1.Quality Evaluation of Black Panacis Quinquefolii Radix Based on Neuroprotective Spectrum-effect Relationship

		                        			ObjectiveTo clarify the neuroprotective effect of black Panacis Quinquefolii Radix(PQR) and explore its active ingredients, with the aim of establishing an activity-oriented quality evaluation method. MethodsTransgenic Tg(HuC∶EGFP) zebrafish was used to establish a neuronal injury model by aluminum chloride immersion. Different doses(10, 20 mg·L-1) of PQR and black PQR ethanol extracts were administered. The neuroprotective effects of PQR and black PQR were compared by analyzing the fluorescent area and intensity of zebrafish neurons. Based on ultra-performance liquid chromatography(UPLC), a fingerprint profile of black PQR was established, followed by principal component analysis(PCA) and orthogonal partial least squares-discriminant analysis(OPLS-DA). Differential components were screened using the criteria of variable importance in the projection(VIP) value>1 and P<0.05. The neuroprotective activity of 14 batches of black PQR was assessed, and Spearman correlation analysis was used to identify saponins related to neuroprotective activity, which were then validated. Based on the above results, active marker components were determined, and an UPLC method was established for their quantitation with clear content limits. ResultsPharmacological efficacy results showed that both PQR and black PQR at different doses could significantly improved neuronal damage in zebrafish. At a dose of 20 mg·L-1, black PQR demonstrated superior efficacy(P<0.05). The fingerprint similarities of 14 batches of black PQR were>0.94, with 26 common peaks identified. Through comparison with the reference standards, 8 components were confirmed, including peak 1(ginsenoside Rg1), peak 2(ginsenoside Re), peak 5(ginsenoside Rb1), peak 9(ginsenoside Rd), peak 16[ginsenoside 20(S)-Rg3], peak 17[ginsenoside 20(R)-Rg3], peak 18(ginsenoside Rk1), and peak 19(ginsenoside Rg5). The results of PCA and OPLS-DA indicated that there were differences in saponins among black PQR samples from different origins, and 12 differential components were screened. All 14 batches of black PQR exhibited good protective effects on zebrafish neurons, with Shaanxi-produced black PQR showing superior protective effects compared to the other three production regions. Spearman correlation analysis revealed that a total of 11 components, including ginsenosides 20(S)-Rg3, 20(R)-Rg3, Rk1 and Rg5, showed a significant positive correlation with the neuroprotective effect in zebrafish(P<0.05). The activity validation results indicated that ginsenosides 20(S)-Rg3, 20(R)-Rg3, Rk1 and Rg5 were the primary components responsible for the neuroprotective effects of black PQR. Quantitative analysis showed that the content of ginsenoside 20(S)-Rg3 in 14 batches of black PQR ranged from 0.17% to 0.52%, and the repair rate of neuronal damage ranged from 42.77% to 97.83%. ConclusionBased on the fingerprint and neuronal protective activity, the spectrum-effect related quality control model of black PQR was established, with ginsenoside 20(S)-Rg3 as the quality control index, and the neuronal damage repair rate≥60% as the evaluation standard, the minimum limit of ginsenoside 20(S)-Rg3 in black PQR should be≥0.20%. 
		                        		
		                        		
		                        		
		                        	
2.Construction of a key technical indicator system for in-hospital treatment and nursing of patients with nuclear radiation injury

		                        			Objective To construct a key technical indicator system for in-hospital treatment and nursing of patients with nuclear radiation injury, and provide a basis for the implementation of such treatment and nursing. Methods The draft of the key technical indicator system for in-hospital treatment and nursing of patients with nuclear radiation injury was determined by literature review, case study, and field investigation. The indicators of the system were determined through two rounds of Delphi consultation and using the precedence chart method. According to the criteria of indicator evaluation, the reliability of expert opinions, and the opinions of the research group, the indicators were refined and evaluated. Results Twenty experts were included for two rounds of consultation via mailed inquiries, with a 100% effective response rate in both rounds. The expert authority coefficients were both 0.945, and the Kendall’s W values were 0.347 and 0.448, respectively (P < 0.05). Following the expert consultations, 1 indicator was deleted, 12 indicators were added, and 6 indicators were modified. The key technical indicator system for in-hospital treatment and nursing of patients with nuclear radiation injury established in this study included 4 first-level indicators, 17 second-level indicators, and 73 third-level indicators. The means of importance assignment for all indicators were > 4.00, and the coefficients of variation were < 0.25. Conclusion The key technical indicator system for in-hospital treatment and nursing of patients with nuclear radiation injury established in this study is scientifically rigorous and practically grounded. The indicators demonstrate strong professional relevance and provide important guidance for in-hospital treatment and nursing of patients with nuclear radiation injury.
		                        		
