1.Mechanism of Shexiang Tongxin Dripping Pills in Ameliorating AngⅡ-induced Vascular Smooth Muscle Cell Dysfunction

ObjectiveTo study the mechanism of Shexiang Tongxin Dripping pills-containing serum (STDP) in ameliorating angiotensinⅡ (AngⅡ)-induced cell phenotype transformation, proliferation, migration, and dysfunction of vascular smooth muscle cells. MethodsAn AngⅡ-induced proliferation and migration model of vascular smooth muscle cells was established. The cells were treated with STDP at 5%, 10%, and 20% for 24 h. The immunofluorescence assay was employed to detect the expression of α smooth muscle actin (α-SMA) and matrix metalloproteinase-2 (MMP-2). The cell-counting kit-8 (CCK-8) assay and 5-ethynyl-2'-deoxyuridine (EdU) staining were employed to detect the proliferation of vascular smooth muscle cells, and the scratch assay was employed to detect the migration of the cells. Western blot was employed to determine the expression levels of pathway proteins such as angiotensin-converting enzyme 2 (ACE2), angiotensin Ⅱ type 2 (AT2), angiotensin Ⅱ type 1 (AT1), as well as proliferation and migration proteins such as typeⅠ collagen (ColⅠ) and osteopontin (OPN). ResultsCompared with the model group, STDP increased the expression of α-SMA, reduced the expression of MMP-2, and inhibited the proliferation and migration of vascular smooth muscle cells (P<0.05). Furthermore, STDP up-regulated the expression levels of ACE2, AT2, and MAS1, while down-regulating the expression level of AT1, PCNA, ColⅠ, MMP-9, Rock1, Rock2, and SRF (P<0.05). Compared with the STDP group, the ACE2 inhibitor reversed the regulatory effects of STDP. ConclusionSTDP inhibits the phenotype transformation, proliferation, and migration of vascular smooth muscle cells and regulates the expression of cell proliferation and migration-related proteins to ameliorate the dysfunction of vascular smooth muscle cells. It exerts the effects by up-regulating the expression of proteins in the ACE2-AT2/MAS pathway and down-regulating the expression of proteins in the AT1-Rock signaling pathway.
2.Shexiang Tongxin Dropping Pills Ameliorate Dysfunction of Cardiac Microvascular Endothelial Cells in Rat Model of Heart Failure

ObjectiveTo study the mechanism by which Shexiang Tongxin dropping pills (STDP) ameliorate the dysfunction of coronary microvascular endothelial cells in the rat model of heart failure. MethodsThe heart failure model was established by ligation of the left anterior descending coronary artery in rats, which were then allocated into sham, model, STDP, and telmisartan (TLM) groups and treated for 21 days. The heart function was detected by echocardiography, and the levels of myocardial injury markers, nitric oxide (NO), endothelin-1 (ET1), and angiotensinⅡ (AngⅡ) were determined by enzyme-linked immunosorbent assay (ELISA). The protein levels of endothelial nitric oxide synthase (eNOS) and inducible nitric oxide synthase (iNOS) were determined by Western blot. The model of cardiac microvascular endothelial cell injury was established by AngⅡ induction and then treated with the STDP-containing serum (5%, 10%, and 20%) for 24 h. The levels of NO and ET1 were measured by ELISA. Western blot was employed to determine the protein levels of eNOS, iNOS, angiotensin-converting enzyme 2 (ACE2), and angiotensinⅡ receptor 2 (AT2). MLN-4760, an ACE2 inhibitor, was used to explore the mechanism underpinning the regulatory effect of STDP on the ACE2-AT2/MAS pathway. ResultsCompared with the sham group, the model group showed decreases in left ventricular ejection fraction (LVEF) and left ventricular fractional shortening (LVFS) (P<0.05), a decline in serum NO level, elevations in serum AngⅡ and ET1 levels, a reduction in p-eNOS/eNOS ratio, and up-regulation in iNOS expression (P<0.05). Compared with the model group, STDP increased LVEF, LVFS, and cardiac output (P<0.05), raised the level of NO and lowered the levels of AngⅡ and ET1 in the serum (P<0.05), increased the p-eNOS/eNOS value, and inhibited iNOS expression (P<0.05). Compared with the AngⅡ group, STDP increased the NO content and decreased the ET1 content in endothelial cells (P<0.05), increased the p-eNOS/eNOS ratio, and inhibited the iNOS expression (P<0.05). The ACE2 inhibitor MLN-4760 reversed the regulatory effects of STDP on p-eNOS, eNOS, and iNOS. ConclusionSTDP improves the cardiac function in the rat model of heart failure, enhances the synthesis and release of NO in cardiac microvascular endothelial cells, reduces AngⅡ and ET1 levels, and regulates the expression of p-eNOS and eNOS, thereby ameliorating the dysfunction of microvascular endothelial cells in heart failure. This mechanism is related to the upregulation of the expression of proteins in the ACE2-AT2/MAS pathway.

