1.Survey and analysis of long term care service needs for home disabled elderly people in Beiliu city

Objective To investigate the current situation of long-term care needs of the elderly with home-based disabil-ity in Beiliu City,and provide data support for the development of Internet plus care services in Beiliu City.Methods A ques-tionnaire survey method was used to design and distribute a survey questionnaire titled"Survey Questionnaire on Home Care Serv-ice Needs of Disabled Elderly People",to investigate the nursing service methods and home care needs of disabled elderly people in Beiliu City.Results Disabled elderly people mainly choose home-based nursing services,with a high demand for basic care,comfortable care,rehabilitation care,psychological care,and other aspects.Conclusion Home care is of great significance for improving the quality of life of disabled elderly people,and requires policy support such as medical insurance to promote the sup-ply of nursing services and meet the care needs of disabled elderly people.
2.Quality comparability analysis on production site change of human coagulation factor Ⅷ

Objective To study the quality comparability of human coagulation factor Ⅷ(FⅧ)produced before and after the change of factory site.Methods A comparative study was carried out on quality quantitative indexes,related im-purities and stability data of FⅧ produced before and after the change of factory site.Results The FⅧ quantitative quality before and after the change of factory site all met the quality standard,and the related impurities including aluminum resi-due,tributyl phosphate residue,polysorbate 80 residue and PEG residue all met the quality standard.Other impurities in-cluding human fibrinogen,fibronectin,plasminogen,IgA,IgM and IgG were extremely low in content and equivalent in quality.The content of VWF(von Willebrand factor)had no obvious change before and after the change of factory site,but was significantly higher than that of other domestic manufacturers'commercial products.The results of accelerated stability and long-term stability tests showed that the titer of FⅧ fluctuated within the methodological error range,and the results all met the quality standard.Conclusion The change of factory site of FⅧ has no effect on the quality.
3.Application of steril wound dressin in the prevention of friction injury by suture at mouth corner in dental im-plantation surgery

The use of suture to pull the lingual gingival flap in mandibular posterior dental implant surgery may damage the contralateral mouth corner of the surgical area.This study explored the effectiveness of sterile wound dressing in preventing the friction injury by suture in oral implant surgery.A total of 506 patients were included,the sterile wound dressing was used in the test group(n=363)but not in the control group(n=143).The postoperative situation of the patients was analyzed,validated and received holistic care after implant surgery.The results suggest that the use of sterile wound dressing can shorten the operation time,prevent suture friction of mouth corer,simplify post-operative care and improve patient satisfaction for posterior dental implant surgery.
4.Medication Analysis of Traditional Chinese Medicine Diagnosis and Treatment Guidelines for Allergic Rhinitis Based on Clinical Efficacy Evaluation

ObjectiveTo explore the role of efficacy evaluation methods in providing evidence for traditional Chinese medicine (TCM) clinical practice guidelines based on a demonstration study of clinical efficacy evaluation of TCM for allergic rhinitis (AR),aiming to enrich the sources of evidence for guideline development. MethodReal-world data of TCM medication for AR were collected and efficacy evaluation was carried out. SPSS 16.0 software was used to calculate the conformity of clinical syndromes,main prescriptions, and medications to the guidelines. Correlation analysis of efficacy and medications was performed according to guideline conformity to compare the differences in medications between real-world clinical applications and current guidelines. ResultA total of 198 cases were collected and the clinical medications were compared with the relevant guidelines for AR. It was found that the clinical syndrome conformity was above 70%,and in addition to the guideline syndrome,there were also syndromes in six meridians and other mixed syndromes in clinical practice. The guideline conformity of the main prescriptions used in clinical practice showed a positive correlation trend with efficacy. There were some differences between the commonly used drugs in clinical practice and those recommended by the guidelines. For example,for lung Qi deficiency and cold syndrome,drugs such as Angelicae Dahuricae Radix, Magnoliae Flos, and Schisandrae Chinensis Fructus were often used. For kidney Yang deficiency syndrome,drugs such as Atractylodis Macrocephalae Rhizoma and Ostreae Concha were often used. For spleen Qi deficiency and weakness syndrome,drugs such as Poria, Cinnamomi Ramulus, and Magnoliae Flos were used. For lung Qi deficiency and cold syndrome in children,drugs such as Chebulae Fructus, Cicadae Periostracum, Zingiberis Rhizoma, and Ginseng Radix et Rhizoma were used. For lung and spleen Qi deficiency syndrome,drugs such as Dioscoreae Rhizoma, Menthae Haplocalycis Herba, Amomi Fructus, and Zingiberis Rhizoma were used. There were also some differences in the commonly used drugs for adults and children. For example,for lung Qi deficiency and cold syndrome,adults often used Angelicae Dahuricae Radix, Magnoliae Flos, and Tribuli Fructus,while children often used Ginseng Radix et Rhizoma, Paeoniae Radix Alba, and Poria. For lung meridian hidden heat syndrome, adults often used Bupleuri Radix, Asari Radix et Rhizoma, Pinelliae Rhizoma, and Xanthii Fructus, while children often used Houttuyniae Herba, Menthae Haplocalycis Herba, Citri Reticulatae Pericarpium, and Massa Medicata Fermentata. ConclusionEffective medication regimens can be screened out based on efficacy evaluation methods,which can help supplement immediate, objective, and clinically relevant evidence of medication for the development of clinical practice guidelines in TCM from the perspective of clinical effectiveness.
5.Isolation and genome-wide evolutionary analysis of a type Ⅲ WU polyomavirus strain

