1.Analysis of factors for international normalized ratio levels>3.0 in patients undergoing warfarin anticoagulation therapy after mechanical heart valve replacement

Objective To investigate the factors influencing international normalized ratio (INR)>3.0 in patients undergoing warfarin anticoagulation therapy after mechanical heart valve replacement. Methods A retrospective analysis was performed on the clinical data of patients who underwent mechanical heart valve replacement surgery and received warfarin anticoagulation therapy at West China Hospital of Sichuan University from January 1, 2011 to June 30, 2022. Based on the discharge INR values, patients were divided into two groups: an INR≤3.0 group and an INR>3.0 group. The factors associated with INR>3.0 at the time of discharge were analyzed. Results A total of 8901 patients were enrolled, including 3409 males and 5492 females, with a median age of 49.3 (43.5, 55.6) years. The gender, body mass index (BMI), New York Heart Association (NYHA) cardiac function grading, INR, glutamic oxaloacetic transaminase, and preoperative prothrombin time (PT) were statistically different between the two groups (P<0.05). Multivariate logistic regression analysis revealed that lower BMI, preoperative PT>15 s, and mitral valve replacement were independent risk factors for INR>3.0 at discharge (P<0.05). Conclusion BMI, preoperative PT, and surgical site are factors influencing INR>3.0 at discharge in patients undergoing warfarin anticoagulation therapy after mechanical heart valve replacement. Special attention should be given to patients with lower BMI, longer preoperative PT, and mitral valve replacement to avoid excessive anticoagulation therapy.
2.Observation on the curative effect of surgical treatment of suppurative spondylitis after percutaneous vertebroplasty

Objectives:To observe the clinical effect of surgical treatment of suppurative spondylitis after percutaneous vertebroplasty(PVP).Methods:The medical records of 14 patients with suppurative spondylitis after PVP treatment admitted to the Department of Orthopedics of our hospital from March 2017 to February 2022 were retrospectively analyzed.There were 6 males and 8 females,aged 53-83 years,with an average age of 69.3±7.8 years.The patients were followed up for 12 months.The operation time,intraoperative blood loss,postoperative complications,postoperative recurrence rate,postoperative visual analogue scale(VAS)and American Spinal Injury Association(ASIA)improvement,postoperative bone graft fusion rate,and postoperative quality of life(short form-36,SF-36)improvement were analyzed to observe the clinical effect and related risks of surgical treatment of suppurative spondylitis after PVP treatement.Results:The operative time of 14 patients was 123-300min,with an average of 205.1±57.4min.Intraoperative blood loss was 200-900mL,with an average of 517.9±181.5mL.Postoperative incision complications occurred in 2 patients(14.3%),of which,1 case was incision effusion,healed after drainage and dressing change;The other case was delayed healing,which healed after regular dressing change,improving nutritional status and blood sugar control.The VAS scores ranged from 3 to 8 points(6.1±1.6 points)before operation,which was 0-2 points(0.9±0.8 points)at fi-nal follow-up,and the difference was statistically significant(t=11.034,P=0.000).The bone graft fusion rate was 92.8%(13/14),and the fusion time was 3 to 7 months(4.6±1.3 months).Spinal infection recurred at 7 months after surgery in the 1 patient with unfused bone graft,who presented internal fixation loosening and kyphotic deformity and was cured after anti-infection,control of complications,and reoperation.According to the ASIA classification,2 patients were grade D and 12 patients were grade E before surgery,which all im-proved to grade E at final follow-up.At final follow-up,the SF-36 scores of the patients were significantly improved compared with those before surgery,and the comprehensive measurement score of physiological health was 31.4±6.9 before surgery and 79.8±6.0 after surgery,and the difference was with statistical signifi-cance(t=19.721,P=0.000).The comprehensive measurement score of mental health was 46.9±7.2 before surgery and 88.5±6.4 after surgery,and the difference was statistically significant(t=16.027,P=0.000).Conclusions:Surgery can effectively treat suppurative spondylitis after PVP treatment and improve the quality of life of pa-tients.
3.Effects of quercetin loaded gelatin microspheres on proliferation and differentiation of MC3T3-E1

