1.Quality evaluation of Mongolian medicine Sendeng-4 based on qualitative and quantitative analysis combined with chemical pattern recognition

		                        			OBJECTIVE To evaluate the quality of Mongolian medicine Sendeng-4 based on qualitative and quantitative analysis combined with chemical pattern recognition, in order to provide the reference for its quality control. METHODS The chemical components in Sendeng-4 were analyzed qualitatively by HPLC-Q-Exactive-MS. The contents of 16 components (methyl gallate, ethyl gallate, epicatechin, dihydromyricetin, genipin-1-O-β-D-gentiobioside, caffeic acid, catechin, corilagin, deacetylasperulosidic acid methyl ester, rutin, geniposide, luteolin, myricetin, quercetin, ferulic acid, and toosendanin) in 15 batches of Sendeng-4 (sample S1-S15) were quantitatively analyzed by HPLC-MS/MS. Cluster analysis (CA), principal component analysis (PCA), and orthogonal partial least squares discriminant analysis were conducted and variable importance projection (VIP) value greater than 1 was used as the index to screen the differential components. RESULTS A total of 73 chemical components were identified in Sendeng-4, including 20 flavonoids, 16 tannins, 14 organic acids, etc. According to the quantitative analysis, the results exhibited that the average contentsthe of above 16 components in 15 batches of Sendeng-4 were 3.683-7.730, 2.391-6.952, 2 275.538-4 377.491, 2 699.188-3 537.924, 858.266-1 377.393, 3.366-11.003, 140.624-315.683,414.629-978.334, 285.501-1 510.457, 27.799-48.325, 3 625.415-6 309.563, 0.506-0.656, 442.337-649.283, 47.093-59.736, 12.942-15.822, 127.738-326.649 μg/g, respectively. According to the results of CA and PCA, 15 batches of samples could be clustered into two categories: S1-S3, S5-S6, S9-S10 and S13 were clustered into one category; S4, S7-S8, S11-S12, S14-S15 were clustered into one category. VIP values of geniposide, epicatechin, deacetylasperulosidic acid methyl ester and genipin-1-O- β-D-gentiobioside were all greater than 1. CONCLUSIONS HPLC-Q-Exactive-MS and HPLC-MS/MS techniques are employed for the qualitative and quantitative analysis of Sendeng-4. Through chemical pattern recognition analysis, four differential components are identified: geniposide, epicatechin, deacetylasperulosidic acid methyl ester, and genipin-1-O-β-D-gentiobioside.
		                        		
		                        		
		                        		
		                        	
2.Discussion on hot topics and promotion paths for post-market surveillance and supervise of medical consumables based on non-active medical devices

		                        			Objective:To explore the research hotspots and effective promotion paths of post market surveillance and supervise of medical consumables with non-active medical devices.Methods:Data mining methods were used to collect related journal literatures and documents from the websites of China regulatory institutions and the China National Knowledge Infrastructure(CNKI),order sub item data of medical device adverse event reports,extract the MeSH element words of literatures and documents,perform bibliometric analysis and visual display.Results:The number of medical devices adverse event reports in China has been increasing year by year,reaching 694 866 in 2022,in the four statistical years from 2019 to 2022,the number of reports on non-active medical devices and IVD reagents also showed a parallel increasing trend,accounting for about 65.00% of the total number of adverse event reports on medical devices in the year.The bibliometric analysis of journal literature shows that research in this field has received varying degrees of participation from regulatory institutions,universities,medical institutions,and enterprises.Regulatory institutions have contributed 46 articles,accounting for 56.79% of the total number of articles,followed by 28 articles from universities.The co-occurrence analysis shows that hot topic is focused in 5 clusters:quality management,risk management,international experiences discussion and adverse event surveillance and re-evaluation and real-world research.China regulatory institutions attach great importance to post market surveillance and supervise,and have issued more than 20 relevant documents since 2006,focusing on specific topics and gradually deepening around safety and effectiveness.Conclusion:The post market surveillance and supervise of medical devices,especially medical consumables based on non-active medical devices,need to be promoted synchronously in three dimensions:regulatory institutions,medical institutions,and enterprises.Universities,research institutes,and industry organizations should work in coordinating to strengthen the collection,identification,and active surveillance of risk signals based on adverse event surveillance,safety evaluation based on risk management,and conducting real-world research,research and develop risk control and corrective and preventive measures.
		                        		
