1.Efficacy of flexible negative pressure ureteral access sheath plus disposable flexible ureteroscope versus SMP in the treatment of 2-3 cm renal calculi

		                        			Objective To explore the efficacy of negative pressure ureteral access sheath combined with disposable flexible ureteroscope(UAS+FRUS)in the treatment of renal calculi of 2-3 cm,so as to provide reference for the treatment.Methods A retrospective analysis was conducted on 130 cases of renal calculi of 2-3 cm treated with surgery in Xiamen Third Hospital during Sep.2021 and Sep.2023,including 68 cases with UAS+FRUS and 62 cases with super-mini percutaneous nephrolithotripsy(SMP).The perioperative indexes and stone-clearance rate(SFR)were compared between the two groups.Results All operations were successful.There were no statistically significant differences in the total SFR and incidence of complications(5.88%vs.9.67%)between the two groups 3 days(88.24%vs.90.32%)and 1 month(91.18%vs.93.55%)after surgery(P>0.05).For patients with lower calyceal calculi with infundibulopelvic angle(IPA)<45°,the SFR of the UAS+FRUS group was significantly lower than that of the SMP group(57.14%vs.100%,P<0.05).The UAS+FRUS group had a longer operation time than the SMP group[(104.94±8.79)minutes vs.(77.98±6.60)minutes,P<0.001],higher hospitalization costs[(23 112.82±1152.34)yuan vs.(21 975.84±1512.24)yuan,P<0.001],less postoperative decrease in hemoglobin[(6.71±2.07)g/L vs.(9.81±4.80)g/L,P<0.001],and shorter postoperative hospitalization time[(3.28±0.51)d vs.(5.58±0.71)d,P<0.001].The UAS+FRUS group had lower postoperative VAS score at 6,24,and 48 hours than the SMP group[(6.38±0.69)vs.(7.87±0.88);(3.62±0.73)vs.(5.81±0.83)and(3.12±0.33)vs.(3.81±0.60)],with statistical significance(P<0.05).Conclusion Both surgical methods have a high SFR in the treatment of renal calculi of 2-3 cm.SMP has the advantages of short operation time,low hospitalization costs,and high SFR for lower calyx calculi,while UAS+FURS has the advantages of little bleeding,minimal trauma,and short hospital stay.Surgeons can make reasonable choices based on the patients'condition and willingness,combined with their own surgical experience.
		                        		
		                        		
		                        		
		                        	
2.Mechanism study of Sargassum in the treatment of thyroid nodule based on network pharmacology

		                        			Objective:Network pharmacology was used to investigate the mechanism of the Sargassum treating thyroid nodule. Methods:The main active ingredients of Sargassum and the targets of active ingredients were obtained by TCMSP, and thyroid nodule disease targets were obtained through GeneCards and OMIM. The STRING database and Cytoscape 3.2.1 software were used to construct the active ingredients-disease-targets network and protein interaction network (PPI). The GO enrichment and KEGG pathway analysis of the targets were analyzed by using R version 4.0.0 software. Results:Two active ingredients were screened from Sargassum and 59 targets were related to thyroid nodule. Biological function analysis showed that the targets of Sargassum included DNA-binding transcription activator activity, RNA polymerase Ⅱ-specific, ubiquitin-like protein ligase binding. Sargassum played an important role in treating thyroid nodule by the gene targets such as RELA, CASP3, IL6, CASP9, EGFR, VEGFA and other targets mediating the signaling pathways such as PI3K-Akt signaling pathway, Kaposi sarcoma-associated herpesvirus infection and other pathways. Conclusion:The potential multi-target and multi-pathway network mechanism of Sargassum in the treatment of thyroid nodule provided theoretical basis for further research.
		                        		
		                        		
		                        		
		                        	
3.miR-10a inhibits cell proliferation and promotes cell apoptosis by targeting BCL6 in diffuse large B-cell lymphoma.