		                        		
		                        		
		                        	
3.Improvement effects of pachymic acid on myocardial injury in coronary heart disease rats by regulating mito-chondrial autophagy mediated by the PINK1/Parkin signaling pathway

		                        			OBJECTIVE To explore whether pachymic acid (Pac) regulates mitochondrial autophagy mediated by the PTEN- induced kinase 1 (PINK1)/Parkin RBR E3 ubiquitin-protein ligase (Parkin) signaling pathway to alleviate myocardial injury in coronary heart disease (CHD) rats. METHODS SD rats were divided into control (Con) group, CHD group, Pac low-dose group (Pac-L group), Pac high-dose group (Pac-H group), Pac-H+PINK1/Parkin signaling pathway inhibitor group (Pac-H+3-MA group), with 10 rats in each group. Except for the Con group, CHD models were established in the remaining groups of rats. After successful modeling, the rats in each group were intraperitoneally injected with the corresponding drugs or normal saline. After continuous intervention for 4 weeks, the left ventricular ejection fraction (LVEF), left ventricular end-diastolic volume (LVEDV), left ventricular end-systolic volume (LVESV), and mean arterial pressure (MAP) of the rats were detected. The levels of creatine kinase isoenzyme (CK-MB), lactate dehydrogenase (LDH), cardiac troponin I (cTnI), and cardiac troponin T (cTnT) in the serum, as well as the levels of tumor necrosis factor-α (TNF-α), interleukin-10 (IL-10), IL-1β, reactive oxygen species (ROS), malondialdehyde (MDA) in the myocardial tissue, and the activities of catalase (CAT) and superoxide dismutase (SOD), as well as the expression levels of p62, cleaved caspase-3, Parkin, PINK1 proteins and the ratio of microtubule-associated protein 1 light chain 3 Ⅱ (LC3Ⅱ)/LC3Ⅰ ratio were measured. The morphology of myocardial tissue and mitochondrial autophagic vesicles were observed, and the number of mitochondrial autophagic vesicles per unit area and the rate of cardiomyocyte apoptosis were counted. RESULTS Compared with CHD group, LVEF, MAP, IL-10 levels, CAT and SOD activities, p62, Parkin, PINK1 protein expressions, LC3Ⅱ/LC3Ⅰ ratio, the numbers of mitochondrial autophagic vesicles per unit area in the Pac-L and Pac-H E-mail:hzdpft@163.com groups were increased significantly (P<0.05); the levels of LVEDV, LVESV, CK-MB, LDH, cTnI, cTnT, TNF-α, IL-1β, ROS and MDA, cell apoptosis rates, and protein expression of cleaved caspase-3 were all decreased significantly (P<0.05); and the changes in various indicators were more pronounced in the Pac-H group (P<0.05); both groups showed varying degree of improvement in myocardial histopathological morphology. Compared with the Pac-H group, the aforementioned indicators in rats from the Pac-H+3-MA group were all significantly reversed (P<0.05). CONCLUSIONS Pac may promote mitochondrial autophagy in cardiomyocytes of CHD rats by activating the PINK1/ Parkin signaling pathway, thereby reducing inflammatory responses and oxidative stress and improving myocardial injury.
		                        		
