1.Differences in gut microbiota among primary school students with different levels of sugar sweetened beverage consumption

Objective:
To explore the differences in the gut microbiota of primary school students with different levels of sugar sweetened beverage intake, so as to provide scientific evidence for better identification of health risks in children and the development of targeted health policies.
Methods:
In June 2022, a total of 192 healthy primary school students from Chengdu were selected using a stratified cluster random sampling method. The sugar sweetened beverage intake was assessed through a dietary frequency questionnaire. Based on the median daily sugar sweetened beverage intake, primary school students were categorized into a low intake group ( n =96) and a high intake group ( n =96). The gut microbiota in fresh fecal samples from the two groups of primary school students was analyzed using 16S rRNA high throughput sequencing, and the diversity and community structure differences in the gut microbiota were compared.
Results:
Children in the low intake group had a sugar sweetened beverage intake of (21.3±1.6) mL/d, while the high intake group had an intake of (269.6±37.3) mL/d. Diversity analysis results showed that there were no statistically significant differences between the low intake and the high intake group in terms of α diversity metrics: Observed_otus index [298.50 (259.75, 342.25), 305.50 (244.25, 367.75)], Goods_coverage index [1.00 (1.00, 1.00), 1.00 (1.00, 1.00)], Chao index [304.18 (260.75, 348.78), 305.88 (245.68, 370.88)], Shannon index [5.88 (5.29, 6.45), 5.71 (4.89, 6.28)] and Simpson index [0.95 (0.91, 0.97), 0.94 (0.88, 0.97)] ( Z =-0.64, -0.76, -0.54, -1.76, -1.67, P >0.05). Furthermore, no statistically significant difference was observed in β diversity between the two groups ( R 2=0.006, P >0.05). At the genus level, the abundance of Blautia [0.033 (0.018, 0.055)] and Fusicatenibacter [0.009 (0.005, 0.015)] were higher in the low intake group compared to the high intake group [0.024 (0.013, 0.041),0.006 (0.003, 0.011)]and differences were statistically significant ( Z =-2.52, -2.81, P <0.05). LEfSe analysis highlighted intergroup differences primarily in Blautia, Fusicatenibacter and Sarcina( LDA= 3.56,3.12,3.53, P <0.05).
Conclusions
There is no significant difference in the diversity and overall structure of the gut microbiota in primary school students with different levels of sugar sweetened beverage intake. However, there are species variations at the genus level. The information can serve as a scientific basis for identifying health risks in primary school students and formulating targeted health strategies.
2.Different methods in predicting mortality of pediatric intensive care units sepsis in Southwest China

Objective:To investigate the value of systemic inflammatory response syndrome (SIRS), pediatric sequential organ failure assessment (pSOFA) and pediatric critical illness score (PCIS) in predicting mortality of pediatric sepsis in pediatric intensive care units (PICU) from Southwest China.Methods:This was a prospective multicenter observational study. A total of 447 children with sepsis admitted to 12 PICU in Southwest China from April 2022 to March 2023 were enrolled. Based on the prognosis, the patients were divided into survival group and non-survival group. The physiological parameters of SIRS, pSOFA and PCIS were recorded and scored within 24 h after PICU admission. The general clinical data and some laboratory results were recorded. The area under the curve (AUC) of the receiver operating characteristic curve was used to compare the predictive value of SIRS, pSOFA and PCIS in mortality of pediatric sepsis.Results:Amongst 447 children with sepsis, 260 patients were male and 187 patients were female, aged 2.5 (0.8, 7.0) years, 405 patients were in the survival group and 42 patients were in the non-survival group. 418 patients (93.5%) met the criteria of SIRS, and 440 patients (98.4%) met the criteria of pSOFA≥2. There was no significant difference in the number of items meeting the SIRS criteria between the survival group and the non-survival group (3(2, 4) vs. 3(3, 4) points, Z=1.30, P=0.192). The pSOFA score of the non-survival group was significantly higher than that of the survival group (9(6, 12) vs. 4(3, 7) points, Z=6.56, P<0.001), and the PCIS score was significantly lower than that of the survival group (72(68, 81) vs. 82(76, 88) points, Z=5.90, P<0.001). The predictive value of pSOFA (AUC=0.82) and PCIS (AUC=0.78) for sepsis mortality was significantly higher than that of SIRS (AUC=0.56) ( Z=6.59, 4.23, both P<0.001). There was no significant difference between pSOFA and PCIS ( Z=1.35, P=0.176). Platelet count, procalcitonin, lactic acid, albumin, creatinine, total bilirubin, activated partial thromboplastin time, prothrombin time and international normalized ratio were all able to predict mortality of sepsis to a certain degree (AUC=0.64, 0.68, 0.80, 0.64, 0.68, 0.60, 0.77, 0.75, 0.76, all P<0.05). Conclusion:Compared with SIRS, both pSOFA and PCIS had better predictive value in the mortality of pediatric sepsis in PICU.
3.Role of spinal cord microglia in chronic pain after chronic heart failure in mice

