1.Effect of ganglioside combined with hyperbaric oxygen on mild to moderate craniocerebral trauma and its influence on serum NSE and MBP levels

		                        			Objective To study the effect of ganglioside combined with hyperbaric oxygen on mild to moderate craniocerebral trauma , and its effect on serum neuron specific enolase ( NSE ) and myelin basic protein ( MBP) levels.Methods Following the random number table method ,110 patients with mild and moderate cranioce-rebral trauma were randomly divided into the observation group and the control group ,55 cases in each group.The observation group was treated with ganglioside combined with hyperbaric oxygen therapy ,while the control group was treated with hyperbaric oxygen alone .The clinical efficacy ,the Glasgow Coma Scale ( GCS) ,the levels of serum NSE and MBP before and after treatment were compared between the two groups .Results The effective rate of the observation group was 94.55%,which was significantly higher than 81.82% of the control group,the difference was statistically significant(χ2 =4.274,P<0.05).The D-value of GCS before and after treatment in the observation group was (2.97 ±0.59)points,which was significantly higher than that of control group (t=17.601,P<0.05). The levels of serum NSE and MBP of the observation group were significantly lower than those of the control group ( t=14.674,10.450,all P<0.05).Conclusion Ganglioside combined with hyperbaric oxygen can promote the recovery of neurological function ,has significant effect and high safety in the treatment of mild to moderate craniocerebral trauma .
		                        		
		                        		
		                        		
		                        	
2.Exercise effects on pain relief and extracellular matrix production in a rat model of intervertebral disc degeneration

		                        			BACKGROUND: Exercise has been proved to accelerate the proliferation of intervertebral disc cells and extracellular matrix production in healthy rats. For the degenerative intervertebral disc, whether exercise also has positive effects on its cell proliferation, extracellular matrix production or pain relief remains unclear. OBJECTIVE: To investigate the effect of exercise on the extracellular matrix production in a rat model of intervertebral disc degeneration.METHODS: A rat model of intervertebral disc degeneration was prepared by Freund's complete adjuvant injection into the intervertebral disc at L5-6 levels. Then, the model rats were allowed to have a rest for 2 weeks. All rats were then randomly divided into exercise and control groups. Rats in the exercise group were forced to run every day, while the controls allowed free activities in the cage. The behavioral tests were performed at 7, 14, 28, 42, 56 and 70 days after modeling; meanwhile, the intervertebral disc samples were collected used for alcian blue staining and immunohistochemical staining to detect the levels of proteoglycan, aggrecan and collagen type Ⅱ in the intervertebral disc cells, respectively.RESULTS AND CONCLUSION: Vocalization threshold on the rat back of punctured disc was significantly decreased, while grooming and wet-dog shaking were significantly increased at 7 days after modeling compared with the baseline (P < 0.05), suggesting that Freund's complete adjuvant injection successfully induces disc degeneration, hyperalgesia and abnormal behaviors. Further, the vocalization threshold and wet-dog shaking in the exercise group showed significant improvement compared with the control group after 14 days of exercise (P < 0.05), while the grooming was significantly reduced until the 28th day (P < 0.01), indicating that exercise can alleviate pain caused by disc degeneration in model rats. At 21 days after modeling, the levels of proteoglycan, aggrecan and collagen type Ⅱ in the nucleus pulposus and annulus fibrosus were significantly decreased compared with the baseline (P < 0.01), indicating the occurrence of disc degeneration. After 14 days of training, the levels of proteoglycan, aggrecan, and collagen type Ⅱ in the nucleus pulposus and annulus fibrosus in the exercise group were significantly increased compared with the control group (P < 0.01). Moreover, after 8-week exercise, the level of proteoglycan in the nucleus pulposus and annulus fibrosus in the exercise group was increased by 4-5 times compared with the control group, and levels of aggrecan and collagen type Ⅱ in the nucleus pulposus in the exercise group also was increased by 3-4 times compared with the control group. To conclude, exercise can promote extracellular matrix increased by production by increasing the levels of proteoglycan, aggrecan, and collagen type II in the degenerative intervertebral disc.
		                        		
