1.Characteristics and Misdiagnosis of Viral Encephalitis Manifested by Isolated Dizziness in 37 Cases

ObjectiveTo study the clinical features of viral encephalitis with isolated dizziness,and to analyze the diagnostic efficacy of vestibular function examination and cerebrospinal fluid cytology in these patients. MethodsTotally 37 cases of viral encephalitis with isolated dizziness and 10 healthy volunteers were included. Clinical data [dizziness handicap inventory (DHI) score,head imaging,electroencephalogram,vestibular function test,cerebrospinal fluid routine,biochemistry,cell morphology,etiology second-generation sequencing,misdiagnosis] were collected. The area under the ROC curve(AUC)of diagnostic value of each type of test was analyzed. The changes of each examination before and after treatment were compared. ResultsWe found 89.19%(33/37)of the patients were misdiagnosed. Vestibular function smooth follow-up test indicated vestibular central lesion (AUC value:0.82)in 64.86%(24/37)of the patients. The number of CSF transformed lymphocytes increased in 86.49%(32/37)of the patients(AUC value:0.93),the CSF large lymphocytes increased in 97.30% (36/37)of the patients (AUC value:0.99),and the mononucleosis was activated in 94.59%(35/37)of the patients(AUC value:0.97). Furthermore,18.92%(7/37)of the patients had increased EEG slow wave(AUC value:0.60),while 13.51%(5/37) of the patients showed cortical swelling on head MR (AUC:0.60). After antiviral treatment,dizziness grade decreased(Z=-4.899,P<0.001),smooth tracking abnormalities decreased(Z=-4.583,P<0.001),the proportion of CSF transformed lymphocytes decreased(t=4.281,P<0.001),and the proportion of large lymphocytes decreased(t=6.905,P<0.001). ConclusionThe misdiagnosis rate of viral encephalitis with isolated dizziness is high. Incorporating into diagnosis the increased large lymphocytes, transformed lymphocytes,activated monocytes in CSF cytology with smooth follow-up test may improve diagnostic efficiency .
2.Neuroprotective effect and mechanism of celastrol and its derivatives in vitro

OBJECTIVE To explore the neuroprotective effect and possible mechanism of celastrol (Cel) and its derivatives (Cel-1, Cel-2) in terms of neuroinflammation and oxidative damage. METHODS Neuroinflammation model of microglial BV2 cells was induced by 1 μg/mL lipopolysaccharide (LPS); oxidative damage model of human neuroblastoma SH-SY5Y cells was induced by 200 μmol/L hydrogen peroxide (H2O2). The toxicity of different concentrations of Cel, Cel-1 and Cel-2 (0.625-20 μmol/L) to the two types of cells was investigated. The levels of nitric oxide (NO), tumor necrosis factor α (TNF-α), interleukin 1β (IL-1β), and IL-6 in BV2 cells induced by LPS at safe concentrations (0.039-0.625 μmol/L) were all detected. The survival rate of SH-SY5Y cells induced by H2O2 was also determined. The expression levels of phosphoinositide 3-kinase (PI3K), p-PI3K, protein kinase B (Akt), p-Akt, cystatinase 3 (caspase-3), B-cell lymphoma 2 (Bcl-2) and Bcl-2-related X protein (Bax) in SH- SY5Y cells induced by H2O2 at 0.156, 0.313, 0.625 μmol/L of active compound 2 were all detected. RESULTS In the concentration gradient range between 0.039 and 0.625 μmol/L, the results of neuroinflammation model experiments showed that Cel, Cel-1 and Cel-2 could reduce the contents of NO, TNF-α, IL-1β, and IL-6 in culture medium of BV2 cells (P<0.05 or P< 0.01); their IC50 values for neuroinflammation were (0.25±0.04), (0.61±0.14) and (0.11±0.02) μmol/L respectively. Meanwhile, all of them could reverse the phenomenon of decreased cell survival rate after H2O2 treatment in the oxidative damage experiments at a certain concentration (P< 0.05 or P<0.01), with neuroprotective EC50 values of (0.43± XJC2023009) 0.08), (0.45±0.04) and (0.28±0.03) μmol/L, respectively.Induced by H2O2, the phosphorylation of PI3K and Akt protein, protein expressions of Bcl-2 and Bcl-2/Bax ratio were all increased significantly (P<0.05 or P<0.01), while the protein expressions of caspase-3 and Bax were decreased significantly (P<0.05 or P<0.01). CONCLUSIONS Cel, Cel-1, and Cel-2 all have significant neuroprotective activities at certain concentrations, and Cel-2 shows the most significant protective effect. The mechanism of action of Cel-2 may be related to regulating the PI3K/Akt and caspase-3/Bcl-2/Bax signaling pathways, reducing the inflammatory response, oxidative stress damage and inhibiting neuronal apoptosis.
3.Effect of Linggui Zhugantang on Mitochondrial Fusion-fission and Sirt3/ AMPK Signaling Pathway in Chronic Heart Failure Rats After Myocardial Infarction

