1.The triglyceride glucose index predicts the risk of nonfatal cardio-cerebrovascular disease in the Beijing community: a prospective cohort study

Objective:To explore the characteristics of the association between the triglyceride glucose (TyG) index and nonfatal cardio-cerebrovascular disease risk in a community population.Method:This was a prospective cohort study. From December 2011 to April 2012, the first investigation was conducted among subjects with more than 40-year old who were from Shijingshan district and Pingguoyuan community in Beijing. The second investigation was conducted from April to October 2015. All the subjects were divided into three groups according to the tertile of the TyG index at baseline. The multivariate Cox proportional risk regression model was established to explore the correlation between the TyG index and nonfatal cardio-cerebrovascular disease risk and the Kaplan-Meier survival curve of the TyG index group was drawn. Subgroup analyses were performed according to age, gender, body mass index, type 2 diabetes mellitus (T2DM), hypertension, and hyperlipidemia to determine the correlation characteristics between the TyG index and nonfatal cardio-cerebrovascular disease among subgroups.Results:A total of 9 577 subjects were finally included to analyze. The mean follow-up time of this study was (34.14±3.84) months. During the follow-up, 363 subjects (3.8%) occurred nonfatal cardio-cerebrovascular disease. The multivariate Cox regression analysis results showed that the hazard ratio ( HR) of nonfatal cardio-cerebrovascular disease in the high TyG index group was 1.54 (95% CI 1.19-1.98), 1.60 (95% CI 1.23-2.10), and 1.57 (95% CI 1.20-2.05) in the three models, compared with the low TyG index group. The Kaplan-Meier analysis showed that the risk of nonfatal cardio-cerebrovascular disease increased from the low-TyG index group to the high-TyG index group ( P=0.015). In the six subgroups analysis, only gender was shown to have a significant interaction effect with the TyG index and nonfatal cardio-cerebrovascular disease risk. In the female population, the risk of nonfatal cardio-cerebrovascular disease is significantly increased with the increase in the TyG index level ( P<0.001). Conclusions:A high TyG index is independently related to the increased risk of nonfatal cardio-cerebrovascular disease in the Beijing community population. Gender has a significant interaction with the TyG index and nonfatal cardio-cerebrovascular disease risk. Therefore, the TyG index may be a useful marker to predict the nonfatal cardio-cerebrovascular disease risk of a community population.
2.Protopanaxadiol-type ginsenoside hydrolases and their application in the preparation of ginsenoside Compound K: a review.

Ginsenoside Compound K (CK) has anti-cancer and anti-inflammatory pharmacological activities. It has not been isolated from natural ginseng and is mainly prepared by deglycosylation of protopanaxadiol. Compared with the traditional physicochemical preparation methods, the preparation of CK by hydrolysis with protopanaxadiol-type (PPD-type) ginsenoside hydrolases has the advantages of high specificity, environmental-friendliness, high efficiency and high stability. In this review, the PPD-type ginsenoside hydrolases were classified into three categories based on the differences in the glycosyl-linked carbon atoms of the hydrolase action. It was found that most of the hydrolases that could prepare CK were PPD-type ginsenoside hydrolase type Ⅲ. In addition, the applications of hydrolases in the preparation of CK were summarized and evaluated to facilitate large-scale preparation of CK and its development in the food and pharmaceutical industries.

3.Value of nomogram based on preoperative ultrasound and inflammatory indexes in predicting axillary high nodal burden in early breast cancer

