1.Application of VR technology in standardized residency training of ureteroscopy in the department of urinary surgery

Objective:To explore the application effect of virtual reality (VR) technology in standardized residency training of ureteroscopy in the department of urinary surgery.Methods:Twenty residents without ureteroscopy experience who rotated in Department of Urinary Surgery, The First Affiliated Hospital of Chongqing Medical University from January to November 2023 were randomly divided into two groups, with 10 residents in each group. The traditional teaching group adopted the theory teaching combining skill training. The VR group utilized theory teaching combining VR-assisted skill training, where the process of ureteroscopy was simulated using VR helmet and handles. A skill examination and satisfaction survey were conducted after training to evaluate the teaching effect. SPSS 22.0 was used for the t-test and chi-square test. Results:Compared with the traditional teaching group, the VR group required fewer cases of skilled operation [(3.40±1.00) cases vs. (7.70±1.00) cases] and shorter operation time [(2.70±0.67) min vs. (3.90±1.37) min] ( P<0.05). The VR group was significantly better than the traditional teaching group in the following dimensions: fluency of scope insertion [(2.30±0.82) points vs. (1.40±0.70) points], degree of mucosal injury [(2.20±0.92) points vs. (1.40±0.70) points], and operational proficiency under the ureteroscope [(2.40±0.70) points vs. (1.60±0.84) points] ( P<0.05); there were no significant differences between the two groups in proficiency level and visual field stability scores ( P>0.05). The VR group had higher satisfaction scores in training method (80% vs. 30%) and more confidence to carry out ureteroscopy independently (70% vs. 20%). Conclusions:Through simulated training with VR technology, resident trainees can significantly shorten the learning curve of ureteroscopy and improve teaching satisfaction.
2.Robot-assisted screw fixation for unstable pelvic ring fractures

Objective:To investigate the clinical efficacy of robot-assisted screw fixation for unstable pelvic ring multifocal fractures.Methods:A retrospective analysis was performed on 76 patients with unstable pelvic fractures treated with orthopaedic robot-assisted screw fixation in the Trauma Center of the Affiliated Central Hospital of Shandong First Medical University from January 2015 to June 2022, including 43 males and 33 females, aged 52.53±13.68 years (range, 16-87 years). There were 43 cases of falling injuries from high places, 22 cases of traffic accidents, 11 cases of crushing injuries and heavy objects. Fifty-five patients were employed before the injury, while 21 were not. Fractures were classified according to the Tile classification, with 72 cases classified as type B and 4 cases as type C. Robot-assisted fixation techniques included internal fixator (INFIX), anterior ring screws, sacroiliac screws, and LC-II screws. Intraoperative blood loss, fluoroscopy frequency, surgical time, and the success rate of initial guidewire placement were recorded. Fracture reduction quality was evaluated using the Matta criteria, and postoperative pelvic function recovery was assessed using the Majeed criteria.Results:A total of 150 surgical procedures were performed on 76 patients, including 34 cases of INFIX fixation, 48 cases of anterior ring screws, 61 cases of sacroiliac joint screws, and 7 cases of LC-II screws. The mean intraoperative fluoroscopy frequency was 46.63±17.50 times (range, 15-93 times). Intraoperative fluoroscopy frequency varied among different fixation techniques, with INFIX group at 16.44±4.32 times, LC-II group at 21.59±5.80 times, anterior ring screws group at 29.44±11.65 times, and sacroiliac screws group at 23.10±11.87 times. The intraoperative blood loss was 20 (10, 47.5) ml (range, 5-300 ml), and the surgical time was 105 (86, 150) min (range, 30-290 min). The mean surgical time varied among different fixation techniques. All patients were followed up for an average of 6.46±2.26 months (range, 3-16 months). Clinical healing was achieved in all patients within 6 months, with an average time of 3.14±0.50 months. At the last follow-up, fracture reduction quality assessed by the Matta score was excellent in 21 cases and good in 43 cases, with an excellent/good rate of 84% (64/76). The Majeed score was 81.82±9.14 points (range, 50-92 points). For patients who were employed before the injury, the Majeed score was 86.55±4.85 points (range, 60-92 points), with 49 cases rated as excellent, 5 cases as good, and 1 case as fair. For patients who were not employed before the injury, the Majeed score was 69.43±5.34 points (range, 50-73 points), with 18 cases rated as excellent, 2 cases as good, and 1 case as fair. The overall excellent and good rate was 97% (74/76). Among patients who underwent INFIX internal fixation, 8 cases experienced lateral femoral cutaneous nerve injury postoperatively, all of which recovered sensation after 3 months; 1 case using LC-II screws experienced screw loosening postoperatively and was advised to reduce activity, then the screw was removed after fracture healing at 6 months postoperatively; 1 case using anterior ring channel screws experienced surgical site infection postoperatively, which was controlled after debridement.Conclusion:In the treatment of unstable pelvic ring multiple fractures, robot-assisted screw fixation has less blood loss, less fluoroscopy times, high success rate of planning guide needle, satisfactory reduction quality and postoperative function.
3.H. L.P.E.S. scoring system combined with a virtual reality technique for evaluation of the stone-free rate of flexible ureteroscopy in the treatment of renal calculi

