One of its primary surgical treatments of tricuspid regurgitation is tricuspid valve biological valve replacement. Catheter tricuspid valve-in-valve implantation is a novel interventional alternative for biological valve failure. The non-invasive lung fluid measuring device remote dielectric sensing (ReDSTM) has been increasingly incorporated into clinical practice as a means of monitoring chronic heart failure in recent years. This report describes the process and outcomes of the first instance of perioperative lung fluid volume evaluation following transcatheter tricuspid valve implantation utilizing ReDSTM technology. The patient has a short-term, substantial increase in postoperative lung fluid volume as compared to baseline.

Objective:To evaluate the role of hippocampal β4-Subunit-Containing Nicotinic Acetylcholine Receptors (Chrnb4) in postoperative delirium in aged mice.Methods:Forty-eight SPF male C57BL/6J mice, aged 18 months, weighing 29-35 g, were assigned into 6 groups using a random number table method: tibial fracture group (TF group, n=6), sham operation group (Sham group, n=6), tibial fracture + adipenine group (TA group, n=9), tibial fracture + control vehicle group (TV group, n=9), sham operation + adipenine group (SA group, n=9), and sham operation + control vehicle group (SV group, n=9). The postoperative delirium model was prepared by tibial fracture under sevoflurane anesthesia. Tibial fracture was simulated by implanting a steel pin into the tibia and then clamping it, while sham group only received a longitudinal incision and suture after anesthesia. A microinjection cannula was implanted into the mouse skull at 5 days before developing the model in TA group, TV group, SA group and SV group. Three mice from each group were randomly selected for microelectrode implantation in the hippocampal CA1 area. Starting from 30 min after surgery, adipenine (62.5 nmol/μl) 2 μl was infused into the cerebral ventricle for 7 consecutive days in TA and SA groups, and vehicle (2 μl) was administered instead at a 24-h interval for 7 consecutive days in TV and SV groups. The expression of Chrnb4 mRNA in the hippocampal tissues was detected by real-time quantitative polymerase chain reaction at 24 h after surgery. On the 7th and 8th days after surgery, the open-field test and O-maze experiment were conducted to assess the impulsive-like behavior in TA, TV, SA and SV groups. After the behavioral test, the expression of glial fibrillary acidic protein (GFAP) and vesicular glutamate transporter 1 (Vglut1) in the hippocampal CA1 region was detected by immunofluorescence staining. The local field potentials in the hippocampal CA1 region were recorded during the open field test. Results:The expression of Chrnb4 mRNA in the hippocampal tissues was significantly up-regulated in TF group compared to Sham group ( P<0.05). Compared with TV group, the percentage of central path distance in the open field test and percentage of time spent in the open arms of the O-maze were significantly decreased, the power of β-waves in the CA1 field potentials was decreased, the expression of GFAP and Vglut1 in the hippocampal CA1 region was down-regulated, and the co-staining area of GFAP + and Vglut1 + was decreased in TA group and SV group ( P<0.05). There was no significant difference in each parameter between SA group and SV group ( P>0.05). Conclusions:Hippocampal Chrnb4 may be involved in the mechanism of postoperative delirium in aged mice, and this process may be related to inhibition of neuron excitotoxicity.

Research of healthy life expectancy (HLE) is needed in an aging society and by the Healthy China Initiative. However, China has not developed the HLE policy goal nor published HLE values. Therefore, it is essential to construct the HLE indicator system to guide HLE estimation and research. Based on the definition of health from the WHO and the concepts from the United Nations Sustainable Development Goal, One Health and Active Health, we systematically reviewed the existing international HLE indicators, and initially proposed a novel multi-dimensional HLE conceptual framework and indicator structure according to international function, disability and health classifications and international disease classification framework of WHO. The conceptual framework includes five dimensions-physical, mental, behavioral, social, and environmental health, and the comprehensive indicator for disability-disability index is added. Furthermore, we proposed the HLE indicator structure, including behavioral factors, physiological and pathological reasons, impairment, functional limitation, disability and death. Then we discussed the approaches, types of data, and potential data sources for such research. The results of this study would facilitate HLE research and contribute to the aging society and Healthy China Initiative.

