1.Pharmacokinetic study of 3 blood-absorbed components of Xiangshao sanjie oral liquid in rats with hyperplasia of mammary gland

OBJECTIVE To explore the pharmacokinetic characteristics of 3 blood-absorbed components of Xiangshao sanjie oral liquid in rats with hyperplasia of mammary gland (HMG). METHODS Female SD rats were divided into control group and HMG group according to body weight, with 6 rats in each group. The HMG group was given estrogen+progesterone to construct HMG model. After modeling, two groups were given 1.485 g/kg of Xiangshao sanjie oral liquid (calculated by crude drug) intragastrically, once a day, for 7 consecutive days. Blood samples were collected before the first administration (0 h), and at 5, 15, 30 minutes and 1, 2, 4, 8, 12, 24 hours after the last administration, respectively. Using chlorzoxazone as the internal standard, the plasma concentrations of ferulic acid, paeoniflorin and rosmarinic acid in rats were detected by UPLC-Q/TOF-MS. The pharmacokinetic parameters [area under the drug time curve (AUC0-24 h, AUC0-∞), mean residence time (MRT0-∞), half-life (t1/2), peak time (tmax), peak concentration (cmax)] were calculated by the non-atrioventricular model using Phoenix WinNonlin 8.1 software. RESULTS Compared with the control group, the AUC0-24 h, AUC0-∞ and cmax of ferulic acid in the HMG group were significantly increased (P<0.05); the AUC0-24 h, AUC0-∞ , MRT0-∞ , t1/2 and cmax of paeoniflorin increased, but there was no significant difference between 2 groups (P>0.05); the AUC0-24 h and MRT0-∞ of rosmarinic acid were significantly increased or prolonged (P<0.05). C ONCLUSIONS In HMG model rats, the exposure of ferulic acid, paeoniflorin and rosmarinic acid in Xiangshao sanjie oral liquid all increase, and the retention time of rosmarinic acid is significantly prolonged.

As color doppler ultrasound listed as the thyroid to regular physical examination project, the positive detection rate of benign and malignant thyroid diseases has increased year by year. The population of patients after thyroid surgery is getting larger and larger, however, the research on how to replace the absent thyroid with exogenous thyroid hormone is not perfect. As the switch of thyroid hormone activation and deactivation, deiodinase plays an important role in maintaining the internal environment of human body. The expression of these enzymes will change with the different needs of each organ in the individual. In the past, many authors have made a detailed description of deiodinase in basic medicine, but the principle of action in clinical aspects is relatively lacking. In this paper, the mechanism of deiodinase was reviewed through literature reading, to explore the basic principle of deiodinase in exogenous thyroid hormone supplementation after thyroid surgery.
3.Discussion of factors influencing preclinical studies based on fecal bacteria transplantation in mice

Fecal microbiota transplantation(FMT)is a therapeutic approach that targets intestinal microorganisms by transplanting fecal microorganisms from healthy individuals into the gastrointestinal tract of diseased individuals,thus restoring the recipient's disordered gastrointestinal microbiota by restructuring the intestinal flora.However,the mechanism of action and adverse effects of FMT in different diseases have not yet been clarified,thus limiting its wide clinical application.Its use still relies on in-depth preclinical studies;however,highly inconsistent or incomplete experimental details provided in current reports,coupled with a lack of authoritative standards and recommendations,seriously affect the interpretation of the study findings and replication of the experimental procedures,as well as hindering the clinical translation of the result.We therefore review and discuss the key steps of recipient selection and graft sample collection,storage,graft material preparation,and grafting route,with the aim of improving the utilization of experimental animals,consumables,and labor,and providing method ological recommendations and references to achieve replicability and standardization of preclinical FMT studies.
4.Comparison of dermoscopic features of toenail psoriasis and fingernail psoriasis

