Objective:To construct a TPC-1 cell model that stably knocks out the HMGA2 by using CRISPR/Cas9 gene editing technology. Methods:Recombinant pLV[2gRNA]-EGFP:T2A:Puro- U6> {hHMGA2 [gRNA# A1]*}- U6>{hHMGA2 [gRNA#A2]*} of lentiviral plasmid vector was constructed: targeting HMGA2 Dual-gRNA sequence was designed, the synthesized Dual-gRNA fragment into pLV [2gRNA]-EGFP was cloned: T2A:Puro-U6 vector, extract a single clone for sequencing verification. the constructed recombinant plasmid vector with lentivirus was packed, and TPC-1 cells were infected, puromycin was used to obtain HMGA2 knock-out single clone, PCR and sequencing verification were performed, and real-time fluorescent quantitative qPCR was used to detect HMGA2 mRNA in cells Knockout efficiency. Results:After sequencing verification, pLV [2gRNA]-EGFP targeting HMGA2: T2A: Puro-U6>{hHMGA2 [gRNA#A1]*}-U6>{hHMGA2 [gRNA #A2]*} plasmid was successfully constructed; A single clone was picked for PCR identification and gene sequencing, TPC-1 cells were successfully obtained with HMGA2 gene completely knocked out; TPC-1 cells with HMGA2 knocked out were detected by real-time fluorescent quantitative qPCR, and they did not express HMGA2 mRNA.Conclusion:CRISPR/Cas9 gene editing technology enables us to construct a human papillary thyroid cancer cell line TPC-1 cell model with stable knockout of HMGA2.

Extracellular vesicles (EVs) are secreted by both eukaryotes and prokaryotes, and are present in all biological fluids of vertebrates, where they transfer DNA, RNA, proteins, lipids, and metabolites from donor to recipient cells in cell-to-cell communication. Some EV components can also indicate the type and biological status of their parent cells and serve as diagnostic targets for liquid biopsy. EVs can also natively carry or be modified to contain therapeutic agents (e.g., nucleic acids, proteins, polysaccharides, and small molecules) by physical, chemical, or bioengineering strategies. Due to their excellent biocompatibility and stability, EVs are ideal nanocarriers for bioactive ingredients to induce signal transduction, immunoregulation, or other therapeutic effects, which can be targeted to specific cell types. Herein, we review EV classification, intercellular communication, isolation, and characterization strategies as they apply to EV therapeutics. This review focuses on recent advances in EV applications as therapeutic carriers from in vitro research towards in vivo animal models and early clinical applications, using representative examples in the fields of cancer chemotherapeutic drug, cancer vaccine, infectious disease vaccines, regenerative medicine and gene therapy. Finally, we discuss current challenges for EV therapeutics and their future development.
4.Analysis of short-term efficacy of overlapping delta-shaped anastomosis in totally laparoscopic left hemicolectomy for digestive tract reconstruction.

