BACKGROUND:Endometrial receptivity is a key factor in embryo implantation in northwest Tibetan cashmere goats, and the expression of genes related to endometrial receptivity and their variable splicing are still unclear. OBJECTIVE:To analyze and explore genes and variable splicing events related to endometrial receptivity in northwest Tibetan cashmere goats. METHODS:On days 5 and 15 of pregnancy(representing pre receptive endometrium group and receptive endometrium group),three northwest Tibetan cashmere goats were randomly selected.Endometrial tissue was collected and stained with hematoxylin and eosin to observe tissue morphology.Immunohistochemical staining was used to detect the expression of endometrial receptive marker proteins leukemia inhibitory factor and vascular endothelial growth factor. After the total RNA was extracted and the quality test was qualified, transcriptome sequencing was performed to search differentially expressed mRNAs,lncRNAs,circRNAs,and miRNAs,perform functional prediction, and analyze alternative splicing mRNAs and lncRNAs related to endometrial receptivity. RESULTS AND CONCLUSION:(1)Compared with the pre receptive endometrium group, the expression levels of leukemia inhibitory factor and vascular endothelial growth factor proteins in the endometrial tissue of the receptive endometrium group were significantly increased.(2)The sequencing results showed that the differentially expressed genes were mostly mRNA and lncRNA genes, including 250 upregulated mRNAs,193 upregulated lncRNAs,135 downregulated mRNAs,and 123 downregulated lncRNAs,which were significantly enriched in the Wnt,Hedgehog, and Hippo signaling pathways.(3)Alternative splicing event analysis uncovered 8 differentially expressed variable splicing transcripts, which were all mRNA transcripts, including 2 downregulated and 6 upregulated, and were significantly associated with vascular endothelial growth factor receptor signaling, cell motility, and embryonic development.

Lactylation, a recently identified post-translational modification of proteins, is induced by lactic acid and can occur at multiple lysine residues in both histone and non-histone proteins. This modification plays a role in disease pathogenesis by affecting transcriptional regulation, mitochondrial metabolism, and immune inflammation. Significant advancements have been made in understanding the mechanisms of lactylation in various ophthalmic diseases, including retinal neovascularization, uveitis, melanoma, and myopia. This paper provides a comprehensive review of the relationship between lactic acid and lactylation, the regulatory mechanisms of lactylation, and the role of lactylation in different ocular diseases. Additionally, it addresses current research limitations and future directions, which is of great significance to elucidate the molecular mechanisms of lactylation in eye diseases and improving the diagnosis and targeted treatment of these conditions.

Misophonia is a psychophysiological and behavioral disorder characterized by an individual's low tolerance to specific sounds, leading to intense negative emotional experiences and physiological responses. Currently, there is no standardized and universally effective treatment for misophonia in clinical practice worldwide. This article reports the case of an 18-year-old male patient with misophonia who showed poor response to sertraline combined with exposure and response prevention therapy. Subsequently, the patient received 8 weeks of Morita therapy (once a week, 50 minutes per session), with symptomatic improvement. By presenting this case, we explore the potential efficacy of Morita therapy in treating misophonia, aiming to provide a reference for its clinical management. OBJECTIVE To provide direction and reference for the adjustment of the discount rate (DR) in China’s pharmacoeconomic guidelines. METHODS Search was conducted on the official websites of the International Society for Pharmacoeconomics and Outcomes Research, health technology assessment agencies in various countries/regions, as well as relevant websites of other upper-middle-income or high-income countries/regions. The recommended DR, adjustment trends, and setting rationales in pharmacoeconomic evaluation guidelines across different countries/regions were then summarized and compared. Based on theoretical derivation and literature analysis, the effects of different DR on the incremental cost-effectiveness ratio (ICER) were examined. RESULTS & CONCLUSIONS Among the 40 included guidelines, the base-case DR ranged from 1.5% to 5%, with 5% being the most common value; the range for sensitivity analysis was 0 to 12%. Thirty-six countries/regions applied the same DR to both costs and health outcomes, while in the Netherlands, Belgium, Poland and Czech Republic, DR for costs was higher than for health outcomes. In recent years, Korea, France and Ireland had lowered their DR in response to economic changes, whereas the Netherlands and Czech Republic had raised their DR for cost. The setting of the DR was primarily based on the public project investment interest rate or referred to recommendations from internationally authoritative institutions and other relevant guidelines. The direction and magnitude of the impact of different DR on the ICER largely depended on the distribution of costs and health outcomes between the intervention and reference measure. The setting and adjustment of DR were closely associated with the economic environment. Based on international experience, the DR in China can be lowered by 0.5% to 1.5%, and localized empirical research can be conducted using internationally common estimation methods.

