1.The association between dietary fiber intake and all-cause mortality and cardiovascular disease mortality in patients with stroke: a retrospective cohort study of NHANES

		                        			Stroke represents the primary cause of death and persistent disability globally, leading to around 5.5 million annual patient fatalities. The objective was to explore the relationship of dietary fiber with all-cause and cardiovascular disease (CVD) mortality risk in patients with stroke. 
		                        		
		                        			SUBJECTS/METHODS:
		                        			We extracted stroke patients’ data from the National Health and Nutrition Examination Survey (NHANES) database. All-cause and CVD mortality were outcomes. Dietary fiber consists of non-digestible forms of carbohydrates, usually polysaccharides that originate from plant-based foods. Covariates including demographic data, vital signs, comorbidities, laboratory parameters, and medication use were screened using the weighted multivariate Cox regression models with backward elimination. Weighted univariate and multivariate Cox regression models were performed to explore the relationship between dietary fiber intake and all-cause/CVD mortality, with hazard ratios (HRs) and 95% confidence intervals (CIs). The association was further investigated in different subgroups. 
		                        		
		                        			RESULTS:
		                        			A total of 1,578 patients with stroke were included, of whom 688 (43.6%) died.Total fiber and vegetable fiber intake were analyzed as categorical variables, and the lowest intake was considered reference groups. High intake of total fiber (HR, 0.73; 95% CI, 0.57–0.94) and high intake of vegetable fiber (HR, 0.63; 95% CI, 0.48–0.82) were related to lower all-cause mortality risk in individuals with stroke. Similar findings were also observed between higher total fiber (HR, 0.56; 95% CI, 0.37–0.85) and vegetable fiber intake (HR, 0.57; 95% CI, 0.36–0.89) with decreased CVD mortality risk. The relationship between higher total fiber intake and lower all-cause mortality risk was discovered in individuals aged ≥ 60 yrs, smoking, non-CVD, and chronic kidney disease (CKD). High total fiber, or vegetable fiber consumption was linked to lower CVD mortality risk in stroke individuals aged ≥ 60 yrs, females, body mass index ≥ 30 kg/m 2 , non-smoking, and CKD. 
		                        		
		                        			CONCLUSION
		                        			Dietary fiber intake and vegetable fiber intake may benefit the prognosis of patients with stroke. 3.Polygonatum Sibiricum Polysaccharides Improve Colonic Injury in a Mouse Model of Chronic Obstructive Pulmonary Disease by Regulating Bile Acid Metabolism in the Colon

		                        			ObjectiveTo investigate the effect and mechanism of Polygonatum neutral polysaccharides from sibiricum (PSP-NP) on colon injury in mice with chronic obstructive pulmonary disease (COPD). MethodsMale C57BL/6J mice were randomly divided into a control group, a COPD model group, and a PSP-NP group. The COPD model was established using smoke exposure combined with intranasal LPS administration. The PSP-NP group was simultaneously treated daily with 200 mg/kg of PSP-NP via intragastric gavage, while the other groups received an equal volume of saline. HE staining was used to observe the pathological changes in the colon. ELISA was employed to detect the levels of LPS in serum and the expressions of ZO-1, Occludin, IL-6, and TNF-α in colon tissue. UPLC-MS was used to detect the types and contents of bile acids in colonic content, and to screen for differential bile acids. Differential microbial flora were identified using 16S rRNA gene sequencing, and correlation analysis was conducted with differential bile acids. PSP-NP was combined with the differential bile acids cholic acid (CA), and deoxycholic acid (DCA) in vitro to analyze the binding capacity of PSP-NP for CA and DCA. PSP-NP was applied to NCM460 normal colonic epithelial cells cultured in CA and DCA. Cell migration ability was assessed using the scratch assay, and the mRNA expression levels of inflammatory cytokines TNF-α, IL-6, and NF-κB were measured by RT-qPCR. ResultsPSP-NP effectively improved colonic damage in COPD model mice, enhanced mechanical barrier function, alleviated inflammatory response, and regulated abnormal changes in colonic flora and bile acid metabolism. Correlation analysis further revealed that PSP-NP regulated colonic bile acid metabolism and reduced the redundancy of secondary bile acids by increasing the relative abundance of Bacteroidota, Verrucomicrobiota, Bacteroides, and Akkermansia, while decreasing the relative abundance of Lactobacillus and Bifidobacterium. Notably, in vitro binding assays demonstrated that PSP-NP bound to differential bile acids DCA and CA, with the strongest binding capacity for DCA at 58.2%. In cellular functional studies, DCA inhibited the migration ability of colonic epithelial cells NCM460 and significantly increased the relative mRNA expression levels of inflammatory factors TNF-α, IL-6, and NF-κB. Importantly, co-treatment with PSP-NP significantly ameliorated the impact of DCA on NCM460 cells. ConclusionsPSP-NP may significantly improve colonic damage in COPD model mice. The mechanism may involve the regulation of colonic bile acid metabolism and bile acid profiles through both microbial modulation and direct binding, thereby reducing the damage caused by secondary bile acids such as DCA to colonic epithelial cells. 
		                        		
