1.Correlation between the distribution of CYP2C19,ABCB1,PON1 genotypes and the risk of clopidogrel resistance in coronary heart disease patients in Tai'an

Aim To study the distribution of CYP2C19,ABCB1,and PON1 genotypes and their correlation with clopidogrel resistance in patients with coronary heart disease in Tai'an.Methods A total of 594 patients with coronary heart disease who were treated with clopidogrel during hospitalization in Tai'an Central Hospital from January 2019 to March 2020 were selected.Fluorescence in situ hybridization was used to detect CYP2C19*2(rs4244285),CYP2C19*3(rs4986893),CYP2C19*17(rs12248560),ABCB1(rs1045642)and PON1(rs662)gene types.Results CYP2C19*2,CYP2C19*3,CYP2C19*17 genotypes in patients with coronary heart disease in Tai'an were mainly with homozygous(GG).The frequencies of CYP2C19*2 GG,CYP2C19*3 GG,CYP2C19*17 CC,ABCB1 CT and PON1 AG were 48.0%,89.6%,97.0%,46.8%and 47.1%respectively.There was no significant difference in CYP2C19*2,CYP2C19*3,CYP2C19*17,ABCB1,PON1 genotype distribution and allele distribution between male and female patients(P>0.05).Significant regional differences in the frequency of CYP2C19 alleles and the distribution of metabolic types were found in patients with coronary heart disease in Tai'an.Among 594 patients included in the study,there were 287 patients with a risk level of clopidogrel resistance ≥ 2 in the composite evaluation of patients,approximately 48.3%of the total number of patients.This indicated that clopidogrel resistance was present in 48.3%of patients on the regular dose of clopidogrel.Of the 287 people with a risk level ≥2,46 had a normal CYP2C19 metabolic type,representing approxi-mately 7.7%of the total number of patients.Conclusion There were gene polymorphisms observed in CYP2C19*2,CYP2C19*3,CYP2C19*17,ABCB1 and PON1 distribution in patients with coronary heart disease in Tai'an,and ABCB1 and PON1 gene polymorphisms would had an impact on the outcome of medication guidance in approximately 7.7%.
2.Osteoimmunology research in rheumatoid arthritis: From single-cell omics approach.

Cellular immune responses as well as generalized and periarticular bone loss are the key pathogenic features of rheumatoid arthritis (RA). Under the pathological conditions of RA, dysregulated inflammation and immune processes tightly interact with skeletal system, resulting in pathological bone damage via inhibition of bone formation or induction of bone resorption. Single-cell omics technologies are revolutionary tools in the field of modern biological research.They enable the display of the state and function of cells in various environments from a single-cell resolution, thus making it conducive to identify the dysregulated molecular mechanisms of bone destruction in RA as well as the discovery of potential therapeutic targets and biomarkers. Here, we summarize the latest findings of single-cell omics technologies in osteoimmunology research in RA. These results suggest that single-cell omics have made significant contributions to transcriptomics and dynamics of specific cells involved in bone remodeling, providing a new direction for our understanding of cellular heterogeneity in the study of osteoimmunology in RA.

3.Sexual orientation and characteristics of sexual behaviors among 200 HIV positive male college students in Beijing

Objective:
To understand sexual orientation and characteristics of sexual behaviors related with HIV infection among HIV positive male college students in Beijing and to provide suggestions for development of sexual health education and HIV prevention strategy for student population.
Methods:
HIV positive male college students diagnosed from 2016 to June 30, 2019 in Beijing were recruited. Questionnaire was used to retrospectively investigate sexual orientation and characteristics of sexual behaviors before HIV infection. Behaviors related to HIV infection were compared between absolutely homosexual and others sexual orientation.
Results:
The average age of male students firstly identified to be HIV positive was (22.18±2.70) years old, 61.5%(123) of them were undergraduate, 69.5%(139) were not registered permanent residency in Beijing, 38.5%(77) of their interviewed sexual partners were from online chat and 83.0%(166) had homosexual behaviors. Sexual orientation score analysis showed that 50.0% of the participants self identified as exclusively homosexual. Compared with other sexual oriental group, exclusively homosexual group had lower mean age of their HIV infection firstly identified( t =2.77, P =0.01), higher rate of Rush use, firstly insertive sexual behavior with male, firstly insertive sexual behavior before 18 years old, sexual partners more than three persons, having regular partners, nonpersistent use of condom, being diagnosed of sexual transmitted disease and the frequency of homosexual behaviors more than 1 time per month ( χ 2=5.15,28.06,4.16,5.34,5.89,7.39,6.68, P <0.05). Rush users had higher rate of STD diagnosis than non users in exclusively homosexual group ( χ 2=6.26, P =0.01).
Conclusion
Risky sexual behaviors associated with HIV infection were higher in exclusively homosexual group then other sexual orientation groups among HIV positive male college students. Family and school should concern with sexual health education byreinforcing health education via network media to improve college students’ awarenees on HIV/AIDS.
4.Prospective effects of protein and animal foods intake on age at menarche among Chinese girls