Performance appraisal of public hospitals have given a guidance for the development of public hospitals at all levels.A Class A tertiary hospital reviewed the problems in the development of the hospital at the present stage and focused on the following four aspects:①insufficient fine management;②No clear orientation of discipline development;③The bottleneck of the improvement of medical operation efficiency;④New challenges in the reform of payment mode.The tertiary hospital launched a fine management practice in May 2022,in order to solve the problems by taking the Department of Surgery as a pilot area,laying the foundation for fine management through information system construction,improving the efficiency of medical operation through management process optimization,improving the overall competitiveness of disciplines through the construction of sub-specialty and Discipline Alliance and adjusting the performance appraisal index system to play the role of performance incentives.The measures effectively improve the overall capacity and efficiency of hospital medical services and help the hospital to achieve high-quality development.
4.Influencing factors of thyroid volume in children aged 8 - 10 in Yunnan Province

Objective:To analyze the influencing factors of thyroid volume in children aged 8 - 10 in Yunnan Province, and provide scientific basis for improving iodine deficiency disorders monitoring.Methods:From March to July 2020, in 129 counties (cities, districts) under the jurisdiction of Yunnan Province, each county (city, district) was divided into 5 sampling areas based on east, west, south, north, and middle. One township was selected from each area, and 40 non-boarding children aged 8 - 10 from one primary school were selected from each township (age balanced, half male and half female) as survey subjects. One random urine sample and household edible salt samples were collected for urine iodine and salt iodine testing, and physical examination and thyroid volume measurement were conducted for children. The influencing factors of thyroid volume were analyzed using Pearson correlation.Results:A total of 24 934 urine samples were collected from children, with a median urine iodine of 233.2 μg/L. A total of 24 933 household edible salt samples were collected from children, the median salt iodine was 24.17 mg/kg, and the qualified rate of iodized salt was 96.63% (24 003/24 839); A total of 24 937 children were examined of their thyroid gland, with a median thyroid volume of 2.62 ml and a goiter rate of 1.12% (280/24 937). Among them, there were 12 410 boys and 12 527 girls, with thyroid volumes of 2.61 and 2.64 ml, respectively. The thyroid volume of boys was positively correlated with age, height, weight, body mass index, body surface area, and salt iodine ( r = 0.15, 0.21, 0.26, 0.18, 0.25, 0.03, P < 0.001). The thyroid volume of girls was positively correlated with age, height, weight, body mass index, and body surface area ( r = 0.17, 0.26, 0.28, 0.17, 0.27, P < 0.001). Conclusion:Children aged 8 - 10 in Yunnan Province are at an iodine excess level; the age, weight, height, body mass index, and body surface area are influencing factors of thyroid volume.

It remains unclear whether heart transplantation is still feasible on patients who develop neurological complications before surgery and underwent successful neurological treatment.This article reported the diagnosis and treatment process of a heart failure patient complicating with acute ischemic stroke,who underwent successful heart transplantation after thrombectomy for acute ischemic stroke,aiming to provide clinical evidence and decision-making plan on diagnosis and treatment options for this group of patients with severe neurological complications.