		                        		
		                        		
		                        	
4.Bacterial Diversity in Phyllosphere and Rhizosphere Soil of Angelica sinensis in Continuous Cropping and Effects of Different Disease Control Measures

		                        			ObjectiveTo investigate the bacterial diversity in the rhizosphere soil and phyllosphere of Angelica sinensis and examine the effects of foliar applications of a composite bacterial agent,salicylic acid,and coronatine on the bacterial diversity,disease incidence,and plant yield,thus providing a theoretical basis and guidance for the artificial construction of functional minimal communities and the regulation of rhizosphere through foliar treatments. MethodsUnder continuous cropping conditions in the field,foliar applications of a composite bacterial agent,salicylic acid,coronatine,and sterile water were conducted. The 100-plant weight was measured via the conventional method,and the incidence of diseases was recorded. The microbial community composition,diversity,and inter-group differences in the phyllosphere and rhizosphere soil of A. sinensis were analyzed by 16S high-throughput sequencing,and the potential microbial functions were predicted. ResultsCompared with the blank control,foliar applications of salicylic acid and coronatine both significantly reduced the yield and root rot incidence of A. sinensis. The foliar application of salicylic acid decreased the content of ferulic acid and increased that of ligustilide. The foliar application of coronatine increased the content of both ferulic acid and ligustilide. The microbial communities and functions in the phyllosphere and rhizosphere soil were significantly different. The phyllosphere had lower microbial diversity,with all bacteria being Gram-negative,mainly Cyanobacteria and Proteobacteria with limited functions. The rhizosphere soil had higher microbial diversity,harboring dominant phyla including Proteobacteria,Actinobacteria,Acidobacteria,and Bacteroidetes with rich functions. All foliar treatments regulated the microbial community in the rhizosphere soil,with a more significant effect on the microbial community in the rhizosphere soil than that in the phyllosphere. The coronatine treatment significantly reduced the abundance of Proteobacteria and nitrate-reducing and aromatic compound-degrading microorganisms in the rhizosphere soil,thus affecting nutrient cycling and autotoxic substance degradation and leading to a yield reduction. Compared with the salicylic acid treatment,the coronatine treatment significantly increased the abundance of Bacillus and Streptomyces in the rhizosphere soil,demonstrating enhanced disease control efficacy. ConclusionFoliar application of coronatine and salicylic acid can significantly regulate the composition and function of bacterial communities in the rhizosphere soil,thereby reducing the disease incidence and the plant yield. 
		                        		
		                        		
		                        		
		                        	
5.Analysis of the anticoagulant effect and influencing factors of warfarin in patients after left ventricular assist device implantation guided by gene test

