Objective To investigate the value of peripheral blood soluble interleukin-2 receptor(sIL-2R),CD4+lymphocyte percentage/CD8+lymphocyte percentage ratio(hereinafter referred to as CD4+/CD8+)and tumor necrosis factor-α(TNF-α)in evaluating the efficacy of chemotherapy in elderly patients with newly treated active pulmonary tuberculosis.Methods A total of 102 elderly patients with newly treated active tu-berculosis admitted to the hospital from December 2019 to December 2022 were enrolled in the study as the observation group,and 102 healthy people aged 60 and older who underwent physical examination in the hos-pital during the same period were enrolled as the control group.The levels of sIL-2R,TNF-α and CD4+/CD8+in peripheral blood were compared between the two groups,and the correlations between sIL-2R,TNF-α and CD4+/CD8+were analyzed.The observation group was treated with 2HRZE/4HR anti-tuberculosis treatment regimen.The levels of sIL-2R,TNF-α and CD4+/CD8+in peripheral blood of patients with different efficacy before treatment,1 month and 6 months after treatment in the observation group were compared.The correla-tion between sIL-2R,CD4+/CD8+,TNF-α levels and therapeutic effect was analyzed.The receiver operating characteristic(ROC)curve was used to analyze the efficacy of indicators in evaluating the efficacy of chemo-therapy in elderly patients.Results The levels of sIL-2R and TNF-α in the observation group were higher than those in the control group,while CD4+/CD8+was lower than that in the control group,and the differ-ences were statistically significant(P<0.05).In the observation group,sIL-2R and TNF-α were negatively correlated with CD4+/CD8+(P<0.05),sIL-2R was positively correlated with TNF-α(P<0.05).After 1 month and 6 months of treatment,the levels of sIL-2R and TNF-α in patients with apparent efficacy were low-er than those in patients with efficacy,and the latter were lower than those in patients with no effect,while the CD4+/CD8+in patients with apparent efficacy was higher than that in patients with efficacy,and the latter was higher than that in patients with no efficacy,and the differences were statistically significant(P<0.05).The levels of sIL-2R and TNF-α were negatively correlated with the efficacy(P<0.05),and CD4+/CD8+was positively correlated with the efficacy(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of sIL-2R,CD4+/CD8+,and TNF-α used in combination to assess efficacy was significantly greater than the AUCs of the single indicators used in the assessment at each time point of treatment(P<0.05),and the AUC of the combination of the indicators was greater after 6 months of treatment than after 1 month of treatment(P<0.05).Conclusion The levels of sIL-2R,CD4+/CD8+and TNF-α are closely related to the ef-ficacy of chemotherapy in elderly patients with newly treated active pulmonary tuberculosis,and the combina-tion of the above indicators has certain reference value in evaluating the efficacy of chemotherapy in patients.
3.Analysis on the glucose and lipid metabolism levels of different sleep duration residents in a community in Urumqi

Objective:To investigate different sleep duration and glucose and lipid metabolism levels in residents of a community in Urumqi.Methods:Using the 2 049 residents′ data of chronic metabolic disease in a community of Urumqi collected in May 2017, 1 822 subjects aged between 19-80 years with complete information were enrolled, their blood pressure, waist circumference, height, weight, body mass index were measured and recorded. Using oral glucose tolerance test to measure fasting and 2 h after meal plasma glucose, uric acid, HbA 1C, total cholesterol, triglyceride, low density lipoprotein-cholesterol (LDL-C), high density lipoprotein-cholesterol (HDL-C) levels were all tested. Results:(1)There were 363 (19.9%), 1 349 (74.0%), and 110 (6.1%) respondents with sleep time≤6.0, 6.1 to 8.0, and>8.0 h/d, respectively. There were statistically significant differences in age, education, and family income in groups with different sleep time ( P<0.05), while their gender, smoking status, and exercise status were not statistically significant ( P>0.05). The rates of overweight, obesity, abdominal obesity, high uric acid, and hypertension in people with different sleep durations were statistically different ( P<0.01). The rates of the above indicators were higher in the group of sleep time≤6.0 h/d than the other two groups. (2) Differences in diastolic blood pressure, systolic blood pressure, body mass index, abdominal circumference, total cholesterol, and LDL-C levels were statistically significant among different sleep duration groups ( P<0.05). Further comparisons of the above indicators among three groups with different sleep durations were performed ( P<0.05). The levels of the above indicators in the sleep time≤6.0 h/d group were higher than those in the other two groups. There were no significant differences in fasting blood glucose, glycated hemoglobin, uric acid, triglyceride, and HDL-C among the three groups. (3) Multivariate logistic regression analysis showed that groups whether or not adjusted of age, family income, and education level, sleep time≤6.0 h/d was related to abdominal obesity, and sleep time≤6.0 h/d was be a risk factor for abdominal obesity [Unadjusted: OR=1.48(95% CI1.04-2.08); Adjusted: OR=1.65(95% CI1.18-2.32; P<0.05]. Conclusion:Sleep time ≤6.0 h/d is associated with abdominal obesity, and sleep time≤6.0 h/d may be a risk factor for abdominal obesity.
4.Research and application progress of mechanical measures for oral plaque control