Objective: At present, though the laparoscopic delta-shaped anastomosis and overlapping delta-shaped anastomosis have been gradually applied to complete laparoscopic radical resection of left hemicolon cancer, the comparative evaluation of their efficacy has not been mentioned in the published literatures. This study aims to explore the safety, feasibility and short-term efficacy of overlapping delta-shaped anastomosis (ODA) in totally laparoscopic left hemicolectomy. Methods: A retrospective cohort study was performed. The clinical and pathological data of patients who underwent totally laparoscopic left hemicolectomy at Department of Colorectal Surgery, Cancer Hospital, Chinese Academy of Medical Sciences and Peking Union Medical College from May 2017 to October 2020 were retrospectively analyzed. The case inclusion criteria were as follows: (1) age of 18-75 years; (2) body mass index (BMI) of 18.5-30 kg/m(2); (3) descending colonic and proximal sigmoid colonic adenocarcinoma was confirmed by preoperative colonoscopy and pathology. The exclusion criteria: (1) multiple primary colorectal cancers; (2) uncontrolled or poorly controlled diabetes mellitus, immune system diseases, or hematological diseases; (3) severe intestinal obstruction; (4) left transverse colonic or splenic flexure colonic adenocarcinoma; (5) distant metastasis of liver, lung and other viscera determined by enhanced computed tomography in the chest, abdomen and pelvis. According to the above criteria, a total of 115 patients with left hemicolon cancer were enrolled. All the patients underwent totally laparoscopic left hemicolectomy. Patients who underwent laparoscopic traditional delta-shaped anastomosis were selected as the control group. Patients who underwent laparoscopic ODA were selected as the ODA group. Effects of these two laparoscopic reconstruction methods on postoperative recovery and perioperative complications were analyzed and compared. Results: A total of 60 patients were enrolled in the ODA group, including 32 males and 28 females, with mean age of (57.3±10.4) years and body mass index (BMI) of (25.0±3.1) kg/m(2). While mean 55 patients were enrolled in the control group, including 31 males and 24 females, with mean age of (56.7±9.9) years and BMI of (24.4±2.9) kg/m(2). There was no statistically significant differences between the two groups in gender, age, BMI, American Society of Anesthesiologist (ASA) classification, TNM staging, preoperative abdominal surgery history, neoadjuvant chemotherapy and nutritional status (levels of hemoglobin, lymphocyte count, prealbumin, and albumin) (all P>0.05). All the patients in both groups received R0 resection without conversion to open laparotomy or conversion to extra-abdominal anastomosis. The digestive tract reconstruction time of the ODA group was significantly shorter than that of the control group [(15.1±1.7) minutes vs. (15.9±2.4) minutes, t=-2.053, P=0.042]. There were no statistically significant differences in the total operation time, intraoperative blood loss, length of skin incision, tumor size, proximal and distal margins, harvested lymph nodes, postoperative first ambulatory time, and postoperative hospital stay (all P>0.05). However, the time to the first flatus and the first defecation in the ODA group was significantly shorter as compared to control group [(1.5±0.5) days vs. (1.7±0.5) days, t=-2.028, P=0.045; (3.1±0.6) days vs. (3.4±0.7) days, t=-2.095, P=0.039], indicating faster intestinal function recovery in patients with ODA. The morbidity of postoperative complication was 6.7% (4/60) in the ODA group and 7.3% (4/55) in the control group and no significant difference was found (χ(2)=0.016, P=0.898). Two cases of incision infection, 1 case of lung infection, and 1 case of intra-abdominal infection occurred in the ODA group, while 3 cases of lung infection and 1 case of intra-abdominal infection occurred in the control group. All these complications were resolved after conservative treatment, and no secondary operation was performed due to complications. Conclusion: Compared with the traditional delta-shaped anastomosis, ODA is associated with a faster recovery of postoperative intestinal function without increasing the morbidity of postoperative complications, and has the satisfactory short-term efficacy.

5.Study on the levels and risk factors of Vitamin D from preconception to pregnancy

Objective To investigate the levels of serum Vitamin D(VD) from preconception to pregnancy in Jiading district of Shanghai, and explore the risk factors of VD concentration deficiency in the second and third trimester of pregnancy. Methods A total of 94 women who planned to have antenatal care and delivery in Maternal and Child Health Hospital of Jiading district of Shanghai from September 2016 to December 2018 were recruited as the study participants. Chemiluminescent immunoassay (CLIA) was used to examine the concentration of women’s serum VD from preconception to pregnancy. A total of 282 serum samples were detected. Results The prevalence of VD deficiency among 94 women from preconception to pregnancy were 40.4%,57.4% and 48.9% respectively. Results of the mixed linear model showed that women who had dyed or permed hair within the past 1 year had significantly lower serum VD levels during pregnancy (P<0.05), and women who often drank milk and ate deep-sea fish during pregnancy had higher VD levels during pregnancy (P<0.05). Conclusions VD deficiency was common among women in Jiading district of Shanghai, and it should be emphasized to supplement VD before and during pregnancy.

Objective To investigate the relationship between apoptotic protein Bcl-2 and Bax in rat hippocampal neurons and cognitive dysfunction in rats with abnormal thyroid function.Methods Thirty healthy Wistar rats of 8-week-old SPF grade were randomly divided into three groups:(1) Normal control group (n=10);(2)hypothyroidism group (n=10);(3) hyperthyroidism group (n=10).All rats were sacrificed at the 4th week by heart blood sampling.The serum T3 and T4,TSH were measured.Morris water maze was used to train rats in each group for 6 days.At the end of the experiment,the hippocampus was taken from the rats,and HE staining was performed.The expression of apoptotic protein bcl-2 and Bax in rat hippocampal neurons was detected by immunohistochemistry.Results ①The escape latency of hyperthyroidism and hypothyroidism group was higher than that of the normal group at different time points (P<0.05).In the test of the target area dwell time,the difference between the hyperthyroid group,the hypothyroid group and the normal group was statistically significant (P<0.05).In the distance test of the target quadrant,The differences between the three groups were statistically significant (P<0.05).In the number of passes through the target quadrant,the difference between hyperthyroidism group,hypothyroidism group and normal group was statistically significant (P<0.05),but there is no significant difference between hyperthyroidism group and hypothyroid group (P>0.05).②Hippocampus tissue HE staining:normal control group hippocampal neurons neatly arranged,the shape of the structure was complete and uniform,the nucleus was round or oval,nucleolus obvious,chromatin uniform level and more clear,nucleus round or oval,obviously,hyperthyroidism group,hypothyroidism group of neuronal structure loose,the number decreased,arranged disorder,deep nuclear staining,shrinkage,nucleolus disappeared,cytoplasm around the empty halo,cell spacing larger.③The positive cells expressing Bcl-2 and Bax protein in the hyperthyroidism group and the hypothyroidism group were increased compared with the normal control group (P<0.05).Compareds with hyperthyroidism group,the expression of Bcl-2 positive cells was increased in hypothyroidism (P<0.05).Conclusions ①The spatial learning and memory abilities of the rats with hypothyroidism and the hyperthyroid are lower than those in the normal control group.②The number of apoptotic protein positive cells in Bcl-2 and Bax neurons of hippocampus in rats with hypothyroidism and hyperthyroid increased,and the proportion of Bcl-2 and Bax was impaired,which indicates that apoptosis occurred in hippocampal neurons.This process may be one of the pathogenesis of cognitive impairment.
8.Relation between serum thyroid stimulating hormone and papillary thyroid carcinoma in Hashimoto's thyroiditis

