2.Retinoblastoma in the Southern Philippines: Clinical outcomes of retinoblastoma patients in a Davao Tertiary Hospital

Background:
Retinoblastoma is the most common intraocular cancer in childhood in the Philippines. Most data
though on demographics, clinical profile, treatment options, and outcomes in the country are from the National
Capital Region.
Objectives:
This study aimed to describe the demographics, clinical profile, treatment done, and outcomes of
retinoblastoma patients seen in a public tertiary referral center in Davao from 2011-2020 to make available literature more representative of the status of retinoblastoma in the Philippines.
Methods:
An analytical cross-sectional study was conducted using the records of retinoblastoma patients seen in a tertiary government hospital located in Davao Region from January 2011 to December 2020.
Results:
There were 157 patients included in the analysis. Seventy-three (46%) were female with 44% coming from the Davao Region. One hundred seven (69%) patients had unilateral disease. Median age at initial consultation for patients with unilateral disease was significantly older than those with bilateral disease (p<0.003). Tumors were extraocular in 82 (40%) eyes. In the intraocular group, 36% of the eyes belonged to International Classification of Retinoblastoma (ICRB) Groups D and E. Enucleation was the most commonly performed treatment. Survival rate was 28%. This is the first report to provide epidemiologic and clinical data on retinoblastoma in the literature, including survival data, from Mindanao. Advanced stages and extraocular cases of retinoblastoma remain high. Delay of consultation contributed to the prognosis and clinical outcome of the disease.
Conclusion. Advanced stages and extraocular cases of retinoblastoma remain significantly high in the country, even in Mindanao.

3.De novo histoid type lepromatous leprosy in a 40-year-old Filipino

Leprosy presents diversely, making diagnosis challenging. Histoid leprosy is a rare variant with atypical features. We present a case of a 40-year-old Filipino male with multiple erythematous to flesh colored, well demarcated hypoesthetic nodules primarily on his back and abdomen. Dermoscopy revealed features of granulomatous disease. Fite-Faraco stain showed globi formation. Skin biopsy showed epidermal atrophy, dense histiocyte infiltrates and collagen trapping. While histoid leprosy is uncommon in patients without a prior history of leprosy and treatment, this case highlights the importance of maintaining a high index of suspicion, especially in endemic regions. Early diagnosis and prompt multidrug therapy are crucial for preventing transmission and improving patient outcomes.

4.Pyoderma gangrenosum in a 54-year-old female treated with oral abrocitinib and healing monitored with dermoscopy: A case report

Pyoderma gangrenosum causes the destruction of skin tissue and then ulceration. There are currently no international treatment guidelines. Abrocitinib, a JAK1 selective inhibitor, has been used as treatment in some case reports.
A 54-year-old female, diagnosed for 20 years with ulcerative colitis and mostly treated with Chinese herbal medicine, admitted for a solitary painful ulcer on the right lateral ankle, measuring 10x8 cm, with swelling of the right lower leg. Antibiotics were given and wound debridement, exploration, then vacuum-assisted closure dressing were done. She was then referred to dermatology service. Wound care was done by light cleansing of the wound with cold-pressed virgin coconut oil (CP-VCO) - soaked gauze, gentle removal of wound and edge scabs, application of 30% Trichloroacetic Acid on the rounded edges of the ulcer, and covered with a gauze saturated with CP-VCO dressing. Abrocitinib 200 mg/tablet was started once a day. After being discharged, abrocitinib and wound care, with an added pharmacist-compounding of 4% monolaurin, 1% monocaprin, and 10% tranexamic acid in a petrolatum base, was maintained with the same dosage and frequency. At the end of the fourth month, the ulcer completely healed into a solitary pinkish to skin-colored linear scar, measuring at 1.8x4 cm. Dermoscopy monitoring of the wound healing showed progressive lines of epithelialization advancing into the ulcer to a scar with a cribriform pattern of epithelialized red dots.
The excellent results from this pyoderma gangrenosum case report using a combination of these treatment modalities are an opportunity to consider in future clinical cases.

