1.Short-term efficacy and safety analysis of PD-1 inhibitor combined with SOX neoadjuvant therapy for locally advanced gastric adenocarcinoma

		                        			Objective:To explore the short-term efficacy and safety for the new adjuvant therapy of programmed cell protein-1(PD-1)inhibitor combined with Oxaliplatin+Tegafur(SOX)in locally advanced gastric adenocarcinoma. Methods:Retrospective collection of clinical data from 72 locally advanced(stage Ⅲ)gastric adenocarcinoma patients admitted to our gastrointestinal surgery department from January 2020 to July 2020,according to different treatment plans,patients were divided into a control group(n=36)and an observation group(n=36).The control group received SOX chemotherapy regimen,while the observation group received PD-1 inhibitor combined with SOX regimen.Both groups of patients underwent chemotherapy for 3 cycles,and were re-evaluated after resting for 3-4 weeks.Patients with no disease progression and tolerable surgery underwent D2 gastric cancer radical surgery.The overall survival(OS),pathological complete response(pCR)rate,progression free survival period(PFS),RO resection rate,objective response rate(ORR),disease control rate(DCR)and adverse drug reaction were compared between the two groups. Results:The pCR rate,ORR and DCR of the observation group were significantly higher than those of the control group[pCR rate:25%(9/36)vs 5.6%(2/36),x2=5.258,P=0.022;ORR:58.33%(21/36)vs 25.00%(9/36),x2=8.229,P=0.004;DCR:88.89%(32/36)vs 69.44%(25/36),x2=4.1 26,P=0.042].The incidence of adverse reactions such as elevated ALT,elevated AST,and hand foot syndrome during neoadjuvant chemotherapy in the observation group was significantly higher than those in the control group(P<0.05).The incidence of other adverse reactions was not significantly difference between the two groups of patients(P>0.05),and there were no neoadjuvant chemotherapy related death events in both groups.The R0 resection rates of the two groups were 97.22%and 91.67%,respectively(x2=1.059,P=0.303).The median OS and median PFS of 72 patients were 36 and 1 3 months,respectively.The 2-year OS rate of the two groups were 82.2%and 71.5%,respectively.Kaplan Meier survival curve analysis showed that the OS rate of the observation group was significantly higher than that of the control group(x2=4.240,P=0.039);the median PFS of the observation group was higher than that of the control group at 14 and 1 1 months,respectively(x2=4.173,P=0.041);COX univariate regression analysis did not identify any factors affecting survival of locally advanced gastric adenocarcinoma. Conclusion:The neoadjuvant treatment of locally advanced gastric adenocarcinoma with PD-1 inhibitor combined with SOX regimen is safe and effective,and the adverse reactions are controllable.

		                        			Patients with deep vein thrombosis require long-term anticoagulant therapy to prevent the spread or recurrence of the thrombus. Self-management can enhance patients' awareness of the disease and adherence to anticoagulant therapy, reducing the recurrence rate. 3.Investigation of Metabolic Profile Changes of Fresh Fruit of Schisandrae Chinensis Fructus After Green Removing Based on 1H-NMR Metabonomics

		                        			ObjectiveTo find the different metabolites of Schisandrae Chinensis Fructus fresh fruits dried in different ways by proton nuclear magnetic resonance (1H-NMR) metabonomics technology, and to explore the possible effect of green removing on its metabolic profile. MethodFifteen samples of Schisandrae Chinensis Fructus fresh fruits were used, and each 3 samples was divided into one group. The samples were processed by five methods, including drying, drying in the sun, microwave, blanching and steaming. These samples were extracted with 50% methanol and analyzed by 1H-NMR. Combined with multivariate statistical analysis, the differences of the metabolic profile between green removing samples and dried samples, sun-dried samples were compared. ResultA total of 27 chemical components were identified by 1H-NMR and Chenomx database, mainly including amino acids, alkaloids, carbohydrates and organic acids. These metabolites were mainly involved in the energy metabolism of fruit postharvest physiology. The results of multivariate statistical analysis showed that there were significant differences among the 5 groups. The dried and sun-dried samples were used as the controls, 17 differential metabolites (asparagine, citric acid, glucose, sucrose, choline glycerophosphate, trigonelline, alanine, lysine, glycerol, leucine, isoleucine, valine, uridine, threonine, methionine, phenylalanine and fructose) were screened by variable importance in the projection (VIP) value and S-plot. One-way ANOVA was used for comparison between groups, the results showed that compared with dried and sun-dried samples, the contents of sucrose and choline glycerophosphate increased significantly (P<0.05, P<0.01) in samples treated with green removing, but the contents of valine, leucine, isoleucine, threonine, phenylalanine and methionine decreased significantly (P<0.01), the content of asparagine increased significantly (P<0.05, P<0.01) in samples treated by microwave, while the contents of trigonelline and uridine decreased significantly (P<0.01) in samples treated by blanching. ConclusionThe consumption, decomposition and/or transformation of active ingredients may be inhibited by interfering with its energy metabolism after the green removing of Schisandrae Chinensis Fructus fresh fruits. 
		                        		