Objective:To evaluate the efficacy and safety of hypomethylating agents (HMA) in patients with myelodysplastic syndromes (MDS) .Methods:A total of 409 MDS patients from 45 hospitals in Zhejiang province who received at least four consecutive cycles of HMA monotherapy as initial therapy were enrolled to evaluate the efficacy and safety of HMA. Mann-Whitney U or Chi-square tests were used to compare the differences in the clinical data. Logistic regression and Cox regression were used to analyze the factors affecting efficacy and survival. Kaplan-Meier was used for survival analysis. Results:Patients received HMA treatment for a median of 6 cycles (range, 4-25 cycles) . The complete remission (CR) rate was 33.98% and the overall response rate (ORR) was 77.02%. Multivariate analysis revealed that complex karyotype ( P=0.02, OR=0.39, 95% CI 0.18-0.84) was an independent favorable factor for CR rate. TP53 mutation ( P=0.02, OR=0.22, 95% CI 0.06-0.77) was a predictive factor for a higher ORR. The median OS for the HMA-treated patients was 25.67 (95% CI 21.14-30.19) months. HMA response ( P=0.036, HR=0.47, 95% CI 0.23-0.95) was an independent favorable prognostic factor, whereas complex karyotype ( P=0.024, HR=2.14, 95% CI 1.10-4.15) , leukemia transformation ( P<0.001, HR=2.839, 95% CI 1.64-4.92) , and TP53 mutation ( P=0.012, HR=2.19, 95% CI 1.19-4.07) were independent adverse prognostic factors. There was no significant difference in efficacy and survival between the reduced and standard doses of HMA. The CR rate and ORR of MDS patients treated with decitabine and azacitidine were not significantly different. The median OS of patients treated with decitabine was longer compared with that of patients treated with azacitidine (29.53 months vs 20.17 months, P=0.007) . The incidence of bone marrow suppression and pneumonia in the decitabine group was higher compared with that in the azacitidine group. Conclusion:Continuous and regular use of appropriate doses of hypomethylating agents may benefit MDS patients to the greatest extent if it is tolerated.

Malocclusion,identified by the World Health Organization(WHO)as one of three major oral diseases,profoundly impacts the dental-maxillofacial functions,facial esthetics,and long-term development of~260 million children in China.Beyond its physical manifestations,malocclusion also significantly influences the psycho-social well-being of these children.Timely intervention in malocclusion can foster an environment conducive to dental-maxillofacial development and substantially decrease the incidence of malocclusion or reduce the severity and complexity of malocclusion in the permanent dentition,by mitigating the negative impact of abnormal environmental influences on the growth.Early orthodontic treatment encompasses accurate identification and treatment of dental and maxillofacial morphological and functional abnormalities during various stages of dental-maxillofacial development,ranging from fetal stages to the early permanent dentition phase.From an economic and societal standpoint,the urgency for effective early orthodontic treatments for malocclusions in childhood cannot be overstated,underlining its profound practical and social importance.This consensus paper discusses the characteristics and the detrimental effects of malocclusion in children,emphasizing critical need for early treatment.It elaborates on corresponding core principles and fundamental approaches in early orthodontics,proposing comprehensive guidance for preventive and interceptive orthodontic treatment,serving as a reference for clinicians engaged in early orthodontic treatment.
8.Role of specific lncSLC25a6 in homocysteine-induced cuproptosis in rat cardiomyocytes

AIM:To investigate the role of specific long noncoding RNA SLC25a6(lncSLC25a6)in homocys-teine(Hcy)-induced cuproptosis in cardiomyocytes.METHODS:Rat cardiomyocytes were cultured in vitro and divided into control group and Hcy group.After 48 h of intervention,the expression levels of cuproptosis-related proteins,ferre-doxin 1(FDX1)and heat shock protein 70(HSP70),were detected by Western blot and immunofluorescence staining.The oxidative stress state of cardiomyocytes was assessed using fluorescence staining,and the intracellular Cu2+levels were measured using a copper ion assay kit.Furthermore,the impact of Hcy on the expression of cuproptosis-related proteins in cardiomyocytes was analyzed following overexpression of lncSLC25a6.RESULTS:Compared with the control group,80 μmol/L Hcy significantly accelerated cardiomyocyte damage,with a notable underexpression of lncSLC25a6(P<0.05).Western blot results indicated that,compared with the control group,the expression level of FDX1 in the Hcy intervention group was significantly reduced(P<0.05),while the expression level of HSP70 was significantly elevated(P<0.05),and the expression level of copper ions in cardiomyocytes of the Hcy group was increased(P<0.05).Immunofluorescence staining showed a significant reduction in FDX1 fluorescence intensity and a significant increase in HSP70 fluorescence in-tensity in the Hcy group.Further overexpression of lncSLC25a6 significantly mitigated Hcy-induced cuproptosis in cardio-myocytes,resulting in elevated expression of FDX1 and reduced expression of HSP70(P<0.05).Pearson correlation analysis demonstrated that the expression level of lncSLC25a6 was negatively correlated with FDX1 protein expression(r=-0.676,P=0.046)and positively correlated with HSP70 expression(r=0.680,P=0.044).CONCLUSION:lnc-SLC25a6 significantly mitigates Hcy-induced cuproptosis in cardiomyocytes,positioning it as a potential therapeutic target for managing Hcy-induced cardiac injury.
9.Clinical characteristics and all-cause mortality influencing factors of 176 patients with Keshan disease

