1.Analysis of an improved workflow of endoscope reprocessing for bedside endoscopic diagnosis and treatment on COVID-19 patients.

Severe cases infected with the coronavirus disease 2019 (COVID-19), named by the World Health Organization (WHO) on Feb. 11, 2020, tend to present a hypercatabolic state because of severe systemic consumption, and are susceptible to stress ulcers and even life-threatening gastrointestinal bleeding. Endoscopic diagnosis and treatment constitute an irreplaceable part in the handling of severe COVID-19 cases. Endoscopes, as reusable precision instruments with complicated structures, require more techniques than other medical devices in cleaning, disinfection, sterilization, and other reprocessing procedures. From 2016 to 2019, health care-acquired infection caused by improper endoscope reprocessing has always been among the top 5 on the list of top 10 health technology hazards issued by the Emergency Care Research Institute. Considering the highly infective nature of COVID-19 and the potential aerosol contamination therefrom, it is of pivotal significance to ensure that endoscopes are strictly reprocessed between uses. In accordance with the national standard "Regulation for Cleaning and Disinfection Technique of Flexible Endoscope (WS507-2016)," we improved the workflow of endoscope reprocessing including the selection of chemicals in an effort to ensure quality control throughout the clinical management towards COVID-19 patients. Based on the experience we attained from the 12 severe COVID-19 cases in our hospital who underwent endoscopy 23 times in total, the article provides an improved version of endoscopic reprocessing guidelines for bedside endoscopic diagnosis and treatment on COVID-19 patients for reference.

4.Principle and perspective of healthcare-associated infection control

The concept of healthcare-associated infections broadens the scope of the previously developed concept of hospital-acquired infections. The principles of infection control can be summarized as follows: fight against resistance and block the spread of infection. To manage Infection control well, one should be familiar with the relevant mode(s) of transmission, appropriate precautions, hand hygiene, and the concepts of incidence, disinfection, sterilization, and resistance. Medical directors should also run the infection control committee competently and play the role of a liaison between departments. Although infectious disease management originated as a system implemented by each medical institution, it is developing into a networked system due to improvements in efficiency, the emergence of widespread information sharing, and the necessity of coherent guidelines. Therefore, infectious disease management is expected to be integrated into the foundation of public and private partnerships in the future. In addition, developments in molecular and digital technologies are expected to have further effects on infection control in the future. In particular, since science and technology are developing exponentially, with innovations emerging on a daily basis, it is necessary for workers in the infection control management field to actively engage with rapidly changing trends.

5.Study on a New Ultraviolet Sterilizer to the Surface Disinfection of the Ultrasound Probe.

We studied the disinfection effect of a new ultraviolet (UV) sterilizer and its utilization on ultrasound probe surfaces. Carrier quantitative germicidal tests, simulated on-the-spot trials, and organic substance influence tests were used to carry out experimental observation. Artificially infected probes were disinfected using the sterilizer or a germicidal lamp for comparison. The total number and types of bacteria were determined and identified. Our results demonstrated the sterilizer had the best disinfection effect among three different disinfection methods in hospital. The sterilizer has been used in a hospital setting for 2 years with no notable damage to the ultrasound probe instrument. It has the advantages of fast disinfection, high disinfection effect, and good compatibility with the ultrasound instrument, worthy of being a promoted application in medical institutions.

6.The Status of Infection Control Nurses and Factors Affecting Infection Control Activities in Healthcare Facilities with more than 150 Beds in 2016 in KOREA

PURPOSE: The purpose of this study was to describe the status of infection control nurses (ICNs) and their activities, and to identify the factors affecting the level of infection control activities. Methods: Data were collected from 199 hospitals from June 24 to July 26, 2016. The structured questionnaires included status of infection control nurses, type and level of infection control activities. METHODS: Data were collected from 199 hospitals from June 24 to July 26, 2016. The structured questionnaires included status of infection control nurses, type and level of infection control activities. RESULTS: Most participating hospital were advanced general hospital (20.1%) and general hospital (67.8%). Among the hospitals, 86.4% had an infection control department (ICD). The average hospital work experience of ICNs was 14.62 years, and their average infection control career was for 4.94 years. Among the ICNs, 85.6% worked in full time and the average number of beds per ICN was 311.21. There were significant differences in the existence of ICD, infection control activities including surveillance, outbreak investigation, negative pressure room, hand hygiene monitoring, disinfection, and sterilization according to hospital size. The level of infection control activities was higher with more number of ICNs, ICN employment as full time, and healthcare institution accreditation status. The explanatory power was 37.5%. CONCLUSION: These results of this study which reflect infection control status of healthcare facilities with more than 150 beds in 2016 will provide baseline data to establish infection control system in small to medium sized hospitals after the Middle East Respiratory Syndrome outbreak in 2015.

