1.Research on the relationship between disease perception and demoralization of patients with chronic heart failure: the chain mediating effect of disease progression ′s fear and positive affect

Objective:To explore the mediating role of disease progression′s fear and positive affect in the relationship between disease perception and demoralization in patients with chronic heart failure. It provided a theoretical basis for targeted interventions for healthcare workers.Methods:From October 2022 to March 2023, 320 patients with chronic heart failure in the First Affiliated Hospital of Zhengzhou University were selected as the research objects by convenience sampling. The General Information Questionnaire, Demoralization Scale Redacted Mandarin Version, Brief Illness Perception Questionnaire, Fear of Progression Questionnaire-Short Form, Positive and Negative Affect Schedule (Positive Affect Schedule) were used to conduct the questionnaire survey exploring the mediating effects of fear of disease progression and positive emotions between disease perception and disorientation by construction structural equation model.Results:There were 268 valid questionnaires. Of the 268 patients, 168 were male and 100 were female, 3.36% (9/268) were ≤40 years old, 55.22% (148/268) were 41-65 years old, and 41.42% (111/268) were >65 years old. Correlation analysis showed that disease perception was positively correlated with disease progression ′s fear, and every dimension of demoralization ( r values were 0.300-0.586, all P<0.01), and negatively correlated with positive affect ( r=-0.374, P<0.01); disease progression′s fear was negatively correlated with positive affect ( r=-0.318, P<0.01), and positively correlated with every dimension of demoralization ( r values were 0.339-0.464, all P<0.01); positive affect was negatively correlated with every dimension of demoralization ( r values were -0.430--0.334, all P<0.01). Structural equation model analysis showed that the direct effect of disease perception on demoralization was significant ( β=0.407, P<0.01), and both mediating effects of disease progression ′s fear and positive affect between disease perception and demoralization in patients with chronic heart failure were significant ( β=0.074, 0.079, both P<0.01). The chain mediating effect of disease progression ′s fear and positive effect was also significant ( β=0.019, P<0.01). Conclusions:Disease perception could directly predict the demoralization of patients with chronic heart failure and indirectly predict the demoralization of patients with chronic heart failure through the mediating effect of disease progression ′s fear, positive affect, and the chain mediating effect of disease progression ′s fear and positive affect.
2.A clinical and electrodiagnostic study of peripheral neuropathy in prediabetic patients

Objective:To explore the clinical and electrophysiological characteristics of peripheral neuropathy in prediabetic patients.Methods:Subjects aged 20-65 years with high-risk factors of impaired glycemia enrolled in Beijing Tiantan Hospital, Capital Medical University from 2019 to 2022 were recruited to conduct oral glucose tolerance test, after excluding other causes of neuropathy or radiculopathy. Patients with impaired fasting glucose or impaired glucose tolerance were defined by American Diabetes Association criteria. These patients were divided into clinical polyneuropathy (PN) and clinical non-PN groups, according to the 2010 Toronto consensus criteria and the presence of PN symptoms and signs or not. Nerve conduction studies (NCS), F wave, sympathetic skin response (SSR), R-R interval variation (RRIV) and current perception thresholds (CPT) were performed and the abnormal rate was compared between different electrodiagnostic methods and between clinical subgroups.Results:Among the 73 prediabetic patients ultimately enrolled, only 20 (27.4%) can be diagnosed as clinical PN according to the Toronto consensus criteria. The abnormal rate of CPT (68.5%, 50/73) was significantly higher than those of F wave (2.7%, 2/73), lower limb NCS (0, 0/73), upper limb NCS changes of carpal tunnel syndrome (26.0%, 19/73), SSR (6.8%, 5/73) and RRIV (5.5%, 4/73; McNemar test, all P<0.001). With sinusoid-waveform current stimuli at frequencies of 2 000 Hz, 250 Hz and 5 Hz, the CPT device was used to measure cutaneous sensory thresholds of large myelinated, small myelinated and small unmyelinated sensory fibers respectively. CPT revealed a 21.9% (16/73) abnormal rate of unmyelinated C fiber in the hands of prediabetic patients, significantly higher than that of large myelinated Aβ fibers [8.2% (6/73), χ2=5.352, P=0.021]. Both abnormal rates of small myelinated Aδ [42.5% (31/73)] and unmyelinated C fibers [39.7% (29/73)] in the feet of prediabetic patients were significantly higher than that of large myelinated Aβ fibers [11.0% (8/73), χ2=18.508, 15.965, both P<0.001]. Compared with the clinical non-PN group, the abnormal rates of CPT [90.0% (18/20) vs 60.4% (32/53), χ2=5.904, P=0.015] and SSR [20.0% (4/20) vs 1.9% (1/53), P=0.016) were significantly higher in the clinical PN group. Conclusions:Peripheral neuropathies in prediabetic patients are usually asymptomatic or subclinical, and predispose to affect unmyelinated and small myelinated sensory fibers. Selective electrodiagnostic measurements of small fibers help to detect prediabetic neuropathies in the earliest stages of the disease.
3.Analysis on personal protection in occupational population at high risk for brucellosis and influencing factor in China