		                        		
		                        		
		                        	
3.The effect of early mechanical ventilation combined with mild hypothermia in the treatment of severe brain injury and its effect on serum C-reactive protein and hemorheology

		                        			Objective To explore the clinical effect of early mechanical ventilation coupled with mild hypothermia in the treatment of severe traumatic brain injury(STBI)and its effect on serum C -reactive protein (CRP)and hemorheology.Methods According to the order of admission,110 patients with STBI were divided into two groups.55 cases in the control group were given conventional symptomatic treatment and early mechanical ventilation, while 55 patients in the observation group were given early mechanical ventilation coupled with mild hypothermia.The clinical efficacy was compared between the two groups.Results The good prognosis rate was 74.55% in the observation group,which was significantly higher than 56.36% of the control group (χ2 =7.65,P <0.05).The serum CRP in the observation group admitted to hospital 2,3,5,7d were significantly decreased compared with the control group(t =7.48,8.04,7.34,7.82,all P <0.05).After treatment,the hear whole blood viscosity was (6.03 ±0.89)mPa·s, low shear whole blood viscosity was (3.18 ±0.19)mPa·s,plasma viscosity was (1.21 ±0.18)mPa·s,hematocrit was (37.56 ±3.23)%,and those were decreased compared with before treatment(t =8.45,7.54,7.04,8.02,all P <0.05).And there were significant differences compared with the control group(t =7.77,6.69,6.44,7.49,all P <0.05).Conclusion The effect of early mechanical ventilation coupled with mild hypothermia in the treatment of STBI has good effect,and it can reduce serum CRP levels,improve blood rheology,which is conducive to patients'prognosis.
		                        		
		                        		
		                        		
		                        	
4.Therapeutic effect of hyperbaric oxygen therapy on patients with severe craniocerebral injury and its influence ;on serum interleukin -1 beta and interleukin -6 levels

		                        			Objective To explore the clinical curative effect of hyperbaric oxygen therapy for severe cranio-cerebral injury,and its influence on the level of serum interleukin -1 beta(IL -1 )and interleukin -6(IL -6). Methods 150 patients with severe craniocerebral injury were selected and divided into two groups by random and single blind method.75 patients of the control group were treated with conventional therapy,75 patients of the observation group were given hyperbaric oxygen on the basis of the conventional treatment,and the clinical efficacy was compared between the two groups.Results The total effective rate of the observation group was 94.67%,which was significantly higher than 77.33% of the control group,the difference was statistically significant(χ2 =7.83,P <0.05).There were no significant differences in GCS and ADL scores between the two groups(t =1.22,2.39,all P >0.05).After treat-ment,the GCS and ADL scores of the observation group were (12.18 ±2.13)points,(67.46 ±12.55)points,which were significantly higher than those before treatment(t =8.01,7.76,all P <0.05),and there were significant differ-ences between the two groups(t =7.23,6.82,all P <0.05).Before treatment,there were no significant differences in the serum levels of IL -6 and IL -1 between the two groups(t =2.10,1.01,all P >0.05).After treatment,the serum levels of IL -1 and IL -6 in the observation group were (127.48 ±16.35)pg/mL,(89.66 ±13.47)pg/mL, which were significantly lower than before treatment(t =8.34,7.89,all P <0.05),there were significant differences between the two groups(t =7.65,7.22,all P <0.05).Conclusion Implementation of hyperbaric oxygen in patients with severe traumatic brain injury on the basis of conventional symptomatic treatment can effectively improve the clinical symptoms and signs,reduce the serum IL -1 and IL -6 levels,promote the recovery of brain function and ability of daily life,and which is worth to be promoted in clinical.
		                        		