7.Accuracy of spring-style and friction-style mechanical implant torque wrench.

PURPOSE: The aim of this study was to compare the accuracy of two different styles of mechanical implant torque wrenches before and after sterilization and repeated use. MATERIALS AND METHODS: Thirty new spring-style and friction-style mechanical implant torque wrenches from five different manufacturers were selected as subjects of study and assessed in terms of accuracy. Three manufacturers produced, spring-style (Dio, Neobiotech, Osstem) torque wrenches, and other three manufactures produced friction-style (Dentium, Osstem, Shinhung) torque wrenches. The procedure of target torque measurement was performed 10 times for each device and a digital torque gauge (MGT-12, Mark-10 Corp, USA) was used to record the measurements. All torque wrenches were used repeatedly for 20 times and then sterilized. This whole procedure was repeated 100 times and all torque wrenches were retested. The collected data was analyzed using a SPSS Statistics Analysis program to evaluate the accuracy of the torque wrenches and Mann-Whitney-U test was used for a comparative analysis. RESULTS: After sterilization and repeated use procedure, results did not show any significant difference between error values in the spring-style torque wrench group. However, the results demonstrated an increase in error values within the friction-style torque wrench group (P<.05). CONCLUSION: There was no difference between the accuracy of new spring-style torque wrenches before and after 100 cycles of sterilization and repeated use (P>.05). There was significant difference between the accuracy of new friction-style torque wrenches before and after 100 cycles of sterilization and repeated use (P<.05). Within the limitation of this study, spring-style torque wrenches were more accurate than friction-style torque wrenches in delivering target torque values.

8.Role of Clinical Endoscopy in Emphasizing Endoscope Disinfection.

Based on the unexpected Middle East respiratory syndrome (MERS) outbreak in Korea, it was established that the virus can spread easily, MERS exposure in hospitals carries an extreme risk for infection as well as mortality, and the sharing of information was essential for infection control. Although the incidence of exogenous infections related to contaminated endoscopes is very low, the majority of published outbreaks have been caused by various shortcomings in reprocessing procedures, including insufficient training or awareness. Ever since the inauguration of "Clinical Endoscopy" as an English-language journal of the Korean Society of Gastrointestinal Endoscopy in 2011, it has published several articles on disinfection of the endoscope and its accessories. Many Science Citation Index journals have also emphasized high-level disinfection of the gastrointestinal endoscope. Many papers have been produced specifically, since the outbreak of carbapenem-resistant Enterobacteriaceae in 2013. The recent review papers concluded that quality control is the most important issue among all the aspects of procedural care, including the efficiency of the gastrointestinal endoscopy unit and reprocessing room. Thorough reprocessing of endoscopes using high-level disinfection and sterilization methods may be essential for reducing the risk of infection.

9.A study on the impacts of infection control education on dental hygienists' perceptions for hepatitis type B and their practices to prevent infection.

PURPOSE: The purpose of this study is to examine the impacts of infection control education on dental hygienists'perceptions for hepatitis B and their behaviors. MATERIALS AND METHODS: Study participants were chosen by random selection among dental hygienists working in Seoul, Korea. A total of 150 questionnaires were sent out for the survey from April 2013 to May 2013, of which 140-excluding incomplete responses-were used for the study. Chi-square tests and t-tests were used (SPSS 19.0), and post-hoc analysis was performed as well. The maximum significance level was 0.05. RESULTS: Average 1.53 times infection control education was taken, but dental hygienists' perceptions for hepatitis B and their behaviors did not show statistical differences whether the education was done or not. Practices to prevent infection showed differences depending on categories, especially disinfection and sterilization were well performed by educated group. The most common reasons for not taking the education and noncompliance with infection prevention guidelines are lack of time and opportunity due to busy schedule. CONCLUSION: 1. The more highly educated, the greater number of patients per day, and the greater size of hospitals, the better infection control education was conducted. 2. Although hepatitis B is one of the most common chronic liver diseases in Korea, no significant correlation between perceptions of hepatitis B and infection control education was found. 3. Dental hygienists who received infection control education performed more efficient practices for protection against infections than those who did not.