Objective:
To investigate the prospective effects of the consumption of protein and animal foods before menarche on the age at menarche among Chinese girls.
Methods:
This paper was based on the data collected in the China Health and Nutrition Survey(CHNS) from 1997 to 2015. A total of 683 girls aged 6 and over who had completed information on age at menarche, height, weight, per capita annual household income, maternal education level and participated in at least one complete dietary survey within 1 to 4 years before menarche were included. Urban-rural stratified multivariable linear regression model was used to examine the effects of protein and animal foods intake before menarche on Chinese girls age at menarche in urban and rural areas.
Results:
After adjusted for total energy intake, body mass index standard deviation score and per capita annual household income, the consumption of meat before menarche was negatively associated with the age at menarche among rural Chinese girls(B=-0.003, P=0.00), but not among urban Chinese girls(B=0.002, P>0.05). Total protein, dairy, eggs and aquatic products intake before menarche were not associated with Chinese girls age at menarche in urban and rural areas(B=0.002, -0.001, 0.003, 0.000; 0.001, 0.001, -0.001, -0.003, P>0.05).
Conclusion
Higher intake of meat before menarche might lead to earlier menarche onset in rural Chinese girls. The consumption of total protein, dairy, eggs, and aquatic products before menarche did not affect the age at menarche in Chinese girls.
5.Rush use and related factors among HIV positive young male students in Beijing

Objective:
This study investigated the use of rush and related factors among HIV positive male students in Beijing, and provided suggestions for education and intervention.
Methods:
Data on HIV positive male students was collected from 2017 to June 30, 2020 in Beijing and analyzed in SPSS 20.0.
Results:
A total of 200 valid questionnaires were collected. The mean age at HIV diagnosis was (22.18±2.70) years. The rate of using rush was 46.00%. The proportion with homosexual behavior before infection was 92.00%. The results of multivariate analysis showed that meeting sexual partners through the Internet( OR=3.84, 95%CI =1.65-8.96), drinking alcohol( OR=3.26, 95%CI =1.51-7.02), group homosexual behavior( OR=4.84, 95%CI =1.71-13.71), and STD diagnosis ( OR=0.23, 95%CI =1.02-4.87) were associated with the use of rush before infection.
Conclusion
The rate of rush use was higher among male students infected with HIV. Government regulation could be strengthened and drug abuse could be prevented through better health education for students.
6.Effect of driving pressure-guided individualized PEEP titration on atelectasis in elderly patients undergoing robot-assisted radical prostatectomy

Objective:To evaluate the effect of driving pressure-guided individualized positive end-expiratory pressure (PEEP) titration on atelectasis in elderly patients undergoing robot-assisted radical prostatectomy.Methods:Fifty elderly patients, aged 65-80 yr, of American Society of Anesthesiologists physical status Ⅱ or Ⅲ, with body mass index of 19-28 kg/m 2, undergoing elective robot-assisted radical prostatectomy under general anesthesia, were divided into 2 groups ( n=25 each) according to the random number table method: traditional lung-protective ventilation group (group C) and driving pressure-guided individualized PEEP group (group D). The method for setting PEEP was as follows: PEEP 5 cmH 2O was used throughout operation in group C. In group D, the optimal PEEP was titrated after intubation and mechanical ventilation and Trendelenburg position-pneumoperitoneum construction, the initial value was the lowest PEEP allowed by the anesthesia machine, the PEEP was increased by 1 cmH 2O (PEEP≤12 cmH 2O) every 4 min, the plateau pressure and PEEP were simultaneously recorded to calculate the driving pressure, and the corresponding PEEP was considered as the optimal PEEP for the individual when the driving pressure reached the minimum.Ultrasound examination was performed after catheterization of radial artery (T 0), after anesthesia induction (T 1), 4 min after developing optimal PEEP ventilation (T 2, 4 min after developing ventilation in group C), after restoration of body position (T 3), before extubation (T 4), and at 2 h after admission to postanesthesia care unit (T 5). Atelectatic aeration loss scores were recorded at T 0, T 1, T 4 and T 5.Bilateral optic nerve sheath diameter was measured at T 0-4.Arterial blood gas analysis was performed at T 0, T 2, T 3 and T 5, PaO 2 and PaCO 2 were recorded, and oxygenation index was calculated.The postoperative pulmonary complications within 3 days after operation were recorded. Results:Compared with group C, atelectasis aeration loss scores at T 4, 5 and PaCO 2 at T 2, 3 were significantly decreased, and PaO 2 and oxygenation index were increased at T 2, 3, 5 in group D ( P<0.05). There were no significant differences in the bilateral optic nerve sheath diameter and incidence of postoperative pulmonary complications between the two groups ( P>0.05). Conclusion:Driving pressure-guided individualized PEEP can increase intraoperative oxygenation and decrease the development of atelectasis in elderly patients undergoing robot-assisted radical prostatectomy.
7.Performance of serum 2019-nCoV IgM/IgG detection in the rapid diagnosis of COVID-19