		                        		
		                        		
		                        	
4.Effects of “Green Removing ”Processing Technology of the Fresh Fruit of Schisandra chinensis after Harvested on the Quality of Medicinal Material

		                        			OBJECTIVE:To stud y the effects of “green removing ”processing technology of fresh fruit of Schisandra chinensis after harvested on the quality of medicinal material ,and to provide new ideas for the scientific and rational processing of Chinese medicinal material. METHODS :Fifteen fresh fruits of S. chinensis were used as samples ,with 3 samples in each group. The sample were processed preliminarily by 5 methods,such as drying at 50 ℃,drying in the sun ,drying at 50 ℃ after“green removing”processing with microwave ,drying at 50 ℃ after“green removing ”processing with blanching ,drying at 50 ℃ after “green removing ”processing with steaming. HPLC fingerprints of 15 batches of dried S. chinensis products were established and similarity evaluation was conducted according to Similarity Evaluation System of TCM Chromatographic Fingerprints (2012 edition). Cluster analysis was used to evaluate the similarity of dried S. chinensis products with different processing methods. At the same time ,HPLC method was adopted to determine the content changes of seven lignans in dried products ,such as schisandrol A , schisandrol B ,schisantherin A ,schisantherin B ,schizandrin A ,schisandrin B and schisandrin C. RESULTS :A total of 7 common peaks were obtained in the fingerprints of 15 batches of dried S. chinensis products. Except that the similarity between the chromatograms of dried samples in the sun and the control fingerprint was relatively low ,the similarities of chromatograms of dried products by other processing methods were greater than 0.900. Cluster analysis showed that 6 samples dried at 50 ℃ after“green removing”processing with microwave and dried at 50 ℃ after“green removing ”processing with blanching were grouped  into the first category ;3 samples dried at 50 ℃ after“green  removing”processing with steaming were grouped into the  second category ;6 samples dried at 50 ℃ and dried in sun were grouped into the third category. The content determination results showed that there was no significant difference in the total content of seven lignans in the samples dried at 50 ℃ and dried in the sun (P>0.05). The total contents of seven lignans in the samples dried at 50 ℃ after“green moving ” processing with microwave ,blanching and steaming were significantly higher than those dried at 50 ℃ and dried in sun (P<0.01). CONCLUSIONS:The quality of S. chinensis samples dried after “green moving ”processing with microwave and blanching is better than those directly dried in sun and dried in oven.
		                        		
		                        		
		                        		
		                        	
5.Genetic analysis of one family with congenital limb malformations.

		                        			To detect potential mutation in a Chinese pedigree affected with congenital limb malformations.
		                        		
		                        			METHODS:
		                        			Clinical data was collected. Genomic DNA was extracted from peripheral blood samples of family members. The zone of polarizing activity regulatory sequence (ZRS) were amplified by PCR and subjected to direct sequencing.
		                        		
		                        			RESULTS:
		                        			Among the 13 individuals in this pedigree, there were 4 PPD patients, who were characterized by varying degrees of deformity. The female patients suffered triphalangeal thumb and preaxial polydactyly, while the male patients only had preaxial polydactyly. Only one patient had foot involvement. TA heterogeneous mutations was discovered in the ZRS (105C>G) in all patients, the same mutation was not detected in 2 healthy family members.
		                        		
		                        			CONCLUSION
		                        			The inheritance pattern of PPD was autosomal dominant inheritance. There was a significant variability of symptoms among family patients. 7.Preliminary evaluation of adjuvant effects of cyclic dinucleotides ( CDN ) on immune responses to nasal spray influenza vaccine

		                        			Objective To analyze the immunostimulatory effects of cyclic dinucleotides ( CDN) on immune responses to a nasal spray influenza split virus vaccine and to evaluate its potential as a mucosal ad-juvant. Methods A H1N1 influenza split virus vaccine combined with different CDN was used for mouse immunization. Each mouse was intranasally immunized twice with 4. 5μg of hemagglutinin (HA) and 10μg of CDN with an interval of 21 d. Titers of hemagglutination inhibition ( HI) antibodies in serum, secretory IgA ( sIgA) in bronchoalveolar lavage fluid and IgG in serum were detected 21 d after the last immunization. Immunostimulatory activities of different CDN were compared. Effects of cyclic di-GMP ( c-di-GMP) and ch-itosan (CSN) on the immunogenicity of H1N1 and H7N9 influenza split vaccines were analyzed and com-pared. H1N1 influenza split vaccine combined with c-di-GMP or CSN was used to immunize mice. Three weeks after the last immunization, these mice were challenged with 10 times the median lethal dose ( LD50 ) of A/Puerto Rico/8/34 (H1N1) influenza virus. Survival rates of the mice were observed for 14 d. Results All three CDN induced high levels of HI antibodies and IgG in serum and sIgA in BALF. HI antibody sero-conversion rates were also higher than those of the control groups. c-di-GMP was superior to CSN in enhan-cing the immunogenicity of H1N1 and H7N9 antigens as higher titers of HI antibodies in serum and sIgA in BALF were induced. 8.Nurses' perceptions and cancer patients' self-evaluation on symptom severity during chemotherapy: a comparative descriptive study

