1.Evaluation of the effect of free fibular flap transplantation in repairing mandibular osteoradionecrosis defect in 151 cases

Objective:To investigate the clinical effect of free fibula flap transplantation in repairing the defect of mandibular osteoradionecrosis (ORN).Methods:A total of 151 mandibular ORN patients undergoing free fibular flap transplantation were selected from August 2005 to September 2020 in the Department of Oral and Maxillofacial Surgery, Sun Yat-sen Memorial Hospital, Sun Yat-sen University. Among them, 109 patients were males and 42 patients were females, aged (54.1±10.1) (ranged 31-85) years old. The clinical data of the patients was collected and the survival rate of the flaps and postoperative function were calculated to evaluate the surgical efficacy. The χ 2 test was used for difference analysis. Results:Among the 151 patients, mandibular ORN caused by radiotherapy for nasopharyngeal carcinoma accounted for 79.5% (120/151). The average time for mandibular ORN appeared was 5(6) years after radiotherapy. Facial artery [57.2%(87/152)] and superior thyroid artery [32.9%(50/152)] were the main anastomotic arteries in the recipient area. There was no significant difference in the necrosis rates of the two flaps [10.3%(9/87) and 12.5% (5/50), respectively, P=0.949]. The main anastomotic veins in the recipient area were the external jugular vein [48.4%(135/279)] and the common facial vein [26.5%(74/279)]. Twenty-five cases (16.6%) had one vein anastomosed, and 126 cases (83.44%) had two veins anastomosed. There was no significant difference in the flap necrosis rate between the two conditions [20.0%(5/25) and 7.1%(9/126), respectively, P=0.100]. Ninety-seven cases (64.2%) used the peroneal musculocutaneous-fascia composite flap to repair the maxillofacial soft and hard tissue defects. Thirteen cases (8.6%) underwent the restorations with digital virtual surgery design, of which 5 cases were repaired with dental implants at the same time. After the operations, lower respiratory tract infection occurred in 17 patients (11.3%), and upper respiratory tract obstruction occurred in 3 cases (2.0%). The survival rate of the flap after operation was 90.7% (136/151), and 21 patients (13.9%) had flap vascular crisis. Delayed healing of maxillofacial wounds occurred in 33 cases (21.9%). After 3 to 24 months of follow-ups, 110 patients (76.9%) had no fistula inside/outside the oral cavity, 118 patients (82.5%) had an improvement in opening mouth of increasing (≥0.5 cm) after surgery, 135 patients (94.4%) had pain relief, 97 cases (67.8%) could eat normal diet, semi-liquid or soft food, and 137 cases (95.8%) were satisfied or basically satisfied with the treatment effects. Conclusions:The free fibular flap transplantation is an effective method to repair mandibular ORN defects. Preoperative vascular assessment is helpful for the selection of recipient vessels. Facial artery, superior thyroid artery, external jugular vein and common facial vein can be used as the main recipient vessels. The repair of the peroneal musculocutaneous-fascia composite flap facilitates the closure of internal and external fistulas. Digital technology can help to restore the maxillofacial shape more accurately, improve the patient′s occlusal and chewing function and enhance the quality of life of mandibular ORN patients.
2.Clinical analysis of five cases of primary sphenoid sinus tumor

Objective:To study the clinical features, diagnosis and treatment of primary sphenoid sinus tumors and to improve the understanding of clinicians.Methods:The clinical data of 5 cases of primary sphenoid sinus tumors diagnosed by pathology from August 2014 to June 2017 in Affiliated Shengjing Hospital of China Medical University were collected and reviewed. The clinical characteristics, diagnosis and treatment were analyzed.Results:Of the 5 patients, 1 patient was first diagnosed in neurology with headache, 2 patients were first diagnosed in ophthalmology with periorbital pain, and 2 patients were diagnosed with nasal symptoms such as decreased olfactory, nasal congestion, blood stasis, and pharyngeal foreign body sensation. In the otolaryngology department, all patients had no history of head and neck surgery before surgery. All patients underwent sinus CT and MRI. They were suspected of malignant ECT examination. There were no distant metastatic lesions and surgical contraindications. General anesthesia and endoscopic sphenoid sinus surgery was used. And assisted image navigation systems were used in 3 patients. Postoperative pathological returns showed chordoma in 1 case, adenoid cystic carcinoma in 1 case, plasmacytoma in 1 case, poorly differentiated carcinoma in 1 case, and focal malignant transformation of inverted papilloma in 1 case. Patients with chordoma, adenoid cystic carcinoma, poorly differentiated carcinoma and inverted papilloma received foci surgery and were transferred to our hospital for oncology and chemotherapy. Patients with plasmacytoma only had surgery, and received no further treatment for personal reasons. Follow-up lasted 16-50 months and ended on October 1, 2018. Patients with poorly differentiated cancer were followed up for 12 months and died after liver metastases, and the other 4 patients lived well. Our department continued to follow up.Conclusions:The clinical manifestations of primary sphenoid sinus tumors are non-specific. Many of them are in advanced stage. Imaging examination plays an important role in the diagnosis of diseases and the development of surgical methods. Surgery is the main method for the treatment of this disease. Postoperative adjuvant chemotherapy and radiotherapy is of great significance to improve the survival rate of patients.
3.Gene Expression Profiling in Nasal Polyp