		                        			OBJECTIVE To evaluate the effectiveness and safety of warfarin anticoagulation therapy guided by gene test in patients undergoing left ventricular assist device (LVAD) implantation, and to analyze the influencing factors of warfarin anticoagulation efficacy. METHODS Patients who underwent LVAD implantation at the Heart and Vascular Center of Northern Jiangsu People’s Hospital from January 2023 to October 2024 and required warfarin anticoagulant therapy were selected as the study subjects. They were divided into genetic testing group (n=51) and empirical treatment group (n=17) based on whether they underwent CYP2C9 and VKORC1 gene test. The gene test group was given warfarin based on the predicted dose calculated by gene test, while the empirical treatment group was given warfarin by clinical doctors based on international normalized ratio (INR) experience, all patients were given warfarin once a day. Follow-up observation was conducted for 6 months to compare the effectiveness [time in therapeutic range(TTR), the time required to reach INR for the first time, the incidence of embolic events, the incidence of INR<1.5 events] and safety (the incidence of major and minor bleeding events,the incidence of INR>3.5 events) of warfarin treatment between two groups of patients. According to whether the patient’s TTR was ≥60%, they were divided into TTR≥60% group (n=20) and TTR<60% group (n=48). Univariate and multivariate binary Logistic regression analysis were used to determine the factors affecting the anticoagulant effect of warfarin in patients. RESULTS The TTR of patients in the gene test group was significantly higher than that in the empirical treatment group (P<0.05). The incidence of INR<1.5 events in the gene test group was significantly lower than in the empirical treatment group (P<0.05). The incidence of minor bleeding events and INR>3.5 events in the gene test group were lower than in the empirical treatment group, but the difference was not statistically significant (P>0.05). The results of multivariate binary Logistic regression analysis showed that gene test was an independent protective factor for warfarin anticoagulant therapy [odds ratio (OR)=10.842, 95% confidence interval (CI): 1.211-27.037, P=0.033], and the combination of statins was an independent risk factor for warfarin anticoagulant therapy [OR=0.196, 95%CI: 0.045-0.861, P=0.031]. CONCLUSIONS Under the guidance of gene test, warfarin anticoagulation therapy for LVAD patients after implantation can improve TTR, shorten the anticoagulation target time, and has good safety; meanwhile, it should be noted that the combination of statins may enhance the anticoagulant effect of warfarin, thereby increasing the risk of bleeding in patients.
		                        		
		                        		
		                        		
		                        	
6.Structural Characterization and Evaluation of Anti-ulcerative Colitis Activity of Homogeneous Polysaccharide from Astragali Radix-Angelicae Sinensis Radix Herb Pair

		                        			ObjectiveTo investigate the immunomodulatory effect of polysaccharides from Astragali Radix-Angelicae Sinensis Radix herb pair(Qi-gui polysaccharides) on lipopolysaccharide(LPS)-induced RAW264.7 macrophages and to characterize the structure of the active component Qi-gui homogeneous polysaccharide(AAPS-4a), and evaluate its protective effect on ulcerative colitis(UC). MethodsThe effects of six Qi-gui polysaccharides(0.01-100 mg·L-1) on the proliferation of RAW264.7 cells were assessed by cell proliferation and activity assay(CCK-8), and enzyme-linked immunosorbent assay(ELISA) was used to investigate the effects of the six polysaccharides(3, 10 mg·L-1) on the secretion levels of tumor necrosis factor(TNF)-α, interferon(IFN)-β, and nitric oxide(NO) in LPS-induced RAW264.7 cells. After screening for active polysaccharides, high-performance size-exclusion chromatography(HPSEC) was used to determine its homogeneity and relative molecular weight, then its characteristic functional groups were identified by Fourier transform infrared spectroscopy(FT-IR), monosaccharide composition was analyzed by high performance liquid chromatography(HPLC), methylation analysis combined with gas chromatography-mass spectrometry(GC-MS) was performed to determine the types and linkage modes of sugar residues, and one- and two-dimensional nuclear magnetic resonance(NMR) were used to identify the sugar residue composition and configuration of the active polysaccharide. Finally, experimental animals were divided into the normal group, model group, AAPS-4a low-dose group(50 mg·kg-1), AAPS-4a high-dose group(100 mg·kg-1), and sulfasalazine(SASP) group (75 mg·kg-1). Except for the normal group, the acute UC mouse model was induced using 3.5% dextran sulfate sodium salt(DSS). Each treatment group was administered the corresponding dose via oral gavage for 7 days, and changes in body weight were recorded. After treatment, the spleen index and disease activity index(DAI) score were calculated, TNF-α and interleukin-6(IL-6) levels in the serum were detected by ELISA, and histopathological changes in colon tissue were observed by hematoxylin-eosin(HE) staining. ResultsAt the cellular level, AAPS-4a exhibited a dose-dependent inhibition of LPS-induced increases in TNF-α, IFN-β, and NO levels(P<0.01). Structural characterization of AAPS-4a revealed that it was a homogeneous polysaccharide with a relative molecular weight of 7.6×103 Da, consisting of mannose(Man), glucose(Glc), and galactose(Gal) in a molar ratio of 1.3∶23.9∶1.0. It was primarily composed of five sugar residues of 1,6-α-D-Glcp, T-α-D-Glcp, 1,3-β-D-Galp, 1,4-α-D-Manp, and 1,2-α-D-Galp. In vivo experiments showed that compared with the normal group, the model group demonstrated markedly increased DAI score and spleen index, significantly reduced colon length, and significantly elevated levels of TNF-α and IL-6(P<0.01). Compared with the model group, the AAPS-4a high-dose group significantly reduced the DAI score and spleen index, as well as TNF-α and IL-6 levels, and improved colonic atrophy(P<0.05, P<0.01). Pathological observations showed that AAPS-4a significantly inhibited inflammatory cell infiltration in colon tissue and alleviated pathological damage. ConclusionAAPS-4a, a neutral homogeneous polysaccharide composed of 1,6-α-D-Glcp, T-α-D-Glcp, 1,3-β-D-Galp, 1,4-α-D-Manp and 1,2-α-D-Galp, is identified as a key bioactive component contributing to the anti-UC effect of the Qi-gui herb pair. 7.Impact of rigid container material type and loading volume on the sterilization of thoracoscopic instruments