Objective This study prepared gelatin three-dimensional porous microspheres and investigated the feasibility of gelatin three-dimensional porous microspheres loaded with quercetin(G-quercetin)as bone tissue scaffold material.Methods Porous gelatin microspheres were prepared by emulsification and loaded with quercetin by lyophilization.Scanning electron microscopy was used to ob-serve morphology of the microspheres.The cytotoxicity of G-quercetin microspheres and their effects on the adhesion,proliferation and differentiation of mouse embryonic osteoblast precursor cells(MC3T3-E1)were detected by immunofluorescence staining,live/dead cell staining and CCK-8 assay,alkaline phosphatase(ALP)staining and alizarin red staining.RT-PCR was used to detect the tran-scriptional levels of osteoblast-related cytokines such as Runx-2,ALP,OPN and OCN.Results The scanning electron microscopy re-sults showed that the prepared three-dimensional microporous material loaded with quercetin gelatin had a porous structure.Cell adhe-sion showed that the cells could spread well on the surface of the microspheres.Compared with the control group,the results of live/dead cell staining and CCK-8 detection showed that the microspheres had no significant cytotoxicity(P>0.05).Compared with the con-trol group,G-quercetin microspheres showed an increase in ALP expression and mineralization in vitro.PCR results also showed a sig-nificant increase in Runx-2,ALP,OCN,OPN(P<0.05).Conclusion The G-quercetin porous microspheres prepared in this experi-ment have good biocompatibility and can promote the osteogenic differentiation of MC3T3-E1 in vitro.It is expected to be used as a new scaffold material for bone tissue engineering.
4.Survey on status of perioperative aspirin use in thoracic surgery in China

Objective:To understand the current status of the use of aspirin in perioperative period of thoracic surgery in China and the awareness of thoracic surgeons on the prevention and treatment of arterial thromboembolic diseases during the perioperative period.Methods:A survey was conducted among thoracic surgeons nationwide using an electronic questionnaire through a software platform from January 20, 2024 to February 29, 2024. The questionnaire mainly included three parts: basic information, aspirin use, and awareness of arterial thromboembolic diseases, with a total of 27 questions.Results:A total of 1318 valid questionnaires were collected, covering all 33 provincial administrative regions except Taiwan. 69.7%(919/1 318) of thoracic surgeons stopped using aspirin for all patients before surgery; differences could be found in the timing of aspirin withdrawal before surgery and resumption after surgery. 65.1%(858/1 318) of surgeons made perioperative medication decisions based on domestic guidelines or consensus, while 20.9%(276/1 318) of surgeons based their decisions on departmental or personal experience. 87.9%(1 159/1 318) of surgeons believed that it is necessary to develop guidelines for the prevention of arterial thromboembolic diseases in the perioperative period of thoracic surgery.Conclusion:The current use strategy of aspirin in the perioperative period of thoracic surgery in China shows a high degree of inconsistency, and there are also significant differences in the awareness of arterial thromboembolic diseases. It is necessary to establish guidelines for the prevention and treatment of arterial thromboembolic diseases in the perioperative period.
5.Early and mid-term outcomes of aortic valvuloplasty in children

Objective:To analyze the short and mid-term efficacy of aortic valvuloplasty with autopericardium on children with aortic valve diseases.Methods:A total of 26 children with aortic valve diseases (stenosis or regurgitation) who underwent aortic valvuloplasty with autopericardium in Fuwai Central China Cardiovascular Hospital from September 2017 to June 2021 were retrospectively analyzed.The short-term and mid-term follow-up data were collected.The maximum aortic valve pressure gradient, subaortic regurgitation area, left ventricular end-diastolic volume (LVEDV) and left ventricular ejection fraction (LVEF) were compared before and after operation.Paired t test was used to analyze the short-term and mid-term efficacy of aortic valvuloplasty with autopericardium on children with aortic valve diseases. Results:All 26 cases were successfully operated, and there were no deaths and serious complications during the follow-up period of (22.96±6.45) months.There was a significant difference between the preoperative and postoperative maximum aortic valve pressure gradient at 1 month ( t=7.85, P<0.05), 6 months ( t=6.43, P<0.05), 1 year ( t=6.16, P<0.05) and 2 years postoperatively ( t=4.22, P<0.05) in children with aortic stenosis or that combined with mild-to-moderate closure.The follow-up data of 9 children with simple aortic stenosis showed that there was a significant difference between the preoperative (8.87±3.57) cm 2 and postoperative aortic regurgitation area at 1 month ( t=6.85, P<0.05), 6 months ( t=5.13, P<0.05), 1 year ( t=6.62, P<0.05) and 2 years postoperatively ( t=5.41, P<0.05). The LVEDV of 26 children was significantly lower at 6 months[(63.54±27.61) mL], 1 year [(53.61±20.20) mL] and 2 years postoperatively [(64.39±17.78) mL] compared with that of preoperative level[(89.42±45.89) mL]( t=3.89, 4.67, 3.58, all P<0.05). The left ventricular pressure and volume decreased, the enlarged heart was narrowed down, and the geometry of the heart was restored.The LVEF of 26 patients also from (61.65±9.67)% before surgery increased to (67.88±4.69)% 6 months after surgery( t=3.68, P<0.05), and increased to (68.62±4.46)% 1 year after surgery( t=4.01, P<0.05), and increased to (67.55±3.09)% 2 years after operation( t=3.01, P<0.05), and the heart function was improved. Conclusions:Aortic valvuloplasty with autopericardium presents an effective short and mid-term efficacy on children with aortic valve diseases, which prevents or delays the aortic valve replacement.