		                        		
		                        		
		                        	
4.Multi-index quantitative detection and quality difference evaluation of Gleditsia sinensis from different producing areas

		                        			OBJECTIVE To establish a multi-index quantitative detection method, and to evaluate the quality difference of Gleditsia sinensis from different producing areas. METHODS The contents of protocatechuic acid, vanillic acid, isoscopoletin, scoparone, isovitexin, fustin, taxifolin, fisetin, quercetin, kaempferol, echinocystic acid, betulinic acid, β -sitosterol and stigmasterol were detected by high performance liquid chromatography-quantitative analysis of multi-components by single marker (HPLC-QAMS). The chromatographic column was Kromasil C18, the mobile phase was 0.2% phosphoric acid-acetonitrile solution (gradient elution), the detection wavelengths were 254, 360, 210 nm for different index components, the column temperature was 30 ℃ , the flow rate was 1.0 mL/min, and the sample injection volume was 10 μL. The contents of extract and total ash were detected according to the method of Chinese Pharmacopoeia. The quality differences of 30 batches of G. sinensis (No. S1-S30) from different producing areas were evaluated by chemometrics, weighted technique for order preference by similarity to an ideal solution (TOPSIS) analysis and Logistic regression model. RESULTS The linear ranges of 14 components were 1.55-77.50, 0.71- 35.50, 0.28-14.00, 0.96-48.00, 1.77-88.50, 0.09-4.50, 4.65-232.50, 1.49-74.50, 0.37-18.50, 1.18-59.00, 7.35-367.50, 3.58- 179.00, 0.49-24.50 and 0.21-10.50 μg/mL, respectively (all r>0.999). The RSDs of precision, stability (24 h) and repeatability were less than 2.00%; the average recoveries were 96.99%-100.13% (all RSDs<2.00%), and the relative correction factor had good repeatability. There was no ZY2019Q014) significant difference in the content of 14 components measured by QAMS method and external standard method (P>0.05). The results of chemometrics showed that 30 batches of samples were clustered into 3 categories: S1 to S11 form one category, S12 to S20 form another category, and S21 to S30 constitute the third category. Echinocystic acid, betulinic acid, taxifolin, kaempferol, isovitexin, scoparone and protocatechuic acid may be the differential components affecting the quality of G. sinensis from different producing areas. The analysis results of the weighted TOPSIS method revealed that relative closeness (Jb) for 30 batches of G. sinensis ranged from 0.144 5 to 0.721 8, with S27 achieving the highest value (Jb) of 0.721 8. The analysis results of the Logistic regression model showed that S21-S30 batches of samples were of superior grade, S1-S11 were of intermediate grade, and S12-S20 were of inferior grade. CONCLUSIONS The established HPLC-QAMS method is simple and accurate. 5.Effect of midazolam on postoperative delirium in elderly patients with preoperative moderate-severe anxiety undergoing radical resection of colorectal cancer

		                        			Objective To investigate the effect of preoperative oral midazolam on postoperative de-lirium in elderly patients with preoperative moderate-severe anxiety undergoing radical resection of colorectal cancer.Methods Eighty elderly patients undergoing laparoscopic surgery for radical resection of colorectal cancer,32 males and 48 females,aged 65-79 years,BMI 21-27 kg/m2,ASA physical status Ⅱ or Ⅲ,the state-trait anxiety inventory(STAI-S)≥38 scores at admission were selected.Patients were divided into two groups using random number method:control group and midazolam group,40 patients in each group.The midazolam group were administrated midazolam 7.5 mg per night till one day before surgery,while the placebo was administrated in the control group.The incidence of delirium 3 days after surgery and the STAI-S scores of one day before surgery were evaluated.The HR and MAP at entry,30 minutes after an-esthesia induction,1 hour,2 hours after anesthesia induction,and 30 minutes after extubation were recor-ded.The total dose of propofol,remifentanil and dexmedetomidine and the using rate of metaraminol were recorded.The visual analog scale scores 30 minutes after extubation,24 and 72 hours after surgery,the u-sing rate of tramadol,and the extubation time were recorded.Results Compared with the control group,the STAI-S scores of one day before surgery,and the incidence of postoperative delirium,the rate of using metaraminol,the VAS scores 30 minutes after extubation and 24 hours after surgery,the rate using of tram-adol were significantly decreased in the midazolam group(P<0.05).There were no significant differences in total dose of propofol,remifentanil,and dexmedetomidine,extubation time between the two groups.Conclusion Preoperative oral midazolam can effectively reduce the incidence of postoperative delirium in elderly patients with preoperative moderate-severe anxiety undergoing radical resection of colorectal cancer.
		                        		
