1.Experimental Study on the Mechanism of ACSL4 Inhibition of Sevoflurane-induced Neuronal Iron Death through the AMPK/mTOR Pathway

Objective To investigate the role and mechanism of acyl-CoA syntbetase long chain family member 4(ACSL4)in Sevoflurane(Sev)induced neuronal cell damage.Methods Human neuroblastoma SH-SY5Y cells were used as the research object,and control group(dimethyl sulfoxide,10 μmol/L),Sev group and Sev+iron death inhibitor Ferrostatin-1(Fer-1,10 μmol/L)group were set up.CCK-8 method was used to detect cell activity in each group.4.1%Sev exposed postoperative cognitive dysfunction model was constructed in vitro and divided into Ctrol group,Sev group,Sev+si-NC group,Sev+si-ACSL4 group,and Sev+si-ACSL4+compound C group according to the transfection category.The contents of Malonaldehyde(MDA),4-hydroxynonenal(4-HNE),Glutathione(GSH)and Fe2+in each group were detected by colorimetry.The level of reactive oxygen species was detected using a 2',7'-dichlorofluorescein diacetate(DCFH-DA)fluorescent probe.Real time fluorescence quantitative PCR(qRT-PCR)was used to detect the mRNA expression of ACSL4,glutathione peroxidase 4(GPX4),and solute carrier family 7 member 11(SLC7A11).Protein immunoblotting was used to detect the expression of ACSL4,GPX4,adenosine 5'-monophosphate activated protein kinase(AMPK),phosphorylated(p)-AMPK,mammalian target of rapamycin(mTOR)and p-mTOR proteins.Results CCK-8 results showed that the cell viability of Sev group(0.41±0.11)was significantly lower than that of control group(0.98±0.07),and the cell viability of Sev+Fer-1 group(0.83±0.09)was significantly higher than that of Sev group(0.41±0.11),and the differences were statistically significant(t=7.572,5.118,all P<0.01).The levels of Fe2+,MDA,4-HNE,ROS and p-AMPK/AMPK ratios,as well as the mRNA and protein expression of ACSL4 in the Sev group cells,were higher than those in the Ctrol group(t=5.900,7.421,4.795,13.517,10.825,9.945,11.334),the GSH conten,p-mTOR/mTOR ratio,and mRNA and protein expression of SLC7A11 and GPX4 were lower than those in the Ctrol group(t=20.438,3.551,11.460,12.211,6.845,8.287),and the differences were statistically significant(all P<0.05)repectively.The levels of Fe2+,MDA,4-HNE,ROS,and p-AMPK/AMPK ratios,as well as the mRNA and protein expression of ACSL4 in the Sev+si-ACSL4 group,were lower than those in the Sev+si-NC group(t=3.818,3.164,3.054,4.465,13.088,7.918,9.737),the cell viability,GSH content,p-mTOR/mTOR ratio,and protein expression of SLC7A11 and GPX4 were higher than those in the Sev+si-NC group(t=2.912,7.248,7.574,20.092,5.915),and the differences were statistically significant(all P<0.05),respectively.The cell viability,GSH content,SLC7A11,and GPX4 protein expression in the Sev+si-ACSL4+compound C group were lower than those in the Sev+si-ACSL4 group(t=4.435,8.521,4.522,8.767),while the levels of Fe2+,MDA,4-HNE,and ROS were higher than those in the Sev+si-ACSL4 group(t=10.046,4.004,2.957,3.752),and the differences were statistically significant(all P<0.05).Conclusion Inhibiting ACSL4 expression attenuates Sev-induced iron death in SH-SY5Y cells by activating the AMPK/mTOR signaling pathway.
2.Effects of total shoulder arthroscopy versus small-incision rotator cuff repair for rotator cuff injury