		                        		
		                        		
		                        	
3.Construction and validation of a predictive model for septic shock based on propensity score matching

		                        			Objective To construct a predictive model for septic shock based on the propensity score matching (PSM) method and validate its effectiveness. Methods A total of 114 patients with sepsis were enrolled as study objects, and were divided into septic shock group (40 patients) and sepsis group (74 patients) according to whether they developed septic shock. PSM was performed with a ratio of septic shock to sepsis of 1∶2, resulting in the inclusion of 30 patients in the septic shock group and 60 patients in the sepsis group after matching. The levels of C-reactive protein (CRP), procalcitonin (PCT), interleukin-6 (IL-6), serum amyloid A (SAA), soluble endothelial protein C receptor (sEPCR), endothelial cell-specific molecule 1 (ESM-1), clusterin (CLU), and the Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ) score and Sequential Organ Failure Assessment (SOFA) score at admission were compared between the two groups. Cox proportional hazards regression analysis was used to identify the factors influencing septic shock, and a predictive model for septic shock was constructed and internally validated using the receiver operating characteristic (ROC) curve. Kaplan-Meier survival curves were plotted to analyze the differences in survival prognosis among patients with different expression levels of the indicators. 5.The influence path of stigma on the time of healthcare-seeking decision in caregivers of elderly patients with dementia and humanistic care strategies

		                        			Objective:To clarify the influence and influence paths of stigma on the time of the healthcare-seeking decision in caregivers of elderly patients with dementia, and to provide a theoretical basis for the construction of corresponding humanistic care strategies.Methods:A total of 176 caregivers of elderly patients with dementia who visited the Affiliated Hospital of Xuzhou Medical University and Xuzhou Oriental People ′s Hospital from February 2021 to February 2022 were selected as the study subjects. The General Information Questionnaire, self-designed Scale of Stigma for Caregivers of Senile dementia patients, Multidimensional Scale of Perceived Social Support, self-designed Elderly Dementia Caregivers′ Perceived Barriers Scale for Healthcare-seeking Decision, and self-designed Scale of the Intention to Seek Healthcare for caregivers of senile dementia patients were used in the survey. AMOS 20.0 was used to establish a structural equation model for path analysis. Results:The higher the stigma of caregivers, the longer the time of the healthcare-seeking decision ( β=0.05, P<0.05). Social support, perceived barriers to the healthcare-seeking decision, and the intention to seek healthcare were the mediating variables of caregivers ′ stigma affecting the time of the healthcare-seeking decision, with a total effect of -0.04, 0.14, and 0.36, respectively, and all P<0.05. Conclusions:The stigma in caregivers of senile dementia patients is an important factor affecting the time of the healthcare-seeking decision. By improving mediating factors including social support, perceived barriers to the healthcare-seeking decision, and the intention to seek healthcare, the implementations of targeted humanistic care strategies are expected to help shorten the time of the healthcare-seeking decision.
		                        		
		                        		
		                        		
		                        	
6.Study on improvement effects of different polar parts from total flavonoids of Scutellaria amoena on non-alcoholic fatty liver model rats