Objective:To analyze the clinical characteristics and all-cause mortality influencing factors of patients with Keshan disease.Methods:Clinical data of patients with Keshan disease from Keshan disease areas in Sichuan Province and Yunnan Province were collected and retrospectively analyzed for clinical characteristics and survival status during regular follow-up. According to the survival status of patients, the survey subjects were divided into a survival group and a death group. All-cause mortality (referring to the death caused by various reasons throughout the follow-up period) was used as the study endpoint. Kaplan-Meier (K-M) survival curve analysis and log-rank χ 2 test were performed, univariate and multivariate Cox regression analysis were used for all-cause mortality factor analysis. Results:A total of 176 patients with Keshan disease were collected, including 92 cases in Sichuan Province and 84 cases in Yunnan Province. Among all the patients, there were 105 males, accounting for 59.66%, and 71 females, accounting for 40.34%. The age was (53.89 ± 13.19) years old. Thirty-five cases died from all causes, with a mortality rate of 19.89%. There were significant differences in age ( t = 2.09, P = 0.038), New York Heart Association (NYHA) cardiac function grading (χ 2 = 14.62, P < 0.001) and ventricular premature contraction (χ 2 = 6.82, P = 0.009) between the survival group and the death group. K-M survival curve analysis showed that patients with Keshan disease complicated by premature ventricular contraction and high NYHA cardiac function grading (Ⅲ and Ⅳ) had higher all-cause mortality (log-rank χ 2 = 8.72, 22.49, P < 0.05). Univariate Cox regression analysis showed that NYHA cardiac function grading and ventricular premature contraction ( HR = 3.09, 2.71, P < 0.05) were predictive influencing factors for all-cause mortality in patients with Keshan disease. Multivariate Cox regression analysis showed that NYHA cardiac function grading ( HR = 6.57, P = 0.002) and ventricular premature contraction ( HR = 2.98, P = 0.050) were independent factors for all-cause mortality in patients with Keshan disease. Conclusions:Among 176 patients with Keshan disease, the number of patients with poor cardiac function (NYHA cardiac function grading Ⅲ and Ⅳ) and arrhythmia is high. NYHA cardiac function grading and ventricular premature contractions are independent influencing factors for all-cause mortality in patients with Keshan disease.
10.Epidemiological characteristics of brucellosis among non-occupational population in Jinan City

Objective:To study the epidemiological characteristics of brucellosis (hereinafter referred to as brucellosis) among non-occupational population in Jinan City, and to provide a basis for formulating further prevention and control measures.Methods:From "the Infectious Disease Monitoring System of the China Center for Disease Control and Prevention", Center for Disease Control and Prevention of Jinan City and its subordinate counties, the epidemiological data and case investigation data of brucellosis in Jinan from 2012 to 2021 were collected and statistically analyzed using descriptive epidemiological methods.Results:From 2012 to 2021, a total of 453 cases of brucellosis were reported in the non-occupational population of Jinan City, accounting for 20.21% (453/2 241) of the total number of cases. The proportion of brucellosis cases in the non-occupational population showed an increasing trend year by year (χ 2tend = 25.50, P < 0.001). Among the non-occupational population, there were 317 males and 136 females, with a gender ratio of 2.33∶1.00; the age was (46.34 ± 17.72) years old, with a minimum age of 1 year and a maximum age of 86 years. The majority of the affected population were farmers, accounting for 71.74% (325/453). The peak onset time was April and May. The main routes of infection were through the digestive tract and respiratory tract, accounting for 36.87% (167/453) and 33.77% (153/453), respectively. The clinical symptoms were mainly fever, muscle soreness, fatigue, and hyperhidrosis. The diagnosis time was 20.0 (7.5, 37.5) d, and the sensitivity of city and district general hospitals to detect and diagnose brucellosis was relatively low. Conclusions:The incidence rate of brucellosis among non-occupational population in Jinan is increasing year by year. The high incidence season is spring, and the high incidence population is male young and middle-aged farmers. Digestive and respiratory transmission are the main transmission routes, with atypical clinical symptoms and a long diagnosis time. The next step is to strengthen the promotion of brucellosis prevention and control knowledge and the training of medical personnel in brucellosis diagnosis.