Objective:To investigate the effects of total shoulder arthroscopy versus small-incision rotator cuff repair on shoulder range of motion, postoperative pain, and complications in patients with rotator cuff injury. Methods:The clinical data of 106 patients with rotator cuff injuries treated in Yiwu Central Hospital between June 2018 and July 2021 were retrospectively analyzed. These patients were divided into an observation group and a control group according to different treatment regimens, with 53 patients in each group. The control group underwent arthroscopic small-incision rotator cuff repair. The observation group underwent total shoulder arthroscopy. Perioperative indicators, pain (Visual Analogue Scale score) before surgery and 1, 3, and 6 months after surgery, shoulder range of motion and shoulder-elbow function before surgery, and 3 and 6 months after surgery and postoperative complications were compared between the two groups.Results:Operative time in the observation group was significantly longer than that in the control group [(98.67 ± 12.31) minutes vs. (85.72 ± 10.53) minutes, t = 5.82, P < 0.01). Intraoperative blood loss, postoperative passive shoulder expansion time, and hospital stay in the observation group were (28.66 ± 3.12) mL, (5.61 ± 1.03) days, and (14.17 ± 2.26) days, which were significantly shorter than (37.68 ± 3.59) mL, (7.22 ± 1.41) days and (17.21 ± 3.04) days in the control group ( t = 13.81, 6.71, 5.86, all P < 0.01). After surgery, the Visual Analogue Scale score in each group significantly decreased compared with that before surgery (both P < 0.05). At 1, 3, and 6 months after surgery, the Visual Analogue Scale score in the observation group was (2.46 ± 0.63) points, (1.53 ± 0.42) points, and (1.03 ± 0.26) points, respectively, which were significantly lower than (3.04 ± 0.74) points, (1.84 ± 0.51) points, and (1.21 ± 0.34) points in the control group ( t = 4.35, 3.42, 3.06, all P < 0.01). At 3 and 6 months after surgery, the ranges of motion of shoulder abduction, external rotation, and internal rotation in each group significantly increased compared with those before surgery (all P < 0.05). The scores of American Shoulder and Elbow Surgeons and the University of California Los Angeles in each group were significantly increased compared with those before surgery (both P < 0.05). After treatment, there were no significant differences in the scores of American Shoulder and Elbow Surgeons and the University of California Los Angeles between the two groups (both P < 0.05). By 6 months after surgery, the incidence of complications in the control group was 3.77%. No obvious complications were observed in the observation group. There was no significant difference in the incidence of complications between the two groups ( χ2 = 0.51, P > 0.05). Conclusion:Compared with small-incision rotator cuff repair, total arthroscopic rotator cuff repair has less invasive trauma and mild pain and is more conducive to postoperative shoulder function recovery.
3.Effect of glycerin fructose combined with mannitol in the treatment of postoperative swelling of extremities fractures

Objective To study the effect of glycerin fructose combined with mannitol in the treatment of postoperative swelling of extremities fractures.Methods In Yiwu Central Hospital from February 2015 to February 2017 treated 100 cases of limb fracture surgery patients as the research object in the course of the study, were randomly divided into control group and experimental group two were 50 cases each.The patients in control group were treated with mannitol after fracture surgery, and the patients in the experimental group were treated with glycerol fructose and mannitol.Comparative analysis of the experimental group and control group of patients with limb swelling degree.Results After different treatment, the experimental group of patients with adverse reactions occurred in 0 cases, the probability was 0.0%, the control group of patients with adverse reactions in the number of cases in the case of the number of patients with adverse reaction probability of 10.0%, the number of patients with adverse reactions in the control group was 5.As a result, the incidence of adverse reactions in the experimental group was significantly lower than that in the control group, with statistical difference(P<0.05).In the experimental group, 6 days after surgery, on the 7 day, the 8 day and the 9 day, the forearm circumference was significantly lower than that of the control group, with statistical difference(P<0.05).On the 8 day after surgery and after 8 days, the leg circumference of the experimental group was significantly lower than that of the control group, with statistical difference(P<0.05).Conclusion The clinical effect of glycerol and fructose combined with mannitol in treatment of limb swelling after surgery for fracture, high safety, reduce the probability of occurrence of adverse reactions to a great extent, is further applied in clinical significance.
4.Protective effect of MG132 on ischemia-reperfusion injury after pancreaticoduodenal transplantation in rats

Objective To investigate the protective effects of Proteasome inhibitor MG132 on ischemia-reperfusion injury (IRI) after pancreas transplantation in rats,and the possible mechanism.Method Fifteen normal SD rats were allocated into the sham operation group.In the allogenic male SD rats,the model of pancreas transplantation was established.The recipients were divided into another two groups (n =15 each) at random:IRI group and the MG132 pretreatment group.Serum amylase level was determined at 1,3 and 6 h after the operation.Pancreas samples were harvested at the same time for pathological study by light microscopy.The expression of NF-κB P65 protein in the pancreas was detected by using Western blotting.The expression of TNF-α and ICAM-1 in the pancreas was detected by using immunohistochemistry and RT-PCR.Result Tissue damage on IRI group was more severe than in sham operation group.The level of serum amylase,and the expression of P65,TNF-α and ICAM-1 were higher in IRI group than those in sham operation group (P<0.05).Tissue damage in MG132 pretreatment group was milder than in IRI group.The level of serum amylase,and the expression of P65,TNF-α and ICAM-1 were lower in MG132 pretreatment group than in IRI group (P<0.05).Conclusion MG132 pretreatment can alleviate the pancreas IRI after pancreas transplantation,probably by inhibiting the activation of NF-κB,and the inhibition effect can down-regulate the expression of TNF-α and ICAM-1.
5.Treatment of posterior condylar tibial plateau fractures

