1.Investigation of an occupational acute carbon monoxide poisoning incident caused by nitrogen generator failure

Objective To analyze the characteristics and causes of an occupational acute carbon monoxide poisoning incident caused by a nitrogen generator malfunction. Methods The workers and the workshop from an electronic component manufacturing company in Guangzhou City, which had an occupational acute carbon monoxide poisoning incident, were selected as the research subjects using the retrospective investigation method. Relevant data of worksite survey of occupational health, clinical records of poisoning patients, and occupational disease diagnosis data were collected for analysis. Results A total of 15 workers who experienced discomfort in this incident, were from the final inspection workshop using high-purity nitrogen (purity of 99.999%). The short-term exposure concentration of carbon monoxide in the workplace air at three oven operating posts using high-purity nitrogen was 64.1, 581.0, and 142.0 mg/m³, respectively, while the concentration in the workplace air at the oven operating posts using regular nitrogen and the nitrogen-generating room was <0.1 mg/m³. Carboxyhemoglobin saturation levels in the 15 patients ranged from 2.000%-17.300%. Occupational acute mild carbon monoxide poisoning was diagnosed in nine patients, occupational acute carbon monoxide exposure reactions was diagnosed in six patients. This poisoning incident was caused by machine malfunction and improper operation, leading to occupational acute carbon monoxide poisoning. Conclusion Companies using nitrogen production machine should enhance their ability to identify carbon monoxide risks, strengthen training and management of workers, establish emergency response plans for nitrogen production and use, ensure proper maintenance of production equipment, and optimize operational procedures to prevent acute poisoning incidents.
2.Differential component analysis between Fructus Tritici Levis and Triticum aestivum based on qualitative and quantitative methods

OBJECTIVE To analyze the compositional differences between Fructus Tritici Levis and Triticum aestivum, and to provide reference for identification and quality control of both. METHODS Twenty batches of Fructus Tritici Levis and three batches of T. aestivum were collected, and their fingerprints were acquired by high-performance liquid chromatography and the similarities were evaluated by the Evaluation System of Similarity of Chromatographic Fingerprints of Traditional Chinese Medicine (2012 version). Cluster analysis (CA), principal component analysis (PCA) and orthogonal partial least squares-discriminant analysis (OPLS-DA) were performed to analyze the difference of Fructus Tritici Levis and T. aestivum from different regions, and the differential components were screened. The contents of the six identified components in Fructus Tritici Levis and T. aestivum were determined. RESULTS The similarities of the fingerprints of Fructus Tritici Levis ranged from 0.928 to 0.996, and the relative similarities of T. aestivum with Fructus Tritici Levis ranged from 0.761 to 0.773. A total of 19 common peaks were calibrated, and six components including linolenic acid, linoleic acid, 5-heptadecylresorcinol, 5-nonadodecylresorcinol, 5- heneicosylresorcinol, and 5-tricosylresorcinol were identified. The results of CA and PCA showed that Fructus Tritici Levis and T. aestivum could be clearly distinguished; the distribution of Fructus Tritici Levis from Anhui province was relatively concentrated. The results of OPLS-DA showed that linolenic acid, linoleic acid, and other six unknown compounds were the differential components between Fructus Tritici Levis and T. aestivum. The average contents of the six identified components in Fructus Tritici Levis were 0.100 9, 1.094 0, 0.005 1, 0.030 9, 0.098 2,and 0.024 8 mg/g, respectively; the contents of linolenic acid and linoleic acid in Fructus Tritici Levis were significantly higher than those in T. aestivum (P<0.05).CONCLUSIONS The established qualitative and quantitative methods are simple and reliable, and can be used for the identification and quality evaluation of Fructus Tritici Levis and T. aestivum. The identified differential components, such as linolenic acid and linoleic acid, can also provide clues for the differentiation and pharmacological study of Fructus Tritici Levis and T. aestivum.

4.Potential and advantages of metal organic frameworks-based hydrogel materials for repairing soft and hard tissues