Objective:To explore the values of ultrasound, pathology combined with inflammatory indicators in predicting high nodal burden (HNB) in patients with early breast cancer and to construct a nomogram to provide reference for individualized diagnosis and treatment.Methods:The ultrasonographic, pathological features and preoperative inflammatory indicators of 378 female patients diagnosed with early breast cancer confirmed by pathology in the South Hospital of the Sixth People′s Hospital Affiliated to Shanghai Jiaotong University from January 2014 to July 2022 were retrospectively analyzed. They were randomly divided into training set ( n=302) and test set ( n=76) in a ratio of 8∶2, and the baseline data of the two groups were compared. The optimal cutoff values of neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR) and lymphocyte to monocyte ratio (LMR) were obtained by ROC curve. In the training set, with axillary high lymph node load (≥3 metastatic lymph nodes) as the dependent variable, independent influencing factors of HNB were identified by univariate and multivariate Logistic regression analyses, and the nomogram was established. The test set data were used to verify the model. The discrimination, calibration and clinical applicability of the model were assessed by the area under the ROC curve (AUC), C-index, the calibration curve, Brier score and the decision curve analysis, respectively. Results:There were no significant differences in all variables between the training set and the test set (all P>0.05). ROC curve analysis results showed that AUCs of NLR, PLR and LMR were 0.578, 0.547 and 0.516, respectively, and the optimal cut-off values were 2.184, 150 and 3.042, respectively. Univariate Logistic regression analysis showed that age, pathological type, histological grade, Ki-67, lymphovascular invasion, NLR, PLR, ultrasonic characteristics (maximum diameter of primary tumor, shape, long/short diameter of lymph node, cortical thickness, cortical and medullary boundary, lymph node hilum, lymph node blood flow pattern) were correlated with HNB of early breast cancer (all P<0.05). Multivariate Logistic regression analysis showed that ultrasonic characteristics (maximum diameter of primary tumor >2 cm, effacement of lymph node hilum, non-lymphatic portal blood flow), lymphovascular invasion, Ki-67>14% and NLR>2.184 were independent risk factors for HNB in early breast cancer ( OR=7.258, 8.784, 6.120, 8.031, 3.394 and 3.767, respectively; all P<0.05) and were used to construct the nomogram model. The AUC of the training set was 0.914 (95% CI=0.878-0.949), C-index was 0.914; The AUC of the test set was 0.871 (95% CI=0.769-0.973), C-index was 0.871, indicating good discrimination. Calibration curve and Brier score were 0.090, indicating high calibration degree of the model. The clinical decision curve indicated good clinical benefit. Conclusions:The nomogram based on ultrasonic characteristics (maximum diameter of primary tumor, lymph node hilum, lymph node blood flow pattern), lymphovascular invasion, Ki-67 and NLR can effectively predict the risk of HNB in patients with early breast cancer, and provide a reference for precision diagnosis and treatment to avoid excessive or insufficient treatment.
4.Construction of intelligent fall warning model for the elderly based on plantar pressure

Objective:To realize the accurate prediction of the fall risk of the elderly by a convolutional neural network prediction model.Methods:Stratified random cluster sampling was used from June 2019 to February 2020, Python′s Matlabplot library and Opencv library were used to perform data preprocessing on the plantar pressure data of 1 093 subjects who had come from medical institutionsand community or elderly care institutions of Chongqing and Nanjing, such as data visualization, compression and clipping, grayscale, Gaussian blur, etc., and then the data were divided into training set (983 cases) and verification set (110 cases), the training set data were used to train the convolutional neural network model, the verification set was used to verify the model, and the ReLU function was used to suppress overfitting to obtain the final prediction model.Results:The sensitivity of the fall warning model to the validation set for predicting falls was 91.2%, the specificity was 81.4%, the accuracy was 91.5%, and the AUC was 0.865.Conclusions:The fall prediction model can predict the fall risk of the elderly in a specific scenario. In the subsequent improvement, the software and hardware construction should be comprehensively improved to further improve the prediction accuracy.
5.Current situation and influencing factors of humanistic care needs of maternal family members in maternal intensive care unit

Objective:To investigate the current situation and influencing factors of humanistic care needs of family members of pregnant women in maternal intensive care unit, and to explore the relationship between humanistic care needs of family members of pregnant women in maternal intensive care unit, relocation stress level and perceived social support ability, so as to provide a basis for clinical nursing staff to implement targeted humanistic care for family members of pregnant women in maternal intensive care unit.Methods:From July to December 2022, 267 family members of pregnant women who were observed in the Maternal Intensive Care Unit of Maternity Hospital Affiliated to Nanjing Medical University/Nanjing Maternal and Child Health Hospital were selected as the research objects by the convenient sampling method. The general information questionnaire, Humanistic Care Needs Scale for Family Members of Pregnant Women in the Obstetric Intensive Care Unit, Family Relocation Stress Scale for Intensive Care Unit Patients and Perceived Social Support Scale were used to carry out a cross sectional investigation.Results:The scores of humanistic care needs, relocation stress scale and perceived social support scale were (175.32 ± 16.04), (35.12 ± 8.11), (57.30 ± 15.43) points, respectively. The length of maternal intensive care unit stay ( B=1.301, P<0.05), the family′s role changed for the first time ( B=2.328, P<0.05), the delivery mode doesn′t match the family′s expectations ( B=-2.407, P<0.05), maternal admission to maternal intensive care unit due to childbirth complications ( B=3.228, P<0.05), relocation stress level of intensive care unit patients′ family members ( B=0.891, P<0.05), and family members′ perceived social support ability ( B=0.461, P<0.05) were the influencing factors of humanistic care needs of maternal family members in maternal intensive care unit factors, which explained 83.2% of the total variation. Conclusions:The humanistic care needs of family members of pregnant women in maternal intensive care unit are at a high level. Medical staff should pay more attention to the family members of pregnant women who stay in maternal intensive care unit for a long time, undergo role change for the first time, have unexpected delivery mode and stay in maternal intensive care unit due to childbirth complications, so as to provide them with more comprehensive humanistic care and establish multiple support system, in order to improve the level of humanistic care for the family members of maternal intensive care unit.
6.Mediating effect of professional values on stress overload and professional quality of life in oncology nurses