5.Treatment satisfaction across different therapeutic modalities of Filipino patients with moderate-to-severe plaque-type psoriasis in a tertiary government hospital: A cross-sectional study

BACKGROUND
Psoriasis is a chronic, incurable inflammatory disease that often frustrates patients and negatively impacts quality of life, affecting treatment satisfaction.
OBJECTIVEThis study evaluated treatment satisfaction, medication adherence, and dermatological quality of life in Filipino patients with plaque-type psoriasis.
METHODSNinety-five patients at a tertiary dermatology clinic completed surveys using the Treatment Satisfaction Questionnaire for Medications (TSQM 1.4), Morisky Medication Adherence Scale-4 (MMAS), and Psoriasis Disability Index (PDI). Disease severity was measured using the Psoriasis Area Severity Index (PASI).
RESULTSPatients reported moderate to high treatment satisfaction, medium medication adherence, and minimal quality of life impairment across all treatments. Convenience satisfaction was significantly correlated with educational attainment. Satisfaction with effectiveness and convenience was strongly linked to medication adherence, while overall satisfaction was tied to disease severity. Additionally, satisfaction with side effects was significantly related to quality of life.
CONCLUSIONAll three treatment modalities remain vital for managing psoriasis in low- resource settings. Emphasizing medication side effects, treatment convenience, and their impact on quality of life can foster a more patient-centered approach.

OBJECTIVE
This study compared the demographics, clinical profile, treatment, and outcomes of retinoblastoma patients seen at medical institutions in the Philippines between two time periods: 2010 to 2015 and 2016 to 2020.
METHODSThis was a multicenter, analytical, cohort study using review of medical charts and databases of retinoblastoma patients seen in 11 medical institutions from 2010 to 2020.
RESULTSThere were 636 patients (821 eyes) included in this study: 330 patients were seen in 2010 to 2015 while 306 in 2016 to 2020. More cases per annum were seen in the latter timeline. The number of patients with unilateral disease was not significantly different between the two time periods (p=0.51). Age at onset of symptom, age at initial consultation, and delay in consult were also not significantly different between the two time periods (p > 0.05). Patients had significantly different distributions of intraocular grades (p < 0.0001) and systemic staging (p < 0.0001) between the two time periods. Enucleation was the most common surgical treatment performed in both timelines. There was significant difference in the status of patients based on the need for systemic chemotherapy (p < 0.01). There was significant difference in outcome between the two time periods, including the proportions of living and deceased patients.
CONCLUSIONThis study compared the most comprehensive data on retinoblastoma patients in the country. There was no improvement in early health seeking behavior based on similar age at initial consult and delay in consult. Enucleation remained the most common treatment mode as opposed to chemotherapy due to similar percentage of patients with unilateral disease, an indication for enucleation rather than chemotherapy.

7.Effectiveness of Virgin Coconut Oil in Treating Dry Eyes

Background:
There are a few dry eye remedies available in the market. Currently, artificial tears and lubricants are still the most common management for dry eyes.
Objective:
We proposed a new method in managing dry eyes.
Methods:
A pre-soaked contact lens in virgin coconut oil (VCOCL) is being used as a vehicle to deliver virgin coconut oil (VCO) in dry eyes. VCOCL was prepared in sterilised conditions where daily soft hydrogel contact lenses were immersed in raw VCO. The efficacy of VCOCL in delivering the VCO to eyes was assessed by measuring the Tear Break-Up Time (TBUT) values, corneal staining of the anterior eye, Schirmer Test values and the measurement of residual VCO volume in tears at baseline and at 15 minutes after insertion on subjects with dry eyes. Pre- and post-data were used to analyse all the measurable variables.
Results:
This study showed a significant difference in the TBUT, corneal staining, and residual VCO volume for both eyes (p<0.05). However, there were no changes in the Schirmer Test value (p>0.05). VCOCL was proven to improve tear quality in dry eye subjects and was able to maintain its presence in the eye even after 15 minutes.
Conclusion
This study suggests a new method for dry eye management.

