1.High-resolution CT features and follow-up analysis of welder pneumoconiosis

Objective To explore the high-resolution computed tomography(HRCT)imaging findings and prognosis for the welder pneumoconiosis.Methods The HRCT imaging findings and follow-up changes in 11 cases of welder pneumoconiosis diag-nosed by occupational diseases were analyzed retrospectively.Results There were 7 cases(63.7%)showed central lobular nodule shadow,5 cases(45.5%)developed patchy ground glass shadows in the center of the lobule("light ink sign"),5 cases(45.5%)showed branching fine line shadow,2 cases(18.2%)showed interlobular septal thickening,and 3 cases(37.5%)showed better ima-ging performance than before.Conclusion HRCT imaging findings of welder pneumoconiosis mainly include diffuse distribution of nodules in the center of lobules,"light ink sign",and branching fine line shadows in both lungs,with rare hilar and mediastinal lymphadenopathy and calcification.When the image shows nodule shadow or"light ink sign"in the center of the lobule,it is more likely to improve after leaving environment exposure.
2.Predictive value of the product of plasma colchicine concentration and poisoning time for the prognosis of colchicine poisoning patients

Objective:To investigate the predictive value of the product of first plasmacolchicine concentration and poisoning time for the prognosis of colchicine poisoning patients, and to provide a basis for early prognosis assessment.Methods:October 2021, patients with colchicine poisoning admitted in the First Affiliated Hospitol of Wenzhou Medical University from January 2017 to September 2021 were collected, including general information such as patient gender, age, oral colchicine dose, poisoning time, the first laboratory test index andplasma colchicine concentration after admission. The patients were divided into survival group and death group according to their prognosis. The differences in clinical indicators such as admission plasma colchicine concentration, blood routine, blood biochemistry, coagulation function, and blood gas analysis were compared between the two groups, and their predictive value for the prognosis of patients were analyzed.Results:A total of 23 patients with colchicine poisoning, aged 20-85 years, were included in this study, of which 15 cases (65.22%) survived and 8 cases (34.78%) died. The first plasma colchicine concentration at admision were 0.42-53.61 ng/ml. The plasma colchicine concentration and the concentration-time product were 10.08-2147.04 h·ng/ml.Compared with the survival group, the plasma colchicine concentration and the concentration-time product in the death group were significantly increased, and the differences were statistically significant ( P<0.05). Univariate logistic regression analysis showed that first plasma concentration and poisoning time>132.48 h·ng/ml, high C-reactive protein, high D-dimer, high absolute value of BE were the risk factors for the prognosis of patients with colchicine poisoning ( OR=12.000, 95% CI: 1.1181-128.836; OR=1.053, 95% CI: 1.009-1.098; OR=1.219, 95% CI: 1.039-1.429; OR=1.360, 95% CI: 1.1.044-1.773; P<0.05). High prothrombin time activity was protective factor affecting the prognosis of colchicine poisoning patients ( OR=0.941, 95% CI: 0.892~0.993; P<0.05). ROC curve analysis showed that the areas under the curves of first plasma concentration and poisoning time, C-reactive protein, absolute value of BE, D-dimer for predicting the prognosis of patients with colchicine poisoning were 0.918, 0.888, 0.867, 0.837, respectively, and the areas under the curves of prothrombin time activityfor predicting the prognosis of patients with colchicine poisoning was 0.788 ( P<0.05) . Conclusion:The product of the first plasma colchicine concentration at admission and poisoning time is closely related to the prognosis of patients with colchicine poisoning, it can be used as a predictor for early evaluation of the prognosis of poisoned patients.

4.Correlation analysis of serum clozapine concentration with serum homocysteine level in schizophrenic patients

Objective To analysis the correlation of serum clozapine concentration with serum homocysteine (Hcy) level and biochemical index in schizophrenic patients. Methods Two hundred and forty one schizophrenic patients treated with clozapine in Beijing Anding Hospital from January 1st 2017 to January 31st 2018 were enrolled in the study. Serum clozapine concentration, Hcy levels and biochemical index were measured. Patients were divided into high concentration group (n=41), normal concentration group (n=86) and low concentration group (n=114) according to serum clozapine concentration. The correlation of serum clozapine concentration with Hcy level and biochemical index was analyzed. Results The levels of Hcy, TG and LDL were higher in high clozazapine concentration group than in low concentration group and in normal concentration group (P<0.05). There was no significant difference between normal group and low concentration group in Hcy, TG and LDL levels (P>0.05). High serum clozapine concentration was positively correlated with serum Hcy (r=0.221, P=0.013), TG (r=0.193, P=0.003) and LDL level (r=0.148, P=0.021). Conclusion Serum levels of Hcy, TG and LDL in schizophrenic patients treated with clozapine are correlated with drug concentration, suggesting that patients with abnormally elevated levels of Hcy, TG and LDL should be monitored.
5.Imaging examination and diagnosis of airway foreign body in children