ObjectiveTo investigate the effects of Linggui Zhugantang on mitochondrial fission and fusion and silencing information regulator 3(Sirt3)/adenosine monophosphate dependent protein kinase (AMPK) signaling pathway in chronic heart failure (CHF) rats after myocardial infarction (MI). MethodSD rats randomly divide into sham operation group (normal saline ,thread only without ligature), model group (normal saline, ligation of the left anterior descending coronary artery proximal to the heart), Linggui Zhugantang group (4.8 g·kg-1) and Captopril group (0.002 57 g·kg-1), with 10 rats in each group. Administere drug continuously for 28 days. Echocardiography detected cardiac function parameters. Hematoxylin eosin (HE) staining observed the pathological changes of the heart. Immunofluorescence detected the levels of reactive oxygen species (ROS). JC-1 detect mitochondrial membrane potential. Colorimetry measure adenosine triphosphate (ATP), superoxide dismutase (SOD), malondialdehyde (MDA), mitochondrial respiratory chain complex activity (Ⅰ-Ⅳ). TdT-mediated dUTP nick end labeling (TUNEL) staining detected the apoptosis rate of myocardial tissue. Western blot detected protein expression levels of Sirt3, phosphorylated AMPK (p-AMPK), phosphorylated dynamic-related protein 1(p-Drp1), mitochondrial fission protein 1(Fis1), mitochondrial fission factor (MFF), optic atrophy protein 1(OPA1). ResultCompared to the sham group, the left ventricular end diastolic diameter (LVIDd) and left ventricular end systolic diameter (LVIDs) were significantly increased in model group (P<0.01), while the left ventricular short axis shortening rate (LVFS) and left ventricular ejection fraction (LVEF) were significantly decreased (P<0.01). There were inflammatory cell infiltration and obvious pathological injury in myocardial tissue. ROS, MDA levels and myocardial cell apoptosis rate were significantly increased (P<0.01), SOD level, ATP content, and membrane potential were significantly decreased (P<0.01). The activity of mitochondrial respiratory chain complexes (Ⅰ-Ⅳ) was significantly decreased (P<0.01). Levels of p-Drp1, Fis1, MFF proteins were significantly up-regulated (P<0.01), while Sirt3, p-AMPK, OPA1 proteins level were significantly down-regulated (P<0.01). Compared with model group, LVIDd and LVIDs were significantly decreased (P<0.01), LVEF and LVFS were significantly increased (P<0.01). Inflammatory cell infiltration and pathological damage of myocardial tissue were significantly relieved. ROS, MDA levels and myocardial cell apoptosis rate were significantly decreased in Linggui Zhugantang group and Captopril group (P<0.01), SOD level, ATP content, and membrane potential significantly increased (P<0.01). The activity of mitochondrial respiratory chain complexes (Ⅰ-Ⅳ) increased significantly (P<0.01),and p-Drp1, Fis1, MFF protein levels were significantly down-regulated (P<0.01), Sirt3, p-AMPK, OPA1 protein were significantly up-regulated (P<0.01). ConclusionLinggui Zhugantang can alleviate oxidative stress and apoptosis damage of myocardial cells, maintain mitochondrial function stability, and its effect may be related to mitochondrial mitosis fusion and Sirt3/AMPK signaling pathway.
4.Hepatitis B virus carriage among pregnant and lying-in womenin Cangnan County from 2011 to 2022