		                        			The BCL6 (B-Cell Lymphoma 6) gene is a proto-oncogene that is often expressed in diffuse large B-cell lymphomas (DLBCLs). BCL6 loss of function can kill DLBCL cells, demonstrating that BCL6 is necessary for the survival of DLBCL cells and could be a therapeutic target. In this study, we found that BCL6 protein levels were consistently upregulated in DLBCL tissues, whereas its mRNA levels varied randomly in tissues, suggesting that a post-transcriptional mechanism was involved in BCL6 regulation. We used bioinformatics analysis to search for miRNAs, which potentially target BCL6, and identified specific targeting sites for miR-10a in the 3'-untranslated region (3'-UTR) of BCL6. We further identified an inverse correlation between miR-10a levels and BCL6 protein levels, but not mRNA levels, in DLBCL tumor tissue samples. By overexpressing or knocking down miR-10a in DLBCL cells, we experimentally validated that miR-10a directly recognizes the 3'-UTR of the BCL6 transcript and regulated BCL6 expression. Furthermore, we demonstrated that negatively regulating BCL6 by miR-10a suppressed the proliferation and promoted apoptosis of DLBCL cells.

4.fMRI Study of Speech Processing in Presbycusis Patients

		                        			Objective To study the brain functional activation under speech stimulation in presbycusis pa‐tients using blood oxygenation level dependent functional magnetic resonance imaging (BOLD - fMRI) .Methods Eighteen normal hearing young volunteers ,10 normal hearing elderly people and 10 presbycusis patients received fM ‐RI under auditory task .Experimental tasks included monosyllabic verbal stimulating at tone of 1 kHz and sound in‐tensity to 90 dB ± 3 dB(after ambient noise detector Smart Sensor AR844 measurement and calibration) randomly transmitted to left and right ear .Following block design ,each stimulus was repeated twice .The lateralization of brain activation analysis observed changes in hemispheric dominance cases (or ear advantage change) .We observed brain activation analysis and changes of hemispheric lateralization advantage (or ear advantages) .SPM5 software was used to deal with the original pictures in the off - line work station to get fictional maps ,volume and intensity of the acti‐vated brain regions of interest ,and calculate the laterality index (LI) .Results To young people ,regardless of left or right ear stimulation ,the contralateral (region of intrest ,ROI) activation volume and intensity significantly increased compared to ipsilateral ,appearing contralateral hemispheric dominance ,and laterality index were - 0 .58 (left ear stimulation) and 0 .37 (right ear stimulation) .Compared to young people ,elderly people with normal hearing and presbycusis contralateral hemisphere conduction advantage was significantly reduced under monaural stimulation ,and the laterality indexes were - 0 .24 ,0 .28 (the aged group) ,- 0 .18 ,0 .25 (presbycusis group) .Young people received the left and the right ear auditory task respectively ,and the ratio of the total volume of bilateral ROI activation was 1 :2 .06 ,appearing to the right ear advantage (left brain dominance) .While the aged and presbycusis group manifes‐ted as right ear advantage decreased (left ear advantage enhanced) ,whose the total volume ratio was 1 :1 .51 for the old normal subjects and 1 :1 .61 for the hearing impaired seniors .Conclusion Auditory verbal tasks activated brain regions in the superior temporal gyrus and middle temporal gyrus were most obvious .Aging may cause these brain regions to be activated weaker ,appearing to the trend of contralateral hemisphere .The aging and deafness lead to the cortical reorganization .This may be one of the reasons for the declined speech recognition .
		                        		
		                        		
		                        		
		                        	
5.Application of high-fluorescence body fluid cell mode on the platform Sysmex XE-5000 automated blood analyzer in diagnosis of meningeal carcinomatosis

		                        			Objective To evaluate the performance of high-fluorescence body fluid cell (HF-BF)mode on the platform Sysmex XE-5000 automated blood analyzer,and analyse its clinical application value in diagnosis of meningeal carcinomatosis.Methods E-valuated the performance of HF-BF by using precision test and methodology comparison test.Retrospectively analyzed 295 test re-sults of cerebrospinal fluid in Zhongshan Hospital Affiliated to Xiamen University from June 2010 to September 2012.Results HF-BF on the platform Sysmex XE-5000 automated blood analyzer had high precision,and exhibited a good consistency with cytolgical examination.The percentage of high-fluorescence body fluid cell(HF-BF%)in the meningeal carcinomatosis group was higher than that in other cerebral diseases groups,had statistically significant differences (P < 0.05 ).The cut-off value for HF-BF% was 4.3%,while the area under a receiver operating characteristic (ROC)curve (AUC)was 0.933,the sensitivity was 95.2%,and the specificity was 92.7%.When HF-BF% was over 4.3%,it was more likely to detect tumor cell in cerebrospinal fluid cytology.Con-clusion HF-BF is an effective reference index for the early diagnosis of meningeal carcinomatosis and has significant clinical appli-cation value.
		                        		
