1.Clinical Observation of Guben Quyu Jiedu Prescription in Treating Nocturnal Hypoxemia of COPD Combined with OSAHS

		                        			ObjectiveTo observe the clinical efficacy and safety of Guben Quyu Jiedu prescription in treating nocturnal hypoxemia of chronic obstructive pulmonary disease (COPD) combined with Obstructive sleep apnea hypopnea syndrome ( OSAHS ) (deficiency of lung, spleen and kidney with blood stasis and toxicity). MethodsThe paper used a forward-looking, random double-blind, placebo-controlled design method to select 96 patients with COPD combined with OSAHS, and their traditional Chinese medicines (TCM) syndrome differentiation was deficiency of lung, spleen and kidney with blood stasis and toxicity. These patients were randomly divided into the observation group and the control group, with 48 cases in each group. Based on conventional Western medicine treatment, the observation group was treated with Guben Quyu Jiedu prescription and the control group was treated with traditional Chinese medicine placebo. Both courses of treatment were 90 days. Then the paper compared the changes in minimum pulse oxygen saturation (SpO2) during the night, apnea index (AHI), OSAHS efficacy evaluation, TCM syndrome efficacy evaluation, and TCM symptom score before and after treatment between the two groups. ResultsThere were 5 withdrawals in the observation group and 8 withdrawals in the control group, so 43 cases in the observation group and 40 cases in the control group completed the trial. Compared with the condition before treatment, the minimum SpO2 during the night and AHI in the observation group were significantly improved at night (P<0.01) and were better than those in the control group (P<0.01). OSAHS efficacy in the observation group was better than in the control group (χ2=7.085, P<0.05). In terms of TCM syndrome efficacy, the total effective rate was 81.40% (35/43) in the observation group, significantly higher than that in the control group, which was 15.00% (6/40) (χ2=36.78, P<0.01). The TCM symptom scores of the two groups were improved compared with the condition before treatment, and the effect of the two groups was similar in the four main symptoms of snoring, choking, lethargy, and cough. However, the observation group was better than the control group in 10 details such as dizziness, headache, chest tightness, chest pain, wheezing, dry mouth, and thirst (P<0.05). ConclusionUsing Guben Quyu Jiedu prescription combined with conventional Western medicine can treat COPD combined with OSAHS hypoxemia at night (deficiency of lung, spleen and kidney with blood stasis and toxicity). In this way, the minimum pulse oxygen saturation (SpO2) of patients, the level of disease control, and the quality of life of patients can be improved, and the clinical symptoms can be relieved. 
		                        		
		                        		
		                        		
		                        	
2.Sleep status and influencing factors among higher vocational college students

		                        			BackgroundSleep disorder can adversely affect human physical and mental health, with important implications for socioeconomic development. And higher vocational college students are subject to various pressures and stimuli exerted by academic studies, job seeking and family socioeconomic status, which may contribute to the onset insomnia symptom or sleep disorder. ObjectiveTo discuss the sleep status and influencing factors among higher vocational college students, and to provide references for improving their sleep status. MethodsIn January and February, 2022, a stratified random sampling method was utilized to enroll 3 300 students from five higher vocational colleges in Wenjiang district of Chengdu city. All the participants were assessed using Insomnia Severity Index (ISI), Patient Health Questionnaire Depression Scale-9 item (PHQ-9) and Generalized Anxiety Disorder Scale-7 (GAD-7). Pearson correlation analysis was conducted to test the correlation of ISI with PHQ-9 and GAD-7 scores, and the Logistic regression analysis was performed to identify the influencing factors of insomnia symptom in higher vocational college students. ResultInsomnia symptom was detected in 81.90% (2 497 / 3 300) of higher vocational college students. The detection rate of insomnia symptom among vocational college students yielded statistical difference in terms of gender, family socioeconomic status, being an only child or having siblings, degree of psychological pressure exerted by academic studies or job seeking, amount of hours spent on Internet per day, frequency of physical exercise, and the presence or absence of anxiety symptom and depression symptom (χ2=21.032, 22.172, 8.983, 75.939, 36.781, 32.350, 54.512, 86.561, P<0.01 or 0.05). Among higher vocational college students, ISI score was positively correlated with GAD-7 and PHQ-9 scores (r=0.620, 0.714, P<0.01), and GAD-7 score was positively correlated with PHQ-9 score (r=0.824, P<0.01). Gender, family socioeconomic status, degree of psychological pressure exerted by academic studies or job seeking, amount of hours spent on Internet per day, frequency of physical exercise, and the presence or absence of anxiety symptom and depression symptom were effective factors in predicting the insomnia symptom in higher vocational college students (P<0.01 or 0.05). ConclusionFemale gender, average and below average family socioeconomic status, moderate-to-severe degree of psychological pressure exerted by academic studies or job seeking, spending 2~5 h, 5~7 h or >7 h on Internet per day, inadequate physical exercise and presence of depressive symptom are the risk factors for insomnia symptom in higher vocational college students.[Funded by Sichuan Provincial Primary Health Development Research Center in 2022, North Sichuan Medical College (number, SWFZ22-C-89)]