Objective:To understand the current status of personal protection in occupational population at high risk for brucellosis in China and provide evidence for the evaluation of implementation of National Brucellosis Prevention and Control Plan (2016-2020).Methods:Four counties in Shanxi Province and Xinjiang Uygur Autonomous Region were selected to conduct a questionnaire survey in occupational population at high risk for brucellosis from December 2019 to July 2020 by using cross-sectional survey methods.Results:A total of 2 384 persons at high risk for brucellosis were surveyed, and the standardized utilization rate of personal protective equipment (PPE) was 20.13% (480/2 384). The utilization rate of glove, mask, rubber shoe, and work cloth were 38.26% (912/2 384), 31.80% (758/2 384), 32.01% (763/2 384) and 30.87% (736/2 384),respectively. There were significant differences in the utilization rate and standardized utilization rate of the four types of PPE among populations in different age, occupation, educational level and area groups (all P<0.001). The utilization rate and standardized utilization rate of PPE were lower in people over 60 years old, women, farmers, and those with lower educational level. The results of multivariate analysis showed that occupation and area were the influencing factors for the standardized utilization of PPE, the standardized utilization rates of PPE were higher in herdsmen and veterinarians. The standardized utilization rate of PPE in Yanggao County and Huocheng County was significantly higher than that in Zuoyun County and Hunyuan County. Conclusions:The utilization rate of the four types of PPE in occupational population at high risk for brucellosis was not high in China, and the standardized utilization rate was low, lower than the requirement in National Brucellosis Prevention and Control Plan, and there were significant differences among different areas. It is urgent to distribute PPE to occupational population at high risk for brucellosis and carry out health education about PPE utilization. Meanwhile, it is necessary to strengthen information exchange or sharing among different areas.
4.Cite Space-based visualization of hot spots and trends in research on swallowing disorders in head and neck cancer patients

Objective:To analyze the research hotspots and trends of swallowing disorders in patients with head and neck tumors both domestically and internationally, to provide reference for future research directions.Methods:Using the China National Knowledge Infrastructure and Web of Science core collection databases as data sources, research literature related to swallowing disorders in head and neck tumor patients published from January 1, 2013, to December 31, 2022, was retrieved. A knowledge graph was drawn using Cite Space 6.2 R3 software, and the graph was interpreted and analyzed from the aspects of publication volume, publication country, and institution, high-frequency keyword co-occurrence and clustering, and emergent words.Results:A total of 427 English literature were included. The overall number of publications is on the rise. The countries with a high volume of collaborative publications were the United States, Australia, the Netherlands, and the United Kingdom. The top three institutions in terms of publication volume were the University of Queensland in Australia, the University of Newcastle in the UK, and the Amsterdam Institute in the Netherlands. A total of 10 cluster clusters and 25 prominent keywords were obtained. The hot topics of research and analysis were "swallowing dysfunction" "associated complications" and "long-term effects", with the forefront being "radiation-induced dry mouth syndrome".Conclusions:Compared to foreign countries, China currently has a relatively small number of publications, loose cooperation between research authors and institutions, and a need for more diverse disciplines and research methods. Focusing on hot topics and cutting-edge content, exploring the mechanism of swallowing disorders in head and neck tumors, implementing targeted interventions, strengthening health education, predicting postoperative dangerous complications, constructing feasible nursing plans, and improving the quality of life of patients will become the development trend of future research.
5.The serial mediating role of information literacy and evidence-based nursing practice competencies between self-efficacy and evidence-based nursing behaviors among otolaryngology nurses

