1.Effect and mechanism of reactive oxygen species-responsive nanoparticles on the regulation of human gingival fibroblast function and inflammation induced by lipopolysaccharide

Objective:
To investigate the effects of PssL-NAC reactive oxygen species (ROS)-responsive nanoparticles on intracellular ROS production, inflammatory factor levels, collagen production, cell function and Toll-like receptor 4 (TLR4), NF-κB nuclear factor-κB (p65) pathway protein expression in human gingival fibroblasts (HGFs) induced by Porphyromonas gingivalis-lipopolysaccharide (P.g-LPS).
Methods:
This study was reviewed and approved by the ethics committee. PssL-NAC microspheres containing oil soluble antioxidant N-acetylcysteine (NAC) were obtained by connecting the hydrophobic end of polycaprolactone (PCL) and the hydrophilic end of polyethylene glycol (PEG) via thioketal (TK) bonds in response to ROS, and self loading in the aqueous and oil phases. After preparation of the PssL-NAC microspheres and aqueous NAC solution, successful synthesis of the nanoparticles was verified by transmission electron microscopy. Then, HGFs were exposed to P.g-LPS (0, 5, or 10 μg/mL), P.g-LPS (0, 5, or 10 μg/mL)+NAC, and P.g-LPS (0, 5, or 10 μg/mL)+PssL-NAC, and the ROS levels in the different groups were observed under confocal microscopy to determine the concentration of P.g-LPS for use in subsequent experiments. The groups were as follows: control group (no treatment), P.g-LPS group (HGFs treated with P.g-LPS), NAC group (HGFs treated with P.g-LPS and NAC), and PssL-NAC group (HGFs treated with P.g-LPS and PssL-NAC). Cell counting kit-8 (CCK-8) assays verified the biosafety of PssL-NAC. The ROS levels in the different groups were detected by DCFH-DA probes and observed via confocal microscopy. Real-time qPCR (RT-qPCR) was used to monitor the gene expression levels of the intracellular inflammatory cytokines interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), collagen 1 (COL1) and collagen 3 (COL3). The effect of PssL-NAC on the migration of HGFs was observed via the scratch test. The protein expression of TLR4-NF-κB, and phosphorylated p65 (p-p65) in the TLR4-NF-κB pathway was evaluated by Western blot.
Results:
PssL-NAC had no significant effect on HGF proliferation (P>0.05). At elevated P.g-LPS concentrations, PssL-NAC maintained intracellular ROS levels approximately twice those in the control group (P<0.001). PssL-NAC significantly decreased P.g-LPS-induced IL-6 (P<0.001) and TNF-α (P<0.001) gene expression and increased COL1 gene expression (P<0.001). After P.g-LPS stimulation, PssL-NAC restored cell migration to the control level (P>0.05) and decreased the protein expression of TLR4 (P<0.001), p65 (P = 0.006), and p-p65 (P = 0.017) in the TLR4-NF-κB pathway.
Conclusion
PssL-NAC maintains the appropriate intracellular ROS concentration, alleviates P.g-LPS-induced inflammation in HGFs through the TLR4-NF-κB pathway, and restores the cell functions of collagen production and migration in an inflammatory environment.
2.Research and thinking on ophthalmologist training in African countries assisted by the Belt and Road Initiative

AIM:To investigate the challenges and needs of individual ophthalmologists participated in the training and their countries in the prevention and treatment of blindness and visual impairment, sum up the training effects, and discuss how to promote the development of eye health in the Belt and Road countries in the future.METHODS:A total of 48 ophthalmologists from 6 countries, including Kenya, Zambia, Nigeria, South Africa, Malawi, Botswana between August 1 and August 30, 2022, were trained and anonymous questionnaire survey was conducted. The questionnaire consists of 15 questions covering three aspects: challenges and strategies related to vision impairment and blindness in the country, training satisfaction, and recommendations for establishing optometric centers in Africa.RESULTS: A total of 48 questionnaires were distributed and 47 valid questionnaires were collected. The ophthalmologists hold the view that the biggest challenge of their countries in prevention and treatment of ocular diseases was high nursing costs, accounting for 36.17%, the biggest challenge faced by ophthalmologists was low wages, accounting for 29.79%. Building more eye specialist hospitals(38.30%)and providing more training opportunities(65.96%)can effectively help the countries and the ophthalmologists. The organizational satisfaction with the training courses reached 98%, the content and the lecturers' satisfaction were 100%.CONCLUSION:There are urgent needs to build more ophthalmic hospitals and provide more professional training opportunities to solve the difficulties in the prevention and treatment of eye diseases of the countries and the ophthalmologists. This training program has high satisfaction and good feedback.
3.Clinical analysis of 72 children with histiocytic necrotizing lymphadenitis

