the median age of 59 years. Six were in the left colon and 5 were in the right colon. One of the 11 cases had lymph node metastasis, 10 cases were well and moderately differentiated adenocarcinoma, and 1 was mucinous adenocarcinoma. Eight cases were of TNM stage T4, 2 of T1 stage and 1 of Tis stage. β-catenin protein was not detected using immunohistochemistry. Sanger sequencing revealed the presence of fragment-deletion mutation in exon 3 of CTNNB1 gene, resulting in loss of β-catenin protein expression.Conclusion:β-catenin deficiency is present in a small number of colorectal cancers and may be associated with exon 3 mutations of CTNNB1 gene.
3.The value of multimodal MRI in the diagnosis of vaginal involvement and parametrial infiltration in patients with cervical cancer

Objective To investigate the consistency analysis between multimodal MRI in determining vaginal involvement and parametrial infiltration in cervical cancer patients and pathological examination.Methods Ninety-two patients with cervical cancer were regressively selected and who underwent preoperative multimodal MRI examination.Using pathological results as the gold standard,the receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficacy of conventional MRI,diffusion weighted imaging(DWI)and dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)alone and in combination in predic-ting vaginal involvement and parametrial infiltration.Results(1)Vaginal involvement:the diagnostic sensitivity of conventional MRI was 65.96%,the specificity was 91.11%,and the area under the curve(AUC)was 0.775[95%confidence interval(CI)0.676-0.873].The sensitivity of DCE-MRI was 61.70%,the specificity was 95.56%,and the AUC was 0.797(95%CI 0.702-0.892).The sensitivity of DWI was 72.34%,the specificity was 88.89%,and the AUC was 0.817(95%CI 0.725-0.908).At the optimal cut-off point,the sensitivity of parallel combined detection was 97.87%,the specificity was 93.33%,and the AUC was 0.956(95%CI 0.907-0.918).(2)Parametrial infiltration:the diagnostic sensitivity of conventional MRI was 67.24%,the specificity was 91.18%,and the AUC was 0.783(95%CI 0.688-0.879).The sensitivity of DCE-MRI was 62.07%,the specificity was 94.12%,and the AUC was 0.781(95%CI 0.686-0.875).The sensitivity of DWI was 75.86%,the specificity was 97.06%,and the AUC was 0.865(95%CI 0.788-0.941).At the optimal cut-off point,the sensitivity of parallel combined detection was 98.28%,the specificity was 91.18%,and the AUC was 0.947(95%CI 0.888-1.000).Conclusion Combined multimodal MRI can improve the positive detection rate of vaginal involve-ment and parametrial infiltration in patients with cervical cancer,allowing for timely treatment adjustments,improved quality of life and longer survival.

5.Content Determination of Pulegone in Schizonepeta tenuifolia and Its Compound Preparation by Hollow Fiber Liquid-phase Microextraction Coupled with HPLC

OBJECTIVE:To establish t he metho d for the content determination of pulegone in Schizonepetae tenuifolia decoction pieces and its compound preparation. METHODS :Hollow fiber liquid-phase microextraction coupled with HPLC (HF-LPME-HPLC) was adopted. Based on single factor tests ,HF-LPME condition of S. tenuifolia decoction pieces and its compound preparation (taking Compound S. tenuifolia granule as an expample ) was optimized by central composite design-response surface methodology using pulegone enrichment multiple as index ,with the concentration of sample phase solution (NaCl),extraction time and stirring speed as factors. Validation test was conducted. HPLC method was adopted to determine the content of pulegone. The determination was performed on Hypersil C 18 column with mobile phase consisted of methanol- 0.3% phosphoric acid (gradient elution )at the flow rate of 1.0 mL/min. The detection wavelength was set at 252 nm,the column temperature was 25 ℃. The sample size was 20 μL. The feasibility of HF-LPME-HPLC method established in this study was validated by using HPLC method stated in the item of S. tenuifolia decoction pieces in 2020 edition of Chinese Pharmacopoeia (part Ⅰ)as reference. RESULTS :The optimum HF-LPME conditions included n-nonanol as the extraction solvent ,sample phase solution with 11% NaCl and pH value of 7,stirring speed of 800 r/min,extraction time of 36 min. Results of HPLC methodology investigation showed that linear range of pulegone were 0.05-5 μg/mL(r=0.999 0). The limits of detection and quantitation were 0.4 and 1.3 ng/mL,respectively. RSDs of intra-day and inter-day precision were 1.8%-4.0% and 1.5%-4.1%(n=3),respectively. RSDs of reproducibility and stability tests (24 h)were all lower than 8%(n=6). Average recoveries of S. tenuifolia decoction pieces and Compound S. tenuifolia granule were 102.6%-105.1% and 97.2%-102.3%,respectively;RSDs were not higher than 4.1% and 6.2%(n=3). The average contents of pulegone in S. tenuifolia decoction pieces determined by pharmacopoeia method and established method were 0.84 mg/g(RSD=4.3% ,n=3)and 0.87 mg/g(RSD=5.5% ,n=3),respectively,with no significant difference (P>0.05). CONCLUSIONS :The established HF-LPME-HPLC method can enrich and concentrate pulegone , shows strong purification ability and high sensitivity ,and can be used to determine the contents of pulegone in S. tenuifolia decoction pieces and its compound preparation.