Objective To discuss the operative procedures and clinical result of posteromedial and posterolateral approaches in treatment of posterior condylar tibial plateau fractures. Methods From January 2006 to June 2008, 21 patients of posterior condylar tibial plateau fractures were treated by posteromedial and posterolateral knee approaches. There were 12 males and 9 females. The age ranged from 28 to 68 years, with a mean of 39.5 years. Of the patients, 13 had resulted from a traffic accident and 8 had caused by a fall. As for the state of posterior condylar tibial plateau fractures, 7 patients had a medial condylar.fracture, 8 patients had a lateral condylar fracture and 6 patients had a bilateral condylar fracture. Results A follow-up lasted 12-24 months (mean 16.2 months ) in 21 patients. There was no infection, no varus or valgus of the knee, no nerve injuries and loosening or breakage of the screw. All cases had attained bone union. According to the Rasmussen functional scoring, the results were excellent in 12, good in 7 and fair in 2. The excellent and good rate of clinical results was 90.5%. Radiologic results were graded with the Rasmussen score to evaluate the reduction of fracture. There were excellent in 13, good in 7 and fair in 1. The excellent and good rate of clinical results was 95.2%. Conclusion Posteromedial and posterolateral approaches can facilitate the reduction and fixation for posterior condylar tibial plateau fractures. It has many advantages such as good exposure, less invasion and the excellent clinical results.
6.Influence of hepatic oval cell on the expression of ERK and P38MAPK signaling pathway protein in liver tissue of murine hepatofibrosis

Objective To observe the influence of hepatic oval cell (HOC) on the expression ERK and P38MAPK signaling pathway protein in liver tissue of murine experimental hepatofibrosis (HF).Method SD rats were fed with 10% ethanol and food with high-fat and low-protein, and were injected subcutaneously with carbontetrachloride once every four days for 8 weeks to establish hepatic fibrosis. HOGs were isolated from male HF rats by collagenase porfusion of the liver. HF rats at 8th week were transplanted with 0. 5 ml HOC suspension medium at a density of 1 × 109 cell /ml via portal vein, and the rats were sacrificed at 8th, 15th, 30th day respectively. Histopathologic changes of liver tissues were observed by HE and Masson. The expression of ERK and P38MAPK signaling pathway protein were determined by Western blotting. Result Hepatofibrosis was reversed and the degree of hyperplasia fibrilcollagen in hepatic fibrosis rats decreased significantly by HOC transplantion. HOC down-regulated the protein expression of Ras, ERK,p-ERK, c-fos, c-jun, STAT3, ALB, FGF-3, PCNA ( F = 91.88,36.28,54.66,93.07,64.76,58.49,52.63,20.45 ,27.03, all P < 0.05 ), up-regulated the protein expression level of HNF-α1, PDGF-Rβ significantly in liver tissues(F = 18.63,25.99,P <0.05). Conclusions HOC improves the degree of hepatofibrosis through inhibiting hyperplasia of collagen fibril in liver tissue of hepatofibrosis rats. With the presence of HOC the expression of c-fos,c-jun,STAT3,5 was not activated by p-P38MAPK. The expression of c-kit and HNF-1α increased and that liver tissue injury alleviated, and hepatofibrosis was improved.
7.Treatment of sacroiliac joint dislocation with percutaneous sacroiliac screw internal fixation

Objective To analyze the clinical effect of percutaneous sacroiliac screw internal fixation in treatment of sacroiliac joint dislocation. Methods From June 2002 to August 2006,16 patients with sacroiliac joint dislocation were treated with percutaneous sacroiliac screw internal fixation under C-arm X-ray tomography.There were 10 males and 6 females at age range of 10-58 years(mean 34.3 years).Results The operation lasted for 30-90 minutes(average 50.5 minutes).All patients were followed up for 12-36 months(average 18.3 months).The results of postoperative normotopia,lateral,ingate and egress Xray and CT scanning showed that all the screws were located within S1 and S2 of all,14 patients obtained satisfactory result of reduction,with no infection,nerve injuries,loosening or breakage of the screw fixation.The function and the sensation of the sacroiliac ioint and low extremities recovered to normal. Conclusions Percutaneous sacroiliac screw intemal fixation is an ideally safe and effective way to treat sacroiliac joint dislocation,for it has many advantages such as minimal invasion,reliable fixation,less complication and quick recovery.It is also very necessary to take caudad and cephalad view under an image intensifier during the operation to assure the accuracy of implantation.
8.The study of clinical anatomy of lingual artery in physiological condition.