Objective:
To investigate the prevalence of hepatitis B virus (HBV) carriage among pregnant and lying-in women in Cangnan County, Zhejiang Province from 2011 to 2022 and identify the influencing factors, so as to provide insights into the guidance of healthcare among HBV carriers during pregnancy.
Methods:
A total of 34 403 women delivered in The Third People's Hospital of Cangnan County from January 2011 to July 2022 were enrolled, and their demographics, HBV carriage and pregnant outcomes were collected. The prevalence of HBV carriage was analyzed among pregnant and lying-in women, and factors affecting HBV carriage were identified using a multivariable logistic regression model.
Results:
A total of 34 403 pregnant and lying-in women were enrolled, with a median age of 27.00 (interquartile range, 7.00) years, and including 8 118 floating populations (23.60%). The overall prevalence of HBV carriage was 3.44%, and the prevalence of HBV carriage was 1.59% from 2011 to 2014, 4.08% from 2015 to 2018 and 6.86% from 2019 to 2022, appearing a tendency towards a rise (P<0.05). Multivariable logistic regression analysis identified estimated age of delivery (20-24 years, OR=1.832, 95%CI: 1.037-3.235; 25-29 years, OR=2.404, 95%CI: 1.372-4.214; 30-34 years, OR=2.914, 95%CI: 1.656-5.129; 35-39 years, OR=3.116, 95%CI: 1.741-5.576; 40 years and older, OR=2.358, 95%CI: 1.145-4.858), floating population (OR=0.670, 95%CI: 0.574-0.782), scarred uterus after cesarean section (OR=1.228, 95%CI: 1.076-1.521) and year of delivery (from 2015 to 2018, OR=2.504, 95%CI: 2.143-2.926; from 2019 to 2022, OR=4.425, 95%CI: 3.779-5.182) as factors affecting HBV carriage among pregnant and lying-in women.
Conclusions
The prevalence of HBV carriage rate appeared a tendency towards a rise among pregnant and lying-in women in Cangnan County from 2011 to 2022. Estimated age of delivery, floating population, year of delivery and scarred uterus after cesarean section are factors affecting HBV carriage.
5.Clinical effects of antegrade anterolateral thigh pedicled flaps in repairing wounds in the perineum or inguinal regions

Objective:To explore the clinical effects of antegrade anterolateral thigh pedicled flaps in repairing wounds in the perineum or inguinal regions.Methods:The study was a retrospective observational study. From January 2022 to May 2024, 7 patients with wounds in the perineum or inguinal regions who met the inclusion criteria were admitted to Fujian Medical University Union Hospital, including 5 males and 2 females, aged 54 to 72 years. The wound area after debridement was 8 cm×6 cm to 16 cm×11 cm. During the operation, antegrade anterolateral thigh pedicled flaps with area of 9 cm×7 cm to 18 cm×13 cm were harvested to repair the wounds. The wounds in the flap donor sites were sutured directly or repaired with split-thickness skin grafts from the thigh. The survival of flaps and the healing of wounds and the survival of skin grafts in flap donor sites were observed after operation. During follow-up, the texture, color, and blood supply of flaps were observed, the muscle strength of the lower extremities on the affected side was evaluated according to Lovett muscle strength grading standard, the muscle tension of the lower extremities on the affected side was evaluated by modified Ashworth scale, and the recovery of lower extremity movement, wound recurrence, and scar formation in the flap donor sites were observed.Results:The flaps all survived successfully after operation. All the wounds in the flap donor sites healed and the skin grafts all survived. During 2 to 29 months of follow-up, the flaps were soft in texture, similar in color to the surrounding normal skin tissue with good blood supply. The muscle strength of the lower extremities on the affected side was evaluated as grade 4 in 2 patients and grade 5 in 5 patients. The muscle tension was grade 0 in all patients with no abnormality in movement. No wound recurred and no obvious scar deformity in the flap donor site was observed.Conclusions:The antegrade anterolateral thigh pedicled flap transplant is one of the effective methods to repair wounds in the perineum and inguinal regions. The procedure is easy to operate, with good postoperative appearances of the donor and recipient sites and good function of affected limbs, which is worthy of clinical promotion.

