1.Analysis of the Spatio-temporal Characteristics and Influencing Factors of Local Hospitalization Propor-tion of Guangdong Province

Objective To analyze the spatio-temporal characteristics and influencing factors of local hospitalization proportion in 57 counties of Guangdong Province.Methods Using local hospitalization proportion and related indicators in 57 counties of Guangdong Province from 2016 to 2019,temporal trend analysis and spatial autocorrelation analy-sis were used to analyze the spatiotemporal characteristics.Bayesian spatio-temporal model was used to explore in-fluencing factors of local hospitalization proportion.Results The local hospitalization proportion in Guangdong Province rose from 79.8%in 2016 to 84.1%in 2019,showing an increasing trend and the overall characteristics of"high in the west and low in the east".The global and local spatial autocorrelation results show that there is a spatial correla-tion in the local hospitalization proportion within the counties of Guangdong Province,and spatial aggregation has formed in some areas.There is a"high"aggregation in western Guangdong,and a"low"aggregation in eastern Guangdong Province.The Bayesian spatio-temporal model shows that the number of people with a bachelor's de-gree or above per 1 000 population and the CMI of county hospitals had an effect on the local hospitalization propor-tion(P<0.05).Conclusion Guangdong Province can improve the medical service capacity of county hospitals sub-stantially and introduce or train high-quality talents through multiple measures,and continue to improve the local hospitalization proportion.
2.Advances in Research on Duodenal Inflammation and Overlapping Symptoms of Functional Gastrointestinal Disorders

Overlapping symptoms are common in functional gastrointestinal disorders(FGIDs).The treatment of FGIDs is based on promoting gastrointestinal motility and improving visceral hypersensitivity,however,the efficacy is limited.Emerging data suggest the presence of low-grade duodenal inflammation with predominant eosinophilic infiltration in functional dyspepsia(FD)patients,accompanied by impaired epithelial barrier,increased permeability and increased pyroptosis,etc.,especially in FD patients overlapping gastroesophageal reflux disease(GERD)and/or irritable bowel syndrome(IBS).The etiologies of FGIDs include infection,stress,food antigens,irregular diet etc.,and can increase mucosal permeability,involve in the gut-brain interaction via activating duodenal mucosal eosinophil-mast cell axis,thus causing the symptoms of FGIDs as well as the overlapping symptoms.This article reviewed advances in research on duodenal inflammation and overlapping symptoms of FGIDs.
3.Consensus on Endoscopic Anterior Fundoplication With MUSE for Gastroesophageal Reflux Disease

Endoscopic anterior fundoplication with the MUSE is an endoscopic therapy that combines ultrasound and endoscopic anti-reflux technology for moderate to severe gastroesophageal reflux disease.Training and learning procedures are required to obtain qualifications for this endoscopic therapy before clinical operations.At present,there is limited high-quality evidence-based medical evidence on MUSE treatment,and lack of expert consensus or guidance for training and the standard of MUSE therapy procedure.This consensus is based on the published literature,and formulated by experts with MUSE clinical experience in China,to provide guidance for the training and clinical standard operation of this technique.

Exogenous lipoid pneumonia is an inflammatory response to the lungs caused by inhaled lipid substances, which is prone to secondary bacterial infection, resulting in the formation of local abscesses, which can be life-threatening in severe cases. This paper reports a case of a 55-year-old patient with diesel aspiration, secondary to Klebsiella pneumoniae (ESBL positive) and Candida glabrata infection resulting in lung abscess formation. He was treated with a variety of antibacterial drugs for anti-infection, non-invasive ventilator ventilation, bronchoalveolar lavage, glucocorticoids, phlegm and other medical treatments. Finally, he underwent middle lobectomy for improvement and was discharged from the hospital, and he recovered well with regular follow-up.

8. Progress in Research on Esophageal Motility Disorders

With the alterations of social operation and lifestyle, the clinical problems related to esophagus have increased and changed greatly. Due to the innovation and progression of high - resolution manometry and endoscopy techniques, as well as the achievements of basic research and clinical practice related to duodenal inflammation, intestinal microbiota, leaky gut syndrome and gut - brain interaction, the understanding of esophageal motility disorders has been gradually improved though more doubts have also been raised. This article reviewed the clinical manifestations, pathogenesis, diagnosis and treatment of primary esophageal motility disorders.

The gastrointestinal tract connect with brain by many methods, such as the vagus nerve, neuroactive monoamine neuromodulators, immune system, intestinal microbiota and microbial metabolites. Circadian rhythms are 24 hours patterns of behavior, such as light-dark cycle, hormones secretion, cell movement and so on. This article reviewed the interactions between circadian rhythms and gut-brain axis through gut motility, gut microbiota, immunity and colorectal cancer.
