1.Analysis of clarithromycin potential adverse drug interactions and establishment of refined prescription pre-review rules

OBJECTIVE To analyze the potential adverse drug interactions (pADIs) of clarithromycin, and establish refined prescription pre-review rules. METHODS Outpatient prescriptions of clarithromycin in combination with other drugs were collected from January 1, 2024 to June 30, 2024 through hospital information system of the Third People’s Hospital of Bengbu. pADIs were identified and their risk severities were graded according to Lexicomp and Micromedex databases. Then, refined prescription pre- review rules for clarithromycin pADIs-related drugs were established according to the identification and risk level results. RESULTS Among 3 046 clarithromycin combined drug prescriptions, 946 cases of pADIs occurred in 812 prescriptions. There were 6, 415 and 525 cases classified as “contraindicated”,“ major” and “moderate”, respectively. The combination drugs with “contraindicated” levels were tamsulosin, rupatadine, domperidone and ticagrelor, while those with “major” levels were mainly theophylline, dexamethasone and amlodipine. Accordingly, 26 refined rules were established, including 4 items of “warning information→prescription interception”, 11 items of “warning information→prescription double signature” and 11 items of “attention information→prescription approval”. CONCLUSIONS There are “contraindicated” and “major” risks associated with clarithromycin and its combination drugs in the hospital, and refined prescription pre-review rules for clarithromycin combined drug prescription have been established successfully.
2.Japan’s stable supply system for pharmaceuticals:its inspiration for enhancing the supply assurance capability of drugs in shortage in China

OBJECTIVE To provide insights for enhancing the supply assurance capability of drugs in shortage in China. METHODS By sorting out top-level design ideas for Japan’s stable supply system for pharmaceuticals, this study elaborates on its current policies for stabilizing drug supply. Policy suggestions were proposed for existing challenges in the supply of drugs in shortage in China. RESULTS & CONCLUSIONS To stabilize drug supply, Japan formulated a systematic 3-tier policy framework that ran through pre-prevention, early risk monitoring, and response mechanisms for supply problems. Pre-prevention measures included establishing a graded classification system for stable supply lists, strengthening government control over the drug production process, and implementing a continuous supply system and cessation reporting system for post-market drugs. Early risk monitoring included self-assessing risks in enterprises, disclosing information concerning supply assurance of enterprises, and providing enterprises with price protection and incentives measures. Response mechanisms for supply problems included establishing the mechanism of increasing production and coordinated supply in enterprises, opening fast-track review and approval channels for changes in raw materials, and establishing emergency systems of transferring drugs to supply. To balance supply assurance and strong regulation of drugs in shortage through refining the incentive and regulatory policies for them, it is recommended to refine the criteria for selecting drugs in shortage, strengthen the circulation of information, optimize price management methods, and clarify problem feedback and solving.
3.Bibliometric analysis of research hotspots and frontier trends in skin adverse reactions caused by epidermal growth factor receptor inhibitors in the treatment of lung cancer

Objective:To analyze the research hotspots and frontier trends in skin adverse reactions caused by epidermal growth factor receptor inhibitors(EGFRIs)in the treatment of lung cancer.Methods:CiteSpace software was used to visually analyze the literature on skin adverse reactions caused by EGFRIs in the treatment of lung cancer in the three databases of CNKI,WANFANG and VIP from the establishment of the database to October 30,2023.The literature was analyzed from the aspects of annual publication volume,funding,journals,authors,keywords,etc.Results:A total of 663 Chinese literature were obtained,with national funding projects accounting for 9.20%.A total of 312 journals were involved,with the highest publication volume being the Chinese Journal of Lung Cancer,accounting for 2.71%.The China-Japan Friendship Hospital and the Beijing University of Chinese Medicine were the top two research institutions in terms of the number of published papers,and they had a close cooperation relationship.The top three authors in terms of publication volume were CUI Huijuan with 18 articles,PENG Yanmei with 11 articles,and ZHANG Xu with 9 articles.Research hotspots focus on EGFRIs related rash,gefitinib,and clinical efficacy.Frontier trends focused on the clinical observation,research progress,and traditional Chinese medicine of skin adverse reactions caused by EGFRIs drugs.Conclusions:The research on skin adverse reactions caused by EGFRIs in the treatment of lung cancer has great development prospects.Authors and institutions can further form a stronger collaborative network to promote interdisciplinary communication,and jointly promote research and development in this field.
4.Prediction of paroxysmal atrial fibrillation based on heart rate variability analysis