Objective:To evaluate and compare dermoscopic features of toenail psoriasis and fingernail psoriasis.Methods:Between June 2020 and January 2022, 61 patients with confirmed toenail psoriasis and 80 with confirmed fingernail psoriasis were enrolled from the Department of Dermatology of Peking University First Hospital. Dermoscopy was performed on 139 affected toenails and 158 affected fingernails among the psoriasis patients, and dermoscopic characteristics were analyzed between the two groups by using the chi-square test.Results:The most common dermoscopic feature of nail psoriasis was pitting (223/297, 75.08%), followed by splinter haemorrhages (164/297, 55.22%), subungual hyperkeratosis (133/297, 44.78%), oil drop sign (126/297, 42.42%), complete onycholysis (121/297, 40.74%), linear margin of the proximal onycholysis (107/297, 36.03%) and linear erythema at the margin of the onycholysis (77/297, 25.93%). Compared with the patients with fingernail psoriasis, those with toenail psoriasis more commonly presented with subungual hyperkeratosis (81[58.27%] vs. 52[32.91%], P < 0.001), punctate/blocky haemorrhages (22[15.83%] vs. 11[6.96%], P < 0.05), longitudinal striae (34[24.46%] vs. 10[6.33%], P < 0.001), longitudinal nail splitting (24[17.27%] vs. 9[5.70%], P < 0.01), brown discoloration (14[10.07%] vs. 2[1.27%], P < 0.01), transverse grooves (17[12.23%] vs. 1[0.63%], P < 0.001) and leukonychia (10[7.19%] vs. 1[0.63%], P < 0.01) ; compared with the patients with toenail psoriasis, those with fingernail psoriasis more commonly presented with splinter haemorrhages (100[63.29%] vs. 64[46.04%], P < 0.01), oil drop sign (81[51.27%] vs. 45[32.37%], P < 0.01), linear erythema at the margin of the onycholysis (55[34.81%] vs. 22[15.83%], P < 0.001), partial onycholysis (50[31.65%] vs. 19[13.67%], P < 0.001) and red spots in the lunula (36[22.78%] vs. 12[8.63%], P < 0.01) . Conclusion:The dermoscopic features of toenail psoriasis were quite different from those of fingernail psoriasis, and features such as subungual hyperkeratosis, longitudinal streaks, and brown discoloration were more commonly presented in patients with toenail psoriasis.
5.Low-dose ATG combined with low-dose PTCY in preventing GVHD after haploidentical transplantation:a retrospective analysis of 90 cases

Objective To retrospectively analyze the efficacy and safety of low-dose antithymocyte globulin(ATG)combined with low-dose post transplantation cyclophosphamide(PTCY)in prevention of graft versus host disease(GVHD)after haploidentical transplantation.Methods Clinical data of 90 patients receiving haplotype matched transplantation in No.920 Hospital of PLA Joint Logistic Support Force from January 2022 to February 2023 were collected,and they were divided into study group(n=47)and control group(n=43)according to different GVHD prevention programs.The patients of the study group were given low-dose ATG combined with low-dose PTCY,and those of the control group received standard dose of PTCY.The implantation status,occurrence of GVHD,survival status and other indicators were analyzed between the 2 groups.Results ① Both groups of patients were successfully implanted,the median duration for neutrophil implantation(11 vs 17 d,P<0.05)and platelet implantation(12 vs 20 d,P<0.05)was significantly shorter in the study group than the control group.The incidence of grade Ⅱ~Ⅳ aGVHD(12.8%vs 34.9%,P<0.05)and grade Ⅲ~Ⅳ aGVHD(6.4% vs 20.9%,P<0.05)was significantly lower in the study group than the control group,so was the non-recurrent mortality rate(6.4%vs 20.9%,P<0.05)and the incidence of hemorrhagic cystitis(12.8% vs 34.9%,P<0.05).② By the end of the study,there were no significant differences in the incidence of mild and moderate and severe cGVHD,recurrence rate,reactivation rates of EBV and CMV,overall survival rate or progression-free survival rate between the 2 groups.Conclusion For haploidentical transplantation,low-dose ATG combined with low-dose PTCY has the advantages of lower incidence of GVHD,non-recurrent mortality,incidence of hemorrhagic cystitis and faster implantation.
6.Control effect of Dasatinib on the treatment of acute myeloid leukemia and adverse events of CD123 targeting CAR-T:a case report and literature review