To compare the evaluation result of 10 symptoms including poor appetite in cancer patients during chemotherapy from the patients and their nurses, in order to improve the assessment and judgment ability of the responsible nurses on the symptoms of patients undergoing chemotherapy through the analysis of the result. Methods? The study was performed in the chemotherapy unit of a cancer ClassⅢGrade A hospital in Beijing. By convenience sampling, a total of 192 patients undergoing chemotherapy from October 2017 to January 2018 and the nurses (n=16) who were primarily responsible for their care were selected as participants. Symptom assessments were completed by the use of the Edmonton Symptom Assessment System (ESAS). Results? Patients and nurses showed poor agreement in Depression, Anxiety and Shortness of breath (ICC< 0.4) and moderate agreement (0.4 ≤ICC< 0.75) in other symptoms. Wilcoxon signed rank test was used to compare the results of symptom assessment between patients and responsible nurses. Among the 10 entries in ESAS, there was no statistical difference between the results in poor quality of life (Z=-1.627, P=0.104), however, there were statistical differences in all the rest 9 entries (P<0.05) among which the nurses scored higher than the patients in the entry of anxiety and depression. Conclusions? Responsible nurses' perception on physical symptoms was better than that of psychological symptoms, and there was overestimation and underestimation in the severity of symptoms. It is necessary for the nurses to teach the patients how to use standard assessment tools and through health education, raise the patients' awareness of reporting their symptoms.
		                        		
		                        		
		                        		
		                        	
9.Detection and clinical application of the isothermal amplification of Mycoplasma pneumonia RNA in bronchoalveolar lavage fluid from children

		                        			Objective To assess the ability of simultaneous amplification and testing ( SAT ) in bacteria detection , bronchoalveolar lavage fluid from children was collected and detected by SAT , combined with in vitro culture experiment , providing theoretical support for the application of SAT in the diagnosis and treatment of mycoplasma pneumonia ( MP ) .Methods A total of 572 bronchoalveolar lavage fluid from children with community acquired pneumonia during October 2015 and December 2017 in Tianjin Children′s Hospital were collected and detected by Mycoplasma pneumonia ( MP ) nucleic acid quantitative assay and SAT technology.Among them, 161 bronchoalveolar lavage fluid were also detected using in vitro culture and the positive ones for MP were analyzed by nucleic acid quantification and SAT after exposing to high concentration of antibiotic .Results The positive rates of MP by nucleic acid quantitative assay and SAT technology in 572 samples were 74.7% (427/572) and 71.9% (411/572), respectively.These two detection methods have high consistency (χ2 =1.142,P=0.285).According to the test results of SAT , the positive rates of male and female were 72.7%(224/308) and 70.8%(187/264), respectively.There was no significant difference of positive rate between different sex (χ2 =0.252, P=0.616).The positive rate of MP in 4-14 years old children (78.1%, 317/416) was higher than that in infants (≤3 years) (56.6%, 94/166) (χ2 =26.811, P=0.000).After adding azithromycin (5 MIC) to MP positive medium for 5 days, the result of nucleic acid quantification was positive but SAT was negative .Conclusions SAT technology is a rapid, sensitive and specific method for detection of MP .In addition, SAT technology could identify the "dead" and"live" bacteria and could evaluate the effect of clinical treatment effectively .
		                        		
		                        		
		                        		
		                        	
10.Curative effect observation of minimally invasive incision combined with plate fixation for calcaneal fractures

		                        			Objective To analyze the curative effect of minimally invasive incision combined with plate fixation for calcaneal fractures.Methods Retrospectively analyzed the clinical data of 30 cases(30 feet) of calcaneal fractures who were treated with minimal incision(sinus tarsi approach combined with posterior lateral incision) from August 2014 to August 2015.Minimally invasive incision combined with plate fixation were performed for the treatment.X-rays were taken in the regular follow-up,B(o)hler and Gissane angle as well as the complications were measured.The final curative effect was comprehensively assessed according to the ankle hind foot score of American Aryhopaedic Foot and Ankle Society(AOFAS).Results All cases were followed up for an average of 14.5 months (from 12 to 16 months), and all fractures healed with no complication such as incision infection or poor healing.The average B(o)hler angle was improved from preoperative(13.42°±4.2°) to (29.3°±3.8°) at the last follow-up with a significant difference(t=-20.424,P=0.000),and the average Gissane angle was significantly improved from preoperative(97.8°±9.7°) to (123.6°±6.5°) at the last follow-up (t=-17.5,P=0.000).The outcomes were evaluated with AOFAS,resulting in an excellent and good rate of 83.3%.Conclusion Treatment of calcaneal fractures with minimal incision has the advantages of less trauma,lower incision complication rate,stronger reduction and fixation,which can achieve satisfactory outcomes.