The clinical data of 120 patients admitted to our hospital with renal stones treated by flexible ureteroscopy (FURS) and the imaging-related data measured by virtual reality technology were retrospectively analysed. The results of the univariate analysis showed that stone surface area (S), renal pelvis volume (P), length of calyces funnel (L), pelvic calyceal height (H) and essence of stone (E) were closely related to stone-free rate. The H. L.P.E.S. score was constructed to predict stone-free rate after FURS based on the above factors, and the area under the receiver aperating characteristic curve for the H. L.P.E.S. and S. O.L.V.E. scoring systems was 0.921 and 0.754 respectively.The H. L.P.E.S. scoring system has higher predictive value.
4.Analysis of psychological status and influencing factors of medical workers amid COVID-19 pandemic; analysis of influencing factors

Objective:The study seeks to explore the factors influencing the psychological status and sleep quality of medical workers amid the ongoing COVID-19 pandemic, in order to provide data sources and theoretical basis for the development of relevant psychological intervention programs.Methods:Employing the convenience sampling method, general information questionnaire (age, gender, marital status, educational background, job status, etc.), Generalized Anxiety Disorder-7 and Patient Health Questionnaire, epidemic stress index scale, and sleep quality questionnaire were distributed to medical staff between February 18 and April 3, 2020, using the PEM mental health care platform of by ZhongShengKaiXin for medical staff issued. Descriptive, single factor, and correlation analyses, as well as multiple linear regression analysis were used to analyze the data.Results:Overall, 24, 845 questionnaires were collected from 23 provinces, of which 24, 687 were valid, with a recovery rate of 99.36%. The findings showed that the proportion of medical personnel with symptoms of anxiety and depression was 50.58% and 51.37%, respectively; 16.11% had poor or very poor anti-stress ability; and 71.78% reported poor or very poor sleep quality. There was a positive correlation between anxiety, depression, anti-stress ability, and sleep quality ( P<0.05). Anxiety was positively correlated with depression, stress tolerance, and sleep quality( r=0.787, 0.667, and 0.486, all P<0.001); depression was positively correlated with stress tolerance and sleep quality ( r=0.709 and 0.586, both P<0.001); and stress tolerance was positively correlated with sleep quality ( r=0.452, P<0.001). Multiple linear regression analysis results showed that age, gender, marital status, educational background, professional title, job status, and participation influenced the anxiety levels of medical personnel in the backdrop of the pandemic ( P<0.001). Depression levels of medical staff were influenced by gender, educational background, job position, and participation ( P<0.001), while gender, marital status, educational background, job position, and participation influenced the stress tolerance levels ( P<0.001). The sleep quality of medical workers was influenced by age, gender, job position, participation in the fight against the pandemic, and professional title ( P<0.001). Conclusions:Amid the ongoing COVID-19 pandemic, medical staff reported poor mental health status and sleep quality, which can be attributed to diverse factors. The research findings can be useful for assisting medical staff to strengthen their self-cognition, while also providing certain psychological counseling data and theoretical basis for management departments.
5.ALKBH5 suppresses migration and invasion of human trophoblast cells by inhibiting epithelial-mesenchymal transition.

OBJECTIVE:
To investigate the effects of ALKBH5 on migration, invasion and epithelial-mesenchymal transition (EMT) of human trophoblast cells.
METHODS:
The expression plasmid of ALKBH5 or a negative control plasmid (ALKBH5-NC) was transfected in human trophoblast HTR-8 /SVneo cells, and the expressions of ALKBH5 mRNA and protein were detected by qRT-PCR and Western blotting. Transwell assay was used to assess the changes in migration and invasion abilities of the trophoblast cells after the transfection. Western blotting was performed to detect the expressions of EMT-related proteins in the cells including vimentin, fibronectin, E-cadherin, N-cadherin, MMP9 and MMP2.

ALKBH5 mRNA and protein expressions were significantly higher in ALKBH5 group than in the control group (
CONCLUSIONS
ALKBH5 is involved in the pathogenesis of preeclampsia by inhibiting EMT of trophoblast cells and hence reducing their migration and invasion abilities.