BACKGROUND:Due to the mechanical properties,unstable drug release,single function and other problems of pure hydrogel materials,in recent years,researchers have prepared a variety of metal organic frameworks-based hydrogel materials by introducing metal organic frameworks into hydrogel,and showed great potential in the field of soft and hard tissue regeneration. OBJECTIVE:To classify the metal organic frameworks-based hydrogel materials based on how metal organic frameworks enhance the properties of hydrogel and further summarize its recent research in the field of soft and hard tissue regeneration,in order to provide ideas and theoretical supports for the subsequent in-depth research on synthesis mechanism and clinical application of the composite material. METHODS:Using"metal organic frameworks,hydrogels,tissue engineering,tissue,bone regeneration,bone,wound"as English and Chinese search terms,we searched Web of Science,PubMed,CNKI,and Wanfang databases.The search period ranged from January 2000 to August 2023.By reading the titles and abstracts,the repetitive studies and unrelated literature of Chinese and English literature were excluded.After the literature quality evaluation,73 articles were included for review. RESULTS AND CONCLUSION:(1)Metal organic frameworks-based hydrogel materials effectively solve the problems of poor mechanical properties,unstable drug release and single function of pure hydrogel.(2)Metal organic frameworks enhance the capacity of repair and regeneration by strengthening the cross-linking of hydrogel,the drug delivery capacity of hydrogel and the multifunction of hydrogel.(3)In terms of hard tissue repair,it has shown good repair effects in animal models of diseases such as bone defects,osteoarthritis,and cartilage defects,suggesting potential application prospects in clinical repair.(4)In terms of soft tissue regeneration,it has the capacities of hemostasis,antibacterial,inflammatory state regulation,oxidative stress state regulation,promoting angiogenesis and other functions,effectively improving the microenvironment of various complex wounds and promoting soft tissue regeneration.(5)Although metal organic frameworks-based hydrogels have many excellent properties,they are still in the initial stage and there are some urgent problems to be solved in the process of clinical transformation,such as the cytotoxicity of metal organic frameworks and large-scale synthesis of metal organic frameworks.(6)With further research,metal organic frameworks-based hydrogels have broad application prospects in the field of soft and hard tissue repair.
5.Continuous renal replacement therapy for neonatal hyperammonemia: 10 cases experience

Objective:To study clinical outcomes, genetic etiology, efficacy and safety of continuous renal replacement therapy (CRRT) for neonatal hyperammonemia.Methods:From September 2016 to June 2023, neonates with hyperammonemia receiving CRRT in NICU of our hospital were retrospectively analyzed. Their perinatal conditions, clinical manifestations, laboratory results, genetic tests, treatments and outcomes were collected. The patients were assigned into survival group and death group according to their conditions at discharge. SPSS 22.0 statistical software was used to analyze the differences between the two groups.Results:A total of 10 patients were enrolled, including 8 males and 2 females. The gestational age was 39.3(38.2,39.8)weeks and birth weight 3 300(3 050, 3 583) g. The age of onset was 2.0(2.0, 4.3) d. The main clinical manifestations included seizures, coma and high blood ammonia level (up to 586-1 250 μmol/L). The patients received CRRT at 3.0(2.0, 8.3) d of age and CRRT lasted for 20.5(16.5, 42.8) h. Before CRRT, average time of coma was 10.0(3.5, 12.8) h and the total duration of coma was 20.5(12.5, 29.0) h. After CRRT, blood ammonia decreased (52.6±22.2) μmol/L every hour for 6 h. The genetic tests showed ornithine transcarbamylase deficiency in 5 cases, methylmalonic acidemia in 2 cases, propionic acidemia in 1 case, carnitine acylcarnitine translocase deficiency in 1 case and transient hyperammonemia in 1 case. 6 patients survived. 4 patients died at discharge, including 2 withdrawal treatment. The duration of coma before CRRT and the total duration of coma in the death group were significantly longer than the survival group ( P<0.05). Conclusions:Inborn metabolic error are common causes of neonatal hyperammonemia. Timely CRRT can safely and effectively reduce blood ammonia levels and may improve clinical outcomes.
6.An Approach for Grading Syndrome Suitability of Dietary Therapy in Traditional Chinese Medicine

Clarifying the applicability (indications or contraindications) of traditional Chinese medicine (TCM) dietary regimens to different syndromes is a guarantee for the rational application and safety of TCM dietary therapy. By considering evidence from ancient literature, modern research and experts' experience, the Grading System for Syndrome Suitability of Dietary Therapy in TCM (GRADE-SSD) has been constructed. According to the currently available highest-quality evidence, GRADE-SSD classified the applicability of TCM dietary therapy to various TCM syndromes into 2 categories and 4 grades: "definitely" applicable/contradicted (Grade A), "very likely" applicable/contradicted (Grade B), "possibly" applicable/contradicted (Grade C), and insufficient evidence to determine (Grade D). When applying, first, systematically search the currently available ancient books and clinical research evidence. For different types of evidence (ancient TCM books, randomized controlled trials, observational studies, and meta-analyses), use the prescribed tools to evaluate the quality of the evidence, and then determine the applicability level of TCM syndromes of dietary therapy based on the principle of "taking the highest" quality. If there is a lack of direct evidence, GRADE-SSD supports a downgraded evaluation based on indirect evidence of "similar prescriptions". When there is a lack of ancient books and clinical research, the GRADE-SSD plan can evaluate the quality of evidence from peer consensus or expert opinions for the applicability level of TCM syndromes of dietary therapy. Based on the requirements for the safety of TCM dietary therapy, when there is both "applicable" and "contradicted" evidence, it should be considered as "contradicted" evidence. GRADE-SSD also provides an evidence certainty update plan based on new evidence in the future. The evaluation results of GRADE-SSD can provide a basis for setting inclusion/exclusion criteria in clinical trials of TCM dietary therapy and stating the applicability of each syndrome in the labels of dietary therapy products.
7.An Empirical Study on the Use of Diagnosis Related Group Tools for Grouping Adjustments in Large Public Hospitals