Effective removal of dental plaque is a key measure to prevent and control common oral diseases and to maintain the long-term effect of dental prostheses. The methods of oral plaque control are mainly depending on mechanical measures, including regular toothbrushing, interdental cleaning and oral irrigation. Toothbrushing is the most common and effective method for mechanical removal of dental plaque.How ever the effect of toothbrushing alone to remove dental plaque is limited. It is necessary to control the interdental plaque using tools such as dental floss and interdental brushes. Oral irrigation is an assistant method to improve oral hygiene. Toothbrushing combined with the use of oral irrigator can significantly improve the removal rate of dental plaque. The present article reviews the research progress of the mechanical plaque control methods and their application results.
5.Status quo and influencing factors of self-management of depression in community residents based on structural equation model

Objective:To explore the current status and influencing factors of self-management of depression in community residents, so as to provide clues and basis for promoting the mental health of community residents.Methods:From June to August 2020, the residents of a community in Jining city in Shandong Province were selected as the survey objects, and 458 permanent residents were selected by simple random sampling method. The self-designed general data questionnaire, Depression Self-management Scale, General Self-Efficacy Scale (GSES) , Simple Coping Style Questionnaire (SCSQ) and Social Support Rating Scale (SSRS) were used to conduct cross-sectional survey, and structural equation was used to analyze the influencing factors of depression self-management of community residents. A total of 458 questionnaires were sent out and 443 were effectively received, with the effective recovery rate of 96.72%.Results:The depression self-management score of 443 community residents was (32.72±5.72) , GSES score was (23.47±5.12) , score of positive coping in SCSQ (2.07±0.51) , score of negative coping in SCSQ was (1.14±0.43) and total score of SSRS (32.89±3.43) . The structural equation model showed that the total effects of self-efficacy, positive coping, negative coping and social support on self-management of depression in community residents were respectively 0.45, 0.42, -0.52 and 0.37 ( P<0.05) . Conclusions:The depression self-management level of community residents is directly or indirectly affected by self-efficacy, coping styles and social support. When conducting mental health publicity and education in the community, it should be based on improving the self-efficacy of residents, coping styles and the level of social support to choose effective methods and content of publicity and education to promote the improvement of the self-management level of individual depression.
6.Clinical analysis of levetiracetam in the treatment of children and adolescents with epilepsy

Objective To explore and compare the clinical efficacy of levetiracetam tablets and compound sodium valproate sustained release tablets in the treatment of children and adolescents with epilepsy.Methods From April 2017 to April 2018,80 children and adolescents with epilepsy treated in Chaonan Minsheng Hospital of Shantou were selected as study objects,and they were randomly divided into two groups by drawing lots,with 40 cases in each group.The observation group was given levetiracetam tablets,and the control group was treated with valproate.The improvement of EEG after therapy,the total effective rate,and the incidence of adverse reactions were observed and evaluated.Results The EEG improvement rates after treatment for 6 months in the observation group and control group were 41.17%,45.71%,respectively,the difference was not statistically significant(x2 =0.508,P >0.05).The EEG improvement rates after treatment for 9 months in the observation group and control group were 70.58%,74.28%,respectively,the difference was not statistically significant (x2 =0.225,P > 0.05).The total effective rate in the observation group was 92.50%,which was 95.00% in the control group,the difference was not statistically significant between the two groups (x2 =0.354,P > 0.05).However,the incidence rate of adverse reactions of the observation group(22.50%) was significantly lower than that of the control group(45.00%)(x2 =6.864,P < 0.05).Conclusion Both levetiracetam tablets and compound sodium valproate sustained release tablets have appreciable efficacy and safety in the treatment of epilepsy in children and adolescents,but levetiracetam therapy has less adverse reactions,which deserves further promotion in monotherapy of epilepsy in children and adolescents.
7.miR-17-5p regulates proliferation, invasion, migration and apoptosis of nasopharyngeal carcinoma CNE2 cells by down-regulating BRMS1Lexpression