Objective To investigate the effect of high serum thyroid stimulating hormone (TSH) on malignant transformation of thyroid tissue in Hashimoto's thyroiditis (HT).Methods The data of 569 patients pathologically confirmed HT admitted from Jan.2009 to Dec.2013 were retrospectively analyzed.According to with or without papillary thyroid carcinoma (PTC),patients were divided into HT group and HT complicated with PTC group.Age,gender,preoperative TSH,free thyroxine,free tri-iodothyronine,thyroidperoxidaseantibody (TPOAb),antithyroglobulinanfibody (TGAb),and tumor multifocality were investigated.Results Among the 569 patients,306 patients had HT without PTC and 263 had HT with PTC.The avaerage age of patients complicated with PTC was (45.52±10.58) years,younger than that of patients without PTC (51.63+9.50) years (P<0.05).In HT patients with or without PTC,31% (81/263) and 22.5% (69/306) of patients had high TSH levels,which was significantly different (P<0.05).Among HT patients complicated with PTC,tumor multifocality was identified in 32%(26/81) of patients with high level of TSH and in 29%(52/182) of patients with normal level of TSH.The difference had no statistical significance (P>0.05).Conclusion High preoperative TSH level may stimulate malignant transformation of thyroid tissue in HT patients.
9.Analysis of the 1000 Hz tympanometry in normal hearing neonates

Objective To analysis the clinical characteristics of 1000 Hz probe tone tympanometry obtained from normal neonates who passed the OAE screening.To calculate the normal range of the variances of the tympanometry,which may serve as the guide for newborn hearing screening and detect middle ear function in neonates.Methods OAE screening were performed to screen the hearing with GSI-70Automated OAE.The 1000 Hz probe tone tympanograms were obtained from 650 neonates who passed the TEOAE screening in both ears and were on the normal physiological conditions after birth with GSI Tympstar Version Ⅱ Middle Ear Analyzer.The means,the standard deviation,the 95% confidence interval were analyzed.Results The 1000 Hz tympanometric data showed the 1Y1B1G tympanogram in 732 ears (56.3%),the 1Y3B1G tympanogram in 145 ears(11.2%),the 0Y0B0G tympanogram in 269 ears (20.7%),other shapes in 154 ears (11.8%) according to the Vanhuyse model.The 1000 Hz tympanometric datas showed the single-peaked tympanogram in 967 ears,the 95% confidence interval of the tympanometric data were as follows:tympanometric peak pressure (Tpp) was from -55.0 to 180.0 daPa,peak compensated static acoustic admittance (Peak Ytm) was 0.03 -1.18mmHo,tympanometric width (TW) was 70.0 - 230.0 daPa.Conclusions The majority of 1000 Hz tympanograms from this study show the single-peaked in normal neonates.The 95% confidence interval of the tympanometric data may serve as a guide for hearing screening and detecting middle ear function in neonates.
10.Investigation on nursing staff' s cognition of nursing-patient communication barrier and technique

Objective To explore the nursing staff' s cognition of barrier, method and technique of nursing-patient communication, to provide reference basis for improving effective nursing-patient communication.Methods 445 nurses sampled randomly from 6 hospitals in Hubei Province were investigated by selfdesigned questionnaire related to nursing-patient communication. Results 87.2% patients had trained about communication according teaching and 26.52% mastered these techniques skillful; 98.88% paid attention to communication with patients and 74.16% believed effective communication was easily attained, but 81.35% ignored it and the influential factors for communication was too busy to operate, and 66.07% nurses held that poor communication was head reason to lead complaint. The higher the title and service year, the deeper the master degree for these nurses, but the younger communicated more frequently with patients(F =4.532,2.891,P < 0.05). Conclusions Nurse should strenthen their communication technique by learning, paying more attention to communicaiont skills and realizing advantage of senior nurses in order to provide effective communication.