To analyze the disease group structure and its trends in key departments of large public hospitals using diagnosis related group (DRG) data, explore the key points of intervention and optimization of disease groups in departments, and further promote the rational allocation of department resources. We retrospectively collected DRG data from two surgical departments in a large public hospital in Beijing from 2017 to 2023. When the case mix index (CMI) of the two surgical departments declined, interventions such as performance appraisal, department education, and hospital publicity were promptly adopted. The changesin CMI values were observed and the trends in disease group weights, time consumption index, cost consumption index, and mortality rate in low-risk groups were analyzed. After the interventions, in surgical department Ⅰ, the proportion of patients with lower-weight diseases, such as major thyroid surgery (KD1), significantly decreased, while that of patients with higher-weight diseases, such as colorectal malignancy surgery (GB2) and pancreatic malignancy surgery (HB1), significantly increased. In surgical department Ⅱ, the proportion of patients with lower-weight diseases, such as chemotherapy (RE1), decreased markedly, while that of patients with higher-weight diseases, including major surgery for malignancy of kidney, ureter, and bladder (LA1), adrenal gland surgery (KC1), surgery for kidney/ureter/bladder except for major malignancy surgery (LB1), and male genital organ malignancy surgery (MA1), increased significantly. Both surgical departments achieved the goal of increasing their CMI values. In terms of efficiency, cost, and quality indicators, the time consumption index and cost consumption index of the two surgical departments were significantly lower than 1, and the mortality rate in low-risk groups was 0. Based on actual conditions and development goals, large public hospitals can achieve improvements in CMI values and optimization of disease group structures through reasonable interventions, thereby enhancing medical efficiency and rational utilization of resources.
8.Diagnostic value of plaque feature index based on coronary CT angiography for lesion specific myocardial ischemia in stable coronary heart disease patients

[Abstract]Objective To explore the predictive value of plaque characteristic index based on coronary CT angiography(CCTA)for disease-specific myocardial ischemia in stable coronary artery disease(CAD).Methods 90 stable CAD patients admitted to Zhongshan Hospital,Fudan University from October 2020 to March 2022 were prospectively enrolled,including 135 target vessels with stenosis degree ranging from 30%to 90%.All participants sequentially underwent CCTA,ICA,and fractional flow reserve(FFR)measurement within 2 weeks.Based on the presence or absence of significant hemodynamic stenosis,all patients were divided into myocardial ischemic population(FFR≤0.8,n=30)and non ischemic population(FFR>0.8,n= 60),135 target blood vessels were divided into a disease-specific myocardial ischemia group(FFR≤0.8,n=36)and a non disease-specific myocardial ischemia group(FFR>0.8,n=99).Univariate and subsequent forward stepwise multivariate logistic regression analyses were used to assess independent predictors of myocardial ischemia,and CCTA-derived plaque characteristics index logistic regression model was created.Receiver operating characteristic(ROC)analysis was used to analyze the diagnostic performance of CCTA-derived plaque characteristics index on detecting myocardial ischemia.Results There were no significant differences in age,body mass index,gender,cardiovascular risk factors,and medication between myocardial ischemia population and non-ischemia population.Compared with the non-lesion-specific ischemia group,plaque length,plaque area,percent area stenosis,total atheroma volume,vessel volume and lipid rich volume,positive remodeling and napkin-ring signs were significantly higher in lesion-specific group,while minimum luminal area(MLA)was significantly lower.MLA(OR=0.303,95%CI 0.178-0.517,P<0.001)and total atheroma volume(TAV,OR=1.006,95%CI 1.002-1.010,P=0.003)were found to be the significant independent predictors of myocardial ischemia.The regression equation of CCTA-derived plaque characteristic index for predicting probability was P=1/[1+e-(-1.194 41×MLA+0.006 058×TAV+0.600 912)].The area under the ROC curve(AUC)of CCTA-derived plaque characteristic index on detecting myocardial ischemia was 0.879(95%CI 0.811-0.928),and the overall diagnostic accuracy,sensitivity,specificity,PPV and NPV were 80.0%,83.3%,78.8%,58.8%,and 92.9%,respectively.Conclusions CCTA-derived plaque characteristic index performs well in diagnosing lesion-specific myocardial ischemia,showing its great clinical application prospect.
9.Comparative analysis of homogeneous phase and vertical auto profile separation phase methods for detecting low-density lipo-protein cholesterol levels