OBJECTIVE:
To provide the clinical anatomy data in physiological condition of the lingual artery in 64-slices spiral computed tomography angiography (CTA) for clinical treatment concerning the lingual artery.
METHOD:
CTA examination of the carotid artery were performed on 80 adult volunteers. The 3D reconstruction images of the carotid artery with hyoid bone were obtained by using 64-slices spiral CT and image postprocessing workstation. The origin, pathway, and anatomic relations of the lingual artery in CTA were studied. The distance from the origin of the lingual artery to the bifurcation of the common carotid artery and tip of the greater horn of hyoid bone were separately measured, and the distance between the segment of the greater horn of hyoid bone of lingual artery and the middle of greater horn of hyoid bone were also measured.
RESULT:
The pathway of the lingual artery had high variation, but the relative position between the segment of the greater horn of hyoid bone of lingual artery and the greater horn of hyoid bone were relatively constant. The lingual artery run forward approximately parallel to the greater horn of hyoid bone into tongue (2.32 +/- 1.29) mm in the superior to the greater horn of hyoid bone or (2.00 +/- 1.68) mm in the inferior to the greater horn of hyoid bone The distances from the origin of the lingual artery to the bifurcation of the common carotid artery and tip of the greater horn of hyoid bone were (12.93 +/- 7.36) mm and (10.40 +/- 5.75) mm separately.
CONCLUSION
The lingual artery could be clearly shown in CTA and the anatomy data in physiological condition of the lingual artery could be obtained by CTA. There was important instructive significance for clinical treatment concerning about the lingual artery.

9.Clinical anatomy of the lingual artery

OBJECTIVE To provide the clinical anatomy data of the lingual artery for clinical application concerning about the lingual artery. METHODS CTA examination of the carotid artery was performed in 80 adult volunteers. The 3D reconstruction images of the carotid artery with hyoid bone were obtained by using 64-slices spiral CT. At the same time, 20 extraoral dissections of the submandibular region were performed on 10 human cadavers. The origin, pathway, and anatomic relations of the lingual artery in CTA and cadavers were studied. The distance from the origin of the lingual artery to the bifurcation of the common carotid artery and tip of the greater horn of hyoid bone were separately measured, and the distance between the segment of the greater horn of hyoid bone of lingual artery and the middle of greater horn of hyoid bone were also measured. And the contrast analysis was carried on the result. RESULTS The lingual artery was mainly come from the external carotid artery (74.4 % in the CTA; 80% in the Cadavers), the next origin was come from the facial artery (23.1 % in the CTA; 20 % in the Cadavers), and it was few to found that the lingual artery had origin in the superior thyroid artery, which was 2.5 % in CTA and none in Cadavers. The path of the lingual artery had high variation, but the position between the segment of the greater horn of hyoid bone of lingual artery and the greater horn of hyoid bone had constancy relatively. The lingual artery run forward with approximation parallel to the greater horn of hyoid bone into tongue,and which located in thesuperior to the greater horn of hyoid bone (2.32?1.29 mm) or in the inferior to the greater horn of hyoid bone (2.00?1.68 mm). The distances from the origin of the lingual artery to the bifurcation of the common carotid artery and tip of the greater horn of hyoid bone in CTA and cadavers were (12.93?7.36) mm, (10.40?5.75) mmand (14.80?6.18) mm, (8.35?5.44) mm respectively. CONCLUSION The lingual artery can be show clearly in CTA and can get the anatomy data in physiological condition with CTA.
10.Experimental Study on Bupleuri in Combination With Saliva Miltiorrhiza in Treatment of Hepatic Fibrosis in Rats

OBJECTIVE:To investigate the effects of Bupleuri plus Saliva miltiorrhiza on preventing the formation of hepatic fibrosis in rats METHODS:To establish an animal model of hepatic fibrosis by feeding high fat diet and 10% alcohol and subcutaneously injecting CCl4 and to detect the zymogram and biochemical parameters and observe the pathological changes of liver tissue RESULTS:Bupleuri and Saliva miltiorrhiza could significantly depress the activities of ALT,AST,MAO,ALP and had no influence on the ?-GT in serum,the serum TG was decreased and TG in tissue was obviously raised and serum GSH was elevated in hepatic fibrosis rat The extract could significantly decreased the levels of PC Ⅰ and PC Ⅲ in hepatic fibrosis rat The histopathological examination showed that the extract could diminish liver degeneration and necrosis,and block the formation of fiber diazoma,the degree of collagen proliferation was significantly lower than that of the control group CONCLUSION:Bupleuri and Saliva miltiorrhea extract could effectively protect the liver cell,and could retard the formation of hepatic fibrosis as well