Objective To preliminarily explore the efficacy of chimeric antigen receptor T cells(CAR-T)targeting CD 123 in the treatment of acute myeloid leukemia(AML)and the role of dasatinib in the treatment of CD123 targeting CAR-T induced side effects.Methods Clinical data of 1 patient with relapsed AML admitted to No.920 Hospital of PLA Joint Logistic Support Force in September,2019 were collected.The patient relapsed after previous multi-line chemotherapy and was treated with CD123 targeting CAR-T therapy.The routine blood changes of the patient after treatment were observed.Dasatinib was used when agranulocytosis occurred,40 mg orally 3 times per day,and was stopped when agranulocytosis was relieved.Changes in blood cells,CAR-T amplification,and disease control were observed.The patient was followed up for over 1 year.Results Flow cytometry for bone marrow showed that minimal residual disease negative result was observed in 30 d after infusion.The patient remained disease-free for over 1 year.After CD 123 CAR-T cells infusion,significant expansion of CAR-T cells was observed,accompanied by granulocyte deficiency and cytokine release syndrome(CRS).After using dasatinib,inhibition of CAR-T cell expansion was observed,accompanied by blood cell recovery,and CRS symptoms were alleviated.After stop of dasatinib,CAR-T cells expanded again and blood cells decreased again.Conclusion CAR-T cells targeting CD 123 have certain efficacy in the treatment for relapsed AML.Dashatinib has a blocking effect on the amplification and function of CAR-T,which can alleviate bone marrow suppression caused by CD 123 targeting CAR-T and avoid severe CRS.
7.Tubeless three-port versus conventional single utility port thoracoscopic surgery in the treatment of thymic tumors: A retrospective cohort study

Objective To investigate the safety and efficacy of totally no tube three-port thoracoscopic surgery (TNTT) for thymic tumor via lateral thoracic approach. Methods The clinical data of patients with thymoma admitted to the Department of Thoracic Surgery of the General Hospital of Northern Theater Command from November 2021 to May 2022 were retrospectively analyzed. The patients were divided into a TNTT group and a single utility port video-assisted thoracic surgery (SVATS) group according to different surgical methods. The clinical data were compared between the two groups. Results A total of 111 patients were collected. There were 44 patients in the TNTT group, including 20 males and 24 females, with an average age of 60.11±8.64 years, and 67 patients in the SVATS group, including 30 males and 37 females, with an average age of 62.40±7.92 years. There was no significant difference between the two groups in the baseline data (P>0.05). The postoperative hospital stay and intraoperative blood loss were shorter or less in the TNTT group (P<0.05), and the visual analogue scale score 48 hours after the operation was smaller in the SVATS group (P<0.05). Conclusion TNTT has a good surgical safety, and can shorten postoperative hospital stay, reduce intraoperative blood loss, and has significant advantages in enhanced recovery after surgery, but SVATS can reduce postoperative pain in patients.
8.Hemodynamic study of personalized Stanford type B aortic dissection based on computational fluid dynamics

Objective To establish a personalized Stanford type B aortic dissection numerical simulation model, and using computational fluid dynamics (CFD) numerical simulation to obtain the hemodynamic behavior and law of the type B aortic dissection at different stages of development. Methods Based on the theory of three-dimensional model reconstruction, we used CT images of a patient with type B aortic dissection in the Xiamen Cardiovascular Hospital of Xiamen University, relevant medical image processing software to reconstruct a personalized aortic three-dimensional model, and CFD to reconstruct the model which was simulated in fluid mechanics. Results The three-dimensional reconstruction model could intuitively observe the changing trend of the false cavity at different stages of the dissection development. Through fluid mechanics simulation, the blood flow rate, pressure, wall shear stress, vascular wall Von Mises stress and other parameters at different stages of the dissection development were obtained. Conclusion The hemodynamic behavior and law of relevant parameters in the development stage of aortic dissection are analyzed. The combination of the values of relevant parameters and clinical medical detection and diagnosis can well predict the development of the disease, and finally provide more theories and methods for the scientific diagnosis of aortic dissection.
9.Epidermoid cyst in the right mandible:a case report and literature review