Objective To analyze the gene expression profiles of nasal polyp and gene expression differences between the nasal polyp and normal nasal mucosa.Methods Total RNA from nasal polyp tissues was purified and synthesized into double-stranded cDNA.The cDNA was labeled and hybridized in a NimbleGen hybridization chamber.The slides were scanned using the Axon GenePix 4000B microarray scanner.Scanned images were analyzed using NimbleScan software.The probes and gene levels were standardized and calculated.Results Compared with normal nasal mucosa tissues,expression of 2.22% (1 000/45 033) of genes was up-regulated in all cases of nasal polyp tissues,while 2.49% (1 123/45 033) of genes were down-regulated in all cases of nasal polyp tissues.We found genes related to the ribosome,proteasome,citrate cycle (TCA cycle),bladder cancer,non-small cell lung cancer,glioma,endometrial cancer,protein processing in the endoplasmic reticulum,chronic myeloid leukemia,and glutathione metabolism were up-regulated.Genes related to olfactory transduction,natural killer cell-mediated cytotoxicity,metabolism of xenobiotics by cytochrome P450,drug metabolism-cytochrome P450,antigen processing and presentation,malaria,graft-versus-host disease,retinol metabolism,linoleic acid metabolism,and pentose and glucuronate interconversions were down-regulated.Conclusion Multiple genes or pathways may be involved in the occurrence and development of nasal polyp.Gene expression profiling provides insight into the mechanism of nasal polyp development.
4.Inhibitory Effects of Deoxyschizandrin on P-glycoprotein in Vitro and in Vivo

Objective To investigate the influence of deoxyschizandrin (Deo) on P-glycoprotein (P-gp).Methods The effect of P-gp on Deo (20,40,80 μg·mL-1) was studied in the Caco-2 cell model in vitro,and the apparent permeability coefficient (Papp) of Deo (20-160 μg·mL-1) on a P-gp substrate,rhodamine123 or cyclosporine A,was calculated.Healthy male Sprague-Dawley rats were randomly divided into five groups:blank control group,verapamil group,low-,medium-and high-dose Deo group (8 rats in each group).Rats in the low-,medium-and high-dose Deo group were intragastrically administered once daily with Deo at 8,16 and 32 mg·kg-1 for 3 consecutive days,while rats similarly received gavagewith verapamil (4 mg·kg-1) in the verapamil group and equal volume of purified water in the blank control group.Thirty minutes after the rats were treated with their respective drugs,rhodamine123 (5 mg· kg-1) was orally administrated.Then the pharmacokinetic profiles of rhodamine 123 were analyzed to evaluate the inhibitory ability of Deo on P-gp in vivo.Results The bidirectional transport rates of Deo (20,40,80 μg·mL-1) were similar,with non-selectivity.Deo (20-160 pg·mL-1)significantly inhibited the basolateral→apical(BL→AP) directional transports of rhodamine 123 and cyclosporine A in Caco-2 cell model (P < 0.05) in a concentration-dependent manner.And Deo (8-32 mg· kg-1) also dose-dependently decreased the peak concentrations (Cm.) and the area under the plasma concentration-time curve (AUC0-t) of Rho123.Conclusion Deo can inhibit P-gp in vitro and in vivo,but it is not a P-gp substrate.
5.Clinical observation of preventive use of antibiotic inguinal hernial repair without tension and its significance