		                        			OBJECTIVE To study the improvement effects of different polar parts fro m total f lavonoids of Scutellaria amoena on non-alcoholic fatty liver disease (NAFLD)model rats. METHODS The total flavonoids of S. amoena (SAF)were extracted by reflux extraction with ethanol ,suspended with water ,and then extracted with ethyl acetate and n-butanol in order to obtain the extraction parts of SAF (recorded as SAFA and SAFB respectively ). Thirty-six rats were randomly divided into normal group (n= 6)and modeling group (n=30). Modeling group was given high-lipid diet to induce NAFLD model. After modeling ,modeling group was randomly divided into model group (normal saline ),fenofibrate group (positive control ,20 mg/kg),SAF group (300 mg/kg),SAFA group (300 mg/kg)and SAFB group (300 mg/kg);they were given relevant intragastical administration ,once a day,for consecutive 6 weeks. After last administration ,the liver index was calculated ;the levels of total cholesterol (TC), triacylglycerol(TG),aspartate transaminase (AST),alanine transaminase (ALT),high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C) in serum ,the levels of superoxide dismutase (SOD),glutathione peroxidase (GSH-Px),malondialdehyde(MDA),interleukin-1β(IL-1β),IL-6 and tumor necrosis factor-α(TNF-α)in liver tissue were detected;the pathomorphological changes of liver tissue were observed. RESULTS Compared with normal group ,the liver index , the levels of TC ,TG,AST,ALT,LDL-C,MDA,IL-1β, IL-6 and TNF-α in serum/liver tissue of model group were all increased significantly (P<0.05), while the levels of HDL-C,SOD and GSH-Px were all decreased significantly (P<0.05). Compared with model group ,except there was no statistical significance in the serum levels of HDL-C and ALT in SAFA group (P>0.05),above indexes in serum/liver tissue of rats in groups of polar parts from total flavonoids of S. amoena were significantly improved (P<0.05);inflammatory cell infiltration and fatty vacuoles in liver tissue were significantly improved. Compared with SAF group and SAFA group ,the levels of TC,TG,AST,MDA,IL-6 and TNF-α were decreased significantly in SAFB group(P<0.05),while the level of SOD was increased significantly (P<0.05);pathomorphological changes of liver tissue were improved more significantly. CONCLUSIONS Each polar part from total flavonoids of S. amoena can improve NAFLD by regulating oxidative stress and inhibiting the secretion of inflammatory factors. 7.Result analysis of serology test and NAT quantitative detection of HBV DNA reactive blood donors

		                        			【Objective】 To investigate the related viral markers of HBV DNA reactive samples among voluntary blood donors in Shiyan area, so as to provide some reference for donor re-entry. 【Methods】 From August 2019 to June 2021, 78 samples with HBV DNA reactivity from voluntary blood donors in our blood station were collected, and they were further detected for HBV viral markers(using chemiluminescence assays), nucleic acid testing(NAT) of virus quantification and sequencing analysis. 【Results】 Forty-seven (60.3%)out of 78 HBV DNA reactive samples were from repeated blood donors, and 56.4%(44/78)of them were over 45 years old. Among the five viral markers of hepatitis B, 37.2%(29/78) were positive for anti-HBs alone, 19.2%(15/78) were positive for anti-HBe+ anti-HBc and anti-HBs+ anti-HBe+ anti-HBc, and 11.5% (9/78) were all items negative.A total of 62.8%(49/78) of samples were detected by NAT quantification and seven samples had been successfully sequenced for HBV. 【Conclusion】 NAT can effectively minimize the missed detection of HBsAg methodology and reduce the risk of HBV transmission through blood. It is widely used in the treatment of chemotherapy-related gastrointestinal reaction, cancer pain, tumor related intestinal obstruction, cancer-related fatigue, etc. It is oftenuse in conventional treatment and nursingprocess, or usedalone, or combined with oraladministration of Chinese and Western Medicine and acupoint massage. This therapy could avoid the gastrointestinal irritation caused by oral administration. 9.Reliability and validity of SF-36 scale in pregnant women during the first trimester of pregnancy