8.Capacity to treat retinoblastoma in the Philippines

Objective:
This study determined the capacity to treat retinoblastoma (RB) in the Philippines.
Method:
This was a cross-sectional study which included all Department of Health (DOH) hospitals and all government and private tertiary hospitals in the Philippines. An online modified questionnaire taken from the St. Jude Children’s Research Hospital study on RB assessment was emailed to 143 hospitals in all 17 regions. Primary outcome measures were human resources capacity, treatment capacity and education and network capacity. Availability, frequency and confidence of use were assessed for treatment capacity. An asset-based tier classification of hospitals was created based on the human resources capacity and treatment capacity.
Results:
This assessment survey had a 49% response rate. There were 3 regions that had zero correspondence. A general ophthalmologist, a subspecialized ophthalmologist, and a general pathologist were the most available physicians in the management of RB. Almost all respondents had the necessary diagnostic technology in their hospitals, but frequency and confidence of use were low. A quarter of the respondents (23%) used a standardized treatment protocol, while only 26% had established a referral network in their area. Tier classification was able to differentiate capacities for particular resources only. Only 23% of the participating hospitals have the capacity to treat RB through its trained human resources, available diagnostic and treatment technology, and education and network programs. Distribution of hospitals identified as tier I, II and III were 18%, 8%, and 41%, respectively.
Conclusion
Capacity to treat RB in the Philippines is evident only in a few hospitals, majority of which are located in the National Capital Region. However, tier 3 hospitals were identified in 10 other regions, making accessibility to RB care possible to patients in the provinces. Although human resource and technology are made available, utilization of these resources is low in many hospitals for the management of RB. Clinical practice guidelines for RB is still lacking. Connecting the different tiers in each region as a form of referral network can improve capacity and management of RB.

9.Clinical presentation, treatment, and outcomes of retinoblastoma in India: A literature review

Background:
India has the highest incidence of retinoblastoma globally and is one of the six Asian countries identified to contribute 43% to the global retinoblastoma cases. Data on clinical presentation, treatment, and outcomes were reviewed which can serve as basis in the creation of clinical guidelines, policies and programs, and resource allocation in the diagnosis and management of retinoblastoma in India.
Methodology:
Articles on retinoblastoma in India gathered from different databases were reviewed for clinical features, treatment, and outcomes.
Result:
Fourteen articles with 3,666 patients involving around 4,945 eyes seen from 1983-2017 were
reviewed. The median age at consult ranged from 14-48 months. The median delay of consult ranged from 2-9 months. Majority of the patients were males (59%) and unilateral disease was present in 63%. Family history was reported in 4%. Retinoblastoma was intraocular in at least 75% of eyes. Systemic chemotherapy was the most used treatment option given to >2,042 eyes. Enucleation was done in >1,695 eyes. The mean follow-up period ranged from 4-50 months. Three hundred fifty six (356) patients were lost to follow-up. The functional vision was retained in 134 eyes. The globe salvage rate for Group A was 100%, 94-100% for Group B, and 50-100% in Group C. The highest globe salvage rate for Group D eyes was 85% and 58% for group E. The overall survival rate was 75% (2,233 patients).
Conclusion
Common among the articles was the relatively high proportion of extraocular disease attributed to delay in consult attributed to financial factors and lack of knowledge on the disease.

10.Botulinum toxin treatment practice recommendations for movement disorders clinics during the COVID-19 pandemic

Background:
Due to the Coronavirus disease 2019 (COVID-19) pandemic, non-emergency services like botulinum toxin A (BoNT) injections were temporarily discontinued.
Objective:
We aimed to create standard practices that optimize the management of patients needing BoNT injections while ensuring both patients' and healthcare professionals' safety.
Methods:
We evaluated published guidelines and best practices to determine their applicability for BoNT injection during this pandemic. We then devised a decision tree algorithm for use in our hospital.
Results:
A decision tree algorithm has been formulated based on patient evaluation and risk stratification, triaging, the urgency of the procedure, and proper precautions on personal protective wear as appropriate for the procedure’s risk stratification.
Conclusion
The algorithm is in place to ensure the safety of both the patient and healthcare professional. It can be adopted and revised by other centers to suit their clinic needs.