Objective:To evaluate the role of spinal cord microglia in chronic pain after chronic heart failure (CHF) in mice.Methods:Eighteen SPF healthy male C57BL/6 mice, aged 8-12 weeks, weighing 20-25 g, were divided into 3 groups ( n=6 each) using a random number table method: sham operation group (Sham group), CHF group, and CHF+ microglia inhibitor PLX3397 group (CHF+ PLX group). The model of chronic heart failure was prepared by ligating the anterior descending branch of the left coronary artery in anesthetized mice. Mice were continuously fed a PLX3397-containing diet from 7 days before preparing the model to 28 days after preparing the model in CHF + PLX group. The cardiac function was evaluated using echocardiography at 28 days after surgery. The mechanical paw withdrawal threshold (MWT) and thermal paw withdrawal latency (TWL) were measured on 1 day before developing the model (T 0) and 1, 3, 7, 14 and 28 days after developing the model (T 1-5). The mice were sacrificed under deep anesthesia after the pain threshold was measured at T 5, and the spinal cord tissues of the lumbar segment (T 2-6 and L 4-6) were removed for determination of the expression of microglial marker Iba1 in the spinal dorsal horn (by immunofluorescence method). Results:Compared with Sham group, the left ventricular ejection fraction and left ventricular short axis shortening rate were significantly decreased, and the left ventricular internal diameter at end-diastole and the left ventricular internal diameter at end-systole were increased, the TWL was shortened and MWT was decreased at T 4-5, and the expression of Iba1 in the spinal dorsal horn was up-regulated in CHF group ( P<0.05). Compared with CHF group, the left ventricular ejection fraction and left ventricular short axis shortening rate were significantly increased, and the left ventricular internal diameter at end-diastole and the left ventricular internal diameter at end-systole were decreased, the TWL was prolong and MWT was increased at T 2-5, and the expression of Iba1 in the spinal dorsal horn was down-regulated in CHF+ PLX group ( P<0.05). Conclusions:The chronic pain after CHF may be related to microglial activation in the spinal cord of mice.
4.Expert consensus on gastrointestinal dysfunction secondary to sepsis with integrating Traditional Chinese Medicine and Western medicine

Acute gastrointestinal dysfunction is a common and important complication of sepsis. As no exiting formal definition and classification of gastrointestinal dysfunction, most of the treatment strategies for gastrointestinal dysfunction are not based on clinical evidence, but on their own clinical experience. Experts of traditional Chinese medicine, integrated traditional Chinese and Western medicine and Western medicine from various disciplines in Shanghai are organized by the Shanghai Society of Integrated Traditional Chinese and Western Medicine and the Emergency Department Branch of Shanghai Physicians Association. After repeated discussion, literature search and formulation of the outline, we developed consensus on gastrointestinal dysfunction secondary to sepsis with integrating Traditional Chinese Medicine and Western medicine by consulting extensively on clinical experts in the fields of emergency medicine, gastroenterology, general surgery, infectious medicine and traditional Chinese medicine, and holding several expert forums and consultation meetings. This clinical expert consensus focused on acute gastrointestinal injury (AGI) classification and inducer of sepsis. In this consensus, the common symptoms, diagnosis, classifications, treatment strategies and suggestions of acute gastrointestinal injury or dysfunction secondary to sepsis were explored from the aspect of both Traditional Chinese Medicine and Western medicine.
5.Role of Caveolin-3/ERK signaling pathway in reduction of myocardial ischemia-reperfusion injury by morphine preconditioning in rats with chronic heart failure: an in vitro experiment

