OBJECTIVE To investigate the mechanism through which Danggui buxue decoction regulates neutrophil extracellular traps (NETs) to improve osteoporosis (OP) in rats with premature ovarian failure (POF). METHODS Female SD rats were randomly divided into normal group, model group, calcitriol group, and Danggui buxue decoction low-dose, medium-dose and high-dose groups, with 9 rats in each group. Except for the normal group, all other groups were administered cisplatin via intraperitoneal injection on days 1 and 8 to establish a POF complicated with OP model. Each group received the corresponding drugs or normal saline intragastrically starting from day 5, once a day, for 4 consecutive weeks. After the last medication, serum levels of estradiol (E2), NETs, 25-hydroxyvitamin D3 [25(OH)D3], receptor activator of nuclear factor-κB ligand (RANKL), and osteocalcin (BGP) were measured. The histopathological changes in bone tissue were observed. The expressions of vitamin D receptor (VDR), myeloperoxidase (MPO), neutrophil elastase (NE) and citrullinated histone H3 (CitH3) in bone tissue were detected; the protein expressions of 25-hydroxyvitamin D-1α-hydroxylase (CYP27B1) and 1α,25-dihydroxyvitamin D3-24-hydroxylase (CYP24A1) were also determined. RESULTS Compared with the normal group, the bone tissue of rats in the model group showed a significant reduction in the number of trabeculae, which was thinner broken and poorly connected, with significant destruction of the reticular structure, and an enlarged marrow cavity. Serum levels of NETs and RANKL, the protein expressions of MPO, NE, CitH3 and CYP24A1 in bone tissue were significantly increased or upregulated, while serum levels of E2, 25(OH)D3 and BGP as well as protein expressions of VDR and CYP27B1 were significantly decreased or downregulated (P<0.05). Compared with the model group, the histopathological changes in the bone tissue of rats in each administration group showed some degree of recovery, with significant improvements observed in most quantitative indicators (P<0.05). CONCLUSIONS Danggui buxue decoction can restore the E2 level in POF complicated with OP rats, and improve OP. The mechanism may be related to its ability to upregulate VD level and inhibit the formation of NETs.

The uniqueness of the location of large general hospitals in urban areas determines that the contradiction between space constraints and medical expansion has become a problem of management bottleneck. The situation that expert visits are too concentrated on a certain working day morning has been significantly improved by carrying out peak load shifting in the visiting arrangements and visiting units of large general hospitals through the Total Quality Management theory and continuous improvement of work mode, greatly alleviating public transportation congestion around the hospital.Through the internal construction of the quality of outpatient clinics,it can effectively improve the patients'sense of access to health care.

Objective To investigate the risk factors of hip osteoarthritis(HOA)after hip arthroscopy in patients with femoro-acetabular impingement(FAI)syndrome, and to reduce and prevent HOA.Methods From September 2018 to Septem-ber 2020,106 patients with FAI underwent hip arthroscopy, including 40 males and 66 females, aged from 20 to 55 years old with an average age of(33.05±10.19)years old.The mechanism of injury included 51 cases for sports injury,36 for traffic ac-cidents,and 19 for blunt object injury. The duration of the disease ranged from 5 to 19 days with an average of(12.02±3.69)days.All patients were followed up for 18 months.Patients were divided into HOA group(23 cases)and non-HOA group(83 cases)according to the occurrence of HOA.Multivariate Logistic regression was used to analyze the risk factors of HOA after hip arthroscopy in FAI patients. Results By univariate analysis, aged from 50 to 70 years old, female, body mass index(BMI)>30 kg·m-2,physical labor, cam type, postoperative infection, last follow-up hip degree of motion(range of motion,ROM)(flex-ion,abduction,adduction,internal rotation)and T?nnis grade 1 and above of the HOA group were higher than those of the non-HOA group(P<0.05),and the relative appendicular skeletal muscle index(RASM)was lower than that of non-HOA group(P<0.05).By multiple Logistic regression analysis, cam type,BMI>30 kg·m-2,last follow-up hip internal rotation ROM and T?nnis grade 1 were risk factors for HOA after hip arthroscopy in FAI patients(P<0.05).Conclusion FAI classification, body mass index, hip ROM and T?nnis grade are all related to HOA after hip arthroscopy in FAI patients.Follow-up and intervention should be strengthened in high-risk FAI patients to reduce the occurrence of HOA.

Objective:This study aims to analyze the spatial-temporal patterns, evolutionary trends, and regional balance of human resources in village clinics in China, providing a practical basis and decision-making reference for the construction of rural healthcare systems. Methods:By employing a variety of spatial-temporal statistical analysis methods, this study examines the spatial-temporal development and dynamic distribution trends of human resource allocation in village clinics. It explores the characteristics of resource allocation balance, and deconstructs the sources of regional, interregional, and intraregional disparities in human resource structures. Results:The total scale of human resources in village clinics decreased, but the number of personnel per thousand population and the proportion of health technicians increased. The human resources of village clinics in different provinces have gradually evolved to Twin Peaks, showing a convergence trend of spatial clubs. There is a significant difference in the allocation of human resources in village clinics, and the internal difference in the eastern region contributes the most, both within and between regions, the difference of health technical personnel has gradually become the main reason for the unbalanced allocation of village clinic space. Conclusion:The allocation of human resources in village clinics in China is facing the challenge of decreasing the total amount. The sustainability of the optimized allocation of personnel structure needs to be further enhanced, and the balance of the allocation of health technical personnel space has also been slightly improved,it is necessary to re-examine the total amount of human resources management in village clinics, promote the optimization of personnel structure and the complementarity of regional advantages, and reduce regional differences in the optimization of personnel structure.