Objective:To explore the necessity of the preventive use of antibiotics and the effects of age and operation time on the efficacy of inguinal hernia repair without tension,and to elucidate the clinical significance of the preventive application of prophylactic antibiotics in inguinal hernia repair without tension.Methods:A total of 228 patients with inguinal hernia repair without tension were selected,amomg them 42 cases with high infection factors were treated with antibiotics (treated group),and 186 cases were not treated with antibiotics(untreated group) during the preoperative period.The prophylactic antibiotics were given 30 min before surgery,and the conventional dose was not used more than 48 h after surgery.All the cases were treated with artificial repair materials for the procedure of inguinal hernial repair without tension.The age,highest body temperature,white blood cell count,operation time,hospitalization time,and postoperative body temperature of all the 228 cases were recorded and analyzed statistically.Results:The preoperative and postoperative white blood cell counts had significant differences between the patients<60 years and the patients≥60 years in untreated group (P<0.05);there were no significant differences in the operation time,hospitalization time and body temperature between the patients<60 years and the patients≥60 years in untreated group (P>0.05).Compared with the patients with the operation time>90 min,the white blood cell count and hospitalization time of the patients with the operation time ≤90 min were increased (P<0.05).There were no significant differences of the body temperature and age between the patients with the operation time >90 min and the patients with the operation time≤90 min (P>0.05).The white blood cell count,operation time,hospitalization time and postoperative body temperature of the patients between treated group and untreated group had no significant differences (P>0.05).Conclusion:The use of antibiotics in the high-risk patients and non-use of antibiotics in the majority of elective inguinal hernia repair without tension can ensure the safe and performability of the patients.
6.Transportation of Schizandrin B in Caco-2 Cell Model

Objective:To investigate the effects of schizandrin B on P- glycoprotein by a classical in vitro cell model. Methods:Caco-2 cell model was used as the carrier,and rhodamine 123 and cyclosporin A were employed as the P-gp substrates, the transmembrane transportation of schizandrin B,rodamine 123 and cyclosporin A were detected by HPLC and a liquid scintillation counting assay,and the apparent permeability coefficient and permeability directional ratio were calculated. Results:The bidirectional transportation rates of schizandrin B(20 μg·ml -1 ,40 μg·ml -1 and 80μg·ml-1 )were similar,and showed non-selective difference. Schizandrin B(20-160 μg ·ml -1 )significantly inhibited the BL → AP directional transportation of rhodamine 123 and cyclosporine A in Caco-2 cell model(P < 0. 05) in a concentration-dependent manner. Conclusion:Schizandrin B is a P-gp inhibitor,while it isn’t a P-gp substrate.
7.Analysis of the Cost-effectiveness of 2 Kinds Regimens in the Treatment of Brain Trauma and Cerebral Hem-orrhage Complicated with Upper Gastrointestinal Hemorrhage

OBJECTIVE:To observe the efficacy and economy of omeprazole and esomeprazole in the treatment of brain trauma and cerebral hemorrhage complicated with upper gastrointestinal hemorrhage. METHODS:The data of 110 patients with trauma cere-bral hemorrhage complicated with upper gastrointestinal hemorrhage were retrospectively analyzed and divided into omeprazole group (56 cases)and esomeprazole group(54 cases). All patients were given conventional treatment. On this basis,omeprazole group was treated with Omeprazole for injection 40 mg by intravenous infusion;esomeprazole group was treated with Esomeprazole injection 40 mg by intravenous infusion,twice a day. The treatment course for 2 groups was 5 d. The efficacy and economy of patients were com-pared. RESULTS:The total effective rates in esomeprazole group were significantly higher than omeprazole group higher,the cost-ef-fectiveness in esomeprazde group(1 397.71)were significantly lower than omeprazole group(1 512.09)(P<0.05),andΔC/ΔE=91.52. CONCLUSIONS:Esomeprazole has good efficacy,safety and economy in the treatment of brain trauma cerebral hemorrhage compli-cated with upper gastrointestinal hemorrhage.
8.Virtual surgical design in accurate mandibular reconstruction with vascularized fibular graft