7.Predicting the invasiveness of lung adenocarcinoma manifesting as ground-glass opacity using spectral CT multi-parameter functional imaging

Objective:To explore the clinical value of quantitative parameters on spectral CT in predicting the invasiveness of lung adenocarcinoma manifesting as ground-glass nodules (GGN).Methods:The clinical and imaging data of 129 patients with pathologically confirmed lung adenocarcinoma who were surgically resected in the First Affiliated Hospital of Zhengzhou University from March to October 2022 were retrospectively analyzed, including 45 males and 84 females, aged from 33 to 81. According to the pathological results, they were divided into the minimally invasive adenocarcinoma (MIA) group ( n=64) and the invasive adenocarcinoma (IAC) group ( n=65). All patients underwent enhanced spectral CT within two weeks before surgery. The iodine density map, Z-Effective (Z eff) map, and electron density (ED) map were reconstructed on the post-processing workstation, and the spectral parameters, including normalized iodine concentration (NIC), arterial enhancement fraction (AEF), Z eff, and ED were measured and calculated. Conventional CT features were analyzed, including maximum diameter, CT value, nodule types, margin, lobulation sign, spiculation sign, bubble sign, pleural retraction sign, abnormal vascular sign, and air bronchial sign. The clinical features, conventional CT characteristics and spectral CT parameters of two groups were compared using the independent sample t test, the Mann-Whitney U test, and the χ 2 test. Multivariate logistic regression analysis was used to evaluate the independent risk factors of lung adenocarcinoma invasiveness, and the model was constructed. Receiver operating characteristic (ROC) curve analysis was performed to evaluate the performance of parameters and models in predicting the invasiveness of lung adenocarcinoma. Results:There were significant differences between the MIA group and IAC group in maximum diameter, CT value, nodule type, margin, spiculation sign, pleural retraction sign, air bronchial sign, venous phase NIC, AEF, venous phase Z eff, arterial phase ED, venous phase ED ( P<0.05). Multivariate logistic regression analysis showed that the maximum diameter (OR=1.183, 95%CI 1.062-1.318), CT value (OR=1.004, 95%CI 1.001-1.007), venous phase NIC (OR=1.185, 95%CI 1.083-1.298), AEF(OR=0.975, 95%CI 0.957-0.994), venous phase Z eff (OR=0.031, 95%CI 0.005-0.196) were independent influence factors for the invasiveness of lung adenocarcinoma. The conventional CT model was established with the maximum diameter and CT value, and the spectral CT model was established with venous phase NIC, AEF, and venous phase Z eff. The combined model was established with all the parameters above. Areas under the ROC curve of the conventional CT model, the spectral CT model, and the combined model for predicting the invasiveness of lung adenocarcinoma were 0.828, 0.854, and 0.902, respectively. Conclusion:The quantitative parameters of double-layer detector spectral CT can be used as an indicator to predict the invasiveness of lung adenocarcinoma manifesting as GGN, and AEF has the highest diagnostic efficacy. Spectral CT combined with conventional CT features can further improve the diagnostic efficiency.
8.Effects of Black Raspberry Supplementation on Methylation Pathways in Vav-cre Asxl1 fl/fl Tet2 fl/fl Double Knockout Mice with Early-stage Myelodysplastic Syndrome

Myelodysplastic syndromes (MDS) are a subset of myeloid malignancies defined by clonality of immature hematopoietic stem cells that leads to faulty blood cell development. These syndromes can lead to an increased risk of infection and may transform into acute myeloid leukemia, making it critical to determine effective treatments for the condition. While hypomethylating agents such as azacitidine and decitabine, as well as stem cell transplants, have been delineated as favored treatments for MDS, not all patients are physiologically receptive to these treatments. However, black raspberries (BRBs) have been shown to exert hypomethylating effects in various malignancies, with minimal adverse effects and thus a broader range of potential candidacies. This study aimed to investigate the potential of BRBs to exert such effects on MDS using Addition of Sex Combs Like/Tet Methylcytosine Dioxygenase 2 (Asxl1/Tet2) double knockout mice (Vav-cre Asxl1fl/fl Tet2fl/fl ), which typically manifest symptoms around 25 weeks of age, mirroring genetic mutations found in humans with MDS. Following a 12-week dietary supplementation of Vav-cre Asxl1fl/fl Tet2fl/fl mice with 5% BRBs, we observed both hyper- and hypomethylation at multiple transcription start sites and intragenic locations linked to critical pathways, including hematopoiesis. This methylation profile may have implications for delaying the onset of MDS, prompting a need for in-depth investigation. Our results emphasize the importance of exploring whether an extended BRB intervention can effectively alter MDS risk and elucidate the relationship between BRB-induced methylation changes, thus further unlocking the potential benefits of BRBs for MDS patients.
9.Study on the policies for the development of traditional Chinese medicine injection in China based on the per- spective of policy tools