Based on the analysis of heart rate variability(HRV),a prediction method for paroxysmal atrial fibrillation(PAF)attacks is proposed.A new adaptive filtering technique is used for smoothing and coarse graining of HRV,followed by entropy-based quantification of HRV complexity at multiple adaptive scales.After the features are normalized by Min-Max,feature subsets are selected by sequential forward selection method,and then input to support vector machine to identify HRV types and predict PAF attacks.Through 5-fold cross-validation on a set of 50 HRV sequences each lasting 5 minutes,the optimal prediction results are obtained:98%accuracy,100%sensitivity,96%specificity,demonstrating excellent performance.In addition,the experiment shows significant changes(P<0.05)in the complexity eigenvalues of HRV far away from and close to PAF at different frequency bands,reflecting alterations in nervous system regulation of cardiac rhythm and a decline in the ability to adapt to external environmental changes such as stress regulation.
5.Research on quality control standard and safety of liquid bandage

Objective To investigate the quality and safety of liquid bandage and establish a reliable quality control method. Methods The quality of liquid bandage was evaluated by appearance, film forming time, viscosity, comfort, waterproof and air permeability. The content was determined by HPLC. The safety of liquid bandage was investigated with skin irritation test and allergy test. Results The self-made liquid bandage was uniform and delicate, with good ductility, fast film formation, good mechanical strength, waterproof and air permeability. The content of active pharmaceutical ingredients in the film met the requirements. It had no obvious irritation to the skin and no allergic reaction. Conclusion The established quality control method was reliable. The liquid bandage had good safety profile.
6.Epidemiological investigation of a school COVID-19 outbreak in Shanghai

ObjectiveTo describe a COVID-19 outbreak due to SARS-CoV-2 Omicron variant in a school and provide suggestions for COVID-19 prevention and control. MethodsData on the COVID-19 outbreak in a school in Putuo District of Shanghai were collected from November 14 to December 20, 2022. Epidemiological characteristics, incidence rate of secondary cases and response measures were described and analyzed. ResultsA total of 27 COVID-19 cases were identified infected with SARS-CoV-2 Omicron BA.5.2 variant, including 14 students (51.9%) and 13 family members / teachers living with those students (48.1%). The first case occurred on November 14, with peak incidence during November 16-18. The median generation interval of the second generation cases was 2 (2,3) days. The index case (case 1, a student) had a clear contact history outside the school, which was found through routine examination in key populations in the school. Immediate management was conducted after the notification. All the second generation cases were exposed students in the same class with case 1, which were identified during the quarantine, whereas the third generation cases were their family members/teachers living with the second generation cases. The incidence rate within the class and school were 36.8% and 3.0%, respectively. No further social transmission was found outside the school. ConclusionEarly detection, multi-sectoral collaboration, prompt control and quarantine measures are effective in containing SARS-CoV-2 transmission. Health promotion, surveillance, ventilation and prophylactic disinfection should be reinforced in schools, office buildings and other gathering places.
7.Desmoid-type fibromatosis of the mesentery: a clinicopatho-logical and genetic analysis of 9 cases.

Nine cases of mesenteric desmoid-type fibromatosis were diagnosed and treated in Taizhou Hospital, Wenzhou Medical University between January 2010 and May 2022, including 2 females and 7 males, aged 16 to 59 years. The lesions were in the mesentery of small intestine with 7 cases, ileocecal junction with 1 cases and transverse colon with 1 case. The tumors had an unclear boundary and no envelope, the section was solid, gray and tough. The mean maximum diameter was (10.7±8.5) cm (range 3.5-33.0 cm). Microscopically, fusiform fibroblasts and myofibroblasts were parallel, bunched or staggered, buried in a large amount of extracellular collagen. The cell morphology was relatively consistent, without obvious atypia, and mitosis was rare. Immunohistochemistry showed that the tumor cells were positive for vimentin (9/9), β-catenin (9/9), while smooth muscle actin (5/9) stains were focally positive. Ki-67 proliferation index was 1%-10%. Cytokeratin Pan, S-100, STAT6, CD117, DOG1, CD34, desmin and anaplastic lymphoma kinase stains were negative. Genetic analysis showed that there were 7 cases of c.121G>A(p.Thr41Ala) mutation of CTNNB1 gene, 1 case of c.121G>A(p.Thr41Ala) and 1 case of c.134C>T(p.Ser45Phe) double mutation, and 1 case of wild type. Tumors were surgically resected in all 9 cases. Eight cases had no recurrence or metastasis, 1 case had recurrence 6 months later, and no recurrence or metastasis after additional surgical resection.