Odontogenic maxillary sinusitis(OMS)is a subtype of maxillary sinusitis(MS).It is actually inflammation of the maxillary sinus that secondary to adjacent infectious maxillary dental lesion.Due to the lack of unique clinical features,OMS is difficult to distinguish from other types of rhinosinusitis.Besides,the characteristic infectious pathogeny of OMS makes it is resistant to conventional therapies of rhinosinusitis.Its current diagnosis and treatment are thus facing great difficulties.The multi-disciplinary cooperation between otolaryngologists and dentists is absolutely urgent to settle these questions and to acquire standardized diagnostic and treatment regimen for OMS.However,this disease has actually received little attention and has been underrepresented by relatively low publication volume and quality.Based on systematically reviewed literature and practical experiences of expert members,our consensus focuses on characteristics,symptoms,classification and diagnosis of OMS,and further put forward multi-disciplinary treatment decisions for OMS,as well as the common treatment complications and relative managements.This consensus aims to increase attention to OMS,and optimize the clinical diagnosis and decision-making of OMS,which finally provides evidence-based options for OMS clinical management.

Endo-periodontal lesions(EPLs)involve both the periodontium and pulp tissue and have complicated etiologies and pathogenic mechanisms,including unique anatomical and microbiological characteristics and multiple contributing factors.This etiological complexity leads to difficulties in determining patient prognosis,posing great challenges in clinical practice.Furthermore,EPL-affected teeth require multidisciplinary therapy,including periodontal therapy,endodontic therapy and others,but there is still much debate about the appropriate timing of periodontal therapy and root canal therapy.By compiling the most recent findings on the etiology,pathogenesis,clinical characteristics,diagnosis,therapy,and prognosis of EPL-affected teeth,this consensus sought to support clinicians in making the best possible treatment decisions based on both biological and clinical evidence.
8.The effect of metformin on intestinal metabolomics in aged septic acute lung injury rats

Objective:To observe the effect of metformin on intestinal metabolites and its protective effect on lung injury in an elderly sepsis rat.Methods:SD rats were fed at the Animal Laboratory Center of Zhengzhou University, fourteen elderly SD rats were randomly divided into three groups: sham surgery (age-Sham, AgS group, n=4), cecal ligation and perforation induced sepsis (age-Cecal ligation and puncture, AgCLP group, n=5), and oral administration of metformin (100 mg/kg) after 1 h of CLP treatment (age-Metformin, AgMET group, n=5). Collected rat feces 24 h after modeling, and analyzed the composition and inter group differences of metabolites in the feces using liquid chromatography tandem mass spectrometry non targeted metabolomics. Collected rat lung tissues and detected the expression levels of inflammation related genes and pathological changes in the tissue. The visualization of metabolic changes between groups were presented using orthogonal partial least squares discriminant analysis, heatmaps, and unsupervised principal component analysis, respectively. MetaboAnalyst 3.0 was used to evaluate the Pathway analysis of metabolites, and this software was based on the KEGG database and the human metabolome database. Results:The expressions of CCL4 ( F=203.00, P<0.001), CXCL1( F=65.69, P<0.001), IL-6 ( F=38.94, P<0.002), TNF-α ( F=14.85, P=0.005) between two groups of rats were significantly different (all P<0.05). However, there was no significant difference in CCL2 expression between AgCLP group and AgMET group. Furthermore, compared with the AgS group, the relative intensities of 17 metabolites such as 7-methylxanthine, N-Arachidonylglycine and Manolide in AgCLP group were significantly increased, whereas the 9 metabolites such as Phenazone, Gly-Phe and Valyproline were significantly decreased, and metformin treatment could reverse these changes of the above metabolites. Correlation analysis showed that the IL-6 and TNF-α levels were positively correlated with the relative strength of 7-Methylxanthine, N-Arachidonylglycine and other metabolites, but negatively correlated with the Phenazone and Gly-Phe. CCL4 and CXCL1 were positively correlated with Manolide, but negatively correlated with Valyproline. Conclusion:The results of this study showed that metformin improved sepsis induced acute lung injury and regulates the host intestinal metabolites, which might provide a potential and effective treatment for elderly sepsis induced acute lung injury.
9.Relationship between sedentary behavior and quality of life in stable chronic obstructive pulmonary disease patients: the mediating role of frailty