OBJECTIVE To investigate the current situation of the review and comment on inpatient medical orders in Beijing municipal hospitals, and to put forward countermeasures and suggestions for further improving related work. METHODS A questionnaire survey was conducted to investigate the current situation of the review and comment on inpatient medical orders in 22 Beijing municipal hospitals. The statistical analysis was conducted for the survey results. RESULTS A total of 22 questionnaires were distributed, with recovery effective rate of 100%. The 22 hospitals carried out inpatient medical order comment, but their proportion varied among hospitals (0.88%-98.54%); medical order comment mainly focused on antibiotics, proton pump inhibitors, anesthetic drugs/class Ⅰ psychotropic drugs, auxiliary drugs and other categories; 205 pharmacists participated in the comment of inpatient medical orders, most of whom hold intermediate or higher professional titles (89.27%); 21 hospitals conducted inpatient medical order comment and feedback the results to relevant departments/responsible persons, but the intervention situation was not the same. Eighteen hospitals had carried out the review of inpatient medical orders; reviewed drug category was roughly the same as the category involved in the medical order comment; review content involved the suitability of administration routes, dosage, etc. The review was conducted mainly through the cooperation of audit software and pharmacists. CONCLUSIONS The comment and review of inpatient medical orders in Beijing municipal hospitals carried out in an orderly manner, and preliminary results have been achieved; at the same time, it is necessary to further increase the ability of participating pharmacists, improve audit standards, optimize pre-audit (No.ZYLX201805) software, and promote rational drug use among hospitalized patients through doctor-pharmacist collaboration.

International Eye Science 2024;24(11):1747-1752
Glial-mesenchymal transition(GMT)is a biological process of transdifferentiation where endothelial cells gradually adopt the phenotypic characteristics of mesenchymal cells under the influence of various factors. GMT is closely associated with retinal fibrosis diseases. Müller cells, the predominant retinal macroglia, undergo activation and transdifferentiation in response to diverse stimuli and pathological conditions. Researches indicate that GMT plays a significant role in the pathogenesis of diseases such as diabetic retinopathy(DR), idiopathic epiretinal membrane(iERM), age-related macular degeneration(ARMD), and proliferative vitreoretinopathy(PVR). Although the exact mechanism of GMT is not well understood, it has showed great promise as potential target. Clarifying the research progress of GMT provides new ideas in the early diagnosis and treatments of retinal diseases, which is clinically and scientifically important for revealing interactive effects of cell transdifferentiation families in retinal diseases.

BACKGROUND:Pain mechanisms in patients with lumbar disc herniation are associated with inflammation, autophagy is closely related to intervertebral disc diseases and inflammatory response, and aberrant miR-206 expression can trigger skeletal diseases. OBJECTIVE:To investigate the mechanism of miR-206 on inflammation, analgesia and autophagy related proteins in nucleus pulposus in rats with lumbar disc herniation. METHODS:Sixty SPF male Sprague-Dawley rats were randomly divided into control group, model group,miR-206 mimics-NC group,miR-206 mimics group,miR-206 inhibitor-NC group and miR-206 inhibitor group.Animal models of lumbar disc herniation were established except for the control group.Ten days after modeling,miR-206 mimics-NC group,miR-206 mimics group,miR-206 inhibitor-NC group and miR-206 inhibitor group were injected with miR-206 mimics-NC(20 μmol/L,10 μL),miR-206 mimics(20 μmol/L,10 μL),miR-206 inhibitor-NC(20 μmol/L,10 μL)and miR-206 inhibitor(20 μmol/L,10 μL),respectively.Administration was given once a day for 4 continuous days. The control group and model group were injected with the same dose of normal saline. The paw withdrawal mechanical threshold of bilateral hind feet was measured by Von Frey filaments, and the paw withdrawal thermal latency of bilateral hind feet was measured by heat pain tester. The morphology of dorsal root ganglia was observed by hematoxylin-eosin staining. The expressions of inflammatory factors phospholipase A2,cyclooxygenase 2,prostaglandin E2,tumor necrosis factor α,and interleukin 1β in nucleus pulposus were detected by qPCR.The expressions of autophagy-related proteins LC3I and Beclin-1 were detected by western blot assay. RESULTS AND CONCLUSION:At 3,7,and 14 days after modeling, the paw withdrawal mechanical threshold and paw withdrawal thermal latency were both decreased in the model group compared with the control group, while the levels of phospholipase A2,cyclooxygenase 2,prostaglandin E2,tumor necrosis factor α,interleukin 1β,LC3I and Beclin-1 increased(P<0.05).The above indexes showed no significant changes in the miR-206 inhibitor-NC group and miR-206 mimics-NC group compared with the model group(P>0.05).Compared with the miR-206 mimics-NC group, the miR-206 mimics group had lower paw withdrawal mechanical threshold and paw withdrawal thermal latency and higher levels of phospholipase A2,cyclooxygenase 2,prostaglandin E2,tumor necrosis factor α,interleukin 1β,LC3I,and Beclin-1 levels(P<0.05).Compared with the miR-206 inhibitor-NC group, the rats in the miR-206 inhibitor group showed opposite changes in the above indicators, and there were significant differences between the two groups(P<0.05).To conclude, inhibition of miR-206 can significantly improve the level of inflammatory factors in nucleus pulposus of rats with lumbar disc herniation, increase pain threshold, and reduce autophagy. The mechanism is related to the inhibition of LC3I and Beclin-1 expression.