Objective:To isolate and culture WU polyomavirus (WUPyV), and to analyze the genome-wide evolutionary patterns, homology and population dynamics.Methods:Real-time quantitative PCR was used to detect the nasopharyngeal aspirate samples of hospitalized children with respiratory tract infection in Beijing Friendship Hospital during 2020 to 2022. Primary human airway epithelial cells cultured at the air-liquid interface were used to isolate and culture WUPyV. Whole genome sequence of the isolated strain was obtained by Sanger sequencing. For phylogenetic and evolutionary dynamics analysis, the whole genome was compared with the published whole genome sequences in GenBank database.Results:The detection rate of WUPyV was 4.7% (31/659) during 2020 to 2022, and a clinical strain BJ0593 of WUPyV type Ⅲc was successfully isolated. The homology of the whole genome and gene fragments of WUPyV was high. The average evolutionary rate of VP2 gene was about 1.256×10 -4 substitution/site every year, and the population dynamics of WUPyV tended to be flat in the last decade. Conclusions:This study successfully isolated a clinical WUPyV type Ⅲ strain for the first time, which provided the basis for further investigation on the molecular evolution and pathogenicity of WUPyV.
6.Application of Health Care Failure Mode and Effect Analysis Combined with Root Cause Analysis in Intelligent Management of Narcotic Use in the Hospital

OBJECTIVE To discuss the application effect of health care failure mode and effect analysis(HFMEA) and root cause analysis(RCA) in the intelligent management of narcotic use in the hospital. METHODS Set up an analysis team to use HFMEA to list potential failure modes in the flow chart of intelligent management of narcotic use in the hospital, and analyze and evaluate the key failure modes in combination with RCA, determine the root cause, and then formulate improvement measures and evaluate the effects after implementation. RESULTS After the implementation of the improvement measures, the risk priority number of the eight potential failure modes decreased significantly(P<0.05). The satisfaction of pharmacists, doctors and nurses with the intelligent management process of narcotic use in the hospital increased from (88.39±2.21)% to (98.04±0.51)%, from (87.79±1.36)% to (97.55±1.24)%, and from (90.79±1.39)% to (95.68±1.30)%, respectively. CONCLUSION HFMEA combined with RCA can reduce the risk of narcotic drug abuse, ensure drug quality and safety, reduce the occurrence of dispensing errors, and ensure the rational clinical use of narcotic drugs.
7.Overview of the Application of Automated Intelligent Drug Cabinet in the Field of Hospital Medicine Management

OBJECTIVE To review the application and development of automatic intelligent drug cabinet in hospitals at home and abroad. METHODS The relevant research articles published from 2010 to 2022 were retrieved from CNKI, Wanfang, VIP, PubMed and other databases. RESULTS The application of automatic intelligent drug cabinet could shorten the execution time of inpatient medical orders, improve the work efficiency of pharmacists and doctors and nurses, improve the quality of drug management in wards and promote the transformation of hospital pharmaceutical care. CONCLUSION The construction of decentralized dispensing mode with automated intelligent drug cabinets as the core has important theoretical guiding significance and broad application prospects for the automation, informatization, intelligence and whole process management of drugs in medical institutions, and also helps to improve the level of medical pharmacy service management.
8.Study on chemical composition difference and analgesic effect of Paeonia lactiflora before and after being processed with wine