Objective:To investigate the mediating role of frailty in the relationship between sedentary behavior and quality of life in patients with stable chronic obstructive pulmonary disease (COPD), so as to provide empirical evidence for improving the quality of life in COPD patients.Methods:This was a cross-sectional survey. Two hundred and twenty-eight patients with stable COPD attending outpatient clinic in Chongming Branch of Shanghai Tenth People′s Hospital from January 2022 to February 2023 were selected by convenience sampling method. The General Information Questionnaire, the International Physical Activity Questionnaire, the Tilburg Frailty Indicator, and the COPD Assessment Test Questionnaire were investigated. Structural equation modeling was used to analyze the mediating role of frailty between sedentary behavior and quality of life, and Bootstrap method was used to analyze the mediating effect values.Results:Finally, 219 valid questionnaires were collected. The effective recovery rate was 96.05%(219/228). There were 168 males and 51 females, aged 69(62, 74) years old. The sedentary time of stable COPD patients was 6(5, 8) h/d, the frailty score was 5(3, 7) points, and the quality of life score was (13.56 ± 2.56) points. Pearson correlation analysis showed that the sedentary time, frailty in stable COPD were positively correlated with quality of life( r=0.420, 0.582, both P<0.01), and the sedentary time was also positively correlated with frailty ( r=0.698, P<0.01). Bootstrap method test results showed that the direct effect of sedentary time was 0.228 (95% CI 0.082-0.374), the indirect effect mediated by frailty was 0.169 (95% CI 0.080-0.215), and the total effect was 0.397, and the 95% CI of both the direct effect and the mediated effect did not contain 0. This model was partially mediated model with a 42.57% mediated effect. Conclusions:The quality of life of patients with stable COPD is at an intermediate level, and sedentary behavior can not only directly affect quality of life, but also indirectly affect quality of life by causing or aggravating debilitation. In clinical work, patients should be encouraged to avoid causing or aggravating debilitation due to prolonged sedentary behavior by standing, changing position frequently or performing light physical activities to promote the improvement of quality of life in the stable phase.
10.Clinical characterization of smoking and nonsmoking patients with chronic obstructive pulmonary disease combined with cardiovascular disease based on propensity score matching

Objective:To compare the clinical characteristics of patients with chronic obstructive pulmonary disease (COPD) and cardiovascular diseases between smokers and non-smokers.Methods:In this retrospective cross-sectional study, a total of 735 patients with COPD and comorbid cardiovascular disease who were hospitalized in the Department of Respiratory and Critical Care Medicine of Peking University Third Hospital from January 1, 2017 to August 1, 2022 were enrolled, and were divided into a smoking group (603 cases) and a non-smoking group (132 cases) according to whether or not they had ever smoked. Clinical data, blood counts, C-reactive protein, procalcitonin, echocardiography, comorbidities, hospitalization costs and outcomes were compared between the two groups. Propensity score matching method was used to balance the baseline information between smoking patients and non-smoking patients for further paired analysis. Data were statistically analyzed using t-test, chi-square test, and rank sum test. Results:The percentage of females was significantly higher in patients in the nonsmoking group than in those in the smoking group (65.9% vs 13.6%, P<0.001), and body mass index (BMI) was also significantly higher than in those in the smoking group [(24.4±4.7) vs (23.5±4.4) kg/m 2, P=0.022]. Inflammation indicators neutrophil-to-lymphocyte ratio (NLR) [3.8 (2.2, 8.1) vs 4.9 (2.9, 8.3), P=0.018], C-reactive protein [1.3(0.5, 8.1) vs 7.9(1.1, 35.1) mg/L, P=0.001] were lower in the non-smoking group than in the smoking group. The ratio of early transmitral flow velocity to early mitral annular velocity (E/Em) values were significantly higher in the smoking group than in the nonsmoking group [10(7, 12) vs 8(7, 10) P=0.030]. The cardiovascular disease composition was dominated by hypertension with a lower percentage of combined coronary heart disease in the nonsmoking group than in the smoking group (22.0% vs 35.0%, P=0.034). The two groups were similar in terms of cost during hospitalization and mortality indicators. Conclusions:Among patients with COPD, smokers exhibit higher levels of systemic inflammation, greater cardiac involvement, and an increased risk of ICU admission than non-smokers. Therefore, they require enhanced daily health management and management during hospitalization.