Objective:To investigate the effects of self-efficacy on evidence-based nursing practice (EBNP) behaviors and its underlying mechanisms.Methods:Between September and October 2023, nurses from the otolaryngology departments of 51 hospitals in Henan Province were selected for this study by convenience sampling. Data were collected using general demographic surveys, Self-Efficacy Scale, Information Iiteracy Questionnaire, EBNP Competency Scale, and EBNP Behavior Scale. Pearson correlation analysis was used to explore the relationships among self-efficacy, information literacy, EBNP competencies, and EBNP behaviors. The mediating effects of information literacy and EBNP competencies between self-efficacy and EBNP behaviors were analyzed using Process v4.2.Results:A total of 580 questionnaires were distributed, with 551 valid responses, yielding a response rate of 95%. Scores for self-efficacy, information literacy, EBNP competencies, and EBNP behaviors were (28.60±6.48), (78.56±11.88), (34.86±8.59), and (28.26±7.64), respectively. Pearson correlation analysis indicated that information literacy was positively correlated with self-efficacy, EBNP competencies, and EBNP behaviors ( r=0.640, 0.687, and 0.661, respectively; P<0.001). Mediation tests confirmed the direct effect of self-efficacy on EBNP behaviors and the individual and serial mediating effects of information literacy and EBNP competencies, with mediating effect sizes of 27.273%, 18.379%, and 44.862%, respectively. Conclusions:The serial mediating roles of information literacy and EBNP competencies between self-efficacy and EBNP behaviors among otolaryngology nurses are validated. Future strategies could involve enhancing otolaryngology nurses' information literacy and EBNP competencies to promote their EBNP behaviors.
6.Analysis on the knowledge of prevention and control and its influencing factors among high-risk occupational groups in key areas of brucellosis in China

Objective:To understand the current status of knowledge of brucellosis prevention and control among occupational groups at high-risk of brucellosis, and to provide a scientific basis for assessing the effectiveness of brucellosis prevention and control in China.Methods:A total of four counties in Shanxi Province and Xinjiang Uygur Autonomous Region were selected as survey counties from 2019 to 2020, and 600 people from the occupational groups at high-risk of brucellosis in each survey county were selected as survey respondents, and basic information and knowledge of prevention and treatment were collected through questionnaires. Single-factor and multi-factor logistic regression models were used to analyze the factors affecting the population's knowledge of prevention and treatment.Results:A total of 2 411 people participated in the survey and 2 384 valid questionnaires were obtained, including 1 405 males and 979 females, with the youngest age being 18 years old, the oldest being 91 years old, and the median being 57 years old. The overall knowledge of brucellosis prevention and control was 17.74% (423/2 384). The knowledge rate was lower among people over 60 years old, farmers, and people with less than elementary school education (13.99%, 14.50%, and 13.78%), and higher among women, herders, and people with elementary school education (20.02%, 36.33%, and 19.58%); the knowledge rate was lower in Hunyuan County (0.51%), and the differences in overall knowledge rates by age, occupation, education level, and region were statistically significant (χ 2 = 18.25, 87.18, 11.05, 197.43, P < 0.001). Multi-factor logistic regression analysis showed that gender, occupation, literacy and region were associated with knowledge of prevention and treatment ( P < 0.05). Conclusions:The overall knowledge of prevention and treatment among high-risk occupational groups in the key areas of China's brucellosis prevention and treatment program is low, with a large gap with the goals of the national brucellosis prevention and treatment program, and gender, occupation, literacy level, and region are the influencing factors of the knowledge of prevention and treatment. There is an urgent need to carry out a variety of health education activities for high-risk occupational groups and to strengthen the exchange of experience on brucellosis prevention and treatment between regions.
7.Study on the characteristics of 18F-flurodeoxyglucose uptake in the microenvironment of hepatic alveolar echinococcosis

Objective:To investigate characteristics of the 18F-flurodeoxyglucose ( 18F-FDG) uptake intensity and ranges in distinct hepatic alveolar echinococcosis lesions. Methods:The clinical data of 39 patients with position emission tomography during Jan 2017 to Dec 2019 in the First Affiliated Hospital of Xinjiang Medical University were enrolled. Among them, there were 17 males and 22 females, aging from 15 to 65 years (median 34 years). Lesions were classified into six groups based on heterogenic scales of calcification and liquefaction: A. non-calcified and non-liquefied ( n=7); B. obvious calcified and non-liquefied ( n=7); C. partial calcified and partial liquefied( n=10); D. obvious calcified and partial liquefied ( n=5); E. partial calcified and subtotal liquefied ( n=5); F. obvious calcified and subtotal liquefied ( n=5). Tumor to background ratio (TBR) and width (W) of lesion infiltrative boundary were measured and calculated. Statistical comparison using Mann-Whitney U test as well as correlation analysis was performed. Results:TBR values [ M( Q1, Q3)] for each group were 4.40(3.66, 7.03), 2.55(1.69, 3.60), 3.73(3.37, 5.21), 2.90(2.75, 3.60), 3.80(3.49, 6.36), 2.49(2.21, 3.97), among which A>B, A>D, A>F, C>B, E>B ( U=3.0, 4.0, 4.5, 11.0, 5.0, all P<0.05); From the perspective of the calcification in each group, it was found that the lighter the calcification was, the greater the TBR value was. W values [ M( Q1, Q3)] for each group were [12.5(10.0, 19.5), 11.2(10.5, 12.5), 12.2(10.9, 13.2), 7.8(7.3, 9.3), 10.0(7.3, 13.4), 7.3(6.8, 7.6)] mm, among which A>D, A>F, B>D, B>F, C>D, C>F (all U=0, all P<0.05); According to the degree of calcification and liquefaction of lesions in each group, the lighter the calcification was, the greater the W value was; The heavier the liquefaction was, the smaller the W value was. A mild strength linear correlation has been observed between the TBR value and W value ( r=0.4136, P<0.05). Conclusions:Less calcification and liquefaction implicated higher 18F-FDG uptake intensity and wider range. Radical resection margins and tissue sampling should be individualized based on different lesion features in surgical treatment.
8.Effects of TTM staged nursing led by specialized nurses in symptom cluster management of radiating muscle group in nasopharyngeal carcinoma