		                        		
		                        		
		                        	
6.The significance of cerebrospinal fluid lactate dehydrogenase measurement in patients with central nervous system diseases

		                        			Objective To evaluate the levels of LDH in cerebrospinal fluid in patients with different types of central nervous sys‐tem diseases .Methods Cerebrospinal fluid LDH were detected by rate assay in control group and disease group(including patients with acute leukemia ,lymphoma ,brain neoplasms and meningitis) .Results In ptients with acute leukemia ,the cerebrospinal fluid LDH level of central nervous system leukemia(CNSL) group was (28 .68 ± 9 .29)U/L ,which was higher than that of non CNSL group ,and the difference were significance(P<0 .05) .In lymphoma patients ,cerebrospinal fluid LDH level of brain metastasis pa‐tients was (125 .20 ± 115 .24)U/L ,which was higher than that of lymphoma patients without brain metastasis ,the difference was significant(P<0 .05) .In patients with brain neoplasms ,cerebrospinal fluid LDH level of meningeal carcinomatosis group was (77 . 37 ± 128 .15)U/L ,which was higher than that of primary brain neoplasms group ,the difference was significant(P<0 .05) .In pa‐tients with meningitis ,cerebrospinal fluid LDH level of tuberculous meningitis group and purulent meningitis group patients were (54 .48 ± 84 .60U/L) and (43 .54 ± 32 .05)U/L respectively ,which were higher than those of viral meningitis group patients ,and the differences were significant (P<0 .05) .Conclusion LDH in cerebrospinal fluid can be used in early diagnosis of CNSL ,brain metastsisly of mphoma ,meningeal carcinomatosis and differential diagnosis of meningitis .
		                        		
		                        		
		                        		
		                        	
7.The effects of Kidney-deficiency on auditory cortex metabolism in the elderly: a 1H-MRS study

		                        			Objective To investigate the effects of kidney-deficiency on auditory cortex metabolism in the elderly by using the proton magnetic resonance spectroscopy(1H-MRS).Methods 50 volunteers,including 20 young subjects and 30 older subjects matched for gender,were measured by pure tone audiometry、“kidney-deficiency”-related test and were executed 3.0 T multi-voxel 1H-MRS tests,The ratios of N-acetylaspartate (NAA),choline (Cho) and lactate (Lac) to creatine (Cr) were measured and were compared between the two groups.The t-test analysis were used for statistic process.The relationship between auditory cortex metabolism and the symptom of kidney-deficiency was also analyzed.Resnlts The result of kidney-deficiency-related test and pure tone audiometry revealed that there was significant difference between the two groups and the older subjects had the more severity kidney-deficiency and high-frequency hearing loss (t=6.335、19.558,P<0.05).The NAA/Cho ratio in the auditory cortex in the older group was significantly lower than that in the younger group (t=2.90,P<0.05).There were no significant differences between two groups for the ratios of NAA/Cr and Cho/Cr (t=1.415、0.927,P>0.05).Decrease of the ratio of NAA/Cho in the older group is significantly related with kidneT-deficiency(r=- 0.773,P=0.025).Conclusion kidney-deficiency play a very important role in the hearing loss and reduction of the number of neurons in auditory cortex of older healthy subjects.It suggests one possible underlying mechanism for the speech perception difficulties exhibited by aging adults.
		                        		
