2.Current status and influencing factors of functional independence of patients with traumatic spinal cord injury in chronic disease management stage
Chinese Journal of Modern Nursing 2022;28(2):200-204
Objective:To understand the current status of functional independence of patients with chronic traumatic spinal cord injury during the chronic disease management period, and analyze its influencing factors. Methods:This study was a cross-sectional study. Using a multi-stage stratified sampling method, a total of 161 patients with traumatic spinal cord injury who were treated in the orthopedics and rehabilitation departments of 9 hospitals in Sichuan Province from January to October 2018 were selected as the research objects. The general information questionnaire and Chinese version of Spinal Cord Independence MeasureⅢ (SCIM-Ⅲ) were used to investigate them. Multiple linear regression analysis was used to analyze the influencing factors. Results:The total score of the SCIM-Ⅲ for 161 patients with traumatic spinal cord injury in chronic disease management stage was (60.93±18.87) . Of which, the scores of self-care, breathing and sphincter management, and movement dimensions were respectively (14.65±4.81) , (29.71±7.70) and (16.57±9.24) . The result of multiple linear regression analysis showed that the degree of spinal cord injury, occupational status after illness and family structure were the factors affecting the functional independence of patients with chronic spinal cord injury during the management period ( P<0.05) . Conclusions:The current status of functional independence of patients with spinal cord injury in chronic disease management stage need to be improved. Nursing staff can actively encourage patients to perform vocational rehabilitation according to its influencing factors, so that they can return to work as soon as possible and to improve functional independence and quality of life in.

Objective:To explore the mediating effect of perceived patient safety culture on job burnout and safety behavior among nurses in Emergency Departments of ClassⅢ hospitals. Methods:This study was a cross-sectional study. From February 26 to March 14, 2021, 455 Emergency Department nurses from 39 hospitals in 9 provinces/municipalities were selected as the participants by a combination of purposeful sampling and convenience sampling. 455 Emergency Department nurses was conducted through WeChat using the General Information Questionnaire, Maslach Burnout Inventory (MBI) , Hospital Survey on Patient Safety Culture (HSOPSC) , and the Nurse Safety Behavior Questionnaire (NSBQ) . A total of 428 valid questionnaires were collected, with a valid recovery rate of 94.07%.Results:Among 428 Emergency Department nurses, the scores of job burnout, perceived patient safety culture and nurse safety behavior were 52 (38, 64) , 150 (139, 166) and 54 (49, 58) , respectively. The job burnout score of Emergency Department nurses was negatively correlated with the perceived patient safety culture score ( r=-0.661, P<0.01) and the safety behavior score ( r=-0.513, P<0.01) . There was a positive correlation between the perceived patient safety culture score and the safety behavior score of Emergency Department nurses ( r=0.482, P<0.01) . The perceived patient safety culture of Emergency Department nurses has a mediating effect between their job burnout and safety behavior, accounting for 34.90% of the total effect. Conclusions:Nurses in Emergency Department of Class Ⅲ hospitals have moderate job burnout, and there is room for improvement in their safety behaviors and perceived patient safety culture. In the relationship among the three, perceived patient safety culture has a certain mediating effect between job burnout and safety behavior of nurses. In Emergency Department nursing management, a series of measures can be taken to prevent or reduce the job burnout of nurses, strengthen the construction of patient safety culture and other measures to improve the safety of nursing behavior.

Chinese Journal of Practical Nursing 2021;37(12):919-924
Objective:To understand the willingness of breast cancer patients to participate in lymphatic reflux training and management after surgery, and analyze its influencing factors. Methods:A total of 157 postoperative patients with breast cancer were selected by convenience sampling method. The investigation tools included general information questionnaire and lymphedema health belief questionnaire.Results:64.3% (101/157) of the patients were willing to participate in lymphatic reflux training and management. Logistic regression analysis showed that ethnic group ( OR=0.06, 95% CI 0.01-0.68), divorce ( OR=0.08, 95% CI 0.01-0.89), benefit cognition ( OR=12.41, 95% CI 4.26-36.19), obstacle cognition ( OR=3.83, 95% CI 1.48-9.91), behavioral clues ( OR=3.72, 95% CI 1.51-9.20) were significant influencing factors of parents' willness of participating in lymphatic reflux training and management( P<0.05). Conclusion:The willingness of patients to participate in lymphatic reflux training and management is at a medium level, which was influenced by ethnic group, marital status, health belief of benefits cognition, obstacle cognition and behavioral clues. Attention should be paid to the implementation during health education for the prevention of lymphedema, and patient′s willness of participating in lymphatic reflux training and management should be improved.

Objective:To explore emergency nurses' experiences and feelings about nursing interruption events and measures to avoid or reduce work interruption events, so as to provide references for emergency nursing managers to strengthen the management of nursing interruption events. Methods:Using methods of descriptive qualitative research and purposive sampling, a total of 12 nurses from 3 nursing units in Emergency Department of a ClassⅢ Grade A hospital in Chengdu were selected for semi-structured interviews from August to September 2019. The obtained interview data were analyzed by thematic analysis. Results:Through thematic analysis, the extracted themes included the source of the interruption events, the nurses' current affairs at the time of occurrence, effects and coping strategies. Conclusions:The occurrence of emergency nursing interruption events has more adverse effects on emergency nurses. Nursing managers should take various measures from multiple perspectives to reduce the occurrence of nursing interruption events and avoid adverse nursing events to ensure patient safety.

Objective To understand the rehabilitation of postoperative patients with breast diseases, and to find out the common problems and caring key points,in order to provide a reference for the continuous nursing for breast disease patients after the discharge.MethodsA total of 1583 patients who underwent surgical treatment in thyroid and breast surgical department in West China Hospital of Sichuan University from June 2015 to July 2016 were selected by purposive sampling method. The all patients were followed up on telephone at the time point of 1 month, 3 months, 6 months and 12 months after discharge. The follow-up was about patients' rehabilitation (wound, functional exercise, self-care ability and drainage), mental state, satisfaction with follow-up and so on.Results Female accounted for 1574 of the follow-up cases (99.4%). The average age of the patients was (47.20±11.97) years old. The proportion of malignant tumors was 74.6%. The average body mass index (BMI) was (22.66±2.5)kg/m2. The average surgery waiting days and the average hospitalization days were (4.12±2.40) and (8.54±3.40) respectively. The follow-up loss rate at 1 month, 3 months, 6 months and 12 months after discharge was 2.27% (36/1583), 1.90% (22/1158), 2.93% (26/888) and 4.89% (9/184) respectively. The differences of benign and malignant diseases between patients with different BMI and age were statistically significant (P<0.05).ConclusionsTelephone follow-up is a simple and effective way to understand the rehabilitation of patients, and is worthy of clinical use. Follow-up results suggests that caregivers should concern about postoperative wound healing, affected upper limb functional rehabilitation, and mental health in patients with breast diseases, so as to take effective measures to improve patients life quality according to actual problems.