Objective:To explore the mediating effect of professional values on the stress overload and professional quality of life in oncology nurses.Methods:Using the convenient sampling method, a total of 544 oncology nurses from 2 cancer hospitals in Beijing were selected as the research objects. General data questionnaire, Stress Overload Scale (SOS) , The Nurses Professional Values Scale-Revised (NPVSR) , revised version of Nurses Professional Values Scale and Chinese version of Professional Quality of Life Scale were used for questionnaire survey and correlation analysis of the data. AMOS 21.0 software was used to establish a structural equation model to verify the mediating effect.Results:The stress overload score of oncology nurses was 48.00 (36.00, 62.00) , and the professional values score was 102.00 (86.25, 110.00) . The compassion satisfaction score for professional quality of life was 36.00 (31.00, 41.00) , the burnout score was 22.00 (17.00, 26.00) , and the secondary traumatic stress score was 22.00 (19.00, 26.00) . The stress overload of oncology nurses was negatively correlated with professional values, negatively correlated with compassion satisfaction, and positively correlated with job burnout and secondary traumatic stress ( P<0.05) . Structural equation fitting index ( CMIF/ DF) was 0.430, goodness of fit index ( GFI) was 1.000, adjusted goodness of fit index ( AGFI) was 0.995 and root mean square error of approximation ( RMSEA) was 0.000. The stress overload and professional values of oncology nurses had a direct predictive effect on their professional quality of life. The stress overload was mediated by professional values, which indirectly affected compassion satisfaction and job burnout. Conclusions:Nursing managers can enhance their professional values and improve their professional quality of life by reducing the stress overload on oncology nurses.
7.Correlation of prolongation of PR interval with carotid atherosclerosis in middle-aged and elderly patients with type 2 diabetes mellitus

Objective:To investigate the relationship between prolonged PR interval and carotid atherosclerosis(CAS)in middle-aged and elderly patients with type 2 diabetes mellitus(T2DM).Methods:A total of 537 middle-aged and elderly inpatients with T2DM in the Southern Branch of the Sixth People′s Hospital of Shanghai Jiaotong University from January 2019 to January 2021 were selected as the research objects. Color Doppler ultrasound was used to detect bilateral carotid artery intima-media thickness(CIMT). The subjects were divided into carotid atherosclerosis group(CAS group, n=352)and non-carotid atherosclerosis group(NCAS group, n=185). The difference in the PR interval of ECG between the two groups was compared. Pearson or Spearman rank correlation analysis was used for evaluating the correlation of PR interval and CAS lesions with various clinical index. The relationship between PR interval and CAS lesions was adopted by multivariate logistic regression analysis. Results:The average PR interval of middle-aged and elderly patients with T2DM was(164.57±23.02)ms. The average PR interval in CAS group was significantly higher than that in NCAS group [(169.76±24.28) vs (154.70±16.42)ms, P<0.01]. The results of multifactorial logistic regression analysis showed that age, low density lipoprotein-cholesterol, serum osteocalcin, and PR interval were independent factors influencing the development of CAS lesions in middle-aged and elderly patients with T2DM( OR=1.079, 1.936, 0.879, 1.039, respectively, P<0.05 or P<0.01)where each 1 ms increase in PR interval was associated with a 3.9% increase in the risk of CAS in middle-aged and elderly patients with T2DM( OR=1.039, 95% CI 1.006-1.073, P=0.020). Multivariate logistic regression analysis showed that middle-aged and elderly type 2 diabetic patients with PR interval≥158 ms were 4.072 times more likely to have CAS lesions than those with PR interval<158 ms( OR=4.072, 95% CI 1.417-11.702, P<0.01). Conclusion:The PR interval of electrocardiogram is correlated with CAS lesions in middle-aged and elderly patients with T2DM. Middle-aged and elderly type 2 diabetic patients with significantly prolonged PR interval should be reminded to screen for CAS lesions early.
8.Monitoring of adverse reaction of Haemophilus influenzae type b vaccine in Songjiang District, Shanghai