6.Screening and Characterization of Effective Components of Immunopotentiating Activity in Senecionis cannabifolii Herba

OBJECTIVE: To screen and characterize effective components of immunopotentiating activity in Senecionis cannabifolii Herba. METHODS: The polysaccharide components were obtained by water extraction and alcohol precipitation method to yield 50% alcohol precipitation sample (SCHE-1) and 80% alcohol precipitation sample (SCHE-2). The cells from mice mononuclear macrophage line RAW264.7 were divided into blank group (medium without serum), negative control group (medium with serum), lipopolysaccharide group (LPS, positive control drug, 1 μg/mL), SCHE-1 and SCHE-2 low-dose and high-dose groups (0.5, 1 mg/mL). The cell viability of RAW264.7 cells was detected by MTT assay. The levels of IL-1β, IL-6 and TNF-α in RAW264.7 were detected by ELISA. These were used to investigate the effects of SCHE-1 and SCHE-2 on the immunological enhancing activity of RAW264.7 cells. The molecular weight and distribution of SCHE-1 were determined by size exclusion chromatography; the monosaccharide composition of SCHE-1 was determined by HPLC pre-column derivatization. Methylation analysis of SCHE-1 was conducted by NaOH method. RESULTS: Compared with negative control group, the activity of RAW264.7 cells was enhanced significantly in SCHE-1 groups and LPS group, which also significantly increased the levels of IL-1β, IL-6 and TNF-α in cell culture fluids (P<0.01). SCHE-1 was an effective component with immunopotentiating activity. The neutral sugar content of SCHE-1 was 40.05%, the uronic acid was 35.62%, and the protein was 8.89%. SCHE-1 was a mixture, molecular weight of which was 62-6 119 Da; monosaccharide was mainly composed of galacturonic acid, arabinose (Ara) and galactose (Gal). The results of methylation analysis showed that the backbone was composed of 1→3, 1→4 and 1→6 linked Gal, and branches were on the O-6 position of the 1→3 linked Gal, and the non-reducing terminals were Ara. CONCLUSIONS: SCHE-1 may be the effective component of immuno potentiating activity, and main component of SCHE-1 is polysaccharide. SCHE-1 may regulate the immune function by activating macrophages to release IL-1β, IL-6 and TNF-α.
7.Investigation on nursing service demand and satisfaction degree of operation patients in department of general surgery outpatient

Objective? To investigate and analyze the nursing service demand and satisfaction of the operation patients in department of general surgery outpatient. Methods? A total of 516 operation patients were selected from the outpatient surgery in Eastern Hospital Affiliated to Tongji University from July to December 2017 were selected by convenience sampling method. Nursing service demand questionnaire and satisfaction questionnaire were applied in the investigation. Results? The average score of demand for nursing services of 516 patients was (3.72±0.62), which was between need and general. The highest requirement before operation was to know the time of the operation and the time needed for the operation. The highest requirement during operation was that I would not feel pain and discomfort. The highest requirement after operation was to tell me how to seek help if I got home uncomfortable. The average score of nursing satisfaction was (4.04±0.48), which was between great satisfaction and satisfaction. The most unsatisfactory thing for patients is that the waiting time is more than 30 minutes. There were significant differences in the needs of pre-operative, intraoperative and post-operative nursing services among patients of different ages (P<0.05). Conclusions? There are many demands for nursing services in outpatients in department of general surgery. Operating rooms should cooperate with multiple departments to provide targeted nursing services to meet the needs of patients. Hospitals should constantly optimize the process of patients' medical treatment, adopt the method of time-division appointment, shorten the waiting time of patients, and improve patients' satisfaction.
8.Evaluation the clinical value of S.O.L.V.E.nephrolithometry scoring system for predicting the stone-free rate of flexible ureteroscopy