		                        			Objective:To evaluate the reliability and validity of Short-form health survey-36 (SF-36) during the first trimester of pregnancy.Methods:From January 2020 to January 2021, pregnant women aged 18―40 during the first trimester visit were admitted to the Obstetric Department of Beijing Obstetrics and Gynecology Hospital, Capital Medical University. Split-half reliability and Cronbach′s α coefficients were used to evaluate the reliability. The convergent and discriminative validity were evaluated by using AMOS 24.0 and the criterion-rated validity was evaluated with correlation analysis and non-parameter test. Exploratory factor analysis and confirmatory factor analysis based on structural equation modeling were used in the evaluation of contract validity.Results:SF-36 scale had good reliability (split-half reliability: R=0.901, Cronbach′s α coefficients=0.878), convergent validity, discriminate validity and the criterion-rated validity ( r=0.907). Second-order confirmatory factor analysis model was not well-fitted ( RMSEA=0.070, χ 2/dF=3.566, GFI=0.813, CFI=0.814, TLI=0.792, NFI=0.761), indicating that the construct validity was poor. Conclusions:The reliability, consolidation validity, discrimination validity and criterion-related validity of Sf-36 scale are good, while the construct validity is poor. 10.Relationship between enhancer of zeste homolog 2 mRNA level and prognosis of gastric cancer patients

		                        			Objective:To analyze the correlation between enhancer of zeste homolog 2 (EZH2) mRNA expression level and prognosis of patients with gastric cancer.Methods:The differences of the EZH2 mRNA levels between gastric cancer tissues and adjacent normal tissues and among patients with different clinical stages were analyzed by The Cancer Genome Atlas (TCGA) database. According to the median expression level of EZH2 mRNA (5.59), 368 patients were divided into high expression group (184 cases) and low expression group (184 cases). Kaplan-Meier method was used to analyze the relationship between EZH2 mRNA expression level and overall survival (OS) of patients. Multivariate Cox regression model was used to analyze the relationship between age, clinical stage, TNM stage, gender, EZH2 mRNA expression level and OS. The relationships between EZH2 mRNA expression level and OS in all gastric cancer patients, human epidermal growth factor receptor 2 (HER2)-positive and HER2-negative patients were analyzed by KM-plotter online database. The KEGG pathway enrichment analysis of tumor samples from TCGA-STAD collection in the TCGA database was carried out by using GSEA software, and enrichment score (ES), normalized enrichment score (NES), nominal P value (NOM p-val) and multiple testing correction (FDR q-val) were calculated. KEGG pathways related to tumorigenesis were screened according to FDR q-val < 0.25. Results:Among 375 cases of gastric cancer tissues in the TCGA database, the level of EZH2 mRNA in 171 cases were low and the level of EZH2 mRNA in 204 cases were high; the level of EZH2 mRNA in 32 cases of adjacent normal tissues were low. The median EZH2 mRNA expression level in adjacent normal tissues was 1.73, and the median EZH2 mRNA expression level in tumor tissues was 5.59. The expression level of EZH2 mRNA in gastric cancer tissues was higher than that in normal adjacent tissues, and the difference was statistically significant ( u = 925, P < 0.05). There was no significant difference in EZH2 mRNA expression level between patients with different clinical stages ( DF = 3, P = 0.25). Kaplan-Meier analysis showed that patients with high expression of EZH2 mRNA had longer OS time than those with low expression of EZH2 mRNA ( HR = 0.623, P = 0.008). Univariate Cox analysis showed that EZH2 mRNA level was one of the independent risk factors for OS in patients ( HR = 0.666, P = 0.024). In the KM-plotter database, patients with high expression of EZH2 mRNA (632 cases) had longer OS time than patients with low expression of EZH2 mRNA (244 cases) ( HR = 0.75, P = 0.002). In patients with positive HER2 (344 cases), patients with high expression of EZH2 mRNA had a shorter OS time ( HR = 1.31, P = 0.042), while in patients with negative HER2 (532 cases), patients with low expression of EZH2 mRNA had a shorter OS time ( HR = 0.57, P < 0.05). GSEA enrichment found that EZH2 mRNA was related to a variety of tumor-related pathways and cell synthesis and metabolism pathways. Conclusion:The expression of EZH2 mRNA is related to the prognosis of patients with gastric cancer, and it may be used as a biomarker for evaluating the prognosis of patients with gastric cancer.