A cross-sectional study was conducted and randomized sampling method was used to choose 100 nurses who participated in standardized training in Chengdu Fifth People′s Hospital. 7.Effect of Ulinastatin Combined with Pantoprazole on Inflammatory Factors and Gastrointestinal Tract in Patients Undergoing Cardiopulmonary Bypass Cardiac Surgery

Objective To investigate the effect of ulinastatin combined pantoprazole on inflammatory factors and gastrointestinal tract in patients undergoing cardiopulmonary bypass ( CBP) cardiac surgery. Methods A total of 200 patients who suffered rheumatic heart disease were scheduled for valve replacement surgery with CPB, were randomly divided into four groups:control group (CON),ulinastatin (UTI),pantoprazole groups (PTZ) and ulinastatin+pantoprazole groups(UTI+PTZ),50 cases in each group.Before CBP,group UTI was given ulinastatin 10 000 U·kg-1,group PTZ was given pantoprazole 40 mg,group UTI+PTZ was given ulinastatin 10 000 U·kg-1and pantoprazole 40 mg,group CON was given 0.9% sodium chloride soution.The gastric mucosa pHi and blood samples would be collected in all four groups at the preoperative (t1),CPB 30 min (t2),after CBP (t3),6 h after surgery (t4),24 h (t5) five time points.The IL-6 and TNF-α would be detected by enzyme linked immunosorbent (ELISA) method,and abdominal distension,abdominal pain,hematemesis,black and defecate occult blood test positive for digestive tract related complications would be collected after the surgery 1,2 days. Results The concentration of TNF-α and IL-6 at t2,t3, t4,t5were higher than those at t1in all four groups(P<0.05).Compared with CON group,the concentration of TNF-α and IL-6 at t2, t3,t4,t5in UTI,PTZ and UTI+PTZ group were significantly decreased (P<0.05).The concentration of TNF-α and IL-6 in UTI and UTI+PTZ group were better than in PTZ group.The pHi at t2,t3,t4was lower than that at t1in four groups(P<0.05),and pHi at t5 was obviously lower than that at t1in group CON (P<0.05).The pHi at t2,t3,t4in UTI,PTZ and UTI+PTZ group was higher than that in CON group ( P<0. 05), and pHi in UTI+PTZ group was better than that in UTI and PTZ group. The postoperative gastrointestinal complications in CON group were higher than those in UTI,PTZ and UTI+PTZ group (P<0.05). Conclusion Ulinastatin combined with pantoprazole for patients undergoing CPB heart surgery,can significantly reduce the release of TNF-α and IL-6、increase gastric pHi and reduce the incidence of gastrointestinal complications.

Obstructive sleep apnea (OSA)is a high incidence of potentially dangerous disease,characterized by intermittent hypoxia or hypercapnia.It is an independent risk factor for ischemic stroke.Currently a number of studies have confirmed OSA closely associated with oxidative stress.In this paper,the complex mechanisms of oxidative stress in the OSA and the occurrence of stroke will be reviewed,such as promoting atherosclerosis,damaging the mitochondria,ischemia -reperfusion injury,ischemic preconditioning.To investigate the relationship between OSA,oxidative stress and stroke from molecular mechanisms.
9.One case report of Jacobsen syndrome combine with Paris-Trousseau syndrome

Objective To investigate the clinical features of a patient diagnosed with Jacobsen syndrome (JBS) and Paris-Trousseau syndrome (PTS) using chromosomal microarray analysis. Method A retrospective analysis including the patients' clinical manifestations, laboratory examination and genetic analysis was carried out and related literature were reviewed. Results A 14 month-old girl with global development retardation was reported. The patient can sit but cannot walk independently. The patient also presented hypsicephaly, ocular hypertelorism, palpebral ptosis, flat nasal bridge, sparse eyebrows, and speech delay. Gesell development scale showed that the patient was global development retarded with a development level of 40 weeks. No o bvious abnormality was found in EEG but the MRI showed cerebral white matter abnormality. This patient was also diagnosed with neonatal thrombocytopenia in other hospital. Genomic CNVs were detected in this girl, and a 15.7Mb loss was found in the 11q23.3q25 region that covers JBS and PTS region. Conclusions Patient diagnosed with JBS and PTS often present with craniofacial abnormalities, cerebral white matter abnormality and neonatal thrombocytopenia. Chromosomal microarray analysis can help diagnosis.
10.Level of uric acid and lipid metabolism in 120 patients with psoriasis and their clinical diagnostic significance

Objective To explore the level of uric acid and lipid metabolism in 120 patients with psoriasis and their clinical diagnostic significance.Methods A total of 120 patients with psoriasis were selected.Uric acid and blood lipids were detected.Levels of TC,TG,HDL-C,LDL-C,ApoA1,ApoB,uric acid were compared between psoriasis group and control group.Lipid metabolism in patients with different types of psoriasis was compared.Results The levels of TC,HDL-C and ApoA1 in the psoriasis group were significantly lower than those in the control group (P < 0.05),and the level of TG was significantly higher than that of the control group (P < 0.05).Levels of TC,HDL-C,LDL-C,ApoA1 and ApoB in psoriasis pustulosa group were significantly lower than the other two groups (P < 0.05).Serum uric acid values of male and female in psoriasis group were significantly higher than the control group (P < 0.05).Conclusion Analysis of uric acid,lipid metabolism in psoriasis patients has very important diagnostic significance.