OBJE CTIVE To provide reference for the adjustment and optimization of the policies related to traditional Chinese medicine(TCM)injection in China. METHODS The policies related to TCM injections issued at the national level were collected from Jan. 1,1990 to May 31,2021. Based on the perspective of policy tool ,the content analysis and quantitative analysis were used to classify ,code and analyze the policy terms according to “policy serial number-chapter number-specific terms ”. RESULTS & CONCLUSIONS Totally 30 policy documents related to TCM injection were included , with a total of 389 codes. Environment-based policy tools were the most widely used (79.95%),followed by supply-oriented policy tools ,accounting for 15.42%. Demand-based policy tools accounted for the least proportion (4.63%). Among environment-based policy tools ,the regulatory and control policy tools (38.05%) received more attention,and the policy publicity (2.06%) received fewer applications. Among supply-oriented policy tools ,there were more applications of science and technology support (10.80%), and fewer applications of capital investment (0.26%). Among demand- based policy tools ,organizational coordination was the most widely used (3.34%),followed by experience demonstration (1.29%),which had not yet involved the relevant policies of international exchange. In order to promote the development of TCM injection ,it is necessary to appropriately reduce the application of environment-based policy tools ,increase the application of policies such as policy publicity ,and improve the external environment for TCM injection ;optimize the internal combination of supply-oriented policy tools ,increase the use of capital investment tools ,and effectively play the role of policy boosting;emphasize the application of demand-based policy XJY21013) tools to form an effective policy pulling force for the healthy development of TCM injection.
10.Medium-term follow-up of percutaneous pulmonary valve implantation using the Venus-P self-expanding valve

Objective:To evaluate the medium-term clinical and hemodynamic outcomes of percutaneous pulmonary valve implantation (PPVI) using a domestic Venus-P self-expanding valve in the treatment of severe pulmonary regurgitation after Tetralogy of Fallot (TOF).Methods:Retrospective study.From December 2017 to December 2020, 13 TOF patients with (17.8±4.7) years old and (50.2±12.3) kg underwent PPVI using the Venus-P self-expanding valve in the Department of Children′s Heart Center, Zhengzhou University People′s Hospital were recruited.The mean valve size was (31.9±3.1) mm.All patients received the transannular patch surgery and developed severe pulmonary regurgitation.After PPVI, 13 patients were followed up for at least 12 months.The operation-related complications, improvement of valve and heart function and the durability and reintervention of the Venus-P self-expanding valve were analyzed.The right ventricular end-diastolic volume index (RVEDVi) before and after operation was compared by the paired t-test, and the New York Heart Association (NYHA) class was compared by the paired Wilcoxon signed rank sum test. Results:PPVI was successfully performed in all 13 patients without death.At 6 months post-PPVI, cardiac magnetic resonance imaging findings showed that RVEDVi was significantly reduced[(145.7±9.6) mL/m 2vs.(100.2±12.2) mL/m 2, P<0.05], and the NYHA class was significantly improved ( P<0.05). One patient presented moderate pulmonary valve regurgitation at 12 months postoperatively.No vegetation was found on echocardiography, and blood culture was negative in this case.Six patients did not have postoperative pulmonary valve regurgitation, and the remaining presented mild or less pulmonary regurgitation.One patient had sudden ventricular tachycardia on the 6 th day postoperatively, which was converted to sinus rhythm after synchronous electrocardiography.Only one case underwent appendectomy 7 months after operation due to acute appendicitis, and the remaining did not require valve-related reintervention after implantation.During the follow-up for (22.8±8.0) months, no patients had perivalve leakage, stent migration and rupture.Complications like embolization and coronary artery compression were not reported. Conclusions:PPVI using the Venus-P self-expanding valve is safe and effective in patients with severe pulmonary valve regurgitation after TOF surgery, showing an acceptable medium-term follow-up outcome.Studies with a large sample size and long follow-up period are still needed to validate our findings.