Objective:To evaluate the rapid diagnostic value of serum novel coronavirus (2019-nCoV) IgM/IgG detection in COVID-19, aiming to further improve the diagnostic and screening system of COVID-19.Methods:Blood samples were collected from 32 patients with COVID-19 (tested positive for 2019-nCoV nucleic acid by RT-PCR and presented with clinical symptoms) and 34 non-COVID-19 patients (tested negative for 2019-nCoV nucleic acid by RT-PCR and clinically confirmed as non-COVID-19 patients). Colloidal gold-based immunochromatography was used for rapid detection of 2019-nCoV IgM/IgG in these samples. The sensitivity and specificity of the test, and the correlation of serum 2019-nCoV IgM/IgG with disease course were analyzed.Results:Among the 32 COVID-19 patients, nine tested positive for 2019-nCoV IgM with a positive rate of 28.1% (9/32) and 25 positive for 2019-nCoV IgG with a positive rate of 78.1% (25/32). The total positive rate was 84.4% (27/32). Two of the 34 non-COVID-19 patients tested positive for 2019-nCoV IgG with a positive rate of 5.9% (2/34), while none of them was positive for 2019-nCoV IgM. The positive rates of serum IgM were 42.9% (3/7), 30.8% (4/13) and 16.7% (2/12) at 10-20 d, 21-30 d and 31-40 d after the patients developed the symptoms of COVID-19, respectively, which showed a decreasing tread with prolonged disease course. The positive rates of serum IgG in COVID-19 patients were 57.1% (4/7), 84.6% (11/13) and 83.3% (10/12) at 10-20 d, 21-30 d and 30-40 d after symptom onset. The rate showed an increasing trend with prolonged disease course and reached the peak in about 21-30 d.Conclusions:Serum 2019-nCoV IgM/IgG detection (using colloidal gold method) had high sensitivity (84.4%) and strong specificity (94.1%) in the diagnosis of 2019-nCoV infection. It had a great value in the diagnosis and screening of COVID-19 and could be used as a valuable complementary method to the COVID-19 diagnostic system due to its advantages of flexibility, rapidity and simplicity.
8.Animal experimental study on the examination of upper digestive tract with medical disposable portable endoscopy

Objective:To evaluate the safety, feasibility and operational performance of self-developed medical disposable portable endoscopy (YunSendo) for upper gastrointestinal endoscopy examination in Ba-Ma mini-pigs.Methods:A total of 10 Guangxi Ba-Ma mini-pigs were used in the experiment, and mucosal injury models were established in advance by biopsy forceps in esophagus, stomach, and duodenum. Each experimental animal underwent medical disposable portable endoscopy and Olympus endoscopy (GIF-Q260J) performed by two endoscopists separately. The time when the endoscope reached the duodenum, the number of detected mucosal injuries and endoscopic pictures of different parts with standard image acquisition were recorded. Endoscopic operational performance and endoscopic image quality were evaluated. Different endoscopists recorded experimental results with blind method. The procedures of the two endoscopic examinations were performed by coin-tossing method. The paired t test was used for statistical analysis. Results:There were no statistically significant differences in the insertion time and total operation time between medical disposable portable endoscopy and Olympus endoscopy ( (171.00±9.96) s vs. (164.00±17.84) s, (285.00±33.94) s vs. (273.40±23.46) s; t=1.289 and 1.281, P=0.230 and 0.232). There were no statistically significant differences in the percentage of time of clear visual field during endoscopy insertion and total operation between medical disposable portable endoscopy and Olympus endoscopy ((91.83±1.85)% vs. (91.52±1.51)%, (93.07±3.10)% vs. (92.06±2.57)%; t=0.401 and 0.689, P=0.698 and 0.508). Moreover, there were no statistically significant differences in the score of comprehensive operation performance, score of clear image number, score of image color recognition, score of image illumination, comprehensive score of image quality and number of detected mucosal injuries ((9.66±0.30) points vs. (9.86±0.15) points, (39.50±0.71) points vs. (39.30±1.06) points, (39.70±0.48) points vs. (39.40±0.70) points, (39.40±0.70) points vs. (39.50±0.71) points, (9.88±0.09) points vs. (9.85±0.20) points, 9.80±0.42 vs. 9.90±0.32; t=2.176, 1.000, 1.152, 0.317, 0.629 and 0.557, all P>0.05). There were no adverse events after operation in medical disposable portable endoscopy group and Olympus endoscopy group. Conclusions:The medical disposable portable endoscopy is safe and feasible for endoscopy examination in live animal models. Different parts of upper gastrointestinal tract and mucosal lesions can be clearly detected. The operational performance and the image quality are excellent, which is similar to Olympus endoscopy (GIF-Q260J).
9.Status and influencing factors of antiretroviral treatment referred by case reporting institutions among HIV/AIDS cases in Beijing, 2017-2019