		                        			ObjectiveTo investigate effect of conducting training of autism spectrum disorder (ASD) early screening skill on improving the ability to early identify ASD of medical staffs in primary care hospitals. MethodsIn September 2021, the training of ASD early screening skills was carried out for medical staffs from 20 primary care hospitals in Chengdu. After training, the training effect was evaluated. The numbers of referrals from primary care hospitals to superior hospitals, confirmed ASD as well as their average diagnostic age of children with ASD before and after training were used as evaluation indicators. ResultsAfter training, the number of children with suspected ASD referred by primary care hospitals was more than that before training [(16.65±11.60) vs. (3.40±2.23), t=5.431, P<0.01], the number of children diagnosed with ASD was more than that before training[(6.85±4.93) vs. (2.45±1.67), t=4.171, P<0.01], and the differences were statistically significant. As for the diagnosed age of ASD children, after training, the average age was lower than that before training [(34.95±11.67) vs. (42.2±14.64), t=-2.553, P=0.019]. 4.Depressive symptoms detection among the urban elderly in Ya'an city and its influencing factors six years after Lushan earthquake

		                        			ObjectiveTo investigate the prevalence and characteristics of depressive symptoms among urban elderly six years after Lushan earthquake in Ya'an, so as to provide references for the mental health interventions for elderly following catastrophic stressful life events. MethodsFrom March to April 2019, a multi-stage stratified cluster random sampling method was adopted to enroll 885 urban elderly people aged 60 and above in Ya'an. A self-compiled questionnaire was used to collect the general demographic information, health-related and disaster-related information, meantime, the elderly was assessed using Geriatric Depression Scale (GDS-30). Thereafter, univariate and multivariate Logistic regression were applied to explore the influencing factors of depressive symptoms in urban elderly. ResultsA total of 783 valid questionnaires were collected, with a questionnaires response rate of 88.47%. Depressive symptoms were detected in 161 cases (20.56%). The prevalence of depression showed statistical differences among the elderly of different gender, age, marital status, family relationship, monthly per capita household income, physical exercise status, health status, self-care ability, sleep status and disaster-affected degree (P<0.05 or 0.01). Logistic regression analysis showed that the urban elderly of the female gender (OR=1.552, P=0.040), monthly per capita household income of 2000~3000 yuan (OR=6.982, P<0.01), monthly per capita household income≤2000 yuan (OR=6.857, P<0.01), lack of physical exercise (OR=1.693, P<0.01), being less capable of self-care (OR=3.838, P<0.01), being incapable of self-care (OR=8.547, P<0.01), complicating multiple curable diseases (OR=4.892, P<0.01) and complicating refractory chronic diseases (OR=5.657, P=0.031) were at high risk of depressive symptoms. The risk of depressive symptoms among the divorced or widowed elderly was greater than that among married elderly (OR=0.063, P<0.01). ConclusionThe prevalence of depressive symptoms is relatively high among the urban elderly six years after Lushan earthquake in Ya'an, moreover, female gender, low monthly per capita household income, lack of physical exercise, being incapable of self-care and poor health status are risk factors affecting the depressive symptom, while being married is a protective factor. 
		                        		