8.Qualitative Analysis of Components of Glandular Hairs in Lonicera Japonica Thunb. Buds by Laser Microdissection Combined with HPLC-TOF/MS

OBJECTIVE To accurately analyze the components of glandular hairs in Lonicera japonica Thunb. buds, and provide reference for quality evaluation and control of Lonicera japonica Thunb.. METHODS Laser microdissection technology was used to obtain glandular hairs of Lonicera japonica Thunb. buds, low-temperature freezing vortex method was used to removal flower buds surface hairs(glandular hairs and non glandular hairs), and the components of flower buds, flower buds removed surface hairs and glandular hairs were detected by HPLC-TOF/MS. Through comparison with standards, database retrieval and mass spectrometry fragmentation, the components were analyzed and identified. RESULTS Thirty-seven components were identified in the flower buds, 31 components were identified in the flower buds removed surface hairs, and 35 components were identified in the glandular hairs. Compared with flower buds, glandular hairs lack 2 phenolic acids, and flower buds removed surface hairs lack 6 flavonoids. CONCLUSION The glandular hair, which contains many secondary substances, is an important place for the synthesis and accumulation of secondary substances in Lonicera japonica Thunb. buds, and is closely related to the quality of Lonicera japonica Thunb..
9.The Biocompatibility of Multi-Source Stem Cells and GelatinCarboxymethyl Chitosan-Sodium Alginate Hybrid Biomaterials

BACKGROUND:
Nowadays, biological tissue engineering is a growing field of research. Biocompatibility is a key indicator for measuring tissue engineering biomaterials, which is of great significance for the replacement and repair of damaged tissues.
METHODS:
In this study, using gelatin, carboxymethyl chitosan, and sodium alginate, a tissue engineering material scaffold that can carry cells was successfully prepared. The material was characterized by Fourier transforms infrared spectroscopy. In addition, the prepared scaffolds have physicochemical properties, such as swelling ratio, biodegradability.we observed the biocompatibility of the hydrogel to different adult stem cells (BMSCs and ADSCs) in vivo and in vitro. Adult stem cells were planted on gelatin-carboxymethyl chitosan-sodium alginate (Gel/SA/CMCS) hydrogels for 7 days in vitro, and the survival of stem cells in vitro was observed by live/died staining. Gel/SA/CMCS hydrogels loaded with stem cells were subcutaneously transplanted into nude mice for 14 days of in vivo culture observation. The survival of adult stem cells was observed by staining for stem cell surface markers (CD29, CD90) and Ki67.
RESULTS:
The scaffolds had a microporous structure with an appropriate pore size (about 80 lm). Live/died staining showed that adult stem cells could stably survive in Gel/SA/CMCS hydrogels for at least 7 days. After 14 days of culture in nude mice, Ki67 staining showed that the stem cells supported by Gel/SA/CMCS hydrogel still had high proliferation activity.
CONCLUSION
Gel/SA/CMCSs hydrogel has a stable interpenetrating porous structure, suitable swelling performance and degradation rate, can promote and support the survival of adult stem cells in vivo and in vitro, and has good biocompatibility. Therefore, Gel/SA/CMCS hydrogel is a strong candidate for biological tissue engineering materials.
10.Epidemiological characteristics of scarlet fever in Putuo District, Shanghai from 2015 to 2020

ObjectiveTo analyze the epidemiological characteristics of scarlet fever in Putuo District, Shanghai from 2015 to 2020, and provide a basis for formulating effective prevention and control measures. MethodsThe data of scarlet fever cases from January 1, 2015 to December 31, 2020 in Putuo District, Shanghai were obtained from the Infectious Disease Reporting Information Management System of the Chinese center for disease control and prevention (CDC). Descriptive epidemiological method was used to analyze the epidemiological characteristics of scarlet fever cases in Putuo District, Shanghai by time, population, and region. ResultsA total of 586 scarlet fever cases were reported in Putuo District from 2015 to 2020, with an average annual reported incidence of 7.39 per 105, an average age of onset of 6.2±1.8 years, and a male/female ratio of 1.6∶1. The incidence of scarlet fever has declined significantly in 2020, which was 5.44 per 105 lower than the average reported incidence in 2015‒2020. The incidence peak was from April to June and November to January of the following year, which was characterized by high incidence in spring and winter. The age distribution of cases was concentrated at the age of 4‒9 years, mainly children in kindergarten and primary school students. Areas with high reported incidence rates were Wanli Street, Changzheng Town, Taopu Town and Changfeng New Village Street. ConclusionThe peak incidence of scarlet fever in Putuo District is from April to June and November to January of the following year, with children in kindergartens and students in primary school as the key populations. We should strengthen epidemic monitoring and guidance of prevention and control for in kindergartens and schools before the high incidence season of scarlet fever, so as to effectively prevent the epidemic of the disease.