Objective To establish S.O.L.V.E.nephrolithometry scoring system,and to evaluate value of S.O.L.V.E.scoring system for predicting the stone-free rate (SFR) of flexible ureteroscopy (FURS).Methods Five reproducible variables were included in S.O.L.V.E.scoring system,such as stone surface area (S),obstruction (O),length of calyces funnel (L),visible number of calyces (V) and essence of stone (E).Variables were measured based on preoperative non-contrast computed tomography of urography.Clinical data of 392 patients who underwent FURS for upper urinary tract stones in our department from January,2017 to Jnne,2018 were retrospectively analyzed.The total study population consisted of 258 male and 134 female patients.The mean age was (49.5 ± 12.6) years old,ranged from 15 to 85 years.There were 292 patients in stone-fiee group,including 197 male and 95 female patients.The average age was (49.2 ± 12.8) years old.37 patients had previous history of renal stone surgery.Median body mass index was 24.7 kg/m2 (18.1-29.0 kg/m2) and median value of preoperative serum creatinine was 72.5 μmol/L (48.9-84.8 μmol/L).The number of patients,whose stone located in the left side and right side were 155 and 137,respectively.The number of patients,whose stones located in ureter,renal pelvis,lower calyceal and non-lower calyceal were 19,16,87,170,respectively.There were 100 patients in non-stone free group,61 men and 39 women.The average age was (50.4 ± 12.0) years old.15 patients had previous history of renal stone surgery.Median body mass index was 25.0 kg/m2 (18.5-28.8 kg/m2) and median value of preoperative serum creatinine was 73.8 μmol/L (46.5-92.5 μnol/L).The number of patients,whose stone located in the left side and right side were 51 and 49,respectively.The number of patients,whose stones located in ureter,renal pelvis,lower calyceal and non-lower calyceal were 7,4,27,62,respectively.The correlation of S.O.L.V.E.scoring system and stone-free rate,postoperative hospital stay,surgical complications,operation time were analyzed.Receiver operating characteristic curves were drawn to detect predictive value of S.O.L.V.E.scoring system for SFR of FURS.Results All cases FURS were performed successfully and the SFR was 74.5% (292/392).Among the variables of the S.O.L.V.E.scoring system in the stone-free group and the non-stone free group,item S were (82.6 ± 69.8) mm2 and (172.6±133.7)mm2,respectively.The item L were (12.7 ± 15.8) mm and (23.9 ± 15.3)mm,respectively.The item V were (0.6 ± 0.7) and (1.3 ± 0.8),respectively.The item E were (817.1 ± 285.5) HU and (902.4 ± 256.1) HU,respectively.The difference was statistically significant (P < 0.01).The item O was (17.7 ± 10.9) mm and (19.3 ± 13.1) mm,respectively,no statistical significance was found (P =0.242).The mean score was 6.3 (ranging 4-11) in this c ohort.The patients were divided into low score (4-5) group,moderate score (6-8) group and high score (9-11) group due to S.O.L.V.E.scoring system,and the stone-free rates were 93.5% (130/139),70.5% (153/217) and 37.5% (9/36),respectively (P <0.01).The operation time of low,moderate,andi high score group were (31.6 ± 10.9),(42.3 ± 18.3),and (58.0 ± 19.2) min,respectively.Additionally,the score was correlated with the operation time(P <0.01),but not with postoperative hospital stay (P =0.133),intraoperative bleeding (P =0.185) and postoperative infectious fever (P =0.839).In logistic regression model analysis,the stone surface area,length of calyces funnel,number of involved calyces were significantly correlated with SFR (P < 0.01).The obstruction degree and essence of stone were not associated with SFR (P > 0.05).The area under receiver operating characteristic curve of S.O.L.V.E.score was 0.782,higher than that of each variable in S.O.L.V.E.scoring system(S,O,L,V,E were 0.738,0.535,0.698,0.735,0.593,respectively).Conclusions The stone surface area,length of calyces funnel,number of involved calyces were significantly correlated with SFR.The S.O.L.V.E.nephrolithometry scoring system can predict SFR after FURS accurately,and provide assistance for making clinical decisions.
9.Analysis on correlation between SIRS occurrence and calculi composition after flexible ureteroscopy lithotripsy

Objective To analyze the correlation between postoperative systemic inflammatory response syndrome (SIRS) and calculi composition following flexible ureteroscopy (f-URS) lithotripsy.Methods The clinical data of 1 130 patients with kidney stones treated with f-URS lithotripsy in the department of urology surgery of our hospital from November 2013 to August 2016 were collected to analyze the correlation between postoperative SIRS occurrence and calculi composition.Results The compositions of urinary stones were mainly calcium oxalate monohydrate(67.7 %),carbonate apatite (14.7 %),anhydrous uric acid (6.4 %),six magnesium ammonium phosphate (6.2 %) and calcium oxalate dihydrate (3.2 %).Only six magnesium ammonium phosphate was related to the development of SIRS(P<0.01).Conclusion Six magnesium ammonium phosphate has a correlation with the development of SIRS following f-URS lithotripsy,and the results of stone compositions analysis can help to prevent the occurrence of SIRS after f-URS lithotripsy.

Objective To analyze the reasons and treatment methods of high transprothetic pressure gradient after aortic valve replacement. Methods The clinical data of 45 patients with high transprothetic pressure gradient after aortic valve replacement were retrospectively analyzed. The patients were followed up for average 24.6 (12 - 40) months. The postoperative effective orifice area (EOA) of artificial valve was measured by transthoracic color Doppler ultrasound. Compared with published referred EOA of different artificial valve, there were 2 kinds results:measured EOA=referred EOA and measured EOA