Objective: To explore the molecular mechanism of miR-17-5p regulating the proliferation and invasion of nasopharyngeal carcinoma cells by regulating the expression of breast cancer metastasis suppressor 1 like (BRMS1-like or BRMS1L) gene. Methods:A total of 40 cases of nasopharyngeal carcinoma tissues and corresponding paracancerous tissues resected from nasopharyngeal carcinoma patients, who were admitted to the General Hospital of Pingdingshan Shenma Medical Group during January 2014 to December 2017, were included in this study; in addition, nasopharyngeal carcinoma cell lines CNE 2, HONE 1, C666-1 and nasopharyngeal immortalized epithelial cell line NP69 were also collected for this study. The expression of miR-17-5p in nasopharyngeal carcinoma tissues and cell lines was detected by qPCR. The targeted relationship between BRMS1L and miR-17-5p was predicted by the StarBase and verified by the Dual luciferase reporter gene assay. Effects of transfection of miR-17-5p mimics and inhibitors on the expression of BRMS1Lin CNE2 cells were detected by WB assay. CCK-8, Transwell and Flow cytometry were used to detect the effects of miR-17-5p/BRMS1L axis on the proliferation, migration, invasion and apoptosis of CNE 2 cells. Results: miR-17-5p was highly expressed in nasopharyngeal carcinoma tissues and cell lines (P<0.05 or P<0.01). Knockdown of miR-17-5p significantly inhibited proliferation, invasion and migration of CNE2 cells but promoted apoptosis (P<0.05 or P<0.01); miR-17-5p targeted BRMS1Land down-regulated its expression. Over-expression of BRMS1Lsignificantly inhibited the proliferation, invasion and migration of CNE2 cells but promoted apoptosis (all P<0.01); while simultaneous over-expression of miR-17-5p and BRMS1L reversed the above effects (all P<0.01). Conclusion: miR-17-5p promoted proliferation, invasion, migration and inhibited apoptosis of CNE 2 cells by down-regulating the expression of BRMS1L.
8.Anti-tumor and immune-modulating effect of decoction in mice bearing hepatoma H22 tumor.

OBJECTIVE:
To investigate the antitumor activity of decoction and study its liver and kidney toxicity and its effect on the immune system in a tumor-bearing mouse model.
METHODS:
Hepatoma H22 tumor-bearing mouse models were randomized into model group, cyclophosphamide (CTX) group, and low-, moderate-, and high-dose decoction groups (JW-L, JW-M, and JW-H groups, respectively). The antitumor activity of decoction was assessed by calculating the tumor inhibition rate and pathological observation of the tumor tissues. Immunohistochemistry was used to detect the expressions of Bax, Bcl-2, Bax/Bcl-2 and caspase-3 in the tumors. The liver and kidney toxicity of decoction was analyzed by evaluating the biochemical indicators of liver and kidney functions. The immune function of the tumor-bearing mice were assessed by calculating the immune organ index, testing peripheral blood routines, and detection of serum IL-2 and TNF-α levels using enzyme-linked immunosorbent assay.
RESULTS:
Compared with that in the model group, the tumor mass in CTX, JW-M and JW-H groups were all significantly reduced ( < 0.05) with cell rupture and necrosis in the tumors. Immunohistochemistry revealed obviously up-regulated expressions of Bax and caspase-3 and down- regulated expression of Bcl-2 protein with an increased Bax/Bcl-2 ratio in CTX, JW-M and JW-H groups. Treatment with decoction significantly reduced Cr, BUN, AST and ALT levels, improved the immune organ index, increased peripheral blood leukocytes, erythrocytes and hemoglobin levels, and up-regulated the levels of TNF-α and IL-2 in the tumor-bearing mice. These changes were especially significant in JW-H group when compared with the parameters in the model group ( < 0.01).
CONCLUSIONS
decoction has a strong anti-tumor activity and can improve the liver and kidney functions of tumor-bearing mice. Its anti-tumor effect may be attributed to the up-regulation of Bax, caspase-3, TNF-α and IL-2 levels and the down-regulation of Bcl-2 expression as well as the enhancement of the non-specific immune function.