OBJECTIVE To investigate the chemical composition difference of Paeonia lactiflora before and after being processed with wine and its analgesic effect. METHODS UPLC-Q-TOF-MS was used to analyze the chemical composition of P. lactiflora before and after being processed with wine. Wister rats were randomly divided into blank group, model group, positive control group (ibuprofen 0.072 mg/g) and P. lactiflora low-dose and high-dose groups (0.005, 0.01 mg/g, by raw material) and P. lactiflora stir-baked with wine low-dose and high-dose groups (0.005, 0.01 mg/g, by raw material), with 6 rats in each group; the dysmenorrhea model of rats was induced by estradiol benzoate combined with oxytocin; the incubation period of body twisting in rats was recorded after the last medication; the contents of prostaglandin F2α(PGF2α), progesterone, nitric oxide (NO) and β -endorphin in uterine tissue homogenate were detected. RESULTS Same 60 chemical compositions were identified in both positive and negative ion modes of P. lactiflora and P. lactiflora stir-baked with wine. The contents of 7 compositions, such as linalool, pedunculagin and syringic acid, were increased after stir-baked with wine, but the contents of other compositions were decreased. Compared with model group, the incubation period of body twisting in all administration groups (except for P. lactiflora low-dose group) was significantly prolonged (P<0.05); PGF2α in uterine tissue homogenate (except for P. lactiflora low-dose group) was significantly decreased; the contents of progesterone (except for P. lactiflora low-dose and high-dose groups), NO (except for P. lactiflora low-dose and high-dose groups) and β-endorphin were significantly increased (P<0.05). CONCLUSIONS The analgesic effect of P. lactiflora stir-baked with wine on dysmenorrhea model rats is better than that of P. lactiflora, which may be related to the increase of 7 compositions such as linalool, pedunculagin and syringic acid after stir-baked with wine.
9.Therapeutic effect of modified Shengjiang Powder acupoint application for hypertension patients with phlegm-heat syndrome

Objective:To evaluate the effect of modified Shengjiang Powder acupoint application on hypertension patients with phlegm-heat syndrome. Methods:A total of 61 patients in Shijiazhuang hospital of traditional chinese medicine (TCM) meeting the inclusion criteria from September 2018 to September 2019 were randomly divided into two groups, the treatment group was 30 cases, the control group was 31 cases. The control group was given Amlodipine Besylate Tablets orally, and the treatment group added modified Shengjiang Powder acupoint application, and both groups were treated for 4 weeks. TCM syndrome scores were performed before and after treatment, and 24 h ambulatory blood pressure were measured with an ambulatory blood pressure monitor, including daytime mean systolic blood pressure (dMSBP), daytime mean diastolic blood pressure (dMDBP), night time mean systolic blood pressure (nMSBP), and night time mean diastolic blood pressure (nMDBP), 24-hour mean systolic blood pressure (24 hMSBP), 24-hour mean diastolic blood pressure (24 hMDSBP) and blood pressure circadian rhythm detection. Results:After treatment, the nMSBP and 24 hMSBP in the treatment group were significantly lower than those control group ( t values were -2.331,-1.511, P values were 0.045, 0.033), the blood pressure circadian rhythm [(8.87 ± 8.33)% vs. (6.94 ± 6.12)%, t=1.034] of the treatment group was significantly higher than those of the control group ( P<0.05); mdizziness, headache, head wraps, vomiting, sputum salivation, upset, mouth pain, insomnia scores of the treatment group after treatment were significantly lower than those in the control group ( P<0.05). Conclusion:The modified Shengjiang Powder acupoint application can reduce the blood pressure level of hypertension patients with phlegm-heat syndrome, especially for the improvement of night blood pressure, and improve the clinical symptoms.
10.High resolution CT findings and clinical features of the novel coronavirus B.1.617.2 variant

Objective:To investigate high resolution CT (HRCT) manifestations and clinical features of patients infected with the novel coronavirus B.1.617.2 variant.Methods:A total of 125 patients with the novel coronavirus B.1.617.2 variant in Guangzhou Eighth People′s Hospital from May 21 to June 9, 2021 was enrolled. There were 52 males and 73 females, aged from 1 to 92 years old with a median age of 47 year. The clinical features and HRCT characteristics were analyzed retrospectively.Results:Of the 125 patients, the main clinical manifestations were fever in 54 patients, cough in 50 patients, pharyngeal discomfort in 39 patients, and diarrhea in 5 patients. HRCT showed pneumonia in 96 cases, which predominantly had ground-glass opacities in 92 cases, ground-glass opacities combined with local consolidation in 22 cases, consolidation in 11 cases, intralobular interstitial thickening in 51 cases, centrilobular structural thickening in 23 cases, and cord-like lesions in 33 cases. One patient had pleural effusion, and no enlarged lymph nodes were observed in all patients. The lesions were distributed in the subpleural and/or peripheral lung in 96 cases and along the bronchovascular bundle (middle and inner zone) in 24 cases. The time interval from onset to positive HRCT was 3 (1, 4) days in 96 patients with pneumonia on HRCT. HRCT showed absorption after the imaging findings reaching the peak in 20 patients, and the time interval between the first positive HRCT findings and the peak imaging was 6 (3, 7) days in these 20 patients.Conclusions:The novel coronavirus B.1.617.2 variant has a strong infectivity, short viral latency. Lung injury is decreaseel. People of all ages are susceptible and the diseasemay have rapid pnegression. HRCT mainly shows ground glass opacities, which are more common in subpleural and/or in the lung periphery, with interlobular thickening.