Objective:To explore the effect of the transtheoretical model and stages of change (TTM) led by specialized nurses in the symptom cluster management of radiating muscle group in nasopharyngeal carcinoma.Methods:From January to October 2021, 58 nasopharyngeal carcinoma patients hospitalized in the First Affiliated Hospital of Zhengzhou University were selected as the study subject by convenience sampling. According to the random number table method, patients were divided into a control group and an experimental group, with 29 cases each. The control group implemented the routine nursing, while the experimental group used TTM staged nursing led by specialized nurses on the basis of the control group. We collected data before discharge and six months after discharge, and observed the changes in incisor distance, degree of mouth opening restriction, and swallowing difficulty in both groups of patients.Results:Six months after discharge, the distance between the incisors in the experimental group was greater than that in the control group, and the grading of mouth opening restriction was better than that in the control group, and the swallowing difficulty score of the patients was higher than that in the control group, and the differences were statistically significant ( P<0.05) . Conclusions:The TTM staged nursing led by specialized nurses can improve the symptom cluster management of radiating muscle group and the quality of life of nasopharyngeal cancer patients after radiotherapy, which is worthy of clinical practice.
9.Nursing intervention based on solution-focused group therapy in young patients with nasopharyngeal carcinoma

Objective:To explore the effects of solution-focused group therapy nursing intervention on body image and anxiety and depression in young patients with nasopharyngeal carcinoma.Methods:Using a convenience sampling method, 60 young patients with nasopharyngeal carcinoma hospitalized in the First Affiliated Hospital of Zhengzhou University from December 2020 to December 2021 were selected as the study subjects, and were divided into an observation group and a control group using the random number table method, with 30 cases each. The control group received conventional care, and the patients in the observation group implemented a focal solution-oriented group counselling nursing intervention on the basis of the control group. The scores on the Body Image Scale (BIS) and the Hospital Anxiety and Depression Scale (HADS) were compared between the two groups before and after the intervention.Results:Before the intervention, there were no statistically significant differences between the BIS and HADS scores of the two groups ( P>0.05). After the intervention, the total score of BIS and each dimension in the observation group and the score of HADS were lower than those in the control group, and the differences were statistically significant ( P<0.05) . Conclusions:The nursing intervention of solution-focused group therapy can effectively alleviate the body image dysregulation condition and reduce the level of anxiety and depression in young patients with nasopharyngeal carcinoma.
10.Effect of WeChat applet in immunotherapy of children with combined allergic rhinitis and asthma syndrome

Objective:To explore the effect of WeChat mini program in immunotherapy management of pediatric patients with combined allergic rhinitis and asthma syndrome (CARAS).Methods:Using the convenient sampling method, 101 children with CARAS who received sublingual immunotherapy in the First Affiliated Hospital of Zhengzhou University from January 2019 to January 2020 were selected as the research objects. The standardized management team of immunotherapy clinic was established. The newly developed specific immunotherapy Wechat applet was used to establish a new model of nurse-patient interaction and communication. The disease knowledge was regularly pushed, medication reminder, nurse telephone intervention and other standardized management were delivered through the Wechat applet to implement continuous nursing for children. The effects of this management model on medication compliance, rhinitis symptoms and asthma control were analyzed.Results:A total of 101 children were included in this study, 3 cases were lost to follow-up, and a total of 98 cases completed the study. After 12 months of intervention, the drug possession score of dust mite drops increased from (83.43±3.68) to (91.78±3.71), the symptom score of allergic rhinitis decreased from (5.83±1.32) to (1.94±0.98), and the score of asthma test questionnaire increased from (18.12±3.99) to (24.97±3.09), the proportion of children with complete control of asthma increased from 48.0% to 90.8%, and the proportion of children without control of asthma decreased from 10.2% to 1.0% (all P<0.05). Conclusion:The immunotherapy management of pediatric patients with combined allergic rhinitis and asthma syndrome based on WeChat mini program can effectively improve the compliance of children and their family, improve the control level of allergic rhinitis and asthma.