		                        		
		                        		
		                        	
6.Patient-derived parotid gland pleomorphic adenoma organoid model and its preliminary histological and functional characterization

		                        			Objective:To establish patient-derived organoid models of pleomorphic adenomas (PA) of the parotid gland and preliminarily characterize their histology, related biomarkers and functions.Methods:Fresh tumor tissue specimens were collected from surgical procedures of Oral and Maxillofacial Department. The harvested tissues were processed and cultured in a head and neck tumor organoid culture system to establish organoid models from parotid gland pleomorphic adenomas. The in vitro growth of PA organoids was recorded by light microscopy. The successfully established organoids were passaged and cryopreserved, and the cryopreserved PA organoids were revived and re-cultured to observe their viability and organoid regeneration ability. Histological characterization, as well as characterization and detection of related markers and functional proteins, were performed on the organoids, comparing them with the patient-derived tissues. Results:The constructed organoid model of pleomorphic adenoma exhibited a dense and compact three-dimensional spherical structure. Hematoxylin and eosin staining indicated morphological similarities between the organoid and its tissue of origin. Immunohistochemistry showed positive cytoplasmic staining for Calponin, cytokeratin 7, and epithelial membrane antigen in both the organoid and the source tumor tissue, suggesting consistent histopathological characteristics between the organoid and its tissue of origin. Periodic acid-Schiff staining of the organoid showed positive staining for glycogen, with positive staining located in the interior and periphery of the organoid, indicating that the organoid possessed secretory functions like the salivary gland.Conclusions:This study successfully constructed organoids of pleomorphic adenoma derived from patient samples. This model faithfully replicates the tissue morphology and biomarkers of the source tissue and exhibits biological functions associated with mucus secretion. 7.Analysis of Cognition Status and Influencing Factors of Medical Research Ethics Review Standards among Physicians in Tertiary Hospitals

		                        			Objective To understand the cognition status of medical scientific research ethics review norms among physicians in tertiary hospitals and the factors influencing their awareness.Methods A survey was conducted using a questionnaire.A total of 2 067 questionnaires were distributed to physicians in tertiary hospitals in Henan Province,with an effective response rate of 81.95%.Results(1)Physicians'awareness rate of Regulations on Ethical Review of Biomedical Research Involving Human Subjects reached 69.01%,while that of Opinions on Strengthening Ethical Governance in Science and Technology was only 34.30%.A total of 310 respondents were aware of all six medical research ethics-related regulations.(2)1 153 respondents were familiar with all four basic principles of medical ethics.(3)78.98%of physicians had received training in medical research ethics knowledge.Learning through medical school and special lectures made it easier for physicians to understand medical research ethics knowledge(P<0.05).(4)The results of binary logistic regression analysis showed that educational level,membership in an ethics committee,and receipt of ethics training significantly influenced physicians'understanding of medical research ethics knowledge(P<0.05).Conclusion Physicians have low awareness of newly issued medical research ethics review norms,targeted training is needed to enhance physicians'research ethics literacy.
		                        		
		                        		
		                        		
		                        	
8.Research on the application of immersive teaching combined with closed-loop assessment in respiratory medicine internship teaching