ObjectiveTo determine the reporting rate of adverse reaction of Haemophilus influenzae type b (Hib) containing vaccine in Songjiang District from 2016 to 2020, so as to provide evidence for the inclusion of Hib vaccine in children's immunization planning and safety monitoring. MethodsThe adverse reaction data of Hib vaccination were collected through the Chinese Disease Prevention and Control Information System and presented by descriptive methods. ResultsFrom 2016 to 2020, the reporting rate of adverse reaction of various Hib-containing vaccines in Songjiang District was determined to be 747.76/105 doses, among which the rates of general reaction and abnormal reaction were 734.31/105 doses and 13.45/105 doses, respectively. The reporting rate of general reaction was significantly higher than that of abnormal reaction (χ2=1 400.18,P<0.001). Moreover, the reporting rate of adverse reaction did not differ significantly by sex or registered residence, whereas it differed significantly by age groups (χ2=366.07,P<0.001). Children ≥12 months old had the highest reporting rate, which was caused by higher rate of general reaction (χ2=360.48,P<0.001) compared with other age groups; in contrast, there was no difference in the reporting rate of abnormal reactions across age groups. The reporting rate of adverse reaction differed significantly across four Hib-containing vaccines (χ2=508.51,P<0.001), among which the reporting rate of pentavalent vaccine was the highest, followed by tetravalent vaccine, and Hib vaccine and meningococcal HI vaccine. This difference was mainly caused by general reaction (χ2=499.19,P<0.001). The reporting rate of booster Hib-containing vaccines was significantly higher than that of basic immunization (χ2=462.85,P<0.001). Furthermore, the reporting rate differed between DTaP-Hib vaccine and DTaP-IPV-Hib vaccine by injection sites(χ2=13.63,P=0.001;χ2=78.48, P<0.001); the reporting rate on the thigh was lower than that on the hip or upper arm. Among the 1 501 reported adverse reactions, 97.21% of the general reactions and 85.19% of the abnormal reactions occurred within 72 hours following immunization. Principal clinical diagnosis was fever, redness and nodules (n=1 454), accounting for 96.87% of the total reported adverse event following immunization(AEFI), followed by allergic rash. ConclusionThe four types of Hib-containing vaccines have high safety. Health care practitioners at points of vaccination should improve the pre-examination, standardized operation and full notification in strict accordance with the requirements. It also warrants strengthening the AEFI monitoring, investigation and response, so as to effectively reduce the incidence and severity of adverse reactions.
9.Analysis of the status quo and hotspots of death anxiety research in China based on bibliometrics

Objective:To retrieve and analyze the status quo and hotspots of death anxiety research, and provide directions for further research in death anxiety.Methods:Articles concerning death anxiety in their titles, keywords and abstracts published up to July 10, 2020 in core journals from CNKI were systematically retrieved. The retrieved results were analyzed bibliometrically using the bibliographic information co-occurrence analysis system (Bicomb) , including year of publication, source journal, keywords, and authors, etc. The high-frequency words were extracted by calculation and customization, and a word-part matrix was derived. The word-part matrix was clustered with GCLUTO 1.0 software, and the current status and research hotspots were derived from the specific articles.Results:A total of 79 death anxiety-related papers were published, with 19 key words appearing ≥ 3 times. Four research hotspots of death anxiety were obtained through analysis: death anxiety of nursing staff, death anxiety of terminal patients, development and adaptation of death anxiety scale, and death anxiety and mental health.Conclusions:Death anxiety research has entered the developmental stage; studies are mostly conducted among the aged and caregivers with easily accessible samples; the types of studies are mainly empirical studies and cross-sectional surveys. In the future, we may further break through the limitation of sample accessibility, strengthen cooperation and communication between authors and institutions, and try to conduct multi-center, large-sample, effective intervention studies to alleviate death anxiety and improve the quality of caregiving.
10.Attitude and knowledge on cancer pain management and their influencing factors among oncology department nurses

Objective:To understand the attitude and knowledge on cancer pain management among oncology department nurses and to explore their influencing factors so as to provide a basis for developing targeted pain training projects.Methods:From April to June 2018, we investigated 291 oncology department nurses from 21 provinces, 4 municipalities directly under the central government and 4 autonomous regions on the spot with the self-designed information questionnaire and the Knowledge and Attitudes Survey Regarding Pain (KASRP) by convenience sampling.Results:A total of 276 valid questionnaires were collected. Among 276 nurses, the accuracy rate of KASRP was (59.2±11.0) % ranged from 31.7% to 85.4%. There were statistical differences in the accuracy rates of KASRP among nurses with different areas, years working in cancer nursing, whether attended the pain training, whether participated in cancer pain management practice and monthly number of patients with cancer pain nursing provided by nurses ( P<0.05) . Correlation analysis showed that the age of nurses had a positive correlation with the score of KASRP ( r=0.237, P<0.001) . Multiple linear regression analysis proved that the influencing factors of attitude and knowledge on cancer pain management of nurses included the age of nurses, attending pain management practice, participating in the pain training ( P<0.05) . Conclusions:Among oncology department nurses in China, there are misunderstandings in attitudes on cancer pain management, and their related knowledge was insufficient which influenced by many factors. On the one hand, it points out the importance and urgency for clinical nurses to develop targeted pain management pain. On the other hand, we should provide innovative training methods and combine pain management practice tightly to effectively improve attitude and knowledge on cancer pain management among of nurses so as to make nurses play an important role well in cancer pain management.