Objective To introduce the application and assess the outcome of ProPlan CMF software in virtual surgery of mandibular resection and reconstruction with vascularized fibular graft.Methods Nineteen patients were performed mandibular resection,and immediate reconstruction with free fibular graft by application of ProPlan CMF software between March 1,2013 and January 31,2014,including 7 cases of osteoradionecrosis,6 cases of adamantoblastoma,4 cases of carcinoma of gingiva,2 cases of intra-osseous carcinoma of mandible.There were 12 males and 7 females,with an age range of 21-73 years (median,54 years).In each case,three-dimensional spiral CT scan of mandible and fibula was obtained before operation.The CT data were imported into the ProPlan CMF software and the virtual surgical planning was performed.After that,the mandibular rapid prototyping and surgical guide plates were made according to customized design.The segmental resection of mandible,titanium plate bending,fibular molding,insetting and fixing were then carried out by using these surgical guide plates.Results Operations were undertaken successfully according to preoperative design in all nineteen patients and negative margin was detected by intraoperative frozen section.The fibula was cut,shaped and fixed accurately.The operations took 4.5-7.0 hours (mean,5.5 hours).Primary healing of incision was obtained without complications except fistula in 2 cases of osteoradionecrosis,which were resolved with local wound care after 4 weeks.19 patients were followed up between 3 months to 1 year and all patients were alive with disease free.All patients were satisfactory with their maxillofacial appearance,occlusion and function.The ranges of mouth opening was 2.0-4.0 cm.The postoperative panoramic radiographs or CT scan showed perfect situation and contour of fibular graft.Healing of the donor site occurred without significant complications.All patients started early postoperative ambulation,and walked normally after 2 or 3 months.Conclusion The application of ProPlan CMF software in virtual surgical design and guide plates creation can simplify the operation,and improve the accuracy of mandibular resection and reconstruction,especially the spatial relationship of the mandible and the fibula graft.It is worth promotion widely in clinical practice.
9.Inhibitory effect of cisplatin combined with Astragalus on human laryngeal carcinoma Hep-2 cells induced by

Objective To study the anti-proliferation and apoptosis induction effect of cisplatin combined with Astragalus on human laryngeal cancer Hep-2 cells.Methods The proliferation inhibition of cisplatin and Astragalus alone or in combination on Laryngeal cancer Hep-2cells was measured by MTT assay.Effects of cisplatin and Astragalus alone or in combination on apoptosis of Hep-2 cells were detected by flow cytometry (FCM).Western blot was used to analyze the expressions of Bcl-2 and Bax proteins.Results The inhibition ratio of proliferation of Hep-2 cells was (53.83 ± 17.12) % in the astragalus group,(69.12 ± 27.12)% in the cisplatin group,and (84.55 ± 27.84)% in the cisplatin combined with Astragalus group,and was significantly greater than the control group (0%) (t =16.87,16.67,40.90,P <0.01),respectively.The apoptotic ratio of Hep-2 cells was (38.2 ± 13.6)% in the astragalus group,(67.2 ± 17.8)% in the cisplatin group,and (86.4 ± 25.1)%] in the cisplatin combined with Astragalus group,and was significantly greater than control group (17.1 ± 1.3) % (t =8.11,12.77,24.92,P <0.05),respectively.The effect in the combination group is better than the other group (t =11.33,9.37,P < 0.01).The expressions of Bcl-2 and Bax proteins were changed.Conclusions The tumor-killing effect of cisplatin on laryngeal cancer Hep-2 cells could be enhanced significantly by the combination application of astragalus by the way of regulating the expression of Bcl-2/Bax.
10.Three treatments of intertrochanteric fractures in the elderly

Objective To compare extramedullary fixation (EF), intramedullary fixation (IF) and dipolar femoral replacement(DFR) in the treatment of intertrochanteric fractures in the elderly patients. Methods From July 1999 to December 2008, 172 old patients with intertrochanteric fractures were treated by either EF, or IF or DFR. The operative procedures, complications and therapeutic effects were compared between the 3 methods. Results The patients included in the present study were followed up for 8 to 36 months (average, 19. 6 months). There were significant differences ( P<0. 05) in incision length, operation time, intraoperative blood loss, functional recovery and complications between the 3 methods. IF had the shortest incision length and the least intraoperative blood loss. DFR had less blood loss than EF. IF had the least operation time, followed by DFR. DFR had the best functional recovery one year postoperatively. IF had shorter rehabilitation and healing time than EF. Conclusions EF is the first choice for intertrochanteric fractures of A 1.1, A 1.2, A 1.3 and A2. 1. IF is more appropriate for the aged and unhealthy patients with intertrochanteric fractures of A1. 1, A1. 2, A1. 3, A2. 1, A2. 2, A3. 1 and A3.2. DFR is fit for the elderly patients with severe comminuted fractures and osteoporosis.