		                        		
		                        		
		                        	
5.Correlation between the MRI-based grading system and F wave as well as H-reflex in patients with lumbar disc herniation

		                        			BACKGROUND:Lumbar spine MRI and electrophysiological test are reliable methods for evaluating nerve root injury caused by lumbar disc herniation. 
 OBJECTIVE:To analyze the correlation between the MRI-based grading system and the latency and frequency of F wave as wel as latency and amplitude of H-reflex in patients with lumbar disc herniation. 
 METHODS:MRI imaging of the lumbar spine was performed with a 3.0-T imager and a dedicated TCL coil to classify lumbar disc herniation and nerve root compression. F wave and H reflex were detected on the patient bilateral tibial nerves using Oxford myoelectricity evoked potential instrument. 
 RESULTS AND CONCLUSION:Spearman correlation analysis showed that the MRI-based grading of patients with lumbar disc herniation had a negative correlation with F wave frequency (r=-0.594 0, P<0.000 1), and a positive correlation with F wave latency (r=0.825 6, P<0.000 1) and H-reflex latency (r=0.875 0, P<0.000 1), but no correlation with H-reflex amplitude (R=0.117 4, P=0.257 3). With MRI grading increased, F wave frequency was decreased, and F wave and H-reflex latency were prolonged gradual y, indicating aggravating nerve root compression.
		                        		
		                        		
		                        		
		                        	
6.Growth and secretion features of microencapsulated human adrenal pheochromocytoma cells in artificial cerebrospinal fluid

		                        			BACKGROUND:Microencapsulated cels are commonly used as a tool to overcome immune rejection after subarachnoid transplantation. However, the effect of microencapsulation on the secretion of human pheochromocytoma cels is unclear. 
 OBJECTIVE:To observe the growth and secretion of primarily microencapsulated cultured human pheochromocytoma cels in artificial cerebrospinal fluid. 
 METHODS: The human pheochromocytoma tissues were digested successively to isolate human pheochromocytoma cels that were then cultured in artificial cerebrospinal fluid. Primary cels were covered with alginate-polylysine-alginate microcapsules, and then the cel morphology was observed with inverted phase contrast microscope. Levels of met-enkephalin and norepinephrine in cel culture medium were detected by enzyme-labeled immunosorbent assay (ELISA). We used cel counting kit-8 colorimetric assay to obtain the growth curve of human pheochromocytoma cels in artificial cerebrospinal fluid. 
 RESULTS AND CONCLUSION:Microcapsulated human pheochromocytoma cels were in suspension and the process outgrowth increased slowly. Compared with non-microcapsulated cels, the proliferation rate of microcapsulated cels increased significantly. ELISA results revealed a significant increase in the levels of met-enkephalin and norepinephrine secreted from the microencapsulated cels compared to the non-microcapsule group. There was a wide variation in contents of met-enkephalin and norepinephrine from different tumors. These findings indicate that microencapsulated human pheochromocytoma cels can survive wel and have good secretion function in artificial cerebrospinal fluid, and human pheochromocytoma cels from different tumor tissues have stable secretory function.
		                        		
		                        		
		                        		
		                        	
7.Effects of Clearing Liver and Purging the Heart Decoction Combined with Methimazole on Hormone Levels in Hyperthyroid Rats

		                        			Objective To investigate the effects of clearing liver and purging the heart decoction( CLPHD) combined with methimazole on hormone levels in hyperthyroid rats. Methods Rats were administered with levothyroxine (600 μg·kg-1 ) for 3 weeks to establish the hyperthyroid model,and the serum contents of T3,FT3,T4,FT4,TSH and TRAb in hyperthyroid rats treated with CLPHD combined with methimazole were detected. Results Methimazole, CLPHD, and the combination therapy significantly reduced food intake,water intake and body temperature of the hyperthyroid rats. All treatments reduced levels of FT3, T4 and FT4 in hyperthyroid rats. Compared with the CLPHD mono-therapy, the combination therapy significantly lowered serum T4 in hyperthyroid rats,but not the TRAb level. 8.Islet-like cells derived from mesenchymal stem cells in Wharton's Jelly of the human umbilical cord co-cultured with rat pancreatic cells for transplantation to control type Ⅰ diabetes mellitus