Objective To investigate the reasons for the inconsistent results between the vertical auto profile(VAP)method and bio-chemical homogeneous phase(BHP)method in detecting plasma low-density lipoprotein cholesterol(LDL-C),and provide experimen-tal basis for the accurate and quantitative detection of plasma LDL-C levels.Methods A total of 360 plasma samples from diabetes mellitus patients combined with carotid plaque admitted to the Department of Endocrinology of Ningbo Yinzhou Hospital of Traditional Chinese Medicine during January,2022 and January,2023 were collected.The LDL-C levels of these samples were detected by the VAP method and BHP method,respectively.The VAP method uses software to automatically calculate the area under the LDL-C curve after centrifugation of the sample as the LDL-C level(LDL-CVAP)and the BHP method directly detects the LDL-C level(LDL-CBHP)by the special surfactant method.360 samples were divided into the consistent group(group A)and inconsistent group(group B)ac-cording to the relative deviation between the LDL-CBHP and LDL-CVAP methods.Group B was further divided into the LDL-CBHP on the high side group(Group B1)and LDL-CBHP on the low side group(Group B2).Groups B1 and B2 were divided into B1-1,B1-2,B1-3 and B2-1 groups based on the degree of relative deviation.The percentages of samples and levels of lipoprotein a cholesterol[Lp(a)-C],intermediate-density lipoprotein cholesterol(IDL-C),Lp(a)-C and IDL-C[Lp(a)-C+IDL-C],very low-density lipo-protein cholesterol(VLDL-C),total cholesterol(TC)and total triglyceride(TG)in each group were compared.Results The LDL-CBHP levels of 360 samples were significantly higher than that of LDL-CVAP(P<0.01).The percentage of samples in group B was significantly higher than that in group A,and that of group B1 was significantly higher than that of group B2(P<0.05).The levels of Lp(a)-C,IDL-C and Lp(a)-C+IDL-C in groups B1-1,B1-2,and B1-3 were significantly higher than those in group A(P<0.01).The relative deviation between LDL-CBHP and LDL-CVAP in 360 samples was significantly positively correlated with the levels of Lp(a)-C,IDL-C,and Lp(a)-C+IDL-C(P<0.01).The maximum correlation coefficient was found in Lp(a)-C+IDL-C.Conclusion The results of plasma LDL-C in diabetes mellitus patients combined with carotid plaque detected by the BHP method are significantly different from those detected by the VAP method,which mainly shows that the results of the BHP method are on the high side.The higher the level of plasma Lp(a)-C+IDL-C,the greater the relative deviation between the BHP method and VAP method.The reason for the high results of LDL-C detected by the BHP method may be related to the fact that LDL-CBHP contains irremovable Lp(a)-C and cholesterol carried by IDL-C.The VAP method can be used as an accurate method for detecting real LDL-C without Lp(a)-C and IDL-C.
10.Research on the diagnostic value of pulmonary artery diameter characteristics and combined TNF-α in patients with HIV associated pulmonary hypertension

Objective To investigate the characteristics of pulmonary artery diameter in patients with human immunodeficiency virus(HIV)infection-associated pulmonary hypertension(PAH),and to explore the clinical significance of pulmonary artery diameter correlation indices combined with tumor necrosis factor-α(TNF-α)for early screening and diagnosis of HIV-associated pulmonary hypertension,aiming to provide valuable clinical insights.Methods A retrospective analysis was conducted on 65 HIV-infected patients admitted to the Eighth Hospital Affiliated to Guangzhou Medical University between May 2017 and December 2023.Based on echocardiog-raphy findings,the patients were categorized into an observation group(23 cases with PAH)and a control group(42 cases without PAH).Measurements of pulmonary artery diameter(PA),distal pulmonary artery diameter(dPA),and aortic diameter were obtained in both groups,and the ratio of dPA to aortic diameter(rPA)was cal-culated.Additionally,serum TNF-α levels were recorded.The clinical significance of each index in early screening and diagnosis of HIV-related pulmonary hypertension was analyzed.Results The PA,dPA,rPA,and TNF-α levels in the observation group were significantly higher compared to those in the control group.Pearson linear correlation analysis revealed a significant positive correlation between TNF-α and both dPA and rPA.Two-column correlation analysis demonstrated a significant association between HIV-PAH and PA,dPA,rPA,as well as TNF-α(P<0.05).The combined measurement of serum TNF-α with dPA and rPA exhibited an area under the curve(AUC)of 0.891 for diagnosing HIV-APAH,with a sensitivity of 0.870 and specificity of 0.881.Conclusion The combination of pulmonary artery diameter and TNF-α provides a more accurate indication of the presence of HIV-PAH,enabling early screening,follow-up,or intervention in high-risk patients.This finding holds significant implications for enhancing patient prognosis.