9.Observation on the efficacy of percutaneous drug penetration in the treatment of young patients with relapsed bacterias and pulmonary tuberculosis

Objective To evaluate the efficacy of percutaneous drug penetration in the treatment of young patients with relapsed positive pulmonary tuberculosis. Methods From June 2013 to May 2016,eighty-four cases of with relapsed bacterias and pulmonary tuberculosis treated in Hebei Chest Hospital were enrolled in the study and were randomly divided into the observation group and the control group, 42 cases in each group. Both groups were treated with conventional anti tuberculosis drug 2HRZES/6HRE, the patients in the observation group were treated with ultrasound conductometric instrument and ultrasound conductive gel patch (isoniazid 0. 1 g) for 1 time /d,20 min each time. The course lasted for 3 months. Results At the end of 1,3 and 8 months treatment,the rates of sputum negative conversion in the observation group were 45. 24%( 19/42),66. 07%( 28/42), 92. 86%( 39/42), which were significantly higher than those of the control group ((23. 81%( 10/42), 40. 48%( 17/42), 73. 81%( 31/42 )), the difference between the two groups was statistically significant (x 2=4. 27,5. 79,5. 49,P<0. 05). At the end of the 8 months,the obvious absorption rate of the lesion was 94. 48%(38/42),and the control group was 69. 05%(29/42). The difference between the two groups was statistically significant (x 2=5. 97,P<0. 05). At the end of 8 months,the curative rate of the observation group was 90. 48%, while that of the control group was 64. 29%. The difference between the two groups was statistically significant (x 2=5. 97,P<0. 05). Conclusion Percutaneous drug penetration has good safety. Combined with systemic chemotherapy,it can improve the treatment effect of young patients with relapsed bacterias positive pulmonary tuberculosis,and is worthy of promotion.
10.Effect of high expression of miR-1269 in non-small cell lung cancer tissues on biological characteristics of lung cancerA549 cells

Objective: To investigate the expression of miR-1269 in non-small-cell lung cancer (NSCLC) tissues, and to explore its effect on the cellular biological characteristics of NSCLC A549 cells and the underlying mechanism. Methods: 34 pairs of NSCLC tissues and the corresponding adjacent para-cancerous tissues obtained from the patients, who underwent surgery in the Department of Breast Surgery, the Fourth Hospital of Hebei Medical University from Jan. 2017 to Jan. 2018, were collected for this study. The expression level of miR-1269 in above tissue specimens was examined by real-time fluorescent quantitative PCR.After transfection with miR1269 mimics and mimics NC (negative control), the proliferation, migration and invasion of A549 cells were detected by MTS, Wound healing and Transwell assay, respectively; and the changes in cell cycle distribution of A549 cells were examined by flow cytometry. The bioinformatics tool was used to predict the possible target gene of miR-1269, and the regulation effect of miR-1269 on target gene was then validated by Western blotting and Dual-luciferase reporter assay. In the meanwhile, the protein expressions of cyclin depen
dent kinase inhibitor p21, Cyclin D2, and EMT-related proteins (E-cadherin and ZEB2) in the transfected A549 cells were measured by Western blotting. Results: The expression level of miR-1269 in NSCLC tissues was significantly higher than that in paracancerous tissues (2.81±2.27 vs 1.61±1.36, P <0.05). The capacities of proliferation, migration and invasion ofA549 cells in miR-1269 mimics transfection group were significantly higher than those in mimics NC group and blank control group (all P <0.01). And the cell proportion at S-phase in miR-1269-mimics group was obviously higher than that in mimics NC group [(46.54±1.57)% vs (23.32±3.15)%, P<0.01]. Bioinformatics analysis showed that miR-1269 could combine with 3’UTR of FOXO1 gene. After transfection with miR-1269 mimics, the expression level and luciferase activity of FOXO1 protein in A549 cells were significantly reduced (all P <0.01). Moreover, the protein expressions of p21 and E-cadherin were significantly decreased after over-expression of miR-1269 (all P <0.05), while the expressions of ZEB2 and Cyclin D2 were up-regulated (all P <0.05). Conclusion: The expression level of miR-1269 in NSCLC tissues was significantly increased, and it could enhance the proliferation, cell cycle progression, migration and invasion ofA549 cells. The possible mechanism may be related to its targeted regulation of FOXO1.