		                        			ObjectiveTo explore the impacts of material type and loading volume of rigid containers on the hydrogen peroxide low temperature plasma sterilization of thoracoscopic instruments, to identify the best rigid containers and loading volume of thoracoscopic instruments. MethodsThoracoscopic instruments sterilized by STERRAD® 100NX hydrogen peroxide low temperature plasma in Shanghai Pulmonary Hospital affiliated to Tongji University from August to September 2024 were selected as the research items. According to the material of rigid containers, the instruments were divided into polyethylene case group (A), stainless steel case group (B) and silicone resin case group (C). In terms of the loading volume, the rigid containers were divided into (loading capacity <80%) groups of 8, 10 and 12 instruments. The results of physical monitoring, the first type of chemical indicator card monitoring, and the five types of card luminal chemical process challenge device (PCD) monitoring of the 9 groups of A8, A10, A12, B8, B10, B12, C8, C10 and C12 were compared and evaluated. ResultsCompared to A8, A10 A12, C8, C10 or C12 groups, the thoracoscope instruments in the stainless steel containers in B8, B10 or B12 group had higher hydrogen peroxide concentrations and shorter elapsed time in the pressure check phases 1 and phases 2, with the differences statistically significant (P<0.05), followed by the silicone resin case group and the polyethylene case group. The nine groups of physical parameter monitoring, the first type of chemical indicator monitoring, and the five types of chemical PCD monitoring for lumen sterilization achieved 100% qualification rates, and there were no significant differences in the qualified rates of sterilization among the 9 groups (P>0.05). ConclusionWhen using hydrogen peroxide low temperature plasma to sterilize thoracoscopic instruments, it is recommended to use stainless steel or silicone resin rigid containers with a controlled loading capacity (≤12) to ensure optimal sterilization quality. 
		                        		
		                        		
		                        		
		                        	
8.Construction and practice of an intelligent management system for preoperative anemia based on multidisciplinary collaboration

		                        			Objective: To improve the efficiency and standardization of preoperative anemia diagnosis and treatment by establishing a systematic intelligent management platform for preoperative anemia. Methods: A multidisciplinary collaborative model was adopted to develop a preoperative anemia management system that integrates intelligent early warning, standardized treatment pathways, and quality control. The system utilizes natural language processing technology to automatically capture laboratory data and establish evidence-based medical decision support functions. A pre-post study design was employed to compare changes in preoperative anemia screening rates, preoperative anemia intervention rates, reasonable use of iron supplements, and perioperative red blood cell transfusion rates before and after system implementation. Results: After system implementation, the standardization of anemia diagnosis and treatment significantly improved: 1) Screening effectiveness: The anemia screening rate increased to 50.00% (an increase of 27.24%); 2) Intervention effectiveness: The anemia treatment rate rose to 56.30% (an increase of 14.02%); 3) Treatment standardization: The reasonable use rate of iron supplements increased to 55.33% (an increase of 21.02%); the red blood cell transfusion rate decreased to 18.29% (a decrease of 4.07%), and the amount of red blood cell transfusions was reduced by 291 units. Conclusion: This system achieves full-process management of preoperative anemia through information technology, significantly enhancing the standardization of diagnosis and treatment as well as intervention effectiveness, providing an effective solution for perioperative anemia management.
		                        		