		                        		
		                        		
		                        	
8.The hemispheric lateralization of the auditory cortex after being stimulated by pure tone: a 1H-MRS study

		                        			Objective To explore the asymmetry of metabolic products of the auditory cortex in normal human brain after being stimulated by the pure tone with the proton magnetic resonance spectroscopy(1 H-MRS). Methods Twelve healthy volunteers, including 7 males and 5 females, were performed MRS tests before and after the stimulation of the pure tone of 90 dB in intensity and 1000 Hz in frequency. The the metabolites peak of the N-acetylaspartate (NAA) ,creatine(Cr) ,choline( Cho) , glutamine/glutamate ( Glx) and GABA in auditory cortex were investigated, The content of metabolic products were conducted with half-quantitative analysis. The differences of the NAA/( Cho + Cr) ,Glx/Cr and GABA/ Cr ratios before and after pure tone stimulation in the bilateral auditory cortex were analyzed. The difference of distribution of the metabolic products between the left and right auditory cortex was also analyzed to explore the asymmetry of the auditory cortex. Results The NAA/( Cho + Cr) and GABA/Cr ratios increased,the Glx/Cr ratio decreased in the left auditory cortex after being stimulated by the pure tone. There were significant differences in the NAA/( Cho + Cr) , Glx/Cr and GABA/Cr ratios before and after pure tone stimulation (P<0.05) ;the GABA/Cr ratio decreased,the Glx/Cr and NAA/( Cho + Cr) ratios increased in the right auditory cortex after being stimulated by the pure tone, and statistical difference of the GABA/Cr ratio was found before and after pure tone stimulation (P<0. 05) ,however the difference of the Glx/Cr and NAA/(Cho + Cr) ratios before and after pure tone stimulation was not significant (P>0.05). There were statistically significant differences in the Glx/Cr ratio of the auditory cortex between two sides after being stimulated by the pure tone. Conclusion The metabolic lateralization exists in auditory cortex of normal human brain after being stimulated by the pure tone, which may be the bases of the functional asymmetry.
		                        		
		                        		
		                        		
		                        	
9.Correlation between serum visfatin levels and gestational diabetes mellitus

		                        			Objective To investigate the variations of serum levels of visfatin in pregnant women with different types of glucose intolerance and the associations with insulin resistance.metabolic parameters and newborn birth weight.Methods Totally,120 pregnant women in late trimester were divided into 3 groups according to the results of 50 g glucose challenge test(GCT)and 75 g oral glucose tolerance test:GDM group(n=45),GCT(+)group(n=20),and NGT group(n=55).Fasting levels of visfatin,insulin,total cholestrol and triglyceride were measured and the newborn birth weight was recorded.Results The serum visfatin level was significantly elevated in GDM group compared with the NGT group[(374.9±23.1)ng/ml vs(308.9±16.8)ng/ml,P<0.05],but no significant difference was shown when compared with the GET(+)group[(346.5±23.1)ng/ml].Furthermore,serum visfatin levels had no relationship with insulin resistance,metabolic parameter and newbom birth weight.Conclusions Visfatin is elevated in GDM women and closely related with GDM,but not with insulin resistance,metabolic parameters and newborn birth weight.
		                        		
		                        		
		                        		
		                        	
10.Correlation between the expression of gastrncolic omentum visfatin mRNA and gestationul diabetes mellitus

		                        			Objective To investigate the relationship between different expression of gastrocolic omentum visfatin (VF) mRNA and gestational diabetes mellitus (GDM) in late trimester pregnancy. Methods One hundred cases of pregnant women in late trimester were divided into two groups, including 45 GDM women( GDM group) and 55 normal glucose tolerance women( NGT group). Semi-quantitative RT-PCR was applied to determine the expression of VF mRNA in gastrocolie omentum. Fast-blood glucose, insulin,cholesterol(TC) and triglyceride (TG) levels were measured. The progestational body mass index(BMI) and the homeostasis model assessment(HOMA) insulin resistance(IR) were calculated in these cases. Results The expression of gastrocolic omentum VF mRNA in GDM group and NGT group were 0.8 ±0.4,0.5±0.3. Fast-blood glucose levels were(4.12±0.14),(3.65±0.13) mmol/L. Insulin levels were (72±5) ,(61±5) pmo]/L TG levels were (5.6±0.3), (3.8±0.3) mmol/L TC levels were(5.6±0.9 ), (3.9±0.3) mmol/L HOMA-IR were 12.5±5.9,9.5±0.8. The progestatianal BMIs were(22.6±0.8), (20.9±0.4) kg/m2. All the indices in the GDM group were significantly higher than NGT group (P<0.05). Furthermore, the expression of gastrocolic omentum VF mRNA had a positive correlation with BMI (r=0.32, P<0.01), but had no association with insulin resistance or metabolic parameters.