Objective:To understand the status of antiretroviral treatment (ART) referred by the case reporting institutions among HIV/AIDS cases and influencing factors in Beijing.Methods:From June 1, 2017 to April 30, 2019, the data of 4 917 people living with HIV/AIDS (PLWHA) collected from National HIV/AIDS Information System and ART institutions were used to understand the status of ART and influencing factors. No records of ART was defined as referral failure and an interval of more than 15 days between diagnosis of HIV infection and ART initiation was defined as delayed ART.Results:Among the 4 917 HIV/AIDS cases, 16.53% (813/4 917) had referral failure. Among the 4 104 PLWHA who received ART, 30.63% (1 257/4 104) had delayed ART, the median of the interval was 27 days ( P 25- P 75:19-42 days). Multivariate logistic regression analysis showed that HIV/AIDS cases who were reported by comprehensive hospitals (compared with ART servicers, OR=1.65, 95% CI: 1.30-2.08), in HIV phase (compared with AIDS phase, OR=1.68, 95% CI: 1.34-2.11), with educational level of junior middle school or below (compared with colleague graduate or above, OR=1.42, 95% CI: 1.13-1.79), unemployed (compared with civil servants or staffs, OR=1.32, 95% CI:1.02-1.70), infected through heterosexual behavior (compared with homosexual behavior, OR=1.40, 95% CI: 1.15-1.72), diagnosed in special survey (compared with testing and counseling, OR=1.87, 95% CI: 1.34-2.60) or blood donation ( OR=3.52, 95% CI: 1.79-6.92), were more likely to have referral failures. And the PLWHA who were reported by CDCs ( OR=1.43, 95% CI: 1.07-1.92) or comprehensive hospitals (compared with ART servicers, OR=1.62, 95% CI: 1.32-1.98), reported by rural reporting institutions (compared with urban reporting institutions, OR=1.24, 95% CI: 1.01-1.52), in HIV phase (compared with AIDS phase, OR=1.42, 95% CI: 1.19-1.70), non-Beijing residence (compared with Beijing residence, OR=1.26, 95% CI: 1.07-1.48), diagnosed in blood donation examination (compared with STD clinics, OR=2.74, 95% CI: 1.21-6.22) were more likely to have delayed ART. Conclusions:The rate of referral failure and delayed ART varied among HIV/AIDS cases with different characteristics. It is still necessary to take effective measures to promote ART in order to reduce referral failure and delayed ART.
10. The association of pre-pregnancy body mass and weight gain during pregnancy with macrosomia: a cohort study

Objective:
To examine the association of pre-pregnancy body mass and weight gain during pregnancy with macrosomia.
Methods:
From January 2015 to December 2015, a total of 20 477 pregnant women were recruited by probabilistic proportional scale sampling with simple randomization in Sichuan, Yunnan and Guizhou Provinces. Basic information of pregnant women, weight gain during pregnancy and weight of newborn were collected. A multiple logistic regression model was used to assess the association between the pre-pregnancy body mass and gestational weight gain indicators with macrosomia.
Results:
20 321 mother-infant were included in the final analysis. 20 321 pregnant women were (30.09±4.10) years old and delivered at (39.20±1.29) weeks, among which 12 341 (60.73%) cases were cesarean delivery. The birth weight of 20 321 infants were (3 292.26±431.67) grams, and 970 (4.77%) were macrosomia. The multiple logistic regression model showed that after adjusting for the age of women, compared to the normal weight group in the pre-pregnancy, the overweight and obesity group elevated the risk of macrosomia, with