		                        		
		                        		
		                        	
5.Epidemiological analysis of 286 clustered cases of rotavirus infection in Hohhot Clustering analysis of 286 cases of rotavirus infection in Hohhot

		                        			Objective   To identify the pathogen and origins of 286 clustered cases of rotavirus infection in Hohhot, and to provide reliable basis for formulating preventive measures of rotavirus infection outbreaks.   Methods   Epidemiological investigation was conducted on 286 rotavirus outbreaks involving 8,560 cases in Hohhot from December 2017 to December 2020. Reverse transcription polymerase chain reaction (RT-PCR) was used to detect the genotypes, and the related risk factors were analyzed.   Results   Of the 286 rotavirus outbreaks, 165 were clustered outbreaks, and the duration was 5 to 8 days, with an average of (7.05±1.06) days, presenting temporal and spatial clustering. The causes of the outbreaks included rotavirus contamination in drinking water, improper management of household infections, and inadequate hospital preventive measures. The total number of cases was 8,560, and the positive rate of rotavirus was 47.17% (4 038/8 560). Group A and B rotavirus strains were detected in vomit or stool samples. Rotavirus infection in children was mostly caused by group A rotavirus (100%) and in adults by group B rotavirus (97.59%), and the common symptoms included diarrhea, vomiting and fever. The infection rate of children (87.67%, 3,540/4,038) was significantly higher than that of adults (12.33%, 4,98/4,038) (χ2=115.520, P<0.05). Rotavirus infection occurred most frequently in autumn and winter.    Conclusion   Clustered outbreaks account for most of rotavirus infection events, and children are the key targets of prevention and treatment. 6.Correlation between BMI, physical activity, resting heart rate and diabetes mellitus

		                        			Objective   To analyze the relationship between BMI, physical activity, resting heart rate and diabetes mellitus.   Methods  A total of 423 patients with diabetes admitted to the Department of endocrinology of our hospital from August 2017 to August 2020 were investigated by questionnaire and physical signs were examined, and the correlation between BMI, physical activity, resting heart rate and diabetes was analyzed.   Results   FPG, 2 h PG and HbA1c were positively correlated with resting heart rate, BMI and physical activity (P < 0.05). Multiple logistic regression analysis showed that BMI (OR = 2.121), elevated resting heart rate (OR = 1.619) and less physical activity (OR = 0.711) were risk factors for diabetes.   Conclusion   The increase of BMI and resting heart rate and the decrease of physical activity will increase the risk of diabetes. 7.Analysis of immune regulation and prognosis of chronic hepatitis B virus infection

		                        			Objective   To investigate the immune regulation and prognosis of hepatitis B virus (HBV) infection in infected patients, and to analyze the relationship between immune regulation and clinical outcome.   Methods   The data of 633 cases of HBV infection in Huhhot from January 2015 to December 2018 were retrospectively analyzed. At the same time, healthy people from the Affiliated Hospital of Inner Mongolia Medical University were selected as the control group. Immune cell regulatory factors and lymphocyte subsets in blood of HBV infected and healthy people were tested. The results of one-year clinical outcomes were calculated. The relationship between immune cell regulatory factors and lymphocytes and the clinical outcomes of patients was analyzed by multiple logistics regression.   Results   The levels of CD4+T cells, CD8+T cells, NK cells, IL-2, and IL-12 in the blood of patients with HBV infection were significantly lower than those in the control group, and decreased as the patients’ clinical outcome deteriorated (P<0.05). The levels of IL-4 and IL-10 in the blood of patients with HBV infection were significantly higher than those in the control group, and increased significantly with the deterioration of the clinical outcome of the patients (P<0.05). There was no significant difference in the levels of INF-γ, B cells and CD4+/CD8+ between the two groups (P>0.05). Low CD4+ T cells, low CD8+ T cells, low NK cells, low IL-2, low IL-12, high IL- 4 and high IL-10 were independent risk factors leading to the deterioration of clinical outcomes (P<0.05).   Conclusion   The immune regulation of patients with HBV infection is in a state of suppression, which is an independent factor affecting the clinical outcome of patients.
		                        		
		                        		
		                        		
		                        	
8.Clinical characteristics and related factors of rheumatoid arthritis complicated with tuberculosis infection.

		                        			OBJECTIVE:
		                        			To investigate the clinical characteristics and high risk factors of Rheumatoid arthritis (RA) complicated with tuberculosis infection.
		                        		