Objective To investigate the clinical features,treatment and prognosis of children with histiocytic necrotizing lymphadenitis(HNL).Methods The clinical data of 72 children with HNL diagnosed and treated in the Department of Infectious Diseases,Children's Hospital Affiliated to Shandong University from January 2019 to December 2023 were reviewed.The clinical symptoms,laboratory examination,imaging examination,pathological findings,treatment and follow-up were retrospectively analyzed.Results Among the 72 children,49 were males and 23 were females.The age was 3 to 15 years old,of which 55 cases(76.4%)ranged from 8 to 13 years.The duration of the first onset was 2-60 days,with an average of 15 days.All the children had fever and lymph node enlargement.Only abdominal lymph node lesions were present in 1 case,and cervical lymph nodes were involved in the rest.In the course of the disease,a few of the children had rash,joint swelling and pain and aseptic meningitis.One patient had coronary artery and jugular dilatation.In the acute stage of the disease,most children had decreased white blood cells and hemoglobin,increased erythrocyte sedimentation rate and lactate dehydrogenase.71 cases were treated with glucocorticoid,and 69 cases were treated for 8-12 weeks.The longest follow-up of the newly diagnosed cases has been nearly 5 years,except for one patient who developed cutaneous lupus,no case has recurred or progressed to other immune diseases.Conclusion Children HNL is more common in school-age children.The overall incidence is higher in boys than girls.Most children with HNL present with cervical lymph node enlargement,and very few patients only involve deep lymph nodes.It can also cause vascular lesions.Treatment with glucocorticoids for 2 to 3 months after diagnosis may reduce recurrence and prevent disease progression.
4.Recent advance in amino acid PET combined with MRI in brain gliomas

Gliomas are the most common primary malignant tumor of the central nervous system, with strong invasion, high disability and mortality, and poor prognosis. The biological distribution characteristics of specific radiopharmaceuticals in amino acid positron emission tomography (PET) could reflect glioma molecular metabolism information, which is complementary to the morphological and functional information of magnetic resonance imaging (MRI); PET combined with MRI play pivotal roles in the diagnosis, biopsy guidance, recurrent monitor and prognosis prediction of gliomas. In this article, the application and research progress of amino acid PET combined with MRI in clinical diagnosis and treatment of brain gliomas are reviewed to provide imaging basis for clinical diagnosis and treatment of gliomas.
5.Research progresses of 18F-FET PET in brain gliomas

Amino acid tracer O-(2-[ 18F]fluoroethyl)- L-tyrosine ( 18F-FET) PET plays a pivotal role in the clinical management of gliomas. In this article, the application of 18F-FET PET in tumor diagnosis and differentiation with other diseases, treatment guidance, treatment response assessment and prognosis prediction of gliomas are reviewed.
6.Effects of 5E rehabilitation nursing model in lung rehabilitation of patients with chronic obstructive pulmonary disease

Objective:To explore the effects of 5E rehabilitation nursing model on self-management behavior and fatigue symptoms of patients with chronic obstructive pulmonary disease (COPD) .Methods:Totally 110 COPD patients admitted in Yantai Yuhuangding Hospital from October 2018 to September 2019 were selected by convenient sampling, and divided into the control group ( n=53) and the experimental group ( n=57) according to the random number table. Excluding missing patients and patients with incomplete data, finally 48 patients were included in the control group and 51 patients were included in the experimental group. Patients in the control group received routine care and health guidance, while patients in the experimental group received care based on the 5E rehabilitation nursing model. The Self-Management Scale for COPD Patients and the Functional Assessment of Chronic Illness Therapy-Fatigue Scale (FACIT-F) were used to evaluate the effects of interventions in the two groups. Results:There was no statistically significant difference in the scores of the Self-Management Scale between the two groups before intervention ( P>0.05) ; the scores of all dimensions the Self-Management Scale of the experimental group were higher than those of the control group 12 weeks after discharge, and the differences were statistically significant ( P<0.05) . There was no statistically significant difference in fatigue scores between the two groups before intervention and 12 weeks after discharge ( P>0.05) . Conclusions:The 5E rehabilitation nursing model can improve the self-management behavior of COPD patients, but it cannot ameliorate the fatigue symptoms of patients. Large-sample, multi-center, and long-term research will be further needed in the future to explore the effects of the 5E rehabilitation nursing model on patients' fatigue symptoms.
7. Value of MRI in prediction of glioma isocitrate dehydrogenase 1 mutation status