Objective:To investigate the role of Caveolin (Cav-3)/extracellular signal-regulated kinase (ERK) signaling pathway in reduction of myocardial ischemia-reperfusion (I/R) injury by morphine preconditioning in rats with chronic heart failure.Methods:Clean-grade healthy adult male Sprague-Dawley rats, weighing 200-250 g, were used in this study.Chronic heart failure was induced by ligating the left anterior descending coronary artery for 6 weeks.Thirty-six Langendorff-perfused hearts with chronic heart failure were divided into 4 groups ( n=9 each) by a random number table method: myocardial I/R group (group IR), morphine preconditioning group (group MP), morphine preconditioning plus methyl-β-cyclodextrin group (group MP+ MβCD), and methyl-β-cyclodextrin group (group MβCD). Global myocardial I/R was induced by 30 min ischemia followed by 120 min reperfusion.In group MP, after 15 min of equilibration, hearts were subjected to 3 cycles of 5 min perfusion with K-H solution containing 1 μmol/L morphine for preconditioning followed by 5 min perfusion with K-H solution, 30 min in total, and after the end of treatment, hearts were subjected to 30 min ischemia followed by 120 min reperfusion.In group MP+ MβCD, hearts were perfused with K-H solution containing 200 μmol/L methyl-β-cyclodextrin at 10 min before preconditioning with morphine, and the other treatments were similar to those previously described in group MP.In group MβCD, hearts were perfused with K-H solution containing 200 μmol/L methyl-β-cyclodextrin at 40 min before ischemia, and the other treatments were similar to those previously described in group IR.At the end of 15 min of equilibration (T 0) and 5 and 10 min of reperfusion (T 1, 2), coronary outflow was collected for determination of actate dehydrogenase (LDH) activity by chemical colorimetry.Myocardial infarct size (IS) and area at risk (AAR) were measured, and IS/AAR was calculated at the end of 120 min reperfusion.Myocardial tissues of left ventricle were taken to detect the expression of Cav-3, ERK1/2 and phosphorylated ERK1/2 (p-ERK1/2) by Western blot, and p-ERK1/2/ERK1/2 ratio was calculated. Results:Compared with group IR, IS, IS/AAR and LDH activity in coronary outflow were significantly decreased, the expression of Cav-3 was up-regulated, and p-ERK1/2/ERK1/2 ratio was increased in group MP ( P<0.05). Compared with group MP, IS, IS/AAR and LDH activity in coronary outflow were significantly increased, the expression of Cav-3 was down-regulated, and p-ERK1/2/ERK1/2 ratio was decreased in group MP+ MβCD ( P<0.05). Conclusions:The mechanism by which morphine preconditioning reduces I/R injury may be related to activation of Cav-3/ERK signaling pathway in rats with chronic heart failure.
6.Sexual orientation and characteristics of sexual behaviors among 200 HIV positive male college students in Beijing

Objective:
To understand sexual orientation and characteristics of sexual behaviors related with HIV infection among HIV positive male college students in Beijing and to provide suggestions for development of sexual health education and HIV prevention strategy for student population.
Methods:
HIV positive male college students diagnosed from 2016 to June 30, 2019 in Beijing were recruited. Questionnaire was used to retrospectively investigate sexual orientation and characteristics of sexual behaviors before HIV infection. Behaviors related to HIV infection were compared between absolutely homosexual and others sexual orientation.
Results:
The average age of male students firstly identified to be HIV positive was (22.18±2.70) years old, 61.5%(123) of them were undergraduate, 69.5%(139) were not registered permanent residency in Beijing, 38.5%(77) of their interviewed sexual partners were from online chat and 83.0%(166) had homosexual behaviors. Sexual orientation score analysis showed that 50.0% of the participants self identified as exclusively homosexual. Compared with other sexual oriental group, exclusively homosexual group had lower mean age of their HIV infection firstly identified( t =2.77, P =0.01), higher rate of Rush use, firstly insertive sexual behavior with male, firstly insertive sexual behavior before 18 years old, sexual partners more than three persons, having regular partners, nonpersistent use of condom, being diagnosed of sexual transmitted disease and the frequency of homosexual behaviors more than 1 time per month ( χ 2=5.15,28.06,4.16,5.34,5.89,7.39,6.68, P <0.05). Rush users had higher rate of STD diagnosis than non users in exclusively homosexual group ( χ 2=6.26, P =0.01).
Conclusion
Risky sexual behaviors associated with HIV infection were higher in exclusively homosexual group then other sexual orientation groups among HIV positive male college students. Family and school should concern with sexual health education byreinforcing health education via network media to improve college students’ awarenees on HIV/AIDS.
7.Rush use and related factors among HIV positive young male students in Beijing