		                        			METHODS:
		                        			Patients with rheumatoid arthritis diagnosed in the hospital of Sichuan Provincial People's Hospital from January 2007 to January 2017 was retrospectively collected, who were enrolled in the study group. A control group was randomly selected from the RA patients hospitalized in the same period without co-infection at a ratio of 1 :2. The general data, clinical data, laboratory test data, treatment plan, etc. of the two groups were collected in detail for single factor statistical analysis. Then multivariate Logistic regression was used to analyze the independent risk factors of RA complicated with tuberculosis infection with statistical significance in univariate analysis.
		                        		
		                        			RESULTS:
		                        			The clinical manifestations of fever (83.3%) were most common, followed by cough (69%) and body mass loss (45.2%). In the infected group, pulmonary tuberculosis accounted for 73.3%. In the infected group the chest CT showed two or more cases, accounting for 59%. There were 9 cases (33.3%) occurring in the typical tuberculosis occurrence site. Compared with the control group, the erythrocyte sedimentation rate (ESR), C-reaction protein (CRP) levels, and the daily average dose of glucocorticoid in 1 year in the infected group were higher than those in the control group. And those differences were statistically significant(P < 0.05). There were no significant differences in gender, age, disease duration, disease activity score, white blood cell (WBC), platelet (PLT), hemoglobin (Hb), immunoglobulin G (IgG), complement (C), Anti cyclic citrullinated peptide antibody (anti-CCP), CD4+T cell count, and immunosuppressant use (P > 0.05). Multivariate Logistic regression analysis showed that CRP levels(OR=1.016, 95%CI:1.002-1.031) and the daily average dose of glucocorticoid in 1 year(OR=1.229, 95%CI:1.066-1.418)were the independent risk factors of RA complica-ted with tuberculosis infection.
		                        		
		                        			CONCLUSION
		                        			RA patients with tuberculosis infection are mainly phthisis. The clinical manifestations of RA combined with tuberculosis infection are lack of specificity, and the chest imaging features of pulmonary tuberculosis are diverse, which are easy to be misdiagnosed. CRP levels and the daily average dose level of glucocorticoid in 1 year were risk factors for RA and tuberculosis infection.

9.Efficacy and influence factors of uterine artery embolization in treatment of intractable postpartum hemorrhage

		                        			Objective To investigate the efficacy and influence factors of uterine artery embolization (UAE)in treatment of intractable postpartum hemorrhage (PPH).Methods 126 patients with intractable PPH were treated by UAE in our hospital.We analyzed the influence factors of failed UAE treatments according to the amount of bleeding,the stability of hemodynamics,with disseminated intravascular coagulation(DIC)or not and active extravasation detected in angiography.Results In 126 intractable PPH patients,13 cases (10.3%) failed to stop bleeding after UAE and the other 113 cases (89.7%)successfully got hemostasis.Logistic regression analysis showed that DIC was a significant factor in failed UAE group (P=0.033,OR 0.107,95%CI 0.014-0.835).Conclusion UAE is an effective method of treating intractable PPH.DIC may be the main cause of the failure of UAE in treatment of intractable PPH.
		                        		
		                        		
		                        		
		                        	
10.The contrast research of DSA rotating reconstruction technology and CT guided biopsy of chest tumor

		                        			Objective To evaluate the value of clinical application in chest tumor biopsy by DSA rotating reconstruction technology and CT guided technology.Methods The 63 cases of chest tumors collected,38 cases of lung tumors,25 cases of mediastinal tumors.Applicating of CT and DSA scan and formulating puncture path,according to their respective descending thoracic tumor puncture guide way.DSA and CT guided traditional way from the success rate of puncture,operation time,X-ray exposure and complications were analyzed.Results 45 cases take CT guidance,18 cases take DSA guidance.60 cases,the pathological results achieved and positive rate was 95.2%.CT group success rate of first puncture in 68.9% (31/45),DSA group 88.9% (16/18),statistically significant.CT group pneumothorax rate 17.8 % (8/45),DSA group pneumothorax rate 11.1% (2/18),no statistical difference.Operation time CT group (23.111 ± 4.281) minutes,DSA group (12.889 ± 3.693) minutes,with significantly statistical differences.CT group exposure (11.563±3.023) mGy,DSA group exposure (9.956±2.193) mGy,statistically significant.Conclusion The chest tumor puncture biopsy by DSA rotating reconstruction technique guided with flexible,fast and convenient,and superior to traditional CT with real-time monitoring,shorten the operation time,reduce the exposure,the success rate of first puncture aspects.