		                        		
		                        		
		                        	
9.Efficacy of flexible ureteroscope lithotripsy with flexible vacuum-assisted urethral access sheath for 1-2 cm lower renal calyceal stones

		                        			[Objective] To compare the clinical efficacy and safety of flexible ureteroscope lithotripsy (FURL) combined with flexible vacuum-assisted urethral access sheath (FV-UAS) and traditional UAS in the treatment of 1-2 cm lower renal calyceal stones, so as to provide reference for clinical practice. [Methods] Clinical data of 157 patients with 1-2 cm lower renal calyceal stones treated with FURL during Mar.2021 and Oct.2023 were retrospectively analyzed, including 80 treated with traditional UAS, and 77 with FV-UAS.General and clinical information of the two groups were compared. [Results] The immediate stone-free rate (SFR) (84.4% vs.67.5%, P=0.013) and final SFR (88.3% vs. 75.0%, P=0.032) of the FV-UAS group were significantly higher than those of the traditional UAS group, with significant difference.The incidence of postoperative complications such as fever, renal colic, and perirenal hematoma was significantly higher in the traditional UAS group than in the FV-UAS group (15.0% vs.5.2%, P=0.042). After treatment with anti-infective and analgesic drugs, both groups were improved, and no severe sepsis or septic shock occurred after surgery.The hospitalization expenses of the FV-UAS group were significantly lower than those of the traditional UAS group [ (18 341±1519)yuan vs.(19 152±1826)yuan, P=0.003]. [Conclusion] Compared to the traditional UAS, the combination of FURL and FV-UAS for the 1-2 cm lower renal calyceal stones has a high SFR and low incidence of complications.Patients experience less pain, recover faster and spend less.It's a new treatment option for inferior calyceal calculi.
		                        		
		                        		
		                        		
		                        	
10.Mechanical stability of intertrochanteric fracture of femur with different internal fixation systems

		                        			BACKGROUND:Intertrochanteric fracture of femur has various fracture types and fixation methods,and the mechanical stability of each fixation system is quite different.It is of scientific clinical significance to use finite element analysis method to carry out biomechanical research on various fixation systems. OBJECTIVE:To compare and analyze the mechanical stability of various internal fixations applied to femoral intertrochanteric fracture A031-A2.1 by finite element method. METHODS:Based on the validated finite element model of femur(Intact),the model was cut and made into A031-A2.1 intertrochanteric fracture of femur.Different internal fixation systems were implanted by simulating clinical operation methods,and fixation models of proximal femoral nail antirotation,dynamic hip screw,percutaneous compression plate and proximal femoral locking plate were established respectively.All nodes under the distal femur of the four groups of models were constrained,and compression loads of 700,1 400 and 2 100 N were applied to the femoral head.Von Mises stress distribution and compression stiffness of each group of models were observed through calculation and analysis,and mechanical stability of each group was compared. RESULTS AND CONCLUSION:(1)Through calculation and analysis,after calculating the compression stiffness by comparing the deformation of each model,the compression stiffness of each model under various loads showed the trend:physiological group>proximal femoral nail antirotation group>proximal femoral locking plate group>percutaneous compression plate group>dynamic hip screw group.The compressive stiffness of the complete physiological group model was significantly higher than that of all surgical group models.(2)The stress index was observed.Due to the stress shielding effect,the stress peak value of each fixed group was higher than that of physiological group,and the maximum peak value was concentrated on each internal fixation.Proximal femoral nail antirotation group had the smallest stress peak,while dynamic hip screw group had the highest stress.The stress distribution trend showed physiological group