Objective:
This study investigated the use of rush and related factors among HIV positive male students in Beijing, and provided suggestions for education and intervention.
Methods:
Data on HIV positive male students was collected from 2017 to June 30, 2020 in Beijing and analyzed in SPSS 20.0.
Results:
A total of 200 valid questionnaires were collected. The mean age at HIV diagnosis was (22.18±2.70) years. The rate of using rush was 46.00%. The proportion with homosexual behavior before infection was 92.00%. The results of multivariate analysis showed that meeting sexual partners through the Internet( OR=3.84, 95%CI =1.65-8.96), drinking alcohol( OR=3.26, 95%CI =1.51-7.02), group homosexual behavior( OR=4.84, 95%CI =1.71-13.71), and STD diagnosis ( OR=0.23, 95%CI =1.02-4.87) were associated with the use of rush before infection.
Conclusion
The rate of rush use was higher among male students infected with HIV. Government regulation could be strengthened and drug abuse could be prevented through better health education for students.
8.Progress in research of influencing factors of prophylaxis drug use after non-occupational exposure to HIV

Non-occupational post-exposure prophylaxis (nPEP), a biological means to block the transmission of HIV, is recommended by European countries, USA and WHO to use in HIV high-risk groups, but its utilization rate is still very low. The information-motivation-behavioral skills model (IMB) can accurately explain the prevalence and change of health behaviors. Based on this model, this paper summarizes the progress in research of the influencing factors for nPEP use to provide a basis for further research to promote the use of nPEP.
9.Knowledge, attitude and practice of drinking water among college students in Hebei Province

Objective:
To understand the knowledge, attitudes and practices of drinking water among college students of Hebei Province, to provide evidence for health education.
Methods:
A total of 715 subjects were selected from a university in Baoding City, Hebei Province in March 2017 by random cluster sampling. Knowledge, attitude and behavior of drinking water among college students were investigated by self-designed questionnaires.
Results:
The awareness rate of water consumption of 1 500-1 700 mL was 19.0%, the awareness rate of drinking modes was 95.0%, the awareness rate of suitable drinking time was 30.1%, the awareness rate on drinking water was higher among girls and medical major sutdents (χ2=22.16, 12.49, P<0.05); the awareness rate on the relationship between insufficient drinking water and stroke, hypertension and coronary heart disease was generally low, with sophornore higher than freshman (χ2=11.10, P<0.05). The most common medium for accessing drinking water knowledge among the respondents was network (60.0%), followed by campus publicity/classroom teaching (48.3%). 99.6% of the respondents believed that drinking water was important to health, 98.3% were willing to change their drinking habits, 93.0% were willing to pay attention to drinking water education, and the proportion of female students was higher than that of male students (χ2=5.46, 6.49, 8.01, P<0.05). In real life, 48.4% of the respondents drank water on an empty stomach in the morning and 40.7% before going to bed. 45.5% of the respondents drank water when they thought of it.
Conclusion
College students in Hebei province are generally lack of appropriate awareness on water drinking, which varied in sex, grade and major. While the attitude towards water drinking is satisfactory, drinking behavior need to be improved.
10.Performance of rapid tests for HIV-1 antibody detection in paired serum and urine specimens of men who have sex with men

Objective:To compare the performance of rapid tests for HIV-1 antibody detection in serum and urine specimens of men who have sex with men (MSM) for investigating suitable technology in the prevention and control of AIDS in Beijing.Methods:A total of 874 cases of MSM were recruited in the AIDS clinic of Beijing Center for Disease Prevention and Control. HIV-1/2 antibody rapid test kit (Kit A, Alere Determine), urine HIV-1 antibody rapid test kit (Kit B, Wantai Biological Pharmacy) and HIV-1/2 antibody Western blot kit (Kit C, IMT) were used for antibody detection. The sensitivity, specificity and consistency of the three rapid test kits for HIV-1 antibody detection in serum and urine specimens were analyzed.Results:Among the 874 cases of MSM, 447 were positive for HIV-1 antibody (51.14%) and 427 were negative. One false negative result occurred by using Kit A and 23 by using Kit B. Taking Kit C as reference, the sensitivity of Kit A and Kit B was 99.78% and 94.85%, respectively; the specificity of both was 100%; the overall consistency was 99.89% ( Kappa=0.998) and 97.37% ( Kappa=0.947), respectively. Conclusions:Although the sensitivity of urine rapid test kit was not as sensitive as serum rapid test kit, it was more suitable for self-test due to its convenience in sampling, high safety and high accessibility. It was suggested that urine rapid test kit should be popularized in MSM population for HIV-1 antibody screening.