Objective To evaluate the value of imaging examination and diagnosis of airway foreign body in children. Methods 218 of airway foreign bodies confirmed by bronchoscopy were retrospectively analyzed in terms of clinical and imaging data. Results 218 patients included 140 males and 78 females. Their ages ranged 6 months to 12 years(mean,19.2 months). All patients had conventional radiographs on frontal chest films,neck lateral films and fluoroscopy. 34 cases underwent 64-slice spiral chest CT examination. According to the location of the foreign bodies,29 cases were in throat and subglottic,55 cases in trachea and 134 cases in bronchus(79 cas-es in the right,55 cases in the left). According to the character of the foreign bodies,5 cases were metal,29 cas-es in bone,93 cases in peanuts,59 cases in seeds,22 cases in fruits and vegetables and 10 cases in others respec-tively. Thirty-fourcases showed a direct sign of the foreign body and 177 cases showed the indirect signs. 34 cases on CT examination directly were showed foreign body in the tracheal bronchi. The diagnostic accuracy of CT exami-nation was 100%. Conclusions X-ray photograph and fluoroscopy were the first choice for aspiratory foreign bod-ies in children. CT examination is an important supplementary method when conventional X-ray examination is neg-ative.
6.MSCT diagnosis of tumors of intra-abdominal cryptorchidism in children

Objective To explore the diagnostic value of MSCT in tumors of intra-abdominal cryptorchidism in children.Methods MSCT findings of 8 children with tumors of intra-abdominal cryptorchidism confirmed by surgery and pathology were analyzed retrospectively.Results Six tumors located in the right,2 (1 tumor of left cryptorchidism turned to the right abdominal) in the left.Eight children showed ovoid soft tissue tumor in abdomen.Three children displayed the long axis of the tumors consistent with regular descending course of embryonic testes.Six teratomas manifested as the cystic and solid mass with fat,calcification (ossification) insidey.Two yolk sac tumors manifested as the large cystic and solid mass with irregular necrosis and abundant tumor vessels.Conclusion The pathologic types of tumors for intra-abdominal cryptorchidism in children are different from adult.Most of them are teratomas or yolk sac tumors,and have some characteristics in MSCT.MSCT is helpful in diagnosis of tumor for intra-abdominal cryptorchidism with medical history.
7.Feature Extraction and Image Recognition of Achyranthes bidentata and Cyathula officinalis

OBJECTIVE:To extract the feature of Achyranthes bidentata and Cyathula officinalis,and to establish image recog-nition method. METHODS:The microscopic image stitching of A. bidentata and C. officinalis was implemented by MATLAB. The color,invariant moment,stripes and the features of vascular bundle in cross section were extracted. The data was organized into da-ta matrix,and then data matrix was standardized by Zscore function;principal components were analyzed through Princomp func-tion. BP nerve network recognition mode was adopted. RESULTS:The microstructures in the micro images of the samples were kept integrated. The measured data of 27 characteristics were acquired in each group of sample. Through principal component analy-sis,the parameters of 11 main components were selected to establish BP never network. The average recognition rate of BP nerve network was 100% between 2 medicinal material relatives (n=50). CONCLUSIONS:The method can be used for micro-image auto Stitching of Chinese medicinal materials and image recognition of A. bidentata and C. officinalis.
8.Application value of MSCT in preoperative diagnosis and postoperative evaluation in children with cavernous transformation of the portal vein

Objective To evaluate the application value of MSCT in preoperative diagnosis and postoperative evaluation in children with cavernous transformation of the portal vein (CTPV).Methods 11 children diagnosed with CTPV by surgery were included in this study in our hospital.All patients underwent both plain and enhanced MSCT scans before surgery,and ultrasound or MSCT scan after surgery.Results In all 11 cases,preoperative CT showed different degrees of splenomegaly and the plain CT showed abnormal soft tissue mass in the hepatic hilar area.Enhanced CT showed stenosis or occlusion of portal vein trunk,earthworm-like blood vessels in the hilar region,and that the left and/or right branch of intrahepatic portal vein were replaced by multiple densely-distributed blood vessels (typeⅡ). Superior mesenteric-intrahepatic left portal shunt (Rex surgery)was performed in all 11 cases with successful results.All patients were followed up for 2-20 months after surgery.Imaging examinations showed anastomotic patency in 9 cases and stenosis in 2 cases.Conclusion MSCT findings of CTPV in children have certain characteristics.MSCT plays an important role in preoperative diagnosis and postoperative evaluation of CTPV in children.
9.The combined application of dissociate skin flap and vacuum sealing drainage on the defect of the large neck neoplasms after surgical procedures.

OBJECTIVE:
To evaluate the effect of the combined application of dissociate skin flap and vacuum sealing drainage (VSD) for the repairing for defect after surgical management of huge neck neoplasms.
METHOD:
Nineteen patients with huge neck malignant tumor involving the skin of the neck were given radical operation, making use of VSD covering the wound surface. After giving 6.65-7.98 mm Hg continuous negative pressure drainage for 72 h, the patients turned to be treated by intermittent negative pressure therapy with 2 min free interval after each treatment period for 5 min. After dismantling the VSD at 7th to 10th day postoperatively, the good wounds covered by granulation tissue were treated by the skin graft operation with dissociate skin flap from thighs; as for the wounds of which the granulation tissue didn't grow well and important cervical tissues was not fully covered by the granulation tissue, VSD was applied again for 1 week, followed by the skin graft operation.
RESULT:
Nineteen patients have received a total of 23 times of VSD wound treatment, one-stage operation time was significantly shortened. The granulation tissue grew faster on the wound after VSD treatment, and the important cervical tissues such as great vessels could be well covered. The infection and tumor recurrence were observed directly after dismantling the VSD. The skin graft transplantation would be performed after 1-3 weeks.
CONCLUSION
The treatment by vacuum sealing drainage combined with skin graft for surgical wounds of huge neck tumor postoperatively has the advantages of simple operation, little injury and promotion of the wound healing, which is an effective way for treatment of neck skin defect by surgical operation for the huge tumor.