		                        			Objective:To explore the application effect of immersive teaching combined with closed-loop assessment in respiratory medicine internship teaching.Methods:A total of 140 students who interned in the Department of Respiratory Medicine from August 2021 to August 2023 were selected as research subjects. They were assigned to a control group and an observation group based on the time of admission, with 70 students in each group. The control group received traditional teaching, while the observation group received immersive teaching combined with closed-loop assessment. After the internship, the theories, clinical diagnosis and treatment, operation skills, empathy ability, and teaching effectiveness of the two groups of students were compared and evaluated.Results:The scores of theories, clinical diagnosis and treatment, and operation skills were significantly higher in the observation group than in the control group ( P<0.001). The observation group scored significantly higher on the empathy scale than the control group [(73.83±6.71) vs. (61.08±6.32); t=15.60, P<0.001]. The recognition rates of the observation group students were significantly higher in learning interest, self-learning ability, comprehensive analysis ability of diseases, clinical thinking ability, independent discovery, analysis, and problem-solving ability, and team collaboration ability, as compared with the control group ( P<0.05). Conclusions:Immersive teaching combined with closed-loop assessment is beneficial for improving the theoretical and practical levels of respiratory medicine students, enhancing their empathy and comprehensive analysis abilities, and improving clinical teaching effectiveness.
		                        		
		                        		
		                        		
		                        	
9.Comprehensive evaluation of Yifei qinghua ointment by multi-component quantitative analysis combined with chemometrics and entropy weight TOPSIS

		                        			OBJECTIVE To comprehensively evaluate the quality of Yifei qinghua ointment by multi-component quantitative analysis combined with chemometrics and entropy weight-technique for order preference by similarity to ideal solution (TOPSIS) method. METHODS The contents of lobetyolin, syringin, calycosin 7-O-β-D-glucopyranoside, ononin, astraisoflavan-7-O-β-D- glucoside, isomucronulatol 7-O-glucoside, astragaloside Ⅳ , deapi-platycoside E, platycoside E, platycodin D3, feretoside, asperulosidic acid, asperuloside, methylophiopogonanone A and methylophiopogonanone B in 14 batches of Yifei qinghua ointment (S1-S14) were determined by high-performance liquid chromatography method. Then, the quality of 14 batches of Yifei qinghua ointment was analyzed by chemometrics (principal component analysis and orthogonal partial least-squares discriminant analysis) and entropy weight TOPSIS method. RESULTS The results of chemometrics showed that 14 batches of Yifei qinghua ointment could be clustered into three categories, S1-S6 as the first category, S7-S10 as the second category, and S11-S14 as the third category. The values of variable importance for projection of calycosin 7-O-β-D-glucopyranoside, ononin, feretoside, astragaloside Ⅳ, astraisoflavan-7-O-β-D-glucoside, lobetyolin, methylophiopogonanone A and platycoside E were higher than 1. The results of the entropy weight TOPSIS method showed that the Euclidean closeness of the optimal solution of 14 batches of Yifei qinghua ointment were between 0.152 9 and 0.736 6, and that of sample S14 was the highest (0.736 6). CONCLUSIONS Among 14 batches of Yifei qinghua ointment, sample S14 has the best quality, and 8 components such as calycosin 7-O-β-D-glucopyranoside and ononin may be differential markers affecting the quality of Yifei qinghua ointment.
		                        		
		                        		
		                        		
		                        	
10.Epidemiological characteristics of severe fever with thrombocytopenia syndrome in China, 2018-2021

		                        			Objective:To understand the epidemiological characteristics and incidence trend of severe fever with thrombocytopenia syndrome (SFTS) in China.Methods:The incidence data of SFTS in China from 2018 to 2021 were collected from Chinese Disease Prevention and Control Information System for a statistical and descriptive epidemiological analysis by using software such as Excel 2016, Joinpoint 5.0.2, SPSS 26.0, and GraphPad Prism 8.0, especially, the SFTS cases reported monthly by key provinces were analyzed.Results:From 2018 to 2021, a total of 8 835 SFTS cases were reported in 25 provinces and the annual incidence showed an upward trend. The distribution of SFTS cases showed clustering, but the cases were mainly sporadic ones. The cases began to increase in March, mainly occurred during April to October (96.79%,8 551/8 835), and peaked during May to July. The cases were mainly distributed in middle-aged and old farmers, and slight more cases were women. The average case fatality rate was 5.38%, which varied greatly with areas. The case fatality rate tended to increase with age.Conclusion:From 2018 to 2021, the epidemiological characteristics of SFTS in China remained stable, but the number of reported cases gradually increased and the distribution showed an expanding trend, to which close attention should be paid.