Background:
Developmental hypothyroidism impairs learning and memory in offspring, which depend on extensive neuronal circuits in the entorhinal cortex, together with the hippocampus and neocortex. The entorhinal-dentate gyrus pathway is the main entrance of memory circuits. We investigated whether developmental hypothyroidism impaired the morphological development of the entorhinal-dentate gyrus pathway.
Methods:
We examined the structure and function of the entorhinal-dentate gyrus pathway in response to developmental hypothyroidism induced using 2-mercapto-1-methylimidazole.
Results:
1,1´-Dioctadecyl-3,3,3´,3´-tetramethylindocarbocyanine perchlorate tract tracing indicated that entorhinal axons showed delayed growth in reaching the outer molecular layer of the dentate gyrus at postnatal days 2 and 4 in hypothyroid conditions. The proportion of fibers in the outer molecular layer was significantly smaller in the hypothyroid group than in the euthyroid group at postnatal day 4. At postnatal day 10, the pathway showed a layer-specific distribution in the outer molecular layer, similar to the euthyroid group. However, the projected area of entorhinal axons was smaller in the hypothyroid group than in the euthyroid group. An electrophysiological examination showed that hypothyroidism impaired the long-term potentiation of the perforant and the cornu ammonis 3–cornu ammonis 1 pathways. Many repulsive axon guidance molecules were involved in the formation of the entorhinaldentate gyrus pathway. The hypothyroid group had higher levels of erythropoietin-producing hepatocyte ligand A3 and semaphorin 3A than the euthyroid group.
Conclusion
We demonstrated that developmental hypothyroidism might influence the development of the entorhinal-dentate gyrus pathway, contributing to impaired long-term potentiation. These findings improve our understanding of neural mechanisms for memory function.

【Objective】 To investigate the serological and molecular characteristics of HBsAg+ /HBV DNA non-reactive (NR) infections. 【Methods】 Samples tested as HBsAg+ and HBV DNA NR were confirmed by individual NAT repeat testing, viral particle concentration by PEG precipitation combined with in-house nested PCR and real-time quantitative PCR, anti-HBc testing, and HBsAg quantification. HBV sequences were compared with those from donors with chronic and occult infection as controls. 【Results】 A total of 792 195 samples were screened between January 2011 and December 2020, of which 53 (1: 14 947) were confirmed HBsAg+ /HBV DNA NR. HBV DNA was detected further in five (9.4%) samples; three S sequences and four Pre Core/Core sequences were obtained. Unique amino acid substitutions (P130T, P135Q/S, R151Q, G153S and S155F) were found in the Core protein that may affect virus packaging and replication. 【Conclusion】 Extremely low HBV DNA level was detected in plasmas of HBsAg+ /HBV DNA NR donors. Barely detectable HBV DNA might be associated with unusual mutations in the Pre Core/Core protein affecting viral replication. More sensitive HBV DNA and/or HBsAg assays may be considered to further reduce the potential HBV transfusion-transmission residual risk.

Objective:To compare the clinical effect of perforator flap of proper palmar digital and hallex fibular flap in repair of soft tissue defect of fingers. Methods:A total of 52 patients with finger soft tissue injury were reviewed from January, 2017 to January, 2019. Twenty-seven patients were repaired by perforator flap of proper palmar digital artery, and 25 by hallex fibular flaps. Both groups of patients had only soft tissue defects without phalangeal fracture. Postoperative follow-up were conducted by phone, WeChat and visit of outpatient clinic. The recovery of finger sensory function was evaluated according to the Trial Standard for Replantation Evaluation of Severed Digit by the Hand Surgery Society of Chinese Medical Association. The flaps were evaluated according to the shape, patient satisfaction and finger function and classified in excellent, good, fair and poor. Data of the 2 groups of patients were collected and statistically analysed by SPSS 22.0. P<0.05 was statistically significant. Results:Both groups entered follow-up for 12-16 months. All flaps in both groups survived. There was statistically significant difference of TPD between the 2 groups[TPD were 4-7(5.3±1.2) mm and 6-10(8.4±1.7)mm, respectively]( t=7.642, P=0.000). According to the Evaluation Criteria of the Hand Surgery Society of the Chinese Medical Association. In the group repaired by perforator flap of proper palmar digital artery, 17 fingers were excellent, 6 good, and 4 fair, with a 85.1% of excellent and good rate. In the group repaired by hallex fibular flap, 8 fingers were excellent, 5 good, 9 fair, and 3 poor, with a 52.0% of excellent and good rate. The difference between 2 groups was statistically significant ( t=6.710, P=0.009). There was a 3.7% incidence of complications in the group repaired by perforator flap of proper palmar digital artery, and 25.9% in the group repaired by hallex fibular flap. The difference between the 2 groups was statistically significant( t=4.167, P=0.032), and there was a higher incidence of complications in the group repaired by hallex fibular flap. Conclusion:The perforator flap of proper palmar digital artery and the hallex fibular flap are good choices in the repair of finger soft tissue defect. However, the postoperative complication of the hallux fibular flap is relatively high. Therefore, it is necessary to make a decision in the choice of flap according to the actual situation in clinical work.