Objective To discuss the possible etiology,pathogenesis,clinical features,diagnosis and treatment of epidermoid cysts of the jaw and to provide a reference for clinical diagnosis and treatment.Methods A case of an epi-dermoid cyst in the right mandible with retained deciduous teeth and succedaneous impacted teeth was reviewed and an-alyzed in combination with the relevant literature.Results A patient presented with a mass in the right mandible that had persisted for 1 month after being found at imaging examination.Tooth 83 was retained,and tooth 43 was unerupted.Swelling was characterized by no obvious tenderness,fluctuation,or table tennis sensation and was observed in the lin-gual alveoli of teeth 83,44,and 45.Imaging revealed a low-density shadow in the apex of teeth 83,44,45,and 46,approximately 1.9 cm × 2.6 cm × 1.6 cm in size,which wrapped around the dental crown of tooth 43.Preliminary diag-noses were as follows:right mandibular mass thought to be a dentigerous cyst;impacted tooth 43;and retained primary tooth 83.The mass in the right mandible was removed,and teeth 43 and 83 were extracted under intravenous and inha-lation anesthesia.During the operation,the mass was observed to have a thin cyst wall and contained bean-like residue.Histopathological examination indicated an epidermoid cyst in the right mandible.At the 1-week follow-up examination,the patient reported no discomfort,and the surgical area showed good recovery.According to the literature,epidermoid cysts are benign cysts originating from ectopic ectodermal tissue that can occur throughout the body but rarely in the oral cavity and are even extremely rarer in the jaw.Epidermoid cysts of the jaw,which have no specific clinical manifes-tations,can be confused with odontogenic cysts such as dentigerous cysts and odontogenic tumors.Dental pulp tests and other techniques can serve as a reference for clinicians.The diagnosis is confirmed via histopathology.Surgical removal is a common treatment,with a good prognosis and a low recurrence rate.Conclusion The principle of treatment for an epidermoid cyst of the jaw is similar to that for a jaw cyst.The prognosis is good when the cyst is removed completely.
10.Analysis of long-term prognosis and risk factors in patients with dilated cardiomyopathy

Objective:To investigate the risk factors and long-term prognosis of major adverse cardiovascular events(MACEs) in patients with dilated cardiomyopathy (DCM).Methods:This study was a single-center retrospective cohort study. Clinical information from 300 patients with DCM hospitalized in Peking Union Medical College Hospital from April 2013 to April 2023 was collected. Based on echocardiography results, the patients were divided into two groups: isolated DCM and DCM with left ventricular non-compaction cardiomyopathy (LVNC). The MACEs, including major heart failure events, severe ventricular arrhythmias, and cardiovascular death, were recorded by outpatient or telephone follow-up. Univariate and multivariate Cox proportional hazard regression models were used to analyze the risk factors affecting the prognosis of patients with DCM. Kaplan-Meier curve and log-rank were used for survival analysis to compare the difference in the incidence of cardiovascular events between the two groups.Results:The included 300 DCM patients were (47.8±16.8) years old, with 197 males (65.7%), of which 237 (79.0%) were isolated DCM and 63 (21.0%) were DCM with LVNC. The follow-up time was 4.0 (1.9, 6.2) years. A total of 142 (47.3%) MACEs occurred, including 117 (39.0%) major heart failure events, 20 (6.7%) severe ventricular arrhythmia events, and 53 (17.7%) cardiovascular death events. Multivariate Cox proportional hazard regression analysis showed that increased left ventricular end-diastolic diameter ( HR=1.21, 95% CI: 1.01-1.44, P=0.042), moderate or severe mitral regurgitation ( HR=1.71, 95% CI: 1.19-2.47, P=0.004), increased ln (N-terminal pro-B-type natriuretic peptide) ( HR=1.30, 95% CI: 1.10-1.54, P=0.002) were independent risk factors for dverse cardiovascular events in DCM patients, and angiotensin-converting enzyme inhibitor (ACEI)/angiotensin receptor blocker (ARB)/angiotensin receptor neprilysin inhibitor (ARNI) treatment ( HR=0.45, 95% CI: 0.26-0.78, P=0.004) was independent protective factor. Kaplan-Meier survival analysis found no significant difference in the risk of MACEs between isolated DCM and DCM with LVNC ( P=0.22). Similarly, there were no significant differences in the incidence of major heart failure, severe ventricular arrhythmia, and cardiovascular death between the two groups (all P>0.05). Conclusion:An increase in left ventricular end-diastolic diameter, moderate or severe mitral regurgitation, elevated N-terminal pro-B-type natriuretic peptide, and non use of ACEI/ARB/ARNI are independent predictors of cardiovascular events in DCM patients. There was no significant risk of MACEs in patients with isolated DCM and DCM with LVNC, and suggested that LVNC may be a unique phenotype and should be accurately managed in combination with genetic background.