Objective: To observe the value of MR in prediction of glioma isocitrate dehydrogenase (IDH) 1 mutation status. Methods: Nineteen-two patients with glioma were divided into IDH mutation positive group and negative group, and their imaging characteristics were retrospectively reviewed, including lesions' site, signal intensity, boundary, growth pattern, degree of enhancement and surrounding edema. Then two-class Logistic model was established. Results: There were significant differences between different grades and location of gliomas between the two groups (both P<0.05). There were no significant differences in tumor signal intensity, boundary and growth pattern (P=0.269, 0.606, 0.139). There were statistically significant difference in degree of enhancement and surrounding edema (all P<0.01). Logistic regression analysis showed that the signal uniformity (X1), boundary (X2) and degree of enhancement (X3) of gliomas were statistically significant (P=0.004, 0.037, 0.001), and the regression equation was: logit (P)=2.668+1.415X1-2.097X2-3.229X3 (χ2=41.583, P<0.001), the sensitivity of the model was 70.70%, and the specificity was 80.40%. Conclusion: MRI can be used to non-invasively predict IDH1 mutation status of gliomas before surgical operation.
8. Progresses in imaging study of MGMT promoter methylation in gliomas

O6-methylguanine-DNA methyltransferase (MGMT) promoter methylation is an important molecular biomarker which plays a key role in tumor development and determine the molecular subtypes and prognosis of gliomas. Radiomics and imaging examinations like MR and PET can obtain information of morphology, function, metabolism and molecular alterations of gliomas, which may help comprehensively and non-invasively predict MGMT promoter methylation status in gliomas. The relationships between imaging features and MGMT promoter methylation status of gliomas were reviewed in this article.
9.Correlation of short-term blood pressure variability and outcome after intravenous thrombolysis in patients with acute ischemic stroke: a Meta-analysis

Objective To evaluate the relationship between short-term blood pressure variability and poor outcome and hemorrhagic transformation after intravenous thrombolysis in patients with acute ischemic stroke.Methods The Databases such as Wanfang,CNKI,Cochrane,Pubmed,EMBASE,and Web of Science were retrieved.The randomized controlled trials,cohort studies and case-control studies about blood pressure monitoring after intravenous thrombolytic therapy in patients with acute ischemic stroke and calculation and analysis of blood pressure variability were enrolled.The deadline for retrieval was December 2017.STATA 13.0 software was used to conduct Meta-analysis.Results A total of 9 non-randomized controlled trials with 19 161 patients were included.Four of them were prospective studies and 5 were retrospective studies.The relationship between short-term blood pressure variability and poor outcome (defined as a modified Rankin scale score >2) were investigated in 8 studies (a total of 19 045 patients).The relationship between short-term blood pressure variability and hemorrhagic transformation were investigated in 6 studies (with 18 456 patients).The results of Meta-analysis showed that short-term systolic blood pressure variability (every 10 mmHg change;1 mmHg =0.133 kPa) and poor outcome (odds ratio [OR] 1.55,95% confidence interval [CI] 1.22-1.86;P >0.001),hemorrhagic transformation (OR 2.39,95% CI 1.71-3.35;P =0.025),and symptomatic intracranial hemorrhage (OR 2.49,95% CI 1.39-4.39;P =0.048) had significant correlations.Conclusion The increased short-term blood pressure variability after intravenous thrombolysis in patients with acute ischemic stroke is associated with poor outcome,hemorrhagic transformation,and symptomatic intracranial hemorrhage.
10.Recent advances in diagnosis and treatment of PEHO syndrome

The progressive encephalopathy with edema,hypsarrhythmia,and optic atrophy (PEHO) syndrome is a unique pediatric neurodevelopmental disorder,characterized by a combination of severe mental retardation,early onset epileptic seizures,pedal edema,optic/cerebellar atrophy,and early death.The affected individuals have neither optic atrophy nor the typical neuroradiological findings has been described as PEHO-like syndrome.At present,there are few reports about PEHO syndrome in China.In this study,we summarizes the incidence,etiology,clinical manifestations,and related genes of PEHO syndrome,and aims to provide assistance for future clinical work.