		                        			BACKGROUND: Mesenchymal stem cells in Wharton's Jelly of the human umbilical cord can induce differentiation into islet-like cells.OBJECTIVE: To verify the possibility of human umbilical cord derived mesenchymal stem cells co-cultured with rat pancreatic cells differentiate into islet-like cells, and to observe the effects of transplantation of islet-like cells on blood glucose of diabetic rats.METHODS: Mesenchymal stem cells in Wharton's Jelly of the human umbilical cord was separated, induced, passaged, and co-cultured with pancreatic cells to induce differentiation into islet-like clusters. Rats were divided into the normal control, model and experimental groups. Rats in the model group were prepared for diabetic models, and those in the experimental group were transplanted islet-like cells after model preparation. RESULTS AND CONCLUSION: There were cells crawled out of cultured Wharton's Jelly of the human umbilical cord, and morphology of adhered cells turned into fusiform shape at 7 days. The isolated cells are characterized by expressing specific surface markers of mesenchymal stem cells, such as CD44, CD29, CD105, but not expressing CD34, CD45 or CD14. The cells were strongly stained by PDX-1 and human insulin at 7 and 10 days. Compared with the simple culture group, the expression of human insulin and concentration of C-peptide were obviously increased; PDX-1 and human insulin mRNA expressions were highly expressed at 7 and 10 days after induction. Compared with the model group, the streptozotocin test of rats in the experimental group was obvious decreased (P < 0.01), but extremely higher than that of the normal control group at 1 week after transplantation (P < 0.01). Brdu positive nuclei and insulin positive kytoplasms could be seen in the experimental group at 8 weeks after transplantation. The results demonstrated that, umbilical cord derived mesenchymal stem cells existed in Wharton's Jelly. The co-cultured cells promote mesenchymal stem cells differentiating into islet-like cells, which can dramatically decrease blood glucose in diabetic rats.
		                        		
		                        		
		                        		
		                        	
9.Value of ultrasound in diagnosis and classification of pilomatricoma

		                        			Objective To explore the value of ultrasound in diagnosis and classification of pilomatricoma.Methods Ultrasonic features of 63 cases with pilomatricoma confirmed by operation and pathology were analyzed and summarized retrospectively.Results According to if there was calcification in the tumor,pilomatricoma cases were classified into two groups(calcific group and non-calcific group).In addition,there were three subgroups in the calcific group based on the size and disposition of calcification:subgroup Ⅰ,completely echogenic mass with strong posterior acoustic shadowing in the subcutaneous layer (n=17,26.98%);subgroupⅡ,hypoechoic nodules with tiny calcification diffused distributed(n=15,23.80%);subgroup Ⅲ,hypoechoic nodules with large calcification(n=8,12.70%).Twenty-three cases (36.51%)were distributed into non-calcific group.Conclusions Ultrasonography is valuable for diagnosis and differential diagnosis of pilomatricoma,and it is an essential examination method for the tumor before operation.
		                        		
		                        		
		                        		
		                        	
10.Clinical application of the Standardized Swallowing Assessment

		                        			Objective To study the reliability, validity, sensitivity, specificity and predictive value of the Standardized Swallowing Assessment (SSA) in assessing swallowing function. Methods Sixty-seven stroke patients with dysphagia participated in the study. They were assessed twice with the SSA within 3 days. In addition, the patients' swallowing was also assessed using videofluoroscopy. Results There was high correlation in all items of the SSA between the two separate assessment sessions. The intraclass correlation coefficient was 0.85 for intrarater re- liability and 0.82 for interrater reliability. The SSA and videoflouroscopy results were highly correlated in terms of their total scores (r = -0.86,P≤0.01). Using the videoflouroscopy result as the standard, the sensitivity and spe- cificity of the SSA were 77.8% and 68.1% , respectively. At the same time, the positive and negative predictive val- ues were 48.3% and 88.9% , respectively. Conclusions The SSA has satisfactory reliability and validity, and higher sensitivity and specificity than videoflouroscopy.
